1.Research on dynamic monitoring of drug consumption based on seasonal Mann-Kendall trend test

OBJECTIVE To investigate a dynamic monitoring of drug consumption (DMDC) model based on the seasonal Mann-Kendall trend test, aiming to provide scientific evidence for the efficient and macroscopic monitoring of drug use. METHODS A monitoring list of key outpatient drugs was established based on the top 20% of drugs ranked by sales volume in the outpatient pharmacy in October 2024. A DMDC model based on the Mann-Kendall trend test was constructed using the monthly usage data of key outpatient drugs from November 2021 to October 2024, aiming to eliminate the impact of seasonal fluctuations and analyze the temporal trends in drug consumption. Taking mucolytic expectorants, triazole derivatives for dermatophytosis, and single-agent hydroxymethylglutaryl coenzyme A (HMG-CoA) reductase inhibitors as examples, the monitoring effectiveness of the DMDC model was demonstrated, and its performance was compared with that achieved by the traditional sequential growth rate ranking method. RESULTS A total of 215 drug varieties were included in the monitoring list, and DMDC models were successfully established for all of them. Among these, 119 showed a significant increasing trend (P<0.05, S′>0). The model successfully monitored the monthly consumption of mucolytic expectorants, triazole derivatives for dermatophytosis, and single- agent HMG-CoA reductase inhibitors. The precision and recall rates of the DMDC model for identifying abnormal drug use were 60.7% and 85.0%, respectively, both significantly higher than those of the sequential growth rate ranking method (8.3% and 15.0%, respectively) (χ2=20.114, P<0.001; χ2=19.600, P<0.001). CONCLUSIONS DMDC model based on the seasonal Mann-Kendall trend test can effectively identify long-term trends in drug consumption, eliminate seasonal interference, enhance monitoring accuracy and management efficiency, and is suitable for the dynamic monitoring of drug consumption.
2.Shikonin Induces Ferroptosis through ROS/JNK Pathway to Intervene in the Malignant Behavior of Pancreatic Cancer

Objective To investigate if Shikonin(SKI)can induce ferroptosis via the ROS/JNK pathway to inhibit the malignant behavior of pancreatic cancer.Methods Human pancreatic cancer PANC-1 or BxPC-3 cells were selected.Drug efficacy experiments were established with a blank control group(Con group)and low,medium,and high dose SKI groups(2,4,8 μmol/L).JNK-related mechanism experiments were categorized into a blank control group(Con group),SKI group,and SKI+JNK inhibitor group(SKI+SP600125 group).ROS-related mechanism experiments were divided into a blank control group(Con group),SKI group,and SKI+ROS scavenger group(SKI+NAC group).Cell viability was assessed using the CCK-8 method to calculate IC50;Transwell experiments evaluated cell migration and invasion capabilities;the C11 BODIPY 581/591 probe was utilized for flow cytometry to detect lipid peroxidation levels,while the FerroOrange fluorescent probe measured ferrous ion levels;ROS levels were determined using a ROS detection kit;the Western blot method identified ferroptosis-related key proteins(SLC7A11,GPX4),apoptosis-related proteins(Caspase3,PARP),and JNK pathway proteins(JNK,p-JNK);an in vivo xenograft tumor model was employed to assess tumor proliferation.Results SKI treatment significantly and dose-dependently inhibited PANC-1 cell viability(IC50:6.04 μmol/L,P<0.0001)and BxPC-3 cell viability(IC50:12.27 μmol/L,P<0.0001),and significantly reduced migrating and invasive cell numbers(P<0.0001),with migration cell numbers dropping to about 30%of the control group at 8 μmol/L SKI treatment(P<0.0001).Mechanistically,SKI induced increased intracellular lipid peroxidation,Fe2+accumulation,and significant ROS production(P<0.0001),significantly downregulated SLC7A11 and GPX4 protein expression(GPX4 protein expression reduced to 40%of that in the control group,P<0.0001),and activated JNK phosphorylation(p-JNK/JNK ratio increased to 2.8-fold,P<0.0001).Pretreatment with the JNK-specific inhibitor SP600125 or ROS scavenger NAC effectively reversed SKI's inhibition of cell viability and downregulation of SLC7A11/GPX4 protein(all P<0.01).SKI also inhibited pancreatic cancer tumor cell proliferation in vivo(P<0.0001).Conclusion SKI induces ferroptosis by activating the ROS/JNK pathway,thereby inhibiting pancreatic cancer proliferation,migration,and invasion.

