1.Research Tackling Paradigm and Technological Layout Strategies Based on Erectile Dysfunction, A Clinical Dominant Disease of Traditional Chinese Medicine

To thoroughly implement the strategic deployment outlined in the Opinions of the Central Committee of the Communist Party of China and the State Council on Promoting the Inheritance and Innovative Development of Traditional Chinese Medicine regarding research on dominant diseases of traditional Chinese medicine and to uphold the development philosophy of equal emphasis on traditional Chinese medicine and western medicine,the China Association of Chinese Medicine has fully played a leading academic role by systematically organizing and conducting a series of academic youth salons on clinical dominant diseases of traditional Chinese medicine. On September 13,2024,the 36th Youth Salon on Clinical Dominant Diseases was successfully held in Nanjing,focusing on the advantages of traditional Chinese medicine and the integrative traditional Chinese medicine and western medicine in the diagnosis and treatment of erectile dysfunction (ED). The conference brought together leading experts from traditional Chinese medicine,western medicine,and interdisciplinary fields,facilitating in-depth multidisciplinary discussions that led to key consensus on optimizing traditional Chinese medicine treatment protocols for ED,researching and developing new drugs of traditional Chinese medicine,and advancing interdisciplinary development in traditional Chinese medicine. This salon systematically sorted out the clinical strengths and distinctive features of traditional Chinese medicine in the diagnosis and treatment of ED. Based on current research foundations and clinical needs,it identified key directions for future scientific layout and scientific research tackling: (1) Standardization of syndrome differentiation system of traditional Chinese medicine for ED. (2) Optimization and standardization of intervention methods of integrated traditional Chinese medicine and western medicine. (3) High-quality clinical research guided by evidence-based medicine. (4) In-depth analysis of the pharmacological mechanisms of traditional Chinese medicine in the treatment of ED. (5) Clinical translation and application promotion of new drugs of traditional Chinese medicine. (6) Interdisciplinary integration and innovation in traditional Chinese medicine. For each research direction,key focus areas,expected objectives,and clinical value were further refined,along with the establishment of a scientifically sound priority funding level evaluation system. Therefore,building on the series of salons on the ED-focused dominant diseases of traditional Chinese medicine,this paper provides standardized guidance for clinical practice of traditional Chinese medicine in ED management,effectively contributing to the high-quality development of traditional Chinese medicine. It serves as a valuable reference for national scientific and technological strategic layout, research and development decision-making in new drugs of traditional Chinese medicine,research topic planning,and clinical guideline formulation.
2.Surface disinfection efficacy of 254 nm continuous ultraviolet and pulsed ultraviolet laser

Objective To compare and analyze the germicidal efficacy of continuous ultraviolet and pulsed ultravio-let(UV)lasers on pathogenic microorganisms.Methods Spore slides of Escherichia coli and Bacillus atrophaeus were irradiated using a 254 nm ultraviolet mercury lamp and UV laser.The carrier quantitative germicidal test was conducted to determine the disinfection efficacy at different irradiation doses.Results When the irradiation dose of the ultraviolet mercury lamp was 245.52 mJ/cm2,the logarithmic killing values of Escherichia coli and Bacillus atro-phaeus spores were 5.00 and 2.92,respectively,and the mean logarithmic killing doses were 49.10 and 84.08 mJ/cm2,respectively;When the UV laser irradiation doses were 208.39 and 206.80 mJ/cm2,the logarithmic killing values for the two microorganisms were 6.29 and 3.32,respectively,and the mean logarithmic killing doses were 33.13 and 62.29 mJ/cm2,respectively.Conclusion Compared with continuous UV radiation,pulsed UV laser has stron-ger penetration ability,better killing efficacy on pathogenic microorganisms at the same radiation dose,and can con-duct targeted disinfection and sterilization on the surface of objects directionally.

