1.Impact of carbapenem-resistant Acinetobacter baumannii colonization on short-term mortality in COVID-19 ICU patients

Objective To investigate the impact of respiratory colonization of carbapenem-resistant Acinetobacter baumannii(CRAB)on 28-day mortality of patients with severe/critical coronavirus disease 2019(COVID-19)in in-tensive care unit(ICU).Methods Clinical data were collected from severe/critical COVID-19 patients admitted to general ICU and respiratory ICU in a tertiary Chinese medicine hospital from December 9,2022 to December 9,2023.According to the detection results of CRAB from sputum culture during ICU stay,patients were divided into respiratory CRAB colonization group and non-CRAB colonization group.Kaplan-Meier survival curve was plotted to compare the 28-day mortality between two groups of patients after ICU admission.Relationship between respiratory CRAB colonization and 28-day mortality was explored by Cox proportional hazards model,and subgroup analysis was performed.Results A total of 88 patients with severe/critical COVID-19 were included in analysis,out of which 27 had respiratory CRAB colonization(CRAB colonization group)and 61 didn't have respiratory CRAB colo-nization(non-CRAB colonization group);37 patients(42.0%)died within 28 days after admission to the ICU.Ka-plan-Meier survival curve showed that the 28-day mortality in CRAB group was higher than that in non-CRAB group(66.7%vs 31.1%,P<0.001).Cox univariate analysis showed that the 28-day mortality risk of patients in the CRAB colonization group was 2.91 folds of those in the non-CRAB colonization group(HR=2.91,95%CI:1.52-5.58;P<0.05).After covariates adjusting for age,sex,underlying diseases,white blood cell count,neutrophil-to-lymphocyte ratio,hypersensitive C-reactive protein,respiratory frequency,oxygenation index,lactate dehydro-genase,urea nitrogen,creatinine,lactic acid,and D-dimer,the risk of death in the CRAB colonization group was still higher than that in the non-CRAB colonization group(HR=2.54,95%CI:1.15-5.58;P<0.05).Subgroup analysis showed that respiratory CRAB colonization and 28-day mortality in severe/critical COVID-19 ICU patients was affected by gender,serum albumin,creatinine,and D-dimer levels(interaction P<0.05 for all).Conclusion The short-term mortality of severe/critical COVID-19 patients in the ICU increases significantly after secondary colo-nization of respiratory CRAB.Effective prevention and control of colonization and infection by multidrug-resistant organisms are crucial for ensuring patient prognosis.
2.Research progress on the effect of influenza vaccination on chronic obstructive pulmonary disease

Chronic obstructive pulmonary disease (COPD) is the fourth leading cause of death worldwide, causing a significant socioeconomic burden. This article reviews the effects of influenza vaccination on COPD and finds that influenza vaccine can significantly reduce the risk of influenza infection, reduce the number of acute exacerbations, and reduce the hospitalization rate in patients with COPD. The vaccine has a favorable safety profile and significant economic benefits, which can reduce medical costs. Currently, influenza vaccination mainly faces challenges such as insufficient patient awareness, insufficient support from the medical system, and socio-cultural and economic factors. Efforts should be focused on reducing the acute exacerbation of COPD patients and providing a scientific basis for the prevention and management of COPD patients.
3.Establishment of an eight-color, twelve-parameter flow cytometric assay and reference intervals for myeloid-derived suppressor cells in human peripheral whole blood of healthy adults in Shanghai region

