Background:
Visceral white adipose tissue (vWAT) inflammation is a critical pathology of obesity-caused heart damage and is closely associated with adipocyte endoplasmic reticulum (ER) dysfunction. Serine (or cysteine) peptidase inhibitor, clade A, member 3C (Serpina3c) has been identified as an adipokine with anti-vWAT inflammatory effects. However, it remains unclear whether Serpina3c deficiency promotion of vWAT inflammation involves adipocyte ER dysfunction and whether it further contributes to heart damage in obesity.
Methods:
Wild type and Serpina3c knockout (Serpina3c–/–) mice were fed a high-fat diet (HFD) for 12 weeks. An adeno-associated virus (AAV) was injected locally into epididymal white adipose tissue (eWAT) of Serpina3c–/– mice to induce eWAT-adipocyte- specific overexpression of Serpina3c (AAV-Serpina3c) or knockdown of hypoxia-inducible factor 1α (AAV-shHIF1α). In vitro experiments were performed in 3T3-L1 adipocytes.
Results:
Serpina3c–/– mice exhibited more severe eWAT, serum and heart inflammation after HFD feeding. Consistently, these adverse phenotypes were mitigated in AAV-Serpina3c and AAV-shHIF1α mice. Mechanistically, ER oxidoreductase 1α (Ero1α) and protein disulfide isomerase (PDI) family members PDIA3 and PDIA4 were found to be target genes of HIF1α. In the obese mice, Serpina3c deficiency caused adipocyte more hypertrophy, and activated HIF1α-Ero1α/PDI mediated ER overoxidation and ER stress in eWAT. Subsequently, this led to increased adipocyte apoptosis and chemokine production and decreased adiponectin expression, which promoted macrophage infiltration and M1 polarization in eWAT, thus exacerbating eWAT inflammation and ultimately facilitating serum and distal heart inflammation.
Conclusion
These findings indicate that Serpina3c is a significant regulator of adipocyte ER redox homeostasis, thus highlighting Serpina3c as a potential therapeutic target for obesity-related eWAT inflammation and heart damage.

Objective To investigate the expression levels of Ubiquitin Carboxy-Terminal Hydrolase-L1(UCH-L1)and Netrin-1 in the serum of patients with spontaneous basal ganglia hemorrhage(ICH)complicated with cerebral edema, and to analyze their impacts on neurological deficits and prognosis. Methods A retrospec-tive analysis was conducted on the clinical data of 173 patients with spontaneous basal ganglia hemorrhage admitted to the Department of Neurosurgery,The First Affiliated Hospital of Bengbu Medical University from September 2023 to January 2025.The serum levels of UCH-L1 and Netrin-1 were measured within 24 hours after the onset. They were divided into three groups according to the size of the cerebral edema volume(CEV):Group A(CEV<10 mL),Group B(CEV 10~30 mL),and Group C(CEV>30 mL).Pearson's correlation analysis was used to analyze the correlation between serum expression levels of UCH-L1 and Netrin-1 with hemorrhage, volume of cere-bral edema, distance of midline shift,Modified Edinburgh-Scandinavian Stroke Scale(MESSS)score,Modified Rankin Scale(mRS)score, and Glasgow Coma Scale(GCS)score.Logistic regression analysis was performed to identify the risk factors for poor prognosis.Receiver operating characteristic(ROC)curve analysis was used to evaluate the predictive value of UCH-L1 and netrin-1 for poor prognosis. Results Significant differences were observed in the serum levels of UCH-L1 and netrin-1 among patients with different volumes of cerebral edema(P<0.05).The larger the volume of cerebral edema, the higher the expression levels of UCH-L1 and netrin-1.The serum levels of UCH-L1 and Netrin-1 were significantly higher in the poor prognosis group compared to the good prognosis group(P<0.05).The serum levels of UCH-L1 and Netrin-1 were positively correlated with MESSS score, hemorrhage volume, cerebral edema volume, distance of midline shift, and mRS score(P<0.05),and negatively correlated with GCS score(P<0.05).Multivariate Logistic regression analysis showed that both UCH-L1 and netrin-1 were independent risk factors for poor neurological prognosis in basal ganglia hemorrhage patients(P<0.05).ROC curve analysis indicated that both markers had important predictive value for poor prognosis. The AUC for serum UCH-L1 level predicting poor prognosis was 0.77[95%confidence interval(CI):0.69~0.85,P<0.01],with a sensitivity of 84.9%and a specificity of 50.6%.The AUC for serum Netrin-1 level predicting poor prognosis was 0.89(95%CI:0.85~0.94,P<0.01),with a sensitivity of 82.1%and a specificity of 68.7%.Conclusions Serum UCH-L1 and Netrin-1 are differentially expressed in patients with spontaneous basal ganglia hemorrhage complicated with different volumes of cerebral edema. They are independent risk factors for poor prog-nosis and are important predictors of neurological function prognosis in these patients.

Objective To specify the occurrence status, distribution characteristics and risk factors of neglected perforations in surgical gloves used during spinal surgery. Methods A total of 932 surgical gloves used during spinal surgery in our hospital from October 2022 to September 2024 were selected. The occurrence and distribution location of neglected perforations were counted.Univariate and binary Logistic regression analyses were respectively conducted on the factors that might lead to neglected perforations of gloves. Results Among the 932 surgical gloves used in spinal surgery selected in this study,a total of 122 gloves(13.09%)with neglected perforations were detected,of which the proportions of neglected perforations at the index finger of the left and right hands in the gloves were the highest, accounting for 37.50%and 37.93%respectively.The results of univariate and binary Logistic regression analyses showed that grade Ⅲ or Ⅳ surgery, open surgery, combined bone grafting operation, operation time≥120 minutes, use of rotating equipment, and gloves worn by instrument nurses were risk factors for neglected perforations(P<0.05,OR>1.0),while wearing double-layer gloves was a protective factor to avoid the occurrence of neglected perforations(P<0.05,OR<1.0).Conclusion The incidence of neglected perforations in surgical gloves used during spinal surgery is relatively high, especially in the area of the glove index finger. For high-risk spinal surgeries(grade Ⅲ or Ⅳ surgery, open surgeries, combined bone grafting procedures)and requiring rotating equipment, health care workers, especially instrumental nurses, are advised to wear double-layer gloves;furthermore,a 120-minute glove change regime is recommended,in order to reduce the incidence of neglected perforation.
