1.Thyroid Hormone Network Regulation in MASLD: Mechanisms and Targeted Therapies

Metabolic dysfunction-associated steatotic liver disease (MASLD) has become the most prevalent chronic liver disease worldwide, affecting approximately 32%-38% of the adult population and posing a growing public health burden. MASLD represents a continuous disease spectrum ranging from simple steatosis to metabolic dysfunction-associated steatohepatitis (MASH), progressive hepatic fibrosis, cirrhosis, and ultimately hepatocellular carcinoma (HCC). The pathological core of MASLD lies in disruption of hepatic lipid metabolic homeostasis, characterized by an imbalance among de novo lipogenesis, fatty acid β-oxidation, and very-low-density lipoprotein (VLDL)-mediated lipid export. This metabolic disequilibrium subsequently drives inflammatory injury and fibrotic progression. Among the multiple regulatory pathways involved, thyroid hormone (TH) signaling has emerged as a central regulator of hepatic metabolic homeostasis. The liver is a major peripheral target organ of TH action, where TH predominantly exerts its metabolic effects through thyroid hormone receptor β (TRβ). Large-scale epidemiological studies and meta-analyses have demonstrated that hypothyroidism is significantly associated with increased MASLD prevalence, more severe histological injury, and advanced hepatic fibrosis, suggesting that dysregulation of TH signaling may participate throughout the entire MASLD disease spectrum. At the molecular level, TH regulates hepatic lipid metabolism by coordinating suppression of lipogenesis, enhancement of mitochondrial fatty acid oxidation, and promotion of VLDL assembly and secretion through integrated genomic actions of the T3-TRβ axis and non-genomic signaling pathways. Across different stages of MASLD, TH signaling exerts stage-dependent protective effects. In the steatosis stage, TH improves metabolic flexibility by modulating insulin sensitivity, glucose metabolism, and lipid droplet clearance, thereby alleviating early lipotoxic stress. During progression to MASH, TH attenuates inflammatory amplification by improving mitochondrial homeostasis, suppressing activation of the NOD-like receptor family pyrin domain containing 3 (NLRP3) inflammasome, and modulating the gut-liver axis microenvironment. In advanced stages, TH signaling influences hepatic stellate cell activation and extracellular matrix deposition, partly through interaction with the transforming growth factor-β (TGF-β)/SMAD pathway, while alterations in intrahepatic TH availability, mediated by dynamic changes in iodothyronine deiodinase 1 (DIO1), contribute to fibrosis progression and hepatocellular dedifferentiation. In hepatocellular carcinoma, coordinated downregulation of TRβ and DIO1 establishes a tumor-associated hypothyroid state that promotes metabolic reprogramming and tumor progression. The clinical relevance of TH signaling in MASLD has been underscored by the recent approval of Resmetirom, a liver-targeted TRβ‑selective agonist, for the treatment of non-cirrhotic MASH with moderate-to-severe fibrosis (F2-F3). This approval represents a landmark transition from mechanistic understanding to metabolism-centered precision therapy in MASLD. Clinical trials have demonstrated that Resmetirom not only improves key histological endpoints, including MASH resolution and fibrosis regression, but also favorably modulates atherogenic lipid profiles, highlighting the therapeutic potential of selectively targeting hepatic TH pathways. This review systematically summarizes the multidimensional regulatory roles of TH across the MASLD disease spectrum and discusses emerging diagnostic and therapeutic implications of TH-based interventions, aiming to inform future mechanistic research and optimize clinical management strategies.
2.Association between modelling cycle and antioxidant pathways in rat models of non-alcoholic fatty liver disease

