1.Effect of deep muscle stimulation combined with electromyographic biofeedback on the spasms of the triceps surae and gait changes after stroke

BACKGROUND:Deep muscle stimulation has the effects of releasing muscle adhesion,relieving muscle spasm,improving and restoring muscle compliance and elasticity.Electromyographic biofeedback therapy can promote nerve recovery and improve lower limb motor function and gait. OBJECTIVE:To observe the effect of the effect of deep muscle stimulation combined with electromyographic biofeedback therapy on the spasm of the triceps surae and gait changes after stroke by using a digital muscle detector and three-dimensional gait analysis system. METHODS:A total of 72 patients who met the inclusion criteria were selected from the Rehabilitation Department of the First Affiliated Hospital of Guangdong Pharmaceutical University from October 2020 to October 2023.And they were enrolled and randomly divided into two groups(n=36 per group):a control group and a combined group.The control group received routine rehabilitation therapies,electromyographic biofeedback and pseudo deep muscle stimulation,while the combined group received true deep muscle stimulation treatment on the basis of the control group,five times per week,for 4 consecutive weeks.The oscillation frequency and dynamic stiffness of the affected gastrocnemius muscle,active range of motion of the ankle dorsiflexion muscle,electromyographic signal of the tibialis anterior muscle,Fugl-Meyer assessment of the lower limbs,and three-dimensional gait analysis parameters were statistically analyzed before and after treatment in two groups. RESULTS AND CONCLUSION:After treatment,oscillation frequency and dynamic stiffness values of the inner and outer sides of the affected gastrocnemius muscle in both groups of patients were significantly reduced compared with before treatment(P<0.05),and the combined group showed a more significant decrease compared with the control group(P<0.05).The active range of motion of the ankle dorsiflexion muscle,electromyographic signal of the tibialis anterior muscle,and Fugl-Meyer scores after treatment were significantly increased or improved compared with before treatment(P<0.05),while the combined group showed a more significant increase or improvement compared with the control group(P<0.05).In terms of gait parameters,the walking speed,frequency,and stride in both groups of patients were significantly increased compared with before treatment(P<0.05),while the combined group showed a more significant increase compared with the control group(P<0.05).The percentage time of support phase on the healthy side was shortened compared with before treatment(P<0.05),while the combined group showed a more significant decrease compared with the control group(P<0.05).In addition,there was no significant difference between the two groups except for the percentage of healthy side support(P>0.05).To conclude,the combination of deep muscle stimulation and electromyographic biofeedback can effectively alleviate triceps spasm in the short term after stroke,improve ankle dorsiflexion function,enhance lower limb motor function,and improve gait.The treatment effect is significant and worthy of clinical promotion and application.
2.Application of artificial intelligence technology in the evaluation of markers for cerebral small vessel disease

Cerebral small vessel disease (CSVD) is a group of cerebrovascular system disease syndromes that involve perforating arteries, capillaries, and small veins. Because of the high incidence of CSVD in the elderly and its great harm, it is particularly important to identify and diagnose the diseases as early as possible. With the development of artificial intelligence technology, intelligent algorithms such as machine learning, deep learning, and computer neural networks are increasingly being applied in the medical field. This article reviews the application of artificial intelligence technology in the evaluation of CSVD imaging markers and blood biomarkers in recent years.
3.Multimodal MRI evaluation of vascular cognitive impairment

Vascular cognitive impairment (VCI) refers to a cognitive impairment syndrome caused by cerebrovascular disease and its risk factors, and encompasses a spectrum from mild cognitive impairment to vascular dementia. It is the second leading cause of dementia after Alzheimer's disease (AD). The clinical manifestations of VCI often overlap with neurological diseases such as AD, and its neuropathological changes are heterogeneous, lacking specificity in clinical diagnosis and treatment, thus delaying the optimal timing of treatment. In recent years, with the continuous development of brain structure-function coupling analysis methods based on multimodal MRI, the correlation between brain structure and function has been comprehensively revealed, which is helpful for exploring the pathological and physiological mechanisms of VCI. This article reviews the research progress on the application of multimodal MRI in VCI in recent years, including the study of brain structure function-coupling based on multimodal MRI.
4.Study on the machanism of Huannao Yicong Deoction targeting HAMP to regulate iron metabolism and improve cognitive impairment in AD model mice