4.The Theory of"Toxin and Heat"in Acute Pancreatitis Based on the"Microbiota-Metabolism-Immunity"Axis

Patients and animal models suffering from acute pancreatitis(AP)have characteristic phenotypes of gut microbiota,which is associated with the severity of AP.Gut microbiota directly or indirectly participates in host immune regulation by catabolizing nutrients into functional products.The imbalance of gut microbiota and its metabolites activate the immune inflammatory response,which attribute to the aggravation of AP.According to the theory of traditional Chinese medicine,the abnormal metabolism and the release of inflammatory factors caused by the imbalance of gut microbiota are classified into the category of"endotoxin",which combine with external toxins(alcohol,food,cold poisoning,etc.)to turn into heat and even fire.Therefore,"microbiota-metabolism-immunity"axis disorder is the molecular biological basis for the"toxin and heat"theory in AP.For traditional Chinese medicine,classical prescriptions and proven prescriptions could treat AP by targeting to regulate the"microbiota-metabolism-immunity"axis,which not only enriches the"toxic and heat"theory,but also provides new ideas for the prevention of AP.
5.Evaluation of Reliability and Validity of the Chinese Version of the Hyperacusis Scale

Objective To evaluate the reliability and validity of the Chinese version of the hyperacusis scale and estimate its applicability in the Chinese population.Methods The patients admitted to the Department of Oto-laryngology of Xijing Hospital from June 2017 to December 2017 were surveyed.A total of 300 questionnaires were sent out and 293 valid questionnaires were collected with effective response rate of 97.67%.All participants comple-ted a basic information survey,pure tone audiometry and fill in the Chinese version of the hyperacusis scale.The re-liability and validity of this scale were evaluated.Results Among the 293 respondents,243(82.93%)were compli-cated with tinnitus.A total of 181 cases(61.77%)were subjectively diagnosed with hyperacusis through question-naire survey.According to the scale evaluation,174 cases(59.39%)were diagnosed with hyperacusis,and the Kappa value of conformity test of the two methods was 0.81(P<0.001).Reliability analysis indicated a Cronbach'sα coefficient of 0.93 for the entire scale.Each dimension had a Cronbach's α coefficient>0.60.The split-half relia-bility coefficient for the entire scale was 0.86,and for each dimension,it was>0.60.Exploratory factor analysis of validity suggested that 25 items could be grouped under 4 factors,such as functional part,social part,emotional part and harmonic sensitivity part.These factors had eigenvalues greater than 1,accounting for 59.42%of the total variance.Post-rotation,the 25 items were distributed across the 4 components,with each item having a factor load-ing>0.40.Confirmatory factor analysis of validity showed model fit parameters as:x2/df=1.98,GFI=0.91,AGFI=0.83,CFI=0.92,TLI=0.89,RMSEA=0.08.Each item had a standardized factor loading>0.40.Con-clusion The Chinese version of the hyperacusis scale demonstrated strong reliability and validity,making it suitable for use among the Chinese population.
6.Exploration of a Machine Learning Based Body Constitution Model for Identifying the Twenty-five People of Yin and Yang through Hand Diagnosis

OBJECTIVE To explore the construction of the physical identification model of twenty-five people of yin and yang by combining the hand diagnosis image analysis of machine learning,in order to enrich the input of physical identification features.METHODS Based on the theory of twenty-five people of yin and yang,the hand images of 542 patients with cardiovascular disease were collected by self-developed hand diagnosis acquisition equipment.The key points of the hand were extracted by the Mediapipe palm key point algorithm,and the palm area was segmented by geometry to obtain the length and color gamut characteristics of different parts.The physician with clinical license was used to determined the patient's yin and yang twenty-five constitution according to the e-lectronic medical record,and the patient's constitution was identified by comparing Random Forest,Logistic Regression,Xgboost algo-rithm and Light GBM.RESULTS This study successfully constructed a TCM constitution identification model based on hand charac-teristics,which can identify ten basic constitution types including gold wood,gold fire,gold water,gold soil,wood fire,wood water,wood soil,water fire,water soil and fire soil.After sample equalization,the Random Forest model performed best in physical identifi-cation,with an accuracy rate of 0.69,which was significantly better than other models.CONCLUSION The constitution identifica-tion model of TCM hand diagnosis based on hand characteristics can realize the constitution identification of the five elements of TCM.However,due to the use of only hand diagnosis data of TCM,the accuracy needs to be improved.Compared with the traditional body mass scale,it is more objective,convenient,highly stable and consistent.In the future,multimodal integration with face diagno-sis and whole body inspection can be carried out to promote the accuracy,intelligence and multi-dimensional evaluation ability of TCM constitution identification technology.