Objective:To construct a rat model of non-alcoholic fatty liver disease(NAFLD)by choline-deficient high fat diet(CDHFD),and to observe the association between feeding cycle and antioxidant pathway.Methods:The study lasted 16 weeks and was divided into 4 cycles.Detection of pathological changes and expression of antioxidant enzymes in rats liver in different cycles.Results:The early stage of liver steatosis and inflammation in model rats was 2~4 weeks,non-alcoholic steatohepatitis(NASH)stage was 4~8 weeks,and liver fibrosis progression was 8~16 weeks.Mechanistic studies had shown that the expressions of antioxidant enzymes Nrf2,SOD and GSH-Px in the liver of NAFLD rats gradually decreased with the extension of the feeding cycle.Conclusion:Different modeling cycles can successfully induce the pathological changes of steatosis,inflammation and liver fibrosis in rat liver,and the pathological changes are time-dependent with the expressions of antioxidant enzymes.
3.Study on the Therapeutic Effect of Banxia Baizhu Tianma Decoction on Phlegm-dampness-blocked Cervical Vertigo and Its Influence on Neck Hemodynamics

Objective:To explore the therapeutic effect of Banxia Baizhu Tianma Decoction on phlegm-dampness-blocking cervical vertigo and its influence on neck hemodynamics.Methods:96 patients with phlegm-dampness block type cervical vertigo admitted to our hospital from January 2022 to January 2024 were divided into control group and observation group,48 cases in each group.The control group was treated with conventional Western medicine,and the observation group was treated with Banxia Baizhu Tianma Decoction.Both groups were treated for 4 weeks and followed up for 6 months.The reduction time and disappearance time of vertigo were recorded.The score of cervical vertigo Symptom and Function Assessment Scale(ESCV)was performed before treatment and 1 day after treatment.The average blood flow level of basilar artery,right vertebral artery and left vertebral artery was detected,and the total clinical effective rate,recurrence rate and adverse reaction rate of the two groups were compared.Results:The reduction time and disappearance time of vertigo in the observation group were shorter than those in the control group(P<0.05).Post-treatment,the ESCV score of both groups was higher than that before treatment;compared with the control group,the ESCV score of the observation group was higher post-treatment(P<0.05).The total clinical effective rate of observation group was higher than that of control group,and the recurrence rate was lower than that of control group(P<0.05).The hemodynamic index were higher after treatment than in the control group(P<0.05).There was no difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:Banxia Baizhu Tianma Decoction has certain curative effect on phlegm-dampness block type cervical vertigo,can effectively relieve vertigo symptoms,reduce recurrence,improve neck hemodynamics,and is safe,worthy of popularization and application.

Central nervous system(CNS)degenerative disorders refer to a spectrum of pathological alterations triggered by struc-tural damage to cerebral neural tissues,clinically manifested as diverse neurological dysfunction syndromes,including multiple sclerosis(MS),neurodegenerative diseases(NDs),and ische-mic stroke.The hallmark pathological features of these disorders involve irreversible neuronal damage and decompensation of functional neural networks,ultimately leading to progressive neurological deficits.Notably,with the accelerating global popu-lation aging,the incidence of these diseases has surged signifi-cantly.According to WHO statistics,they now rank among the top three global causes of disability and mortality.Current re-search has confirmed that the pathogenesis of CNS degenerative disorders exhibits high heterogeneity,encompassing multifaceted pathophysiological processes such as genetic predisposition,oxi-dative stress,protein misfolding,and metabolic dysregulation.This intricate pathogenic network not only complicates clinical differential diagnosis but also poses substantial challenges to the development of precision therapeutic strategies.Importantly,re-cent studies have revealed that mitochondrial homeostasis disrup-tion-induced inflammatory cascades(termed mitochondrial in-flammation)play a pivotal regulatory role in neurodegenerative progression.Key molecular mechanisms include impaired mito-phagy,aberrant mitochondrial DNA(mtDNA)release and NL-RP3 inflammasome activation.This review systematically deci-phers the molecular regulatory network of mitochondrial inflam-mation,with a focus on its biological effects in critical pathologi-cal events such as blood-brain barrier disruption,microglial hy-peractivation and neuronal apoptosis.The overarching aim is to provide a theoretical foundation for developing innovative thera-peutic strategies targeting mitochondrial homeostasis restoration.
5.Exploration of the integrated general and specialist comprehensive management model for concomitant diseases of breast cancer: 2 cases analysis and literature review