Objective:To establish a flow cytometric assay for myeloid-derived suppressor cells (MDSC) in human peripheral whole blood and reference intervals for healthy adults in Shanghai region.Method:A whole blood eight-color and twelve-parameter flow cytometric assay was designed, utilizing fluorescently labeled antibodies against CD45, CD3, CD19, CD123, CD56, CD16, HLA-DR, CD33, CD11b, CD14, CD15 and CD20.A total of 246 healthy participants who met the health standards from the health check-ups conducted at the Tongji University Affiliated Shanghai East Hospital between May 8 to December 2, 2024 were enrolled. Peripheral venous whole blood was collected using EDTA-K 2 anticoagulant vacuum tubes for MDSC detection. A single-platform flow cytometry based relative count technique was used to quantify the percentage of each MDSC subpopulation. Kolmogorov Smirnov (K-S) test was used to test the distribution of specimens. Mann-Whitney U test and Kruskal-Wallis (K-W) test were used to evaluate whether reference intervals should be established separately based on gender or age. According to the clinical significance of MDSC, bilateral reference intervals were taken. Non parametric methods were used to take the percentile P2.5 and P97.5 to represent the rank of the lower and upper reference limits, respectively. Results:The results showed that a gating strategy was designed to exclude granulocytes, lymphocyte lineage cells, and natural killer cells. The K-S test results showed that the MDSC in each group of healthy individuals were distributed in a skewed manner. The U test showed significant gender differences ( P0.05) in the distribution of total myeloid-derived suppressor cells (T-MDSC) and polymorphonuclear myeloid-derived suppressor cells (PMN-MDSC). The K-W test showed no significant differences in MDSC among different age groups (21-30 years old, 30-40 years old, 40-50 years old, and 50-60 years old). T-MDSC reference interval is 0.056%-0.485%, PMN-MDSC reference interval is 0.035%-0.406%, Monocytic myeloid-derived suppressor cells (M-MDSC) reference interval is 0.000%-0.221%, early-stage myeloid-derived suppressor cells (E-MDSC) reference interval is 0.004%-0.125%. Reference interval verification was conducted on 20 healthy individuals, with a pass rate of 100%. Conclusion:A whole blood eight-color and twelve-parameter flow cytometric assay was established in this experiment. Based on the flow cytometry single platform method, reference intervals for healthy adults in Shanghai region were established.
4.Construction of assessment model on the basis of dynamic programming algorithm for medical device and its management effect

Objective:To construct an assessment model on the basis of dynamic programming algorithm for medical device,so as to improve the management level for medical devices.Methods:A assessment model for medical device was constructed on the basis of the presupposition,basic ideas,solution steps and algorithm elements of dynamic programming algorithm,and the management of medical devices was transferred and carried out according to the state of the model at the multi-stage when decided problem.A total of 200 used medical devices in clinical application in Zhongshan City People's Hospital from January 2022 to December 2023 were selected.The conventional management method was adopted to manage these medical devices from January to December 2022,and the assessment model(dynamic programming algorithm)on the basis of dynamic programming algorithm for these medical devices was adopted to manage them from January to December 2023.The management level for medical devices,the score for management indicators,the incidences of low risk,medium risk and high risk of adverse event were compared before and after the dynamic programming algorithm was applied,and the difference of the scores of assessing comprehensive business ability of management team was also compared.Results:The standard placement rate,standard recording rate and availability rate of equipment were respectively 85.5%,84.0%and 84.5%after the dynamic programming algorithm was applied,which were higher than those before it was applied,and the failure rate of equipment was 2.5%,which was lower than that before it was applied,and the differences were statistically significant(x2=13.888,12.516,14.240,14.414,P<0.05).The scores of resource allocation,information basis,technical support and guarantee for operation environment of devices were respectively(92.54±4.69),(93.65±3.65),(94.58±3.65)and(92.47±3.65)after the dynamic programming algorithm was applied,which were higher than these before it was applied,and the differences were statistically significant(t=14.574,18.419,11.438,12.981,P<0.05).The ratios of the adverse events with low risk,medium risk and high risk were respectively 6%,5%and 2%,which were lower than these before it was applied,and the differences were statistically significant(x2=12.754,12.876,11.348,P<0.05).The scores of comprehensive business ability of the technical support team,research team,promotion team and auxiliary management team for medical device were respectively(94.56±4.69),(92.65±3.14),(94.69±4.98)and(95.68±4.14)after the dynamic planning algorithm was applied,which were higher than these before it was applied,and the difference was statistically significant(t=12.170,18.682,12.449,18.269,P<0.05).Conclusion:The application of the assessment model on the basis of dynamic programming algorithm for medical devices in management for medical device can improve the management level for device,and reduce the incidence of adverse events,and enhance the comprehensive business ability of management team for device.
5.Improvement of blood deficiency of rats by Dietary Yangrong Decoction via PI3K/AKT pathway

