1.Evaluation of computed tomographic anatomy of the foot in Jeju horses

Objective:
The objective of this study was to establish reference ranges related to the feet of normal Jeju horses by measuring the angles and length of the hoof, and the Hounsfield Unit (HU) value of soft tissues in the foot.
Methods:
Sixteen normal Jeju horses (mean age: 4.25 years; mean body weight: 282.06 kg) without lameness were examined using CT. After CT scanning, the angles and lengths of the hoof, such as hoof width and dorsal hoof wall angle, and the HU values of the soft tissues, such as the deep digital flexor tendon, were measured.
Results:
We identified significant differences in the HU values of overall soft tissues between the forelimbs and hindlimbs, and between the medial and lateral parts (p < 0.05). Length measurements showed no differences between left and right limbs or between medial and lateral parts, but significant fore–hind differences were found in most parameters (p < 0.05).Angle measurements revealed significant differences between left and right limbs and between fore- and hindlimbs, with an additional medial–lateral difference in the left hindlimb (p < 0.05).
Conclusions
and Relevance: The CT-based reference values established in this study provide valuable breed-specific data that may support assessment of hoof conformation and interpretation of CT findings in Jeju horses.

Purpose:
In Korea, the Act on Hospice and Palliative Care and Decisions on Life-Sustaining Treatment (LST) was implemented on February 4, 2018. We aimed to investigate relevant factors and clinical changes associated with LST decisions after law enforcement.
Materials and Methods:
This single-center retrospective study included patients who completed LST documents using legal forms at Asan Medical Center from February 5, 2018, to June 30, 2020.
Results:
5,896 patients completed LST documents, of which 2,704 (45.8%) signed the documents in person, while family members of 3,192 (54%) wrote the documents on behalf of the patients. Comparing first year and following year of implementation of the act, the self-documentation rate increased (43.9% to 47.2%, p=0.014). Moreover, the number of LST decisions made during or after intensive care unit admission decreased (37.8% vs. 35.2%, p=0.045), and the completion rate of LST documents during chemotherapy increased (6.6% vs. 8.9%, p=0.001). In multivariate analysis, age < 65 (odds ratio [OR], 1.724; 95% confidence interval [CI], 1.538 to 1.933; p < 0.001), unmarried status (OR, 1.309; 95% CI, 1.097 to 1.561; p=0.003), palliative care consultation (OR, 1.538; 95% CI, 1.340 to 1.765; p < 0.001), malignancy (OR, 1.864; 95% CI, 1.628 to 2.133; p < 0.001), and changes in timing on the first year versus following year (OR, 1.124; 95% CI, 1.003 to 1.260; p=0.045) were related to a higher self-documentation rate.
Conclusion
Age < 65 years, unmarried status, malignancy, and referral to a palliative care team were associated with patients making LST decisions themselves. Furthermore, the subject and timing of LST decisions have changed with the LST act.

5.Miniature Pony with 64,XY SRY-Positive Disorder of Sexual Development: Karyotyping and Genetic Marker Test

A 6-year-old miniature pony mare was examined because of the abnormal appearance of the perineal region. A physical examination revealed an enlarged and penis-like clitoris in the vulva. The owner reported that the horse had frequently exhibited aggressive behavior. Blood and hair root samples were collected for the karyotyping and genetic marker tests. Cytogenetic and molecular analysis showed that the pony had a 64,XY SRY-positive disorder of sexual development (DSD). The owner opted for no surgical removal of the internal genitalia. This report describes the karyotyping and genetic marker analysis of 64,XY SRY-positive DSD in a miniature pony in South Korea.

Purpose:
The female sex is reported to have a higher risk of adverse events (AEs) from cytotoxic chemotherapy. Few studies examined the sex differences in AEs and their impact on the use of medical services during adjuvant chemotherapy. This sub-study aimed to compare the incidence of any grade and grade ≥ 3 AEs, healthcare utilization, chemotherapy completion rate, and dose intensity according to sex.
Materials and Methods:
This is a sub-study of a multicenter cohort conducted in Korea that evaluated the impact of healthcare reimbursement on AE evaluation in patients who received adjuvant chemotherapy between September 2013 and December 2016 at four hospitals in Korea.
Results:
A total of 1,170 patients with colorectal, gastric, or non–small cell lung cancer were included in the study. Female patients were younger, had fewer comorbidities, and experienced less postoperative weight loss of > 10%. Females had significantly higher rates of any grade AEs including nausea, abdominal pain, stomatitis, vomiting, and neutropenia, and experienced more grade ≥ 3 neutropenia, nausea, and vomiting. The dose intensity of chemotherapy was significantly lower in females, and they also experienced more frequent dose reduction after the first cycle. Moreover, female patients receiving platinum-containing regimens had significantly higher rates of unscheduled outpatient visits.
Conclusion
Our study found that females experienced a higher incidence of multiple any-grade AEs and severe neutropenia, nausea, and vomiting, across various cancer types, leading to more frequent dose reductions. Physicians should be aware of sex differences in AEs for chemotherapy decisions.
7.Healthcare Workers and Patient Loss During the COVID-19 Pandemic: Interaction Between Grief, Viral Anxiety, and Depression