With the extended survival period of breast cancer patients and the increasing health demands, the concomitant diseases of breast cancer have gradually attracted the attention of both doctors and patients, and it is imperative to conduct comprehensive management of these diseases, in which the general practitioners, as the more comprehensive and complex medical talents, have not yet played their due roles. In this article, we report two cases of comprehensive management of concomitant diseases of breast cancer through collaboration of general practitioners and specialists (integrated general and specialist care). The role and function of general practitioners in this process were deeply analyzed, and the establishment of a consultation-liaison general practice model to further promote the role of integrated general and specialist care in integrated oncology care was advocated.
6.Experience of Lyu Zhiping in the Treatment of Chronic Kidney Disease with Harmonizing Therapy

Chronic kidney disease(CKD)will eventually progress to end-stage kidney disease and has become one of the common obstinate diseases in China,which becomes a serious threat to human life.Traditional Chinese medicine(TCM)shows unique advantages in treating CKD.In the view of Professor Lyu Zhiping,disharmony,in particular the disharmony of yin and yang of kidney,contributes to the core pathogenesis of CKD.Disharmony between kidney and other organs,and disharmony between kidney and exogenous pathogens together with pathological products lead to the complication of yin and yang,superficial and interior,deficiency and excess syndromes in the progression of the disease.Professor Lyu recommended that the clinical medication for CKD should be based on harmonizing therapy by harmonizing shaoyang meridian,harmonizing the spleen and stomach,and harmonizing collaterals and resolving blood stasis,thus to protect kidney function and slow down the progression of CKD.Based on years of clinical experience,professor Lyu formulated the Shen Qi Shenkang Prescription,which aims at primarily tonifying the kidney,and assisting by harmonizing the lung and spleen,clearing heat,removing dampness and eliminating turbid,thus to treat symptoms and root cause simultaneously,and then stronger clinical efficacy for CKD can be achieved.
7.Data Mining in Medication Rules of Chinese Herbal Concentrated-Granule for the Treatment of Hypertension

Objective To analyze the medication rules of Chinese herbal concentrated-granule for the treatment of hypertension nationwide using data mining and knowledge graph methods,thus to provide evidence for expanding its application in treating hypertension.Methods From January 2022 to March 2023,Chinese herbal concentrated-granule prescriptions for hypertension prescribed by traditional Chinese medicine experts nationwide were collected.Microsoft Excel was used to analyze the frequency,properties and flavors,meridian tropism,and efficacy categories of the medicinals.Hierarchical clustering was applied for cluster analysis,and the Neo4j graph database was utilized to construct an information knowledge graph illustrating the relationships between regions and medicinals.Results A total of 2 600 Chinese herbal concentrated-granule prescriptions were included,involving 370 medicinals.The top six frequently-used medicinals were Poria(Fuling),Glycyrrhizae Radix et Rhizoma(Gancao),Atractylodis Macrocephalae Rhizoma(Baizhu),Citri Reticulatae Pericarpium(Chenpi),Paeoniae Radix Alba(Baishao),and Angelicae Sinensis Radix(Danggui).The properties of the medicinals were mainly warm and mild,and their flavors were usually sweet,pungent,and bitter.The medicinals frequently have the meridian tropism of the spleen,lung,and liver meridians.Hierarchical clustering yielded seven clusters.The information knowledge graph of region-medicinals relationships revealed that medicinals used in five or more regions were Baishao,Baizhu,Chenpi,Pinelliae Rhizoma(Banxia),Fuling,Gastrodiae Rhizoma(Tianma),Bupleuri Radix(Chaihu),Chuanxiong Rhizoma(Chuanxiong),Salviae Miltiorrhizae Radix et Rhizoma(Danshen),Danggui,Gancao,and Astragali Radix(Huangqi).Conclusion Chinese herbal concentrated-granule for hypertension usually consist of tonifying medicinals,with sweet and warm properties,and having the meridian tropism of the spleen meridian.And the medicinals composed of the prescriptions often have the actions of calming the liver and suppressing yang,strengthening the spleen and removing dampness,and nourishing the liver and kidney.
8.Analysis of the Characteristics of Rare Disease Outpatient Visits in a Children′s Hospital: A Single-Center Retrospective Study