The research status and common methods of the Four Tantras are summarized by bibliographic methods.The"philosophy,biology,society,geography,psychology and medicine"model of Tibetan medicine inheritance innovation in the Four Tantras was used to explore the historical necessity of interdisciplinary integration of the Four Tantras.It combines the latest technology and methods at the forefront of systems science,philosophical science,psychological science,astronomy,biological science,geographic science,ecological science,medical science,as well as modern biotechnology and digital technology into the innovation of Tibetan medical heritage.It is also put forward the idea of multidisciplinary intersection and large integration research of the Four Tantras and Tibetan medicine,as well as a new paradigm of systematic Tibetan medicine research that reflects a systemic and holistic view.The culture,theoretical basis and scientific connotation of Tibetan medicine contained in the Four Tantras are preliminarily clarified,which provides basic ideas for the inheritance and innovation of Tibetan medicine.
4.A retrospective matching study of partial nephrectomy and radical nephrectomy for pathological T3a stage renal cell carcinoma

Objective:To evaluate the long-term oncological outcomes of partial nephrectomy(PN)in patients with renal cell carcinoma(RCC)who were clinically staged as clinical T1(cT1)preoperatively but upstaged to pathological T3a(pT3a)after surgery.Methods:A total of 427 RCC patients postopera-tively diagnosed as pT3aN0M0 at Peking University Third Hospital from February 2013 to December 2022 were retrospectively reviewed.Among them,33 cT1 patients upstaged to pT3a RCC received PN(PN group),while 394 non-upstaged pT3a RCC patients underwent radical nephrectomy(RN,RN group).Propensity score matching was performed at a 1∶1 ratio based on baseline characteristics.The Kaplan-Meier method was used to assess overall survival(OS),cancer-specific survival(CSS),and disease-free survival(DFS),with Log-rank tests and Cox regression models for multivariate analysis.Results:Before matching,the PN group(n=33)had significantly higher rates of perirenal fat invasion(PFI,45.5%vs.15.2%)and segmental renal vein involvement(42.4%vs.20.8%),but lower rates of renal sinus invasion(RSI,21.2%vs.73.6%)and renal vein tumor thrombus(0%vs.15.2%)compared with the RN group(n=394,all P<0.05).After matching,baseline characteristics were comparable between the PN group(n=33)and RN group(n=33).No significant differences were observed in operative time,blood loss,mean hospital stay,complication rate,positive margin rate,or conversion to open surgery between the two groups(P>0.05).However,the PN group showed significantly higher estimated glomerular filtration rate(eGFR)postoperatively[76.9(55.4,87.3)mL/(min·1.73 m2)vs.61.7(56.8,73.5)mL/(min·1.73 m2),P<0.05],indicating better renal function preserva-tion.No significant differences were found in OS,CSS,or DFS between the groups(P>0.05).Multi-variate ana-lysis identified renal vein invasion(RVI),higher Fuhrman grades(Ⅲ-Ⅳ),and sarcoma-toid differentiation as independent risk factors for DFS and CSS in the pT3a RCC patients(P<0.05).Conclusion:For cT1 RCC patients upstaged to pT3a,PN preserves renal function more effectively while achieving com-parable oncological outcomes to RN.RVI,higher Fuhrmann grade,and sarcomatoid differentiation are independent risk factors for pT3N0M0 RCC patients.
5.IGSF11: A Novel Target for Cancer Immunotherapy.

Immune checkpoint blockade therapy has demonstrated remarkable efficacy in treating various malignancies; however, its clinical application remains challenged by low response rates and immune-related adverse events. Immunoglobulin superfamily member 11 (IGSF11), an inhibitory immune checkpoint molecule, serves as a specific ligand for the V-domain immunoglobulin suppressor of T cell activation (VISTA). Through the IGSF11/VISTA axis, it suppresses T cell function and represents a promising novel target for cancer immunotherapy. IGSF11 is widely expressed across multiple tumor types, though its regulatory mechanisms vary depending on the malignancy. Studies have confirmed that blocking the IGSF11-VISTA interaction or specifically inhibiting IGSF11 exerts antitumor effects. While IGSF11 is closely associated with patient prognosis, its prognostic significance differs among cancer types. This review systematically summarizes the structural characteristics of IGSF11, its regulatory mechanisms, interaction with VISTA, and functional role within the tumor microenvironment.

