1.Interventional Effect of Active Ingredients of Chinese Medicine and Compound Formulas on Epithelial-mesenchymal Transition in Lung Cancer: A Review

Lung cancer is the leading cause of cancer-related deaths worldwide, and tumor metastasis is a key factor contributing to the mortality of most lung cancer patients. Aberrant activation of epithelial-mesenchymal transition (EMT) is a major driver of lung cancer progression and metastasis. EMT is characterized by the loss of apical-basal polarity and intercellular adhesion in highly differentiated, polarized, and organized epithelial cells, which acquire motility, migratory potential, and invasive properties. During this process, cells undergo cytoskeletal remodeling and transform into a mesenchymal phenotype, accompanied by associated changes in cellular markers. The EMT process is highly complex and is tightly regulated by intricate networks involving multiple transcription factors, post-translational controls, epigenetic modifications, and non-coding RNAs. Therefore, therapies targeting the mechanisms of malignant transformation and their associated pathways in lung cancer are of significant clinical importance. In recent years, EMT has attracted increasing attention as a potential target for cancer therapy. Chinese medicine, with its characteristics of multi-target action, low side effects, and good therapeutic efficacy, has demonstrated an important role in anticancer treatment. A series of studies have investigated the role of Chinese medicine in inhibiting EMT in lung cancer. Active ingredients of Chinese medicine, including flavonoids, glycosides, phenols, terpenoids, saccharides, and alkaloids, as well as Chinese medicine compound formulas, have shown significant regulatory effects on EMT. Their mechanisms mainly involve multiple pathways, targets, and links, including signaling pathways, exosomes, microRNAs (miRNAs), and the tumor-associated immune microenvironment. This article summarizes the mechanisms by which EMT promotes malignant tumor progression and reviews the current research on how Chinese medicine active ingredients, monomers, and compound formulas inhibit EMT and suppress lung cancer cell migration and invasion. This study is expected to provide comprehensive theoretical information for basic and translational research on lung cancer.
2.Thrombocytopenia in Liver Cirrhotic Patients Induced by Linezolid

Objective To investigate the risk factors and countermeasures of linezolid-induced thrombocytopenia.Methods Clinical pharmacists participated in the pharmacy practice of linezolid-induced thrombocytopenia in a patient with liver cirrhosis.They analyzed the risk factors,took timely countermeasures,and conducted pharmaceutical care.Results The liver cirrhosis,low creatinine clearance and basic platelet values were closely related to linezolid-related thrombocytopenia,platelet recovered after discontinuation.Linezolid(0.3 g,Ⅳ,q12h)was used again under serum drug concentration monitoring,and the infection was effectively controlled,no thrombocytopenia occurred.Conclusions Clinical pharmacists should conduct pharmaceutical care for patients,pay close attention to adverse reactions,and timely adjust the treatment plan to better ensure the effectiveness and safety of treatment.
3.Effects of enhanced core stabilization training based on real-time ultrasound imaging in patients with chronic nonspecific low back pain

Objective:To investigate the efficacy of enhanced core stabilization training based on real-time ultrasound im-aging in patients with chronic nonspecific low back pain(CNLBP).Method:From May,2020 to July,2022,30 patients with CNLBP were randomly divided into two groups with 15 patients in each group.The treatment course was 4 weeks,with real-time ultrasound imaging feed-back combined with core stabilization training or core stabilization training alone.The pain was assessed with visual analogue scale(VAS),the lumbar spine mobility was assessed with the finger-to-floor distance(FFD),and the HUMAC NORM multi-joint isokinetic testing and rehabilitation system was used to evaluate the spine flexion absolute error(AE).The musculoskeletal ultrasonography was used to evaluate the thickness and con-traction rate of transversus abdominis(TrA).Result:After training,the scores of VAS,FFD,and AE in the flexion position of 2 groups were improved compared with the scores before treatment,and the differences were statistically significant(P<0.05),and the improvement in the experimental group was more obvious than that in the control group(P<0.05).There was no statistical significance in the thickness of TrA between the two groups after training.However,the contrac-tion rate of TrA in the observation group was significantly higher than that in the control group,and the dif-ference was statistically significant(P<0.05).Conclusion:Enhanced core stabilization training based on real-time ultrasound imaging has a better short-term therapeutic effect on CNLBP,which is more helpful in reducing pain intensity,enhancing muscle contraction efficiency,and improving the dysfunction of CNLBP patients.

