1.Analysis of Dengue virus nucleic acid testing screening among blood donors in Xishuangbanna Dai Autonomous Prefecture, China

Objective: To investigate the prevalence of Dengue virus (DENV) infection among voluntary blood donors in Xishuangbanna Dai Autonomous Prefecture, and to evaluate the necessity of implementing nucleic acid testing (NAT) for blood donors during the rainy season (May-October). Methods: Prior to initiating donor screening, the Xishuangbanna Central Blood Center conducted in-house validation of reagent performance and participated in external quality assessment (EQA) organized by the National Center for Clinical Laboratories (NCCL). During the surveillance period (August-October 2024), a total of 2 919 donor samples were screened using a 6-sample mini-pool NAT strategy. Daily internal quality controls were recorded. Samples that tested positive in pooled screening were deconvoluted and retested in duplicate; only those reactive in both replicate wells were sent to the NCCL for confirmatory testing. At NCCL, samples underwent re-testing using five domestic NAT reagents, as well as serological assays for NS1 antigen and DENV-specific IgG/IgM. Confirmed positive samples were further characterized by serotyping, envelope (E) gene sequencing, and phylogenetic analysis using the maximum likelihood method. Results: The DENV NAT reagent demonstrated consistent detection of 40 copies/mL controls in individual donor (ID)-NAT test (mean CT: 35.61±0.40). During the 63-day quality control monitoring, DENV detection remained stable (mean CT: 22.53±0.72). The center achieved full marks in EQA assessments for 2023 and 2024. Three reactive pools were identified in initial screening, and subsequent individual testing confirmed three DENV RNA-positive donors (sample numbers: 2401, 2402, and 2403). The confirmatory test results from NCCL were: all five NAT platforms consistently detected DENV RNA in the three samples; for serological tests, 2 samples (2402, 2403) were positive for NS1 antigen, while all three samples were negative for both IgG and IgM antibodies. DENV serotyping reagents identified DENV-2 in all cases, which were further confirmed as DENV-2 Genotype Ⅱ-Cosmopolitan by E gene sequencing. Phylogenetic analysis indicated that samples 2401 and 2402 clustered with Southeast Asian strains (Thailand/MZ636802.1, Laos/PQ775621.1), while sample 2403 closely matched a previously reported local Yunnan strain (PV544686.1). Conclusion: DENV-2 infection was detected among blood donors in Xishuangbanna during the rainy season, indicating concurrent risks of imported and local transmission. We recommend implementing pooled NAT screening for blood donors in high-risk areas during dengue epidemic seasons, along with strengthened laboratory quality control, to enhance blood safety.
2.Clinical and genetic characteristics of SCN2A gene related developmental delay

Objective:To explore the genotype and the clinical phenotype of SCN2A-related developmental delay in children. Methods:A case series study was adopted. Collect clinical data from 10 cases of children with SCN2A gene variants diagnosed with global developmental delay/intellectual disability who were admitted to the Children′s Hospital between July 2019 and March 2023. Summarize the clinical phenotype and genotype based on clinical data such as general information, clinical manifestations, imaging examinations, laboratory tests, genetic testing results, and comprehensive pediatric neuropsychological development assessment. Results:A total of 10 patients were recruited, including 7 males and 3 females, with an age range of 27 days to 5 years and 9 months. 9 patients underwent children′s neuropsychological and behavioral assessments, and the results were consistent with global developmental delay, including 2 mild cases, 4 moderate cases, and 3 severe cases. 3 cases had autism spectrum disorder, and 2 cases had epilepsy. 6 patients underwent complete head MRI examination, and 4 of them showed abnormalities, including delayed myelination, widening of the local extra brain space in the frontal lobe, and abnormal frontal lobe morphology. All 10 cases had point variants. Among them, 9 cases are de novo and 1 case is maternal inheritance. Out of 10 cases, there were 5 cases with copy number variations, but all of them were of unknown significance. Among the 10 variants, 8 have been reported and 2 have not been reported, namely c.4145A>T(p.N1382I) and c.4937T>A(p.I1646N). In this study, 4 out of 10 patients with SCN2A variants had variation sites located in the S4 segment of domain which constitute Nav1.2, the sodium ion channel encoded by SCN2A. The developmental quotient level was lower when the variation sites were located in the S4 segment of domain, and the difference was statistically significant ( t=-3.101, P=0.017), indicating that the severity of developmental delay may be related to the localization of amino acids corresponding to variant sites within the protein domain. Conclusion:SCN2A mutations are strongly associated with diverse neurodevelopmental disorders. In this study, the phenotypic spectrum of SCN2A variants encompassed epilepsy, global developmental delay, and autism spectrum disorder. Affected individuals exhibited early-onset developmental delays, predominantly moderate to severe in severity. Voltage-sensing domain dysfunction in sodium channels may constitute a critical pathomechanism underlying neurodevelopmental impairments. Further electrophysiological characterization and molecular mechanistic studies are warranted todelineate the genotype-phenotype correlations between specific variant loci and clinical severity.
3.Effects of metronidazole and vulvar cleaning with 0.1% benzalkonium bromide solution on vaginal environment, etiology, and interleukin-6 and Toll-like receptor 2 levels in lavage fluid of patients with bacterial vaginosis

