Objective: To observe the expression of long non-coding RNA myocardial infarction associated transcript (LncRNA-MIAT) in tumor necrosis factor-α (TNF-α) induced endothelial cells (ECs) inflammation in vitroand to study the impact of LncRNA-MIAT on inflammatory regulation. Methods: LncRNA-MIAT expression in ECs was induced by TNF-α at different time and concentration. Expressions of intercellular adhesion molecule-1 (ICAM-1) and LncRNA-MIAT in inflammatory ECs were examined by quantitative real time polymerase chain reaction (qRT-PCR) and Western blot analysis. Moreover, ECs was transfected by siRNAMIAT to observe the effect of LncRNA-MIAT knock-down on ICAM-1 expression. Results: LncRNA-MIAT expression showed the increasing trend by elevated time and concentration of TNF-stimulation. Compared with TNF-α stimulation at 0h, 6h and 12h, LncRNA-MIAT expressions were increased at 24h and 48h of TNF-αstimulation respectively, allP<0.05; compared with TNF-α concentration at 0ng/ml and 0.125ng/ml, LncRNA-MIAT expressions were elevated by TNF-α stimulation at 1.000ng/ml and 10.000ng/ml respectively, allP<0.05. With siRNAMIAT knock-down, TNF-α induced ICAM-1 protein expression was significantly reduced in ECs,P<0.05. Conclusion: LncRNA-MIAT might be involved in ECs inflammatory response and it may play a role to promote inflammation.

A 67-year-old female patient underwent the hypersensitivity test of ultravist solution before doing adrenal enhancement CT. Fifteen minutes after intravenous injection of 2 ml of ultravist solution,the patient developed palpitation,chest pain,vomiting,weakness,and hyperhidrosis. Her blood pressure was 90 / 60 mmHg. She was considered to have contrast agent anaphylactic shock. The patient received intravenous injection of dexamethasone 5 mg and IV infusion of 0. 9％ sodium chloride injection 500 ml immediately. Two minutes later,her blood pressure elevated to 145 / 90 mmHg,but the chest pain was not relieved. The results of electrocardiogram showed tilted elevation on ST segment in II,III,aVF leads and declivous depression on ST segment in leads I and aVL. She was diagnosed as acute myocardial infarction. The patient immediately received aspirin enteric-coated tablet 300 mg and clopidogrel hydrogen sulfate 300 mg by chewing,hypodermic injection of low-molecular-weight heparin calcium injection 4100 U, and continuous intravenous pumping of isosorbide dinitrate injection 50 mg at speed of 2 mg/ h. One hour later, the patient′s thoracalgia was remission. She received the treatments of anti-coagulation,stable plaque,anti-hypertension,and expanding coronary artery sequentially. Thirteen days later,the hypersensitive test of ultravist solution was underwent again,the patient did not show any obvious discomfort for 24 hours. It was indicated that the patient′ s acute myocardial infarction appeared in the first hypersensitive test was the anaphylactoid reaction induced by ultravist solution.

Objective:To study protective effect of nitrates pharmacological postconditioning (PI‐PostC) on ischemia‐reperfusion (I/R) injured myocardium in rats .Methods :A total of 32 Wistar rats were randomly and equally divided into sham operation group ,I/R group ,ischemia postconditioning group (IPostC group) and PI‐PostC group .Myo‐cardial ischemic area ,infract size ,plasma level of cardiac troponin I (cTnI) and cell apoptotic index were measured and compared among all groups .Results:Compare with sham operation group ,there was significant rise in plasma cTnI level [ (6.39 ± 2.35)μg/L vs .(70.33 ± 6.94)μg/L vs .(41.19 ± 4.50)μg/L ,(37.47 ± 4.20)μg/L ,P<0.01 all] in I/R group ,IPostC group and PI‐PostC group ,and those of IPostC group and PI‐PostC group were significant‐ly lower than that of I/R group , P<0.01 both .Compared with I/R group ,there were significant reductions in my‐ocardial ischemic area [ (53.31 ± 3.87)% vs .(46.46 ± 2.13)% vs .(42.08 ± 4.84)% ] ,infarct size [ (52.19 ± 3.44)% vs .(41.02 ± 2.93 )% , (38.13 ± 2.05 )% ] and cell apoptotic index [ (26.92 ± 1.91 )% vs . (20.54 ± 3.05)% ,(19.49 ± 2.41)% ] in IPostC group and PI‐PostC group ,P<0.01 all .Compared with IPostC group ,ische‐mic area significantly reduced (P<0.05) in IP‐PostC group ,but there were no significant changes in infarct size , cTnI level and cell apoptotic index between IPostC group and IP‐PostC group , P>0.05. Conclusion:Nitrates phar‐macological postconditioning possesses the same protective effect on myocardial I/R injury as ischemia postcondition‐ing ,and it can reduce myocardial ischemic area more effectively .