6.Electrotactic migration and mechanisms of stem cells

BACKGROUND:With the deepening of research on stem cell technology,how to make its accurate homing has become a major problem in clinical application.In addition to the induction of drugs and chemokines,electric fields are also widely used to guide the directional migration of stem cells.They can enhance their migration speed and orientation.OBJECTIVE:To summarize the effect of an electric field on the characteristics of stem cell migration and analyze the possible mechanism of action.METHODS:PubMed and CNKI databases were searched to collect relevant literature up to March 2024,with Chinese and English search terms"stem cells,direct current electric field,pulsed electric field,migration,electric field device,mechanism."Literature that was not available in full text and unrelated to the topic was excluded.RESULTS AND CONCLUSION:A total of 58 articles were included according to the screening requirements,including 15 Chinese articles and 43 English articles.In the literature,the effects of different parameters of the electric field on the migration of adipose-derived mesenchymal stem cells,bone marrow mesenchymal stem cells,neural stem cells,epidermal stem cells,human embryonic stem cells,and human induced pluripotent stem cells and its mechanism were studied in a migration device.(1)As a simple,non-invasive,and stable intervention method,the electric field plays an active role in guiding the directional migration of stem cells.(2)Different types of stem cells had different directions of electrotaxis migration,and the migration speed and directionality of most stem cells increased with the increase of electric field intensity.(3)Different electric field devices have different focuses in observing stem cell migration,and the relevant devices can be selected according to the purpose of the experiment.(4)The mechanism of electrotaxis migration of different stem cells is not completely the same.MAPK pathway,ROCK activation,and PI3K function are involved in the migration process of most stem cells,and other protein complexes and signaling pathways are involved in the regulation of this process.(5)In addition to different electric field parameters,cell senescence and culture environment also affect the results of electrotaxis migration.In summary,as an important signal that affects the migration characteristics of stem cells,the application of electric field combined with other emerging materials has shown certain potential in tissue engineering.It is expected to play a more important role in guiding stem cells to home,promoting bone tissue regeneration and repair,and making greater breakthroughs in the research of the nervous system,autoimmune system,tumors,and other diseases.
7.The predictive value of preoperative CALLY index for the short-term prognosis in patients undergoing radical gastrectomy

Objective To explore the predictive value of the preoperative C-reactive protein-albumin-lymphocyte ratio(CALLY index)for short-term prognosis in patients undergoing radical gastrectomy.Methods The clinical data of 228 gastric cancer patients who underwent radical gastrectomy were retrospectively analyzed.Based on follow-up results,patients were divided into the good prognosis group(n=205)and the poor prognosis group(n=23).The differences in clinical characteristics and laboratory test results between the two groups were compared.Cox proportional hazards regression model was used to analyze the impact of CALLY index on poor prognosis in patients.The receiver operating characteristic(ROC)curve was used to determine the clinical value and optimal cutoff value of CALLY index in predicting poor prognosis of patients.Kaplan-Meier method was used to analyze the difference in disease-free survival(DFS)between patients with different CALLY index levels.Results After a follow-up of 3 to 53 months(median follow-up of 36 months),23 patients developed poor prognosis(including 3 deaths and 20 recurrences or metastases).Compared with the good prognosis group,tumors in the poor prognosis group were larger,the proportion of poorly differentiated tumors was higher and white blood cell count(WBC),neutrophil count(NEU),C-reactive protein(CRP)were higher,and lymphocyte(LYM),albumin(ALB),prealbumin,total cholesterol(TC)and CALLY indexl were lower.Multivariate Cox regression analysis showed that the elevated CALLY index(HR=0.306,95%CI:0.102-0.921,P=0.035)was an independent protective factor for poor prognosis,while larger tumors and poorly differentiated tumors were independent risk factors for poor prognosis.ROC curve analysis showed that the area under the curve for predicting poor prognosis in gastric cancer patients undergoing radical gastrectomy was 0.862(95%CI:0.810-0.904),with a sensitivity of 95.65%,specificity of 69.76%,and an optimal cutoff value of 2.21.Kaplan-Meier analysis showed that patients with a CALLY index>2.21 had a longer disease-free survival than those with a CALLY index≤2.21(mean DFS:48 months vs.42 months,Log-rank χ2=4.123,P=0.042).Conclusion The preoperative CALLY index is a simple,effective and easily accessible predictor for predicting short-term prognosis in patients undergoing radical gastrectomy.A high CALLY index indicates a better prognosis of patient.
8.Non-invasive detection of rat skin scars using terahertz time-domain spectroscopy technology

Objective To explore the difference in terahertz signal characteristics between rat skin scar and normal skin using terahertz time-domain spectroscopy technology,thereby providing a novel non-invasive detection technique for the pathological examination of skin scars.Methods A rat model of whole-layer skin defect was prepared,and a reflectance terahertz time-domain spectroscopy system was used to obtain the terahertz signal maps of normal skin and scarred area.Results The terahertz signals of normal skin showed two obvious reflection time-domain signal peaks,and the characteristics of the reflection peaks at different reflection points were relatively regular.The terahertz signals of skin scar also had two obvious reflection time-domain signal peaks,but the highest peak was lower than that of normal skin.In addition,principal component analysis revealed that skin scar signals and normal skin signals were clustered together separately.The terahertz signal at different sites differed significantly(P<0.05).Conclusion Terahertz time-domain spectroscopy technology can be applied to the non-invasive detection of skin scarring,exhibiting a good application prospect in biomedicine.
9.Analysis of Pathogenic Gene Carriers and Prenatal Diagnosis Results in 13472 Cases of Spinal Muscular Atrophy