Objective:To investigate the effects of metronidazole and vulvar cleaning with 0.1% benzalkonium bromide solution on vaginal environment, etiology, and interleukin-6 and Toll-like receptor 2 levels in lavage fluid of patients with bacterial vaginosis.Methods:A total of 114 patients with bacterial vaginosis who were admitted to Jinan 2 nd People's Hospital from February 2021 to February 2024 were included in this prospective study. These patients were randomly divided into a control group and an observation group, with 57 patients in each group, following a 1:1 ratio. The control group received treatment with metronidazole, while the observation group received metronidazole in combination with vulvar cleaning using a 0.1% benzalkonium bromide solution, for a total of 14 days. The clinical efficacy was evaluated by comparing the vaginal environment (including vaginal pH, cleanliness, and microbial density), microbiological outcomes (lactobacilli detection rate and pathogenic bacteria detection rate), and interleukin-6 and Toll-like receptor 2 levels in the vaginal lavage fluid between the two groups before and after treatment. Adverse drug reactions were recorded. Patients were followed up for 3 months after treatment to record recurrence rates. Results:The total response rate in the observation group was significantly higher than that in the control group [92.98% (53/57) vs. 78.95% (45/57), χ2 = 4.65 P < 0.05]. After treatment, improvements of vaginal pH, vaginal cleanliness, and bacterial density were greater in the observation group than those in the control group ( χ2 = 4.61, Z = 4.92, 4.29, all P < 0.05). After treatment, the detection rate of lactobacilli in the observation group was significantly higher than that in the control group [63.16% (36/57) vs. 42.11% (24/57), χ2 = 5.07, P < 0.05]. The detection rate of pathogenic bacteria in the observation group was significantly lower than that in the control group [5.26% (3/57) vs. 17.54% (10/57), χ2 = 4.25, P < 0.05]. After treatment, lavage fluid levels of interleukin-6 [(8.70 ± 2.13) ng/L vs. (13.52 ± 3.36) ng/L] and Toll-like receptor 2 [(5.06 ± 1.34) ng/L vs. (7.84 ± 1.22) ng/L] in the observation group were significantly lower than those in the control group ( t = 9.15, 11.58, both P < 0.05). There was no statistically significant difference in the incidence of adverse reactions between the two groups ( χ2 = 1.37, P > 0.05). After 3 months of treatment, the recurrence rate of bacterial vaginosis in the observation group was significantly lower than that in the control group [4.55% (1/22) vs. 27.78% (5/18), χ2 = 4.19, P < 0.05]. Conclusions:The combination of metronidazole and vulvar cleaning with a 0.1% benzalkonium chloride solution is effective for treating bacterial vaginosis. This approach helps regulate the vaginal environment, increases the levels of lactobacilli, reduces the levels of pathogenic bacteria, and decreases the levels of interleukin-6 and Toll-like receptor 2 in the lavage fluid. Additionally, it decreases the risk of recurrence of bacterial vaginosis and is highly safe.