Aim To explore the effects of Huannao Yicong decoction(HYD)on the learning and memory ability and brain iron metabolism in APP/PS1 mice and the correlation of HAMP knockout mice and APP/PS1 double transgenic model mice.Methods The ex-periment was divided into five groups,namely,HAMP-/-group(6-month HAMP gene knockout mice),APP/PS1 group(6-month APP/PS1-double-transgenic mice),HAMP-/-+HYD,APP/PS1+HYD,and negative control group(6-month C57BL/6J mice),with six mice in each group.The dose was ad-ministered(13.68 g·kg-1 weight),and the other groups received distilled water for gavage once a day for two months.After the administration of the drug,the mice in each group were tested for learning and memory in the Morris water maze;Biochemical detec-tion was performed to detect iron ion content in each mouse brain;Western blot and RT-qPCR were carried out to analyze hippocampal transferrin(TF),transfer-rin receptor1(TFR1),membrane iron transporter1(FPN1)divalent metal ion transporter 1(DMT1)and β-amyloid protein(Aβ)protein and mRNA expression levels in each group.Results Compared with the normal group,both HAMP-/-mice and APP/PS1 mice had reduced the learning and memory capacity,in-creased iron content in brain tissue,Aβ protein ex-pression increased in hippocampus of HAMP-/-group and APP/PS1 group mice(P<0.01),the protein and mRNA expression of TF,TFR1 and DMT1 increased in hippocampal tissues of HAMP-/-and APP/PS1 groups(P<0.01),and the FPN1 protein and mRNA expres-sion decreased(P<0.01).Compared with the HAMP-and APP/PS1 groups,respectively,HAMP-/-+HYD group and APP/PS1+HYD group had improved learning and memory ability,decreased iron content,decreased Aβ protein expression(P<0.01),decreased TF,TFR1,DMT1 protein and mR-NA expression(P<0.01),and increased expression of FPN1 protein and mRNA(P<0.01).Conclusions There is some association between HAMP-/-mice and APP/PS1 mice,HYD can improve the learning and memory ability of HAMP-/-and APP/PS1 mice and reduce the Aβ deposition.The mechanism may be related to the regulation of TF,TFR1,DMT1,FPN1 expression and improving brain iron overload.
5.Influences of echinacoside on myocardial cell apoptosis and inflammatory injury in rats with coronary heart disease by regulating mTOR/STAT3 signaling pathway

Objective:To investigate influences of echinacoside(ECH)on myocardial cell apoptosis,inflammatory injury and mammalian target of rapamycin(mTOR)/signal transducer and activator of transcription 3(STAT3)signaling pathway in rats with coronary heart disease(CHD).Methods:A total of 12 SD rats were taken as control group(NC group),48 CHD rat models were successfully constructed,and randomly divided into model(Model)group,ECH group(50 mg/kg),mTOR activator MHY1485 group(10 mg/kg),ECH+MHY1485 group(50 mg/kg ECH+10 mg/kg MHY1485).After 4 weeks,electrocardiogram was recorded through biological function experimental system.Kits were used to detect myocardial C-reactive protein(CRP)and IL-6 levels.Myocardial pathology and apoptosis were measured by HE and TUNEL staining.qRT-PCR was used to detect myocardial Bcl-2,Bax mRNA expressions.Western blot was used to detect myocardial mTOR/STAT3 pathway protein expression.Results:Compared with NC group,Model group rats had obvious necrosis,edema and inflammatory cell infiltration,ST segment deviation,IL-6,CRP levels,cardiomyocyte apoptosis rate,Bax mRNA expression,mTOR and p-STAT3/STAT3 protein levels were significantly increased(P<0.05),Bcl-2 mRNA expression was significantly decreased(P<0.05).Compared with Model group,myocardial injury in ECH group was alleviated,ST segment shift,IL-6,CRP levels,cardiomyocyte apoptosis rate,Bax mRNA expression,mTOR and p-STAT3/STAT3 protein levels were were significantly decreased(P<0.05),Bcl-2 mRNA expression was significantly increased(P<0.05).The above indexes of MHY1485 group rats showed opposite trend.MHY1485 eliminates beneficial effects of ECH on CHD rats.Conclusion:ECH may reduce myocardial inflammatory injury and apoptosis in CHD rats by down-regulating mTOR/STAT3 pathway.
6.Investigation of tick species and potential pathogenic ricks in certain areas of Wuwei City,Gansu Province