Objective:To explore the carrier rate and the genetic distribution characteristics of spinal muscular atrophy(SMA)pathogenic genes in Huizhou,and analyze the prenatal diagnosis results of fetuses from couples who are both carriers,in order to provide scientific reference for genetic counseling and prenatal diagnosis.Meth-ods:13472 peripheral blood samples were collected for the survival motor neuron gene 1(SMN1)testing at Huizhou First Maternal and Child Health Care Hospital from August 2021 to October 2024.And prenatal diagnosis was conducted on high-risk pregnant couple who were both carriers of SMA pathogenic genes.Fluorescence quantitative polymerase chain reaction(PCR)was used to detect the copy numbers of SMN1 exon 7 and 8(E7,E8),screen for SMA pathogenic gene carriers,and calculate the carrier rate.For samples identified as homozy-gous deletions and prenatal diagnosis samples,further validation of copy number variations in E7 and E8 of the SMN1 gene was performed using multiplex ligation-dependent probe amplification(MLPA)technology.Results:Among the 13472 screened individuals,268 carriers of the SMA pathogenic gene were detected,with a carrier rate of approximately 1/50(1.99%,268/13472).Among them,there were 251 cases of E7 and E8 heterozygous dele-tion,3 cases of E7 heterozygous deletion and E8 homozygous deletion,and 14 cases of pure E7 heterozygous de-letion;2 cases of E7 and E8 homozygous deletion were detected.One case had obvious motor developmental dis-orders in the child,and the other case had a normal phenotype in the pregnant woman.Among 20 couples who were both SMA carriers,17 pregnant women underwent prenatal diagnosis.The results showed that 4 cases were normal E7 and E8 types,7 cases were E7 and E8 heterozygous deletion types,all of whom continued to conceive.6 cases were E7 and E8 homozygous deletion type,namely SMA patients,and the pregnancy was terminated by pregnant women.Conclusions:This study reports the carrier rate of SMA pathogenic genes in the population of Huizhou for the first time,and the combined use of MLPA for prenatal diagnosis of high-risk couples can effective-ly prevent the birth of SMA children,which is of great significance for the prevention and control of SMA birth de-fects.
10.Clinical analysis of 8 cases of laparoscopic combined with colonoscopic transanal total mesorectal resection

Objective:To explore the feasibility and safety of laparoscopic combined with colonoscopic transanal total mesorectal resection (laparoscopic combined with colonoscopic taTME) in the treatment of rectal cancer.Methods:The descriptive case series analysis method was adopted. From October 2023 to February 2024, the Department of Colorectal Surgery of Li Huili Hospital, Ningbo Medical Center, performed laparoscopic combined with colonoscopic taTME on 8 patients with rectal cancer. Among the 8 patients, there were 5 males and 3 females, aged from 56 to 74 years old, with a body mass index (BMI) of 20.3-26.7 kg/m2. All patients were pathologically diagnosed with rectal adenocarcinoma. The long diameter of the tumors was 2.0-6.5 cm, the lower edge of the tumors was 3-5 cm away from the anal verge. In terms of tumor TNM staging, there were 2 cases in stage I, 3 cases in stage II, and 3 cases in stage III. The surgical conditions, postoperative curative effects, and the occurrence of complications were observed.Results:All 8 patients successfully completed laparoscopic combined with colonscopic taTME, and there was no conversion to laparotomy. The operative time was 260 to 335 minutes, the intraoperative blood loss was 50 to 100 milliliters, and the distance from the tumor to the anal margin was 0.8 to 2.0 centimeters. All patients in the group underwent protective end ileostomy, and none of them underwent permanent enterostomy. Specimens were removed from the right lower abdomen in 7 cases and through the anus in 1 case. There was no residual cancer cells at the pathological resection margins postoperatively. All patients ambulated on the first day after the operation, and began to eat on the 2nd to 3rd day after the operation. Anastomotic leakage occurred in 1 patient after the operation, and the condition improved after conservative treatment. The length of hospital stay was 21 days. The other 7 patients were discharged from the hospital 8 to 12 days after the operation. Two patients completed the ileostomy closure surgery 3 months after the operation and recovered well. The patients were followed up until April 2024, during which there were no cases of tumor recurrence or death.Conclusion:For appropriate cases, laparoscopic combined with colonoscopic taTME is safe and feasible.