6.New research direction of organ dysfunction caused by hemorrhagic shock: mechanisms of mitochondrial quality control

Hemorrhagic shock (HS) is one of the leading causes of death among young adults worldwide. Multiple organ dysfunction in HS is caused by an imbalance between tissue oxygen supply and demand, which is closely related to the poor prognosis of patient. Mitochondrial dysfunction is one of the key mechanisms contributing to multiple organ dysfunction in HS, while mitochondrial quality control regulates mitochondrial function through a series of processes, including mitochondrial biogenesis, mitochondrial dynamics, mitophagy, mitochondrial-derived vesicles, and mitochondrial protein homeostasis. Modulating mitochondrial quality control can improve organ dysfunction. This review aims to summarize the effects of mitochondrial dysfunction on organ function in HS and discuss the potential mechanisms of mitochondrial quality control, providing insights into the injury mechanisms underlying HS and guiding clinical management.
7.Correlation between Weight Gain during Late Pregnancy and Glycated Albumin,Visfatin Level and Neonatal Body Composition in Gestational Diabetes Mellitus Patients

Objective:To evaluate the association between weight gain in late gestation and glycated albumin (GA),visfatin level and neonatal body composition in gestational diabetes mellitus (GDM) patients who are non-obese.Methods:200 pregnant women were enrolled.The preconception body mass index(BMI) of all the research object was less than 25 kg/m2.According to the speed of body mass growth from 28th week of gestation to delivery,the subjects were divided into two groups:excessive weight gain group (n =77) and appropriate weight gain group(n=123).Variance of serum GA,visfatin levels and neonatal birth weight(BW),the percent of body fat in newborn(F%) between two groups were compared,and the relationship between weight gain in late pregnant GDM patients and above values was analyzed.Results:①Serum GA,HbA1 c,visfatin and total cholesterol,triglyceride,low density lipoprotein,very low density lipoprotein levels in excessive weight gain group were higher than those in appropriate weight gain group(P < 0.05).②Mean gestational age,newborn Apgar score immediately,the ratio of baby boys and vaginal delivery rate in two group had no significant difference(P>0.05).Compared with the appropriate weight gain group,the excessive weight gain group had higher average birth weight and lower blood glucose levels.The proportion of large for gestational ages(LGA) was increased in appropriate weight gain group,and the ratio of abdomen circumference (AC) and head circumference (HC) and the percent of body fat in newborn(F%) were significantly increased in this group(P <0.01).③The speed of body mass growth in late pregnancy in GDM patients was positively related with serum GA,visfatin,HbA1c levels and newborn birth weight and the percent of body fat in newborn(r=0.524,0.470,0.456,0.329,0.473,P<0.001).Conclusions:The monitoring of the serum GA and visfatin levels may have certain clinical value to evaluate the weight gain in late pregnancy of GDM patients.The rapid body mass growth in those patients may accelerate the visceral fat accumulation and worsen existing glucolipid metabolic disorders,and is also closely related to the increase of neonatal BW and F%.
8.Clinical Study on Sanyinjiao Acupoint Sticking with Gushen Yutai Tie in Treatment of Threatened Abortion

Objective To observe the clinical efficacy of Sanyinjiao acupoint sticking with Gushen Yutai Tie on threatened abortion. Methods A total of 216 women diagnosed with threatened abortion were randomly assigned to trial group (108 cases) and control group (108 cases). Both groups were treated with TCM syndrome differentiation treatment and progesterone soft capsule, and the trial group was treated with Gushen Yutai Tie acupoint sticking at Sanyinjiao (SP6) additionally. One treatment course was five days, and both groups were treated for three courses. Vaginal bleeding and lower abdominal pain, lumbar pain, nausea and vomiting were observed, pulsatility index (PI) and resistance index (RI) of the uterine artery were detected after treatment. Results The total effective rate in the trial group was 97.22% (105/108), which was significantly higher than that of 90.74% (98/108) in the control group (P<0.05). Symptom scores in the trial group were also significantly higher than those in the control group (P<0.01). PI and RI in the ineffective (pregnancy failure) group were much lower than that of effective (threatened abortions with normal outcome) group (P<0.01). Conclusion The treatment that Gushen Yutai Tie acupoint sticking at Sanyinjiao combined with TCM syndrome differentiation and progesterone soft capsule had a better effect to improve pregnancy success rate in threatened abortion. PI and RI in uterine artery could be used to evaluate the outcome of pregnancy.
