Objective: To evaluate the efficacy and safety of ruxolitinib combined with low-dose hormone for patients with acute graft-versus-host disease (aGVHD) after allogeneic hematopoietic stem cell transplantation (allo-HSCT). Methods: Thirty patients with aGVHD after allo-HSCT admitted to the Department of Hematology of the General Hospital of Western Theater Command from November 2021 to November 2024 were retrospectively analyzed. All patients were treated with low-dose hormone (methylprednisolone 0.3-1 mg kg -d ) combined with ruxolitinib 5-10 mg d . The efficacy and adverse reactions were observed during the follow-up period to analyze the survival outcomes of the patients. Results: A total of 30 patients with aGVHD after allo-HSCT were included in this study, consisting of 15 (50%) males and 15 (50%) females with a median age of 34 year-old (ranging from 14 to 62). Classification by disease type: there were 18 cases of acute myeloid leukemia, 4 cases of acute lymphoblastic leukemia, 4 cases of aplastic anemia, and 4 cases of myelodysplastic syndrome. Classification by aGVHD severity: there were 27 cases (90%) of Ⅱ-Ⅳ degree aGVHD and 11 cases (36.7%) of Ⅲ-Ⅳ degree aGVHD. Ruxolitinib in combination with low-dose glucocorticoid treatment yield responses in 28 (93.3%) patients, of which 27 (90%) achieved complete remission (CR), while 1 (3.3%) showed partial remission (PR). One patient (3.3%) had no response (NR), and 1 patient (3.3%) exhibited progressed disease (PD). Overall survival (OS) at 1 year of transplantation was 73.9% (95%CI 49.5% to 87.7%), progression-free survival (PFS) was 93.3% (95%CI 75.9% to 98.3%), non-relapse mortality (NRM) was 20.6% (95%CI 7.9% to 47.4%), and median survival time was 27.6 months. Conclusion: Ruxolitinib combined with low-dose hormones is safe and effective in the treatment of aGVHD after allo-HSCT.

Objective To explore the association between plasma fibrinogen(FBG)levels and the risk of in-hospital mortality among patients with septic shock.Methods The clinical data of 563 patients diagnosed with septic shock in the Intensive Care Unit(ICU)of Shenzhen Second People's Hospital from August 1,2018,to December 31,2020,were collected.Patient demographic information,basic vital signs,and blood routine and biochemical indices upon admission were gathered.Moreover,the Acute Physiology and Chronic Health Evaluation Ⅱ(APACHEⅡ)scores were calculated.Binary logistic regression analysis was conducted to explore the correlation between plasma fibrinogen levels and in-hospital mortality in patients with septic shock.Additionally,a generalized additive model(GAM)and smoothed curve fitting were employed to investigate the nonlinear relationship between plasma fibrinogen and in-hospital mortality.Receiver operating characteristic(ROC)curves were constructed for FBG and APACHEⅡ scores to predict in-hospital mortality in septic shock patients.The area under the curve(AUC)was computed to compare the predictive efficacies of the two.Furthermore,a segmented linear regression model was utilized for quantification.Results Binary logistic regression analysis demonstrated a significant negative correlation between plasma fibrinogen levels and in-hospital mortality among patients with septic shock(P<0.05).GAM modeling and smoothed curve fitting disclosed a nonlinear association between plasma fibrinogen levels and in-hospital mortality,with an inflection point at 5.54 g/L.The segmented linear regression model indicated that,to the left of the inflection point(FBG≤5.54 g/L),for every 1 g/L decrease in plasma fibrinogen,the risk of death increased by 24.5%(OR=0.755,P=0.003).Conversely,to the right of the inflection point(FBG>5.54 g/L),the relationship was not statistically significant(OR=1.049,P=0.685).The findings of the subgroup analyses indicated that the characteristics of the subgroups did not alter the relationship between blood fibrinogen levels and in-hospital mortality.Conclusion There is a nonlinear relationship between FBG levels and in-hospital mortality in patients with septic shock,which has predictive value for evaluating the risk of in-hospital mortality in this patient cohort.

