Objective To study the safety and effectiveness of arc edge needle therapy for moderate to severe tenosynovitis of the flexor digitorum tendon in the thumb.Methods A total of 62 patients with moderate to severe stenosing tenosynovitis of the flexor digitorum thumb who met the inclusion criteria were collected.The patients were randomly assigned 1:1 to the arc edge needle group and the needle knife group using a random number table method.Among them,31 patients in the arc edge needle group were treated with arc edge needles,while 31 patients in the needle knife group were treated with traditional needle knife release surgery.Both groups were treated once.Record the number rating scale(NRS),range of motion(ROM),Quinell grade,and disease efficacy score(WDES)of patients before and after treatment at various stages(1 week,2 weeks,4 weeks),and at follow-up after 24 weeks to evaluate the treatment effect.Results During the 24 weeks follow-up of 62 patients after treatment,the Quinell grading comparison between the two groups showed that the arc edge needle group was better than the needle knife group(P<0.05);The total effective rate of disease efficacy score(WDES)was 96.77%in the arc edge needle group and 83.87%in the needle knife group.The clinical total effective rate of the arc edge needle group was better than that of the needle knife group(P<0.05);After treatment,both groups showed significant improvement in pain numerical score(NRS)and joint range of motion(ROM),and the degree of improvement increased over time(P<0.05);And the inter group comparison at 1 week,2 weeks,4 weeks and 24 weeks time points after treatment showed that the curved blade needle was superior to the needle knife group in improving pain and joint mobility(P<0.05).Conclusion Both the 45° tenosynovitis incision and release surgery with arc edge needle and traditional needle knife release surgery are effective in treating moderate to severe tenosynovitis of the thumb flexor digitorum tendon.However,the 45° tenosynovitis incision and release surgery with arc edge needle has significant advantages in improving pain,interphalangeal joint mobility,and safety.

Objective:To explore efficacy and safety of endovascular therapy combined with hyperbaric oxygen (HBO) intervention in acute anterior circulation macrovascular occlusion stroke.Methods:A total of 150 patients with acute anterior circulation macrovascular occlusive stroke admitted in No.902 Hospital of the CPLA Joint Logistic Support Force were selected and randomly divided into endovascular treatment group and combined treatment group (endovascular+ HBO). Endovascular therapy was adopted in the treatment of all patients. Modified thrombolytic flow grading (mTICI) was used to evaluate vascular recanalization after operation, and the incidences of complications of hemorrhage transformation and vascular re-occlusion were recorded. National institutes of health stroke scale (NIHSS) was used to evaluate the neurological functions on admission, at 24 hours after the operation, and one course after HBO therapy. The prognosis was evaluated by observing the changes of blood homocysteine (HCY) and high-sensitivity C-reactive protein (hs-CRP) levels before and after treatment. The patients had been followed up by the modified Rankin Scale (mRS) for 90 days. At the same time, 75 patients with acute anterior circulation macrovascular occlusive stroke without taking endovascular treatment were selected as the control group.Results:Among 150 patients, 126 cases had a grade of mTICI≥2b, while 24 cases had a grade of mTICI<2b. Postoperative vascular re-occlusion occurred in 3 cases, and symptomatic intracranial hematoma occurred in 6 cases. The NIHSS score at 24 hours after the operation was significantly lower than that on admission, and the NIHSS score was further improved after HBO treatment. Ninety days after the operation, mRS showed 98 cases of good prognoses, while 52 cases of poor prognoses. The levels of HCY and hs-CRP were significantly lower than those before the treatment.Conclusion:Endovascular therapy for acute anterior circulation macrovascular occlusive stroke is safe and effective. Postoperative HBO therapy can significantly improve the prognosis and the quality of life of patients.

Percutaneous vertebroplasty has many advantages such as minimal trauma, simple mode of operation, rapid and correct therapeutic effect, high safety, and wide indications. Therefore it has been generally paid close attention and accepted. Percutaneous vertebroplasty has conspicuous curative effect, especially in vertebral compression fracture. Polymethyl methacrylate is the most used filling materials at present; There is not significant difference between unipedicular and bipedicular vertebroplasty; Using vertebral body venography and appropriate dense bone cement can reduce leakage of bone cement; Bolster for self-replacement can rebound vertebral body height, and balloon kyphoplasty and sky bone expander kyphoplasty can be avoided; In order to reduce refracture of vertebral body, anti-osteoporosis drugs should be used in the treatment of percutaneous vertebroplasty.

Objective To observe the effect of "Jingu Tongxiao Wan" on blood stasis and cold coagulation type gonarthrosis.Methods 160 patients with blood stasis and cold coagulation type gonarthrosis were divided into the "Jingu Tongxiao Wan" group(group A)with 120 cases(treated with "Jingu Tongxiao Wan" 6 g/time,2 times/d)and "Guci Pian" group(group B)with 40 cases(treated with "Guci Pian" 3 tablets/time,3 times/d).Indexes of effect and safe were observed after one month.Results 23 cases were healed,63 cases were excellent,9 cases were utilitied,9 cases were invalid,total effective rate was 93.8% in the group A.While,in the group B,7 cases were healed,21 cases were excellent,9 cases were utilitied,3 cases were invalid,total effective rate was 92.5%.There was no significant difference between two groups(P>0.05).Scores of sings and symptoms before and after treatment were 8.98±2.24 and 2.15±1.71 respectively in the group A,and 8.29±2.35 and 2.25±1.25 respectively in the group B.The scores after treatment were significantly different from that before treatment in each group(P<0.01),and no significant difference between scores of the group A and group B after treatment(P>0.05).Conclusion The treatment of "Jingu Tongxiao Wan" shows an obvious effect and safe on blood stasis and cold coagulation type gonarthrosis.

BACKGROUND: The morbidity of cervical spondylosis complicated with osteoporosis is rather high in senile patients, which affects the life quality.OBJECTIVE: To observe the therapeutic effects of gushukang, the Chinese herbal patent for kidney tonification on senile cervical spondylosis complicated with osteoporosis.DESIGN: Random control study based on patients.SETTING: Orthopedics department in a provincial hospital of traditional Chinese medicine.PARTICIPANTS: Fifty cases of senile cervical spondylosis complicated with osteoporosis collected from the inpatients and outpatients in Henan Hospital of Traditional Chinese Medicine from .June 2002 to December 2003.METHODS: Fifty cases of senile cervical spondylosis complicated with osteoporosis were randomized into experimental group(30 cases) and the control(20 cases) . In experimental group, gushukang was prescribed, 10 g/time and twice a day. In the control, jingfukang was prescribed, 6 g/time,twice per day. The course of treatment was 6 months.MAIN OUTCOME MEASURES: To observe the improvement of therapeutic effects, X-ray manifestation, bone density, serum calcium and phosphorus before and after treatment in two groups.RESULTS: The therapeutic effects were obtained in both groups. The effective rate was 93％ in treatment group, better than the control (75％) ( P ＜ 0.05). Concerning to remote ultrasonic conductive speed in radius(SOS), it was(3 934 ± 125) and(3 990 ± 105) m/s before and after treatment respectively in experimental group; and was(3 935 ± 110) and(3 940 ± 123) m/s in the control, indicating significant difference( P ＜ 0. 05) . The difference in SOS before and after treatment in the control was not significant ( P ＞ 0.05).There was no remarkable difference in serum calcium and phosphorus before and after treatment in two groups( P ＞ 0.05).CONCLUSION: Gushukang, Chinese herbal patent for kidney tonification,provides better therapeutic effects on senile cervical spondylosis complicated with osteoporosis.