To analyze the characteristics of outpatient rare disease admissions at Xi′an Children′s Hospital based on the two batches of the China′s Rare Disease Catalogs. Based on the hospital information system, the outpatient visits of children with diseases included in the two batches of the rare disease catalogs from January 2018 to December 2023 at our center were retrieved and analyzed. A total of 93 602 outpatient visits of children with rare diseases were recorded, accounting for 0.71% of the total outpatient admissions during the same period. Among the diseases included in the two batches of the rare disease catalogs, 129 disease types were admitted, with a disease coverage rate of 62.32% (129/207). Specifically, 83 disease types (83/121, 68.60%) from the first batch of the catalog were admitted, with a total of 18 454 outpatient visits, showing an increasing trend year by year; 46 disease types (46/86, 53.49%) from the second batch of the catalog were admitted, with a total of 75 148 outpatient visits. The department of neurology admitted the most disease types, with a total of 54 types (54/207, 26.09%). Outpatient visits for single disease types exhibited multi-specialty characteristics, with 51.21% of the cases involving two or more specialties. Our center has admitted a relatively large number of children with rare diseases, covering most of the disease types that onset during childhood in the two batches of catalogs. There are obvious multi-specialty characteristics in outpatient admissions. The study results can provide data support for further optimizing the outpatient admission process for children with rare diseases and reasonably setting up multidisciplinary outpatient services.
9.Clinical characteristics of co-infection of Talaromyces marneffei and non-tuberculous Mycobacterium in HIV-negative patients

Objective To analyze the clinical characteristics of co-infection of Talaromyces marneffei(TM)and non-tuberculous Mycobacterium(NTM)in human immunodeficiency virus(HIV)-negative patients.Methods Clinical data of 8 HIV-negative patients with co-infection of TM and NTM in a hospital from 2019 to 2022 were co-llected.Clinical manifestations,auxiliary examination,treatment and prognosis were retrospectively analyzed.Results Among the 8 patients,5 were females and 3 were males,with an average age of(52.25±12.31)years old.All patients presented TM and NTM disseminated infection.The major involved organs were lung(100%),lymph nodes(87.5%),and skin(75.0%).Clinical symptoms mainly included cough and expectoration(87.5%),fatigue(62.5%),joint and lumbosacral pains(62.5%),fever(50.0%),as well as skin and soft tissue abscess(50.0%),etc.Anti-interferon-γ(INF-γ)autoantibodies were detected in 4 patients and the results were positive.All 8 patients(100%)had pulmonary lesions,with chest CT mainly showing spots,patches,and striped shadows in both lungs.Among them,7 cases(87.5%)had increased and enlarged mediastinal lymph nodes,4 cases(50.0%)had pleural thickening and pleural effusion,2 cases each(25.0%for each)were accompanied by pulmonary mass shadows,bronchial stenosis,as well as increased and enlarged hilar lymph nodes.One case each(12.5%for each)had pulmonary cavity formation,bronchiectasis,and pericardial effusion.Conclusion The co-infection of TM and NTM in non-HIV patients presents disseminated infection,with multiple clinical symptoms.Chest imaging shows a wide variety of pulmonary lesions.It is prone to miss diagnosis in clinic,and the effect is not ideal after treatment for single pathogen infection.