This study aims to determine the therapeutic effect and then elucidate the molecular mechanism of Dietary Yangrong Decoction(DYRT)on blood deficiency syndrome based on net-work pharmacology and study its active compounds.The molecular mechanism of DYRT in the treatment of blood deficiency syndrome was predicted by network pharmacology.The blood defi-ciency of rats was established using cyclophosphamide and acetophenohydrazine jointly,which was divided into a blank control group,a model group,a Men's Yangrong Decoction group(MYRT)(positive control group),a medicinal group and food-based Yangrong Decoction group.The active components were analyzed by liquid chromatography-mass spectrometry(HPLC-MS),the periph-eral hemogram was detected by an automatic biochemical analyzer,the levels of hematopoietic regulatory factors in serum were determined by ELISA,and the relative expression of EPO in kid-ney and PI3K,AKT,GM-CSF in bone marrow were measured by RT-PCR.The results showed that DYRT had 57 active ingredients and 128 potential targets for the treatment of spleen deficien-cy syndrome.1 835 items were obtained by GO enrichment analysis,and 20 pathways were ob-tained by KEGG enrichment analysis;DYRT and MYRT were identified by HPLC-MS to contain 12 same blood active compounds.Animal experiments showed that,compared with the model group,the number of red blood cells,the number of platelets and the content of hemoglobin in the DYRT group were significantly increased,and the number of white blood cells was significantly decreased(P<0.05).The contents of EPO,IL-3,IL-6 and GM-CSF in serum were significantly in-creased,and the content of TNF-α was significantly decreased(P<0.05).The relative expression levels of EPO in kidney and PI3K,AKT and GM-CSF mRNA in bone marrow were significantly increased(P<0.05).Compared with the MYRT group,the number of white blood cells in the DYRT group was significantly reduced,the content of GM-CSF in serum was significantly in-creased,and the content of EPO and GM-CSF mRNA was significantly increased(P<0.05).In conclusion,DYRT contains the same active compounds as MYRT,which can regulate hematopoi-etic factors,thereby activating the PI3K/AKT pathway to restore hematopoietic function and im-prove the blood deficiency state of the body.

8.Impact of high-risk cytogenetic abnormalities on the efficacy of allogeneic hematopoietic stem cell transplantation with post-transplantation Cyclophosphamide in pediatric patients with acute myeloid leukemia

Objective:To explore the influence of initial high-risk cytogenetic abnormalities on the outcomes of children with acute myeloid leukemia (AML) after post-transplant Cyclophosphamide (PTCy)-based allogeneic hematopoietic stem cell transplantation (allo-HSCT).Methods:A retrospective cohort study.AML children who underwent PTCy-based allo-HSCT after the first complete remission at Wuhan Children′s Hospital, Tongji Medical College, Huazhong University of Science and Technology between April 2017 and April 2024 were enrolled.Patients were divided into intermediate-risk and high-risk groups based on their initial cytogenetic features.These patients were further divided into complex karyotype, 11q23 rearrangement, and other karyotype groups.Clinical characteristics and survival outcomes were compared among these groups.Measurement and count data were analyzed using Wilcoxon rank-sum/Kruskal-Wallis and χ2 tests, respectively.Survival and risk factor analyses were performed using Kaplan-Meier and Cox proportional hazards methods, respectively. Results:A total of 51 AML children who underwent allo-HSCT were included in this study.The median age at transplantation was 3.2 years and the median follow-up time was 4.6 years.There were 26 cases in the intermediate-risk group and 25 cases in the high-risk group; 8 cases in the complex karyotype group, 14 cases in the 11q23 rearrangement group, and 29 cases in the other karyotype groups.By the end of the follow-up on November 30, 2024, 11 patients relapsed, 8 patients died, and 13 patients developed grades Ⅱ-Ⅳ acute graft-versus-host disease (GVHD).The 3-year overall survival (OS), relapse-free survival (RFS), and grades Ⅱ-Ⅳ acute GVHD-free and relapse-free survival (GRFS) were 84.0% (95% CI: 74.4%-94.8%), 74.5% (95% CI: 63.4%-87.5%), and 58.8% (95% CI: 46.7%-74.0%), respectively.The 3-year OS of the high-risk group was significantly lower than that of the intermediate-risk group (71.8% vs.96.2%, P=0.022), while differences in 3-year RFS and GRFS between the 2 groups were not statistically significant (68.0% vs.80.8%, P=0.400; 52.0% vs.65.4%, P=0.420).The 3-year OS, RFS and GRFS of the complex karyotype group were significantly lower than those of 11q23 rearrangement and other karyotype groups (50.0% vs.85.7%, 93.1%, P=0.009; 37.5% vs.85.7%, 79.3%, P=0.022; 25.0% vs.64.3%, 65.5%, P=0.049).Multivariate analysis showed that a complex karyotype was an independent prognostic factor affecting 3-year OS and GRFS [OS: HR=6.79 (95% CI: 1.13-43.80), P=0.044; GRFS: HR=3.72(95% CI: 1.13-12.20), P=0.030]. Conclusions:High-risk cytogenetic features are significant predictors of survival outcomes in pediatric AML patients undergoing PTCy-based allo-HSCT.