To understand the distribution of ticks in the Wuwei Region,enrich tick species data,and provide a basis for the prevention of tick-borne diseases,tick were collected using flagging and tick-picking methods during the highest activity period from April to September in 2021 and 2022 in the mountainous areas of Wuwei City.The ticks were identified based on morpho-logical and molecular biological characteristics,and characteristic sequences were obtained.A systematic evolutionary tree was constructed using the neighbor-joining method in MEGA 11.0 software.In total,7 342 ticks collected in Wuwei,which be-longed to 5 species from 4 genera with in the Ixodidae family,which included Dermacentor nuttalli,Hyalomma asiaticum,Ixodes canisuga,Haemaphysalis longicornis and Haema-physalis danieli.Ticks of the same species clustered together into the same branch of an evolutionary tree.In the Wuwei Re-gion,five common tick species are found across various habi-tats,with each habitat featuring different distributions of tick species and populations.The Dermacentor nuttalli is the dom-inant tick species in this area.
7.Prevalence of Angiostrongylus cantonensis infection in snails for sale in select regions of Yunnan Province from 2019 to 2023

This study was aimed at investigating the prevalence and characteristics of Angiostrongylus cantonensis infection in snails for sale in select regions of Yunnan Province between April and September in 2019-2023.A total of 1 584 snails were collected from 13 regions,and third stage larvae with A.cantonensis infection were identified with the digestion method.The A.cantonensis infection rate in snails was 3.66%.The infection rate of Pomacea canaliculata was highest(6.41%),that of Cipangopaludina cathayensis was 2.74%,and that of Bellamya aeruginosa was 2.56%.The A.cantonensis infection rate in each snail species decreased each year,except in P.canaliculata.The A.cantonensis infection rate was highest in July(7.87%).Snails infected with A.cantonensis were found in regions except Chuxiong.The highest infection rate in these 12 re-gions occurred in Honghe(9.52%).In conclusion,A.cantonensis infection was identified in snails for sale in select areas of Yunnan province,thus posing hidden food safety risks.Strengthened supervision and management of snail food products,in-creased education,environmental improvement,and deratization deratize are necessary.Moreover,molecular biological detec-tion provides a sensitive tool for the prevention and control of A.cantonensis.
8.Sulforaphane alleviates acute liver injury induced by diquat in mice by activating Keap1/Nrf2 signaling pathway

Objective:To investigate the protective effect and possible mechanism of sulforaphane (SFN) on acute liver injury in mice induced by diquat (DQ) poisoning.Methods:Forty-eight male C57BL/6 mice were divided into Control group, DQ model group (DQ group), SFN intervention group (DQ+SFN group), and SFN control group (SFN group) using a random number table method, with 12 mice in each group. Acute liver injury mice model was established by one-time intraperitoneal injection of 1 mL of 40 mg/kg DQ solution at once. SFN group was injected with 1 mL of ddH 2O. After 4 hours of molding, 0.5 mL of 5 mg/kg SFN solution was injected into the peritoneal cavity of the DQ+SFN group and SFN group, once daily for 7 consecutive days. DQ group and Control group were injected with an equal amount of ddH 2O. Then, the mice were euthanized to collect liver tissue and blood samples, and the levels of plasma biomarkers alanine aminotransferase (ALT) and aspartate aminotransferase (AST), as well as oxidative stress indicators such as superoxide dismutase (SOD), glutathione (GSH), and malondialdehyde (MDA) in liver tissue were measured. The changes of liver structure were observed under transmission electron microscopy. The apoptosis and reactive oxygen species (ROS) level in liver tissue were observed under fluorescence microscope. Western blotting was used to detect the protein expressions of nuclear factor E2-related factor 2 (Nrf2), hemeoxygenase-1 (HO-1), Kelch-like ECH-associated protein 1 (Keap1), and cleaved caspase-9 in liver tissue. Results:Compared with the Control group, the liver mitochondria in the DQ group showed severe swelling, partial dissolution of the matrix, and cristae rupture and loss; the levels of plasma AST and ALT significantly increased, the MDA content in the liver increased, the activities of SOD and GSH decreased, the level of ROS significantly increased, the number of apoptotic cells in the liver significantly increased, the protein expressions of Nrf2 and HO-1 significantly decreased, and the protein expressions of Keap1 and cleaved caspase-9 significantly increased. Compared with the DQ group, the mitochondrial damage in the DQ+SFN group was reduced, the levels of plasma AST and ALT were significantly reduced [ALT (U/L): 58.22±4.39 vs. 79.94±3.32, AST (U/L): 177.64±8.40 vs. 219.62±11.60, both P < 0.01], the liver MDA content decreased, and the activities of SOD and GSH increased [MDA (μmol/g: 5.63±0.18 vs. 5.96±0.29, SOD (kU/g): 102.05±4.01 vs. 84.34±5.34, GSH (mmol/g): 16.32±1.40 vs. 13.12±1.84, all P < 0.05], the production of ROS in liver tissue was significantly reduced [ROS (fluorescence intensity): 115.90±10.89 vs. 190.70±10.16, P < 0.05], and apoptotic cells were significantly reduced (cell apoptosis index: 4.39±1.00 vs. 10.71±0.56, P < 0.01), the protein expressions of Nrf2 and HO-1 were significantly increased, while the protein expressions of Keap1 and cleaved caspase-9 were significantly decreased (Nrf2/β-actin: 1.15±0.04 vs. 0.93±0.05, HO-1/β-actin: 1.75±0.12 vs. 0.78±0.04, Keap1/β-actin: 1.00±0.14 vs. 1.28±0.13, cleaved caspase-9/β-actin: 1.31±0.12 vs. 1.81±0.09, all P < 0.05). However, there was no statistically significant difference in various indicators between the SFN group and the Control group. Conclusion:SFN can activate the Keap1/Nrf2 signaling pathway to alleviate DQ induced acute liver injury in mice.
9.Risk factors and predictors of radioactive iodine refractory in differentiated thyroid cancer with distant metastasis

