1.Predictive value of high-sensitivity cardiac troponin T for death in old patients with stable coronary heart disease

Objective To investigate the value of hs-cTnT in predicting all-cause death in the elderly with SCAD.Methods A prospective cohort observation study was conducted on 274 old adults with SCAD hospitalized in our department from January 2016 to January 2019.Their hs-cTnT level was measured,and according to the results,they were divided into lower(≤13.0 ng/L,94 cases),middle(14.0-22.0 ng/L,94 cases)and upper(≥23.0 ng/L,86 cases)tertile groups.The general clinical data were compared among the three groups.Kaplan-Meier survival curve was drawn to analyze the survival differences among groups.Cox proportional hazards regression analysis was applied to identify risk factors for mortality.ROC curve analysis was applied to evaluate the predictive value of hs-cTnT for all-cause mortality.Results During a me-dian follow-up period of 32 months,62(22.63%)patients died among the 274 patients,account-ing for 75.8%dying of non-cardiovascular diseases.There were statistically differences in the three tertile groups in terms of age,male ratio,proportions of hypertension,chronic obstructive pulmonary disease and chronic kidney disease,number of comorbidities,estimated glomerular fil-tration rate,albumin and hemoglobin levels,left ventricular ejection fraction,left ventricular mass index,and mortality rate(P<0.05,P<0.01).COX proportional hazards regression model showed the upper tertile group had significantly lower cumulative survival rate than the middle and lower tertile groups(Plog rank<0.01).Multivariate Cox proportional hazards regression analysis indicated that hs-cTnT≥23.0 ng/L level was still a risk factor for death in both model 2(HR=3.749,95%CI:1.703-8.256,P=0.001)and model 3(HR=2.990,95%CI:1.358-6.581,P=0.007).ROC curve analysis revealed that the AUC value of hs-cTnT level in predicting death was 0.736,with a cut-off value of 25 ng/L.Conclusion For elderly SCAD patients,despite the existence of multiple comorbidities and the priority of non-cardiovascular death,hs-cTnT,a marker reflecting myocar-dial injury,is still a predictor for risk of death in the population.

3. Comparison of predictability on adverse events among different assessment tools in elderly discharged inpatients

Objective:
To investigate the predictability on adverse events(re-hospitalization and death)in elderly discharged inpatients among frailty phenotype(FP), frail scale(FS)and clinical frailty scale(CFS), in order to screen the simple and feasible frailty assessment tools for elderly discharged inpatients.
Methods:
Elderly discharged inpatients aged 65 years and over were recruited for this prospective cohort study.And they underwent frailty assessment by using FP, FS and CFS, respectively.Patients were followed up for more than 6 months after discharge, and adverse events including re-hospitalization or death after discharge were recorded.Cox regression model was adopted to evaluate the relationship between frailty and death or re-hospitalization.Predictive effects of three assessment methods on adverse events were compared by using receiver operating characteristic(ROC).
Results:
A total of 527 elderly patients aged(84.1±6.0)years with males of 61.9%(326/527)were successfully followed up.The detection rate of frailty by FP and FS evaluation tools was 26.0%(137/527)and 26.0%(137/527), respectively.The detection rate of moderate and severe frailty by CFS was 25.2%(133/527). The Cox regression model showed that the mortality was significantly higher in frailty patients by FP, FS, and CFS than in the non-frailty patients(
4.Effects of hyperbaric oxygen on the osteogenic differentiation of adipose-derived stem cells

Objective To investigate the effects of hyperbaric oxygen (HBO) on the osteogenic differentiation of adipose-derived stem cells.Methods Adipose-derived stem cells were isolated,cultured and passed to next generations.The fourth adipose-derived stem cells were collected and seeded to the 96-well plates (2 × 105 cells/well) for culture in osteogenic induction medium,and were randomly divided into the control group and the hyperbaric oxygen (HBO) group.In the entire experiment,the control group was only kept in the 5% CO2 incubator at a temperature of 37 ℃,while the HBO group was treated with HBO for 24 hours a day at the same culture conditions as the control group.(The HBO treatment cells were placed in the HBO chamber,which was washed with pure oxygen at a ventilation rate of 10 L/min for 15min.When oxygen flow rate was down-regulated to 2-3 L/min,the chamber was compressed for 20-25 min to a pressure of 2.5 ATA,which was maintained for 60 minutes.Then,uniform decompression was implemented for 20-25 minutes.At days 7,14 and 21,alkaline phosphatase activity of the cells was detected by alkaline phosphatase kit.At days14 and 21,osteocalcin concentration was detected by using the osteocalcin quantitative test kit,and at day 21,the calcified nodules were stained with Alizarin red.The data obtained were analyzed by 2 independent samples t test,paired sample t test and repeated test ANOVA.Results The cultured cells could express such mesenchymal stem cell markers as CD90 and CD105,but could not express hematopoietic markers such as CD34.After 7,14 and 21 days of culture,the activity of alkaline phosphatase in the HBO group was significantly increased,as compared with that of the control group A (P < 0.01).Following culture for 14 and 21 days,the expression level of osteocalcin in the HBO group was significantly elevated,as compared with that of the control group (P < 0.05).At day 21,the detection results by alizarin red staining showed that the number of calcified nodules for the HBO group was increased markedly as compared with that of the control group,and the calcified size became larger.Conclusions Treatment of HBO could increase osteogenic differentiation of adipose-derived stem cells.

Endoplasmic reticulum plays a key role in both basic structure formation and function performance of microenviron-ment. Endoplasmic reticulum homeostasis unbalance caused by endoplasmic reticulum stress has become a hot research topic in recent years. This paper focuses on the role of endoplasmic retic-ulum stress in ischemic stroke. Research progress of related sig-naling pathways were reviewed, especially mechanisms through which endoplasmic reticulum stress trigger the inflammatory reac-tion, so as to provide a new research method for prevention of is-chemic stroke.