Objective To summarize the magnetic resonance imaging(MRI)features of spinal adnexal tuberculosis in order to improve its early diagnosis.Methods Totally 21 spinal adnexal tuberculosis patients confirmed at some hospital from January 2019 to October 2023 had their clinical data and MRI images analyzed retrospectively to determine the basic clinical characteristics and MRI features.Results Of the 21 patients,8 ones had sudden lower extremity weakness and pyramidal tract signs,and the remaining 13 ones had no significant symptoms of neurologic deficit.The lesion involvement ranged from C4 to L5 vertebrae,with the involvement of lumbar segments in 13 cases,thoracic segments in 6 cases and cervical segments in 2 cases.There were 15 cases that had tuberculosis involving in only a single spinal adnexa and peripheral soft tissue,4 cases in 2 vertebrae and 2 cases in 3 vertebrae.There were 9 cases involving in pedicles,8 cases in vertebral plates and some cases involving in sphenoid process,transverse process or facet joints.The MRI features of spinal adnexal tuberculosis included the tuberculosis-infected bone showing slightly low signals on T1WI while slightly high signals on T2WI,edge enhancement by enhanced scan and edema and abscess of paravertebral soft tissue and intra-and extradural tuberculous abscess displayed clearly by the fat suppression and diffusion weighted imaging sequences of T2WI.Conclusion MRI effectively detects the abnormal signs of bone and soft tissue of spinal adnexal tuberculosis.The MRI findings of spinal adnexal tuberculosis are of characteristics,and MRI can be used as the first choice for imaging examination and differential diagnosis to realize early detection of spinal adnexal tuberculosis.[Chinese Medical Equipment Journal,2025,46(1):55-59]

Objective To explore the correlation between amide proton transfer weighted(APTw)imaging of the brain and the clinical psychological scale assessment in patients with Alzheimer's disease(AD).Methods A total of 30 AD patients(AD group)and 33 gender-and age-matched healthy volunteers(control group)were selected and all patients underwent brain MRI,magnetic resonance angiography(MR A)and APTw imaging examinations.According to the APTw images,the magnetization transfer ratio asym-metry(MTRasym)at 3.5 ppm values of each brain region were measured.The differences in MTRasym(3.5 ppm)values of each brain region between the AD group and the control group were compared,and then the diagnostic efficacy of MTRasym(3.5 ppm)values with significant differences were further analyzed.The correlations between the MTRasym(3.5 ppm)values of each brain region and scores of the mini-mental state examination(MMSE)and Montreal cognitive assessment(MoCA)were analyzed.Results Compared with the control group,the MTRasym(3.5 ppm)values of the left hippocampal head in the AD group were significantly increased(P＜0.05).The area under the curve(AUC)of the receiver operating characteristic(ROC)curve of MTRasym(3.5 ppm)values of the left hippocampal head was 0.656(P＜0.05).MTRasym(3.5 ppm)values of the left hippocampal head,right temporal white matter,and bilateral occipital white matter were significantly negatively correlated with MoCA score(P＜0.05).Conclusion APTw imaging can effectively reflect the changes of protein concentration of the brain regions in AD patients,and is associated with cognitive func-tion,providing a new approach and method for early non-invasive diagnosis and disease monitoring of AD.

Ulcerative colitis（UC） is a chronic non-specific inflammatory bowel disease characterized by inflammation and ulceration of the colonic mucosa and submucosa， and its complex pathogenesis involves immune abnormality， oxidative stress and other factors. The nuclear transcription factor E₂-related factor 2（Nrf2）， encoded by the Nfe212 gene， plays a central role in antioxidant responses. It not only activates various antioxidant response elements such as heme oxygenase-1（HO-1） and quinone oxidoreductase 1（NQO1）， but also enhances the activity of glutathione-S-transferase（GST） and superoxide dismutase 1（SOD1）， effectively eliminating reactive oxygen species（ROS） accumulated in the body， and mitigating oxidative stress-induced damage to intestinal mucosa. In addition， Nrf2 can reduce the release of inflammatory factors and infiltration of immune cells by regulating immune response， cell apoptosis and autophagy pathways， thereby alleviating intestinal inflammation and promoting the repair and regeneration of damaged mucosa. Based on this， this paper reviews the research progress of Chinese materia medica in the prevention and treatment of UC by modulating the Nrf2 signaling pathway. It deeply explores the physiological role of Nrf2， the molecular mechanism of activation， the protective effect in the pathological process of UC， and how active ingredients in Chinese materia medica regulate the Nrf2 signaling pathway through multiple pathways to exert their potential mechanisms. These studies have revealed in depth that Chinese materia medica can effectively combat oxidative stress by regulating the Nrf2 signaling pathway. It can also play a role in anti-inflammatory， promoting autophagy， inhibiting apoptosis， protecting the intestinal mucosal barrier， and promoting intestinal mucosal repair， providing new ideas and methods for the multi-faceted treatment of UC.
