1.Sex-specific All-cause Mortality is Associated with Adiposity in the Malaysian Elders Longitudinal Research (MELoR) Study

Background:
While adiposity has been found to be protective against adverse outcomes in older adults, the role of muscle in this association remains underexplored. This study sought to evaluate sex-specific mortality associated with available adiposity indices in older adults and the potential role of muscle strength in this relationship.
Methods:
Individuals aged ≥55 years were recruited from 2013 to 2015. Adiposity indices obtained were body mass index (BMI), percentage body fat (%BF), waist-to-hip ratio (WHR), and waist circumference (WC). Vital status up to June 2022 was determined through the National Registry Department.
Results:
Of the 1,347 included participants, mean age of 68.45±7.21 years, 57.1% female, and 11.2% deaths were recorded. Male who were underweight had increased mortality compared to male with normal BMI (hazard ratio [HR]=3.17, 95% confidence interval [CI] 1.35–7.47). Mortality was greater in male with %BF within the highest quartile (Q4) compared to the lowest quartile (Q1) (HR=4.72, 95% CI 2.07–10.78). Increased mortality in both male and female in Q4 for WHR compared to Q1 was influenced by age, as was increased mortality in female in Q4 for %BF. WC did not predict mortality in male or female. Increased mortality risk was present in male with normal muscle strength and increased %BF, and reduced muscle strength in male with low BMI.
Conclusions
Adiposity measured with BMI, WHR and WC had limited value in determining mortality risk at 9-year follow-up among individuals aged ≥55 years. Increased mortality was, however, observed in male with higher %BF but this could not be attributed to muscle strength.

FACTORS ASSOCIATED WITH PAROXYSMAL VERTIGO AMONG URBAN-DWELLERS AGED 55 YEARS AND OVER IN THE MALAYSIAN ELDERS LONGITUDINAL RESEARCH (MELOR) STUDY
Background: Paroxysmal Vertigo (PV) negatively affects life quality and increases in prevalence with age, but the risk factors remain inconclusive.
Objective: The present study aimed to investigate the risk factors of PV among a multi-ethnic urban population in late mid-life and late life within a middle-income Southeast-Asian country.
Methods: Cross-sectional data from the first wave of the Malaysian Elders Longitudinal Research (MELoR) study were utilized for this study involving randomly sampled participants aged 55 years and over. Socio-demographics, comorbidities, and psychological status were obtained through home-based computer-assisted interviews.
Results: Based on an analysis of 1530 participants, mean age (SD) = 68.81 (7.49), 64.9% female; 32.9% Malay, 34.6% Chinese), the lifetime prevalence of PV was 12.1%. The risk factors associated based on multivariate analysis were hypertension, osteoarthritis, depression, and vitamin B12 Deficiency. Chinese ethnicity (OR = 0.582; 95% CI = 0.390 to 0.870) and smoking (OR = 0.377; 95% CI = 0.223 to 0.637) were protective factors against PV.
Conclusions: While the risk factors reported are similar to other studies, the lower prevalence of PV among ethnic Chinese and smokers had not previously been reported. Our study highlighted potential genetic and lifestyle links to PV which should be evaluated in future studies.
3.The effect of chlorhexidine in reducing oral colonisation in geriatric patients: a randomised controlled trial.

INTRODUCTIONInadequate oral care has been implicated in the development of aspiration pneumonia in frail geriatric patients and is a major cause of mortality, due to the colonisation of microbes in vulnerable patients. This type of pneumonia has been associated with an increase in respiratory pathogens in the oral cavity. The aim of this study was to evaluate the effects of chlorhexidine compared to routine oral care in edentulous geriatric inpatients.
METHODSA double-blind, parallel-group randomised controlled trial was carried out. The intervention group received oral care with chlorhexidine 0.2%, while the control group received routine oral care with thymol. Nurses provided oral care with assigned solutions of 20 mL once daily over seven days. Oral cavity assessment using the Brief Oral Health Status Examination form was performed before each oral care procedure. Data on medication received and the subsequent development of aspiration pneumonia was recorded. An oral swab was performed on Day 7 to obtain specimens to test for colonisation.
RESULTSThe final sample consisted of 35 (control) and 43 (intervention) patients. Chlorhexidine was effective in reducing oral colonisation compared to routine oral care with thymol (p < 0.001). The risk of oral bacterial colonisation was nearly three times higher in the thymol group compared to the chlorhexidine group.
CONCLUSIONThe use of chlorhexidine 0.2% significantly reduced oral colonisation and is recommended as an easier and more cost-effective alternative for oral hygiene.

4.Normative data for hand grip strength and key pinch strength, stratified by age and gender for a multiethnic Asian population.

INTRODUCTIONHand strength is a good indicator of physical fitness and frailty among the elderly. However, there are no published hand strength references for Malaysians aged > 65 years. This study aimed to establish normative data for hand grip strength (HGS) and key pinch strength (KPS) for Malaysians aged ≥ 60 years, and explore the relationship between hand strength and physical ability.
METHODSHealthy participants aged ≥ 60 years with no neurological conditions were recruited from rural and urban locations in Malaysia. HGS and KPS were measured using hand grip and key pinch dynamometers. Basic demographic data, anthropometric measures, modified Barthel Index scores and results of the Functional Reach Test (FRT), Timed Up and Go (TUG) test and Jebsen-Taylor Hand Function Test (JTHFT) were recorded.
RESULTS362 subjects aged 60-93 years were recruited. The men were significantly stronger than the women in both HGS and KPS (p < 0.001). The hand strength of the study cohort was lower than that of elderly Western populations. Significant correlations were observed between hand strength, and residential area (p < 0.001), FRT (r = 0.236, p = 0.028), TUG (r = -0.227, p = 0.009) and JTHFT (r = -0.927, p < 0.001).
CONCLUSIONThis study established reference ranges for the HGS and KPS of rural and urban elderly Malaysian subpopulations. These will aid the use of hand strength as a screening tool for frailty among elderly persons in Malaysia. Future studies are required to determine the modifiable factors for poor hand strength.