5.Isorhamnetin alleviates pathological damage in influenza A virus strain PR8-induced pneumonia by activating the Nrf2/HO-1 pathway and suppressing apoptosis

Background: Influenza A viruses (IAVs) are the major pathogens associated with respiratory infections which can result in extensive pathological damage in lungs and serious complications. Isorhamnetin, an abundant natural flavonoid in fruits and medicinal plants, has recently been shown to have strong antioxidative, anti-inflammatory, and antiviral effects. Objective: This study investigated the pharmacological effects of isorhamnetin on viral pneumonia and explored the underlying mechanisms by in vivo and in vitro experiments. Materials and methods: In the present study, the protective effect of isorhamnetin against IAV was evaluated by the cytopathogenic effect assay, cell counting kit-8 assay, real-time polymerase chain reaction, and immunofluorescence assay in vitro. Then the pathological damage associated with pneumonia was examined by calculating the pulmonary index and performing micro-CT and hematoxylin-eosin staining in vivo. Thereafter, the related protein or gene levels of factors in the mitogen-activated protein kinase (MAPK) and nuclear factor erythroid 2-related factor 2 (Nrf2)/heme oxygenase-1 (HO-1) signaling pathways were determined by Western blot and immunofluorescence staining. Results: Isorhamnetin exerted significant anti-influenza effects and inhibited the expression of viral RNA in A549 cells, counteracting oxidative stress and apoptosis by suppressing the production of reactive oxygen species and caspase-3. The in vivo experiment results showed that isorhamnetin (20 and 40 mg/kg) caused a significant decrease in the pulmonary index, ameliorated pathological damage in the lung tissue, decreased viral load and NA activity, and reduced cytokines and nuclear factors. Furthermore, isorhamnetin could counteract the B cell lymphoma-2/B cell lymphoma-2–associated X protein (Bax) imbalance induced by PR8, suppress activation of the MAPK pathway, and upregulate the expression of Nrf2 and HO-1. Conclusions: Isorhamnetin can protect against viral pneumonia by activating the Nrf2/HO-1 pathway and suppressing the MAPK path-way. This study deciphers the pharmacological mechanism of isorhamnetin in alleviating pathological damage in viral pneumonia and provides rationale for the application of isorhamnetin in influenza treatment.

8.Pharmacological effects and mechanisms of Xuanfei Baidu Decoction in the treatment of viral pneumonia

Objective: This study aims to investigate the therapeutic effects and underlying mechanisms of Xuanfei Baidu Decoction (XFBD) in a mouse model of dampness-heat toxin pneumonia. By exploring how XFBD exerts its effects, we seek to deepen our understanding of its role in treating pulmonary diseases and to address the current knowledge gap regarding its mechanisms of action, thereby supporting its clinical application. Methods: Ultra-high-performance liquid chromatography and high-resolution mass spectrometry (HRMS) were employed to analyze the chemical constituents of XFBD. The protective effects of XFBD were evaluated using a dampness-heat toxin-induced mouse model, established through dampness-heat exposure and HCoV-229E infection. XFBD was administered orally, followed by assessments including lung index measurement, micro-CT imaging, viral load quantification, cytokine analysis, and histological evaluation via hematoxylin-eosin staining. Proteomics and single-cell transcriptomic analyses were conducted to explore the potential mechanisms underlying XFBD’s pharmacological effects. A cellular model of HCoV-229E infection was developed to investigate changes in the cAMP/PKA signaling pathway. Molecular docking and surface plasmon resonance (SPR) experiments confirmed the strong binding affinity between key XFBD components and PKA. Finally, PKA activators and inhibitors were applied in vitro to validate these mechanistic findings. Results: In vivo studies demonstrated that XFBD significantly reduced the lung index, improved the structural integrity of lung and tongue tissues, and decreased levels of proinflammatory mediators, including IL-6, IL-8, and TNF-α. Proteomic and single-cell transcriptomic analyses showed that the differentially expressed proteins after XFBD treatment were primarily associated with inflammatory responses and immune regulation. The cAMP/PKA signaling pathway was identified as a key mechanism underlying these therapeutic effects. Notably, Western blot, ELISA, molecular docking, and SPR analyses confirmed that XFBD elevated cAMP levels and p-PKA expression, thereby activating the cAMP/PKA signaling pathway in vitro. Conclusion: This study demonstrated that XFBD significantly alleviates symptoms in mice with dampness-heat toxin pneumonia. Its therapeutic effects are mediated, at least in part, through activation of the cAMP/PKA signaling pathway. These findings provide compelling evidence that XFBD is an effective herbal remedy against HCoV-229E infection.
9.Improvement effects of pachymic acid on myocardial injury in coronary heart disease rats by regulating mito-chondrial autophagy mediated by the PINK1/Parkin signaling pathway