Objective:To explore value of pulsed wave (PW) and tissue synchronization imaging (TSI) evaluate cardi-ac systolic synchronization in patients with ischemic cardiomyopathy (ICM) .Methods:A total of 50 ICM patients were enrolled as ICM group and 35 healthy volunteers without organic heart disease were regarded as healthy control group ,PW was used to measure mitral diastolic blood flow duration/RR interval (LVFT/RR) to evaluate synchroni-zation of left atrial and left ventricular synchronization ;interventricular mechanical delay (IVMD) was measured to evaluate left and right ventricular synchronization ;TSI software was used to measure mitral annular mean systolic peak velocity (LV-Sm) ,systolic time to peak (Ts) and Ts standard deviation (Ts-SD) of all segments in order to e-valuate left ventricular systolic synchronization .Results:Compared with healthy control group ,there were signifi-cant reductions in LVFT/RR [ (44.74 ± 1.58)% vs .(41.08 ± 4.65)% ] and LV-Sm [ (9.72 ± 0.53) ms vs .(4.09 ± 1.06) ms] ,and significant rise in IVMD [ (15.51 ± 5.52) ms vs .(41.96 ± 4.20) ms] and Ts-SD [ (16.47 ± 4.16) ms vs .(34.13 ± 11.68) ms] in ICM group , P<0.01 all;Ts of anterior wall ,inferior wall et .6 positions were significantly longer ,and its synchronization significantly reduced , P<0. 05 all ,percentages of the most de-layed systolic part of left ventricle were no significant differences between two groups (P> 0.05) .Conclusion:Compared with healthy control group ,left ventricular total systolic function ,left atrial&ventricular ,left&right ventricular synchronization and systolic synchronization within left ventricular significantly reduce in ICM patients .

BACKGROUND:Erythropoietin and bone marrow mesenchymal stem cells have been shown to affect myocardial apoptosis. However, few studies concerned their combined application to sepsis-related myocardial injury. OBJECTIVE:To observe the effects of the combination of erythropoietin and bone marrow mesenchymal stem cells on pathology and apoptosis of sepsis rat cardiomyocytes. METHODS:A total of 50 Sprague-Dawley rats were randomly selected and assigned to five groups (n=10). Sepsis models were established by cecal ligation perforation method. Rat models in the bone marrow mesenchymal stem cellgroup, erythropoietin group and erythropoietin+bone marrow mesenchymal stem cellgroup were respectively treated with bone marrow mesenchymal stem cells, erythropoietin and erythropoietin+bone marrow mesenchymal stem cells immediately after model induction via intraperitoneal injection or caudal vein. Model group received cecum ligation and puncture. Control group did not undergo any treatment after the abdomen was opened. Model and control groups were infused with an equal volume of physiological saline via caudal vein. At 24 hours, experimental animals were sacrificed by anesthesia. Myocardial specimens were col ected. Myocardial appearance was observed using hematoxylin-eosin staining. Bax, Caspase-3 and Bcl-2 were tested by western blot assay. RESULTS AND CONCLUSION:Hematoxylin-eosin staining:cardiomyocytes were regularly arranged, showing integrated structure in the control group. Extensive myocardial fiber breakage, disordered arrangement, cardiomyocyte swel ing or shrinkage, and vacuolar degeneration were observed in the model group. Moreover, myocardial interstitial vascular congestion, edema, and inflammatory cellinfiltration were visible. Myocardial tissue was similar between erythropoietin and bone marrow mesenchymal stem cellgroups, with the presence of mild inflammatory cellinfiltration and scattered normal cardiomyocytes. In the erythropoietin+bone marrow mesenchymal stem cellgroup, cardiomyocytes were slightly damaged. Interstitial vascular congestion was not apparent, and a few inflammatory cells infiltrated. Western blot assay results demonstrated that Bcl-2 protein expression was significantly higher (P<0.01), but Bax and Caspase-3 protein expression was lower (P<0.05) in the erythropoietin+bone marrow mesenchymal stem cellgroup than in the erythropoietin, model and control groups. The combination of erythropoietin and bone marrow mesenchymal stem cells in treatment of sepsis-related myocardial injury could lessen myocardial pathological changes, and inhibit cardiomyocyte apoptosis. The mechanisms maybe exert by upregulating anti-apoptotic protein expression and downregulating apoptotic protein expression.