Objective:To investigate the risk factors and prognostic indicators of radioactive iodide refractory (RAIR) in differentiated thyroid cancer (DTC) with distant metastasis (DM).Methods:From January 2007 to November 2023, 140 DM-DTC patients who received 131I therapy in JiangYuan Hospital Affiliated to Jiangsu Institute of Nuclear Medicine were retrospectively conducted. According to the effect of 131I treatment, 84 cases in the RAIR group and 56 cases in the radioactive iodide efficient (RAIE) group were finally included. The general clinical data, B-Raf proto-oncogene, serine/threonine kinase (BRAF) V600E and telomerase reverse transcriptase promoter (TERTp) mutations were compared between the two groups by independent-sample t test, Mann-Whitney U test and χ2 test. Multiple logistic regression was used to analyze the influencing factors of RAIR. Results:There were significant differences between the RAIR group and the RAIE group in age, age≥55 years, tumor maximum diameter, cumulative dose of radioactive iodide, type of metastasis and preoperative thyroglobulin (pre-Tg) ( χ2 values: 7.78 and 9.03, t values: 2.44-2.74, z=-3.92, all P<0.05). The TERTp mutation rate in RAIR group was 39.39%(26/66), which was significantly higher than that in RAIE group (2.17%(1/46); χ2=20.97, P<0.001). The BRAF V600E mutation rate was 41.79%(28/67) in the RAIR group and 40.00%(20/50) in the RAIE group, with no significant difference ( χ2=0.04, P=0.846). Logistic regression analysis found that high pre-Tg level and TERTp mutation were independent risk factors for RAIR occurrence. Conclusion:TERTp mutation and high pre-Tg level are independent risk factors for RAIR occurrence, and they may be potential indicators for predicting RAIR.
10.Professor CHEN Yi's Clinical Experience in the Treatment of Insomnia with the Theory of"Harmony and Unity"

[Objective]To summarize Professor CHEN Yi's clinical experience in treating insomnia based on the theory of"harmony and unity".[Methods]Through learning from Professor CHEN,collation and summary of relevant literature and Professor CHEN's expositions and cases,the understanding of the etiology and pathogenesis of insomnia is elaborated,and the unique treatment ideas and experiences rooted in the theory of harmony and"unification"are discussed,and verified by medical cases.[Results]Professor CHEN believes that the occurrence of insomnia is due to imbalance of viscera Yin and Yang,as well as deficiency of Qi and blood.The attached medical case was a patient in the physiological stage before and after menopause,with the imbalance of Yin and Yang,Yin and Yang Yin deficiency and fire as the main pathogenesis,and the treatment method of nourishing Yin and reducing fire,calming the heart and calming the nerves,further harmonizing Qi and blood was used.[Conclusion]Professor CHEN's theory of"harmony and unity"has a remarkable curative effect,which is worth learning and promoting.