Objective:To explore the current status of surgery for portal hypertension to grasp current status and future development of surgery in China.Methods:This study is jointly sponsored by China Hepatobiliary & Pancreatic Specialist Alliance & Portal Hypertension Alliance in China (CHESS).Comprehensive surveying is conducted for basic domestic situations of surgery for portal hypertension, including case load, surgical approaches, management of postoperative complications, primary effects, existing confusion and obstacles, liver transplantation(LT), laparoscopic procedures and transjugular intrahepatic portosystemic shunt(TIPS), etc.Results:A total of 8 512 cases of portal hypertension surgery are performed at 378 hospitals nationwide in 2021.Splenectomy plus devascularization predominated(53.0%)and laparoscopy accounted for 76.1%.Primary goal is preventing rebleeding(67.0%) and 72.8% of hospitals used preventive anticoagulants after conventional surgery.And 80.7% of teams believe that the formation of postoperative portal vein thrombosis is a surgical dilemma and 65.3% of hospitals practiced both laparoscopy and TIPS.The major reasons for patients with portal hypertension not receiving LT are due to a lack of qualifications for LT(69.3%)and economic factors(69.0%).Conclusions:Surgery is an integral part of management of portal hypertension in China.However, it is imperative to further standardize the grasp of surgical indications, the handling of surgical operation and the management of postoperative complications.Moreover, prospective, multi-center randomized controlled clinical studies should be performed.

We wished to establish an expert consensus on late stage of critical care (CC) management. The panel comprised 13 experts in CC medicine. Each statement was assessed based on the Grading of Recommendations Assessment, Development, and Evaluation (GRADE) principle. Then, the Delphi method was adopted by 17 experts to reassess the following 28 statements. (1) ESCAPE has evolved from a strategy of delirium management to a strategy of late stage of CC management. (2) The new version of ESCAPE is a strategy for optimizing treatment and comprehensive care of critically ill patients (CIPs) after the rescue period, including early mobilization, early rehabilitation, nutritional support, sleep management, mental assessment, cognitive-function training, emotional support, and optimizing sedation and analgesia. (3) Disease assessment to determine the starting point of early mobilization, early rehabilitation, and early enteral nutrition. (4) Early mobilization has synergistic effects upon the recovery of organ function. (5) Early functional exercise and rehabilitation are important means to promote CIP recovery, and gives them a sense of future prospects. (6) Timely start of enteral nutrition is conducive to early mobilization and early rehabilitation. (7) The spontaneous breathing test should be started as soon as possible, and a weaning plan should be selected step-by-step. (8) The waking process of CIPs should be realized in a planned and purposeful way. (9) Establishment of a sleep-wake rhythm is the key to sleep management in post-CC management. (10) The spontaneous awakening trial, spontaneous breathing trial, and sleep management should be carried out together. (11) The depth of sedation should be adjusted dynamically in the late stage of CC period. (12) Standardized sedation assessment is the premise of rational sedation. (13) Appropriate sedative drugs should be selected according to the objectives of sedation and drug characteristics. (14) A goal-directed minimization strategy for sedation should be implemented. (15) The principle of analgesia must be mastered first. (16) Subjective assessment is preferred for analgesia assessment. (17) Opioid-based analgesic strategies should be selected step-by-step according to the characteristics of different drugs. (18) There must be rational use of non-opioid analgesics and non-drug-based analgesic measures. (19) Pay attention to evaluation of the psychological status of CIPs. (20) Cognitive function in CIPs cannot be ignored. (21) Delirium management should be based on non-drug-based measures and rational use of drugs. (22) Reset treatment can be considered for severe delirium. (23) Psychological assessment should be conducted as early as possible to screen-out high-risk groups with post-traumatic stress disorder. (24) Emotional support, flexible visiting, and environment management are important components of humanistic management in the intensive care unit (ICU). (25) Emotional support from medical teams and families should be promoted through"ICU diaries"and other forms. (26) Environmental management should be carried out by enriching environmental content, limiting environmental interference, and optimizing the environmental atmosphere. (27) Reasonable promotion of flexible visitation should be done on the basis of prevention of nosocomial infection. (28) ESCAPE is an excellent project for late stage of CC management.