Objective: To observe influence of porphyromonas gingivalis (Pg, a main pathogenic bacterium of periodontal disease) on vascular intima adhesion factors and metalloproteinases. Methods: A total of 60 rats were randomly and equally divided into blank control group, Pg low dose group (Pg 2ml) and Pg high dose group (Pg 5ml) according to number table. The latter two groups respectively received intramuscular injection of corresponding dose Pg every three days without antibiotic intervention for 12 weeks. Then their venous blood was taken to measure levels of intercellular adhesion molecule-1 (ICAM-1), vascular cell adhesion molecule-1 (VCAM-1), matrix metalloproteinase (MMP)-3 and MMP-9 in three groups. Results: Compared with blank control group, there were significant increase in contents of ICAM-1 [（175.79±14.30）ng/ml vs.（182.62±15.07）ng/ml, （189.39±14.93）ng/ml]、VCAM-1 [（256.49±37.17）ng/ml vs.（271.58±32.85）ng/ml , (286.66±30.66)ng/ml] 、 MMP-3 [（3.23±0.69）ng/ml vs.（3.61±0.82）ng/ml, (3.97±0.83)ng/ml]及 MMP-9 [（1.30±0.39）mg/L vs.（1.48±0.39）mg/L, 1.67±0.45）mg/L ]（P <0.05，or <0.01），Compared with Pg low dose group, there were significant increase in levels of above indexes in Pg high dose group (P<0.05). Conclusion: Porphyromonas gingivalis can significantly increase serum contents of vascular intima adhesion factors and metalloproteinases, aggravating pathological development of coronary heart disease.

Objective To observe the change of the concentration of plasma B-type natriuretic peptide (BNP) before and after treadmill exercise test (TET),and to judge the accuracy of BNP changes ( ABNP)in diagnosing coronary heart disease (CHD). Methods A total of 188 patients with suspected CHD underwent TET and the levels of plasma BNP before and after TET was detected. All the patients were divided into CHD group( group A, n = 96) and non-CHD group(group B, n = 92) according to coronary ar-teriography. Meanwhile, 26 healthy volunteers were enrolled (group C). Results The levels of plasma BNP after TET increased significantly (P<0.01 ). The levels of ABNP of group A were obviously higher than those of group B and C(P<0.01 ). When the levels of △BNP was 8.5 pg/ml, the sensibility, specificity, pos-itive predictive value and negative predictive value of diagnosing CHD was 93.5%, 57.1%, 70.5% and 88.9% respectively. Area under curve (AUC) was 0.815, and 95% confidence interval was 0.768-0.896.Conclusions The levels of plasma BNP after TET increase obviously. It exists higher sensibility and speci-ficity in diagnosing CHD by △ABNP set 8.5 pg/ml.

Objective To evaluate the protective effect of prior ?-adrenergic blocker therapy to patients who have PCI therapy.Methods We analyzed 210 consecutive patients undergoing ?-adrenergic blocker,of whom 105 did ?-adrenergic blocker therapy,105 had not.CK-MB、E、NE were tested before PCI,and 6~8 h,16~24 h after PCI.Procedural complications in hospital and 1-year outside outcomes were evaluated.Results (1)There was no significant difference between the two groups on CK-MB、E、NE.(2)Both the in-hospital(2.1% vs 4.1%)and one year (5.5% vs 7.1%)mortality was lower in ?-group than in no-? group.Conclusion Prior ?-adrenergic blocker therapy can reduce the incidence of cardic events.

Objective:To investigate the predictive value of QT interval dispersion (QTd),cor rected QT interval dispersion (QTcd) and corrected QT interval(QTc) of car diac events in patients with congestive heart failure (CHF).Methods: QTd、QTcd、QTc and QTcd/QTc of 106 CHF patients were reviewed. Results:QTd、QTcd、QTc and QTcd/QTc of patients with and without cardiac events had significant difference respectively(P<0.05、<0.01、<0.05、<0.001 respectively).Conclusion: QTd、QTcd、QTc and QTcd/QTc are independent factors in predictin g cardiac events of CHF patients. In which,QTcd/QTc is the most accurate factor.