OBJECTIVE To explore whether pachymic acid (Pac) regulates mitochondrial autophagy mediated by the PTEN- induced kinase 1 (PINK1)/Parkin RBR E3 ubiquitin-protein ligase (Parkin) signaling pathway to alleviate myocardial injury in coronary heart disease (CHD) rats. METHODS SD rats were divided into control (Con) group, CHD group, Pac low-dose group (Pac-L group), Pac high-dose group (Pac-H group), Pac-H+PINK1/Parkin signaling pathway inhibitor group (Pac-H+3-MA group), with 10 rats in each group. Except for the Con group, CHD models were established in the remaining groups of rats. After successful modeling, the rats in each group were intraperitoneally injected with the corresponding drugs or normal saline. After continuous intervention for 4 weeks, the left ventricular ejection fraction (LVEF), left ventricular end-diastolic volume (LVEDV), left ventricular end-systolic volume (LVESV), and mean arterial pressure (MAP) of the rats were detected. The levels of creatine kinase isoenzyme (CK-MB), lactate dehydrogenase (LDH), cardiac troponin I (cTnI), and cardiac troponin T (cTnT) in the serum, as well as the levels of tumor necrosis factor-α (TNF-α), interleukin-10 (IL-10), IL-1β, reactive oxygen species (ROS), malondialdehyde (MDA) in the myocardial tissue, and the activities of catalase (CAT) and superoxide dismutase (SOD), as well as the expression levels of p62, cleaved caspase-3, Parkin, PINK1 proteins and the ratio of microtubule-associated protein 1 light chain 3 Ⅱ (LC3Ⅱ)/LC3Ⅰ ratio were measured. The morphology of myocardial tissue and mitochondrial autophagic vesicles were observed, and the number of mitochondrial autophagic vesicles per unit area and the rate of cardiomyocyte apoptosis were counted. RESULTS Compared with CHD group, LVEF, MAP, IL-10 levels, CAT and SOD activities, p62, Parkin, PINK1 protein expressions, LC3Ⅱ/LC3Ⅰ ratio, the numbers of mitochondrial autophagic vesicles per unit area in the Pac-L and Pac-H E-mail:hzdpft@163.com groups were increased significantly (P<0.05); the levels of LVEDV, LVESV, CK-MB, LDH, cTnI, cTnT, TNF-α, IL-1β, ROS and MDA, cell apoptosis rates, and protein expression of cleaved caspase-3 were all decreased significantly (P<0.05); and the changes in various indicators were more pronounced in the Pac-H group (P<0.05); both groups showed varying degree of improvement in myocardial histopathological morphology. Compared with the Pac-H group, the aforementioned indicators in rats from the Pac-H+3-MA group were all significantly reversed (P<0.05). CONCLUSIONS Pac may promote mitochondrial autophagy in cardiomyocytes of CHD rats by activating the PINK1/ Parkin signaling pathway, thereby reducing inflammatory responses and oxidative stress and improving myocardial injury.