Objective:
Coronavirus disease-2019 (COVID-19) had a significant impact on the mental health of healthcare workers. Related assessments should be included in plans for future pandemics. We investigated the connections between grief, viral anxiety, depression, and preoccupation in the context of COVID-19 in healthcare workers, to determine which factors will need to be incorporated.
Methods:
A total of 267 healthcare workers who had experienced the death of at least one patient during the COVID-19 pandemic were asked to respond to questionnaires assessing grief, viral anxiety, depression, loneliness, and preoccupation with COVID-19, based on their emotional state during the 2 weeks immediately after the death. Multivariate linear regression, causal mediation analysis and structural equation modeling were used to examine the psychological processes underlying grief.
Results:
Linear regression showed that viral anxiety (β=0.287, 95% confidence interval [CI]: 0.235–0.338, p<0.001) and depression (β= 0.157, 95% CI: 0.073–0.241, p<0.001) had independent associations with preoccupation with COVID-19. Causal mediation analysis revealed that both viral anxiety (proportion mediated: 0.51, 95% CI: 0.37–0.66, p<0.001) and depression (proportion mediated: 0.77, 95% CI: 0.50–1.08, p<0.001) were mediators between grief and preoccupation with COVID-19. In a structural equation model, viral anxiety and depression mediated most of the effects of grief on preoccupation with COVID-19, and loneliness was a mediator between grief and depression.
Conclusion
Contingency plans for the next pandemic are being formulated. Effective measures to protect the mental health of healthcare workers should be included, and such measures should consist of assessments for grief, viral anxiety, depression, and loneliness.
8.Large Mesenteric Pedunculated Lipoma in a Horse

A seven-month pregnant 15-year-old Thoroughbred mare presented with acute abdominal pain to Jeju National University Equine Hospital. At presentation, a nasogastric intubation revealed 10 L of gastric reflux; rectal palpation and ultrasound revealed dilated loops and thickening of the walls of the small intestine. An exploratory laparotomy revealed strangulation of the small intestine due to a large abdominal mass. The mass was double-ligated and resected blindly due to the short pedicle. An enterectomy was not performed as intestinal motility was detected following the mass removal. Histopathological examination confirmed that the mass was a lipoma, measuring 24 cm × 16 cm × 16 cm in size. On day 8 post-surgery, the mare was discharged without complications. This case report describes the diagnosis and treatment of strangulation of the small intestine by a pedunculated lipoma, thus providing useful information on lipoma in horses.
9.Diagnosis of Coxofemoral Joint Luxation in a Whooper Swan (Cygnus Cygnus) Using Computed Tomography and Radiography

A wild Whooper swan (Cygnus Cygnus) with limping due to an injured left pelvic limb in an accident was rescued on the seashore and transferred to the Jeju Wildlife Rescue Center on November 23rd, 2020. On physical examination, its body condition score was 1 out of 5 due to starvation and dehydration.The left coxofemoral joint was also examined by careful palpating and estimating the damage. Moderated soft tissue swelling and crepitus surrounding the hip joint were confirmed. Radiography and computed tomography (CT) were used together for an accurate diagnosis of the joint. By radiographs readings, it was difficult to accurately confirm the condition of the proximal femur due to superimposition of the synsacrum and internal organs. However, signs such as avulsion fracture of the femoral head and a few fragments around the joint were revealed by CT imaging. Besides, through three-dimensional (3D) image analysis of CT, the dislocated area and condition of the left hip joint could be accurately and easily confirmed. The diagnostic process showing in this paper could be used as a good reference for diagnosing coxofemoral joint luxation in wild swan.
10.Transcondylar Screw Fixation in a Horse with a Subchondral Cystic Lesion in the Medial Femoral Condyle in South Korea

A 2-year-old male Thoroughbred presented with the right hindlimb lameness for the past 3 months. The horse showed swelling around the right stifle. Radiography revealed radiolucency in the medial condyle of the right femur, indicative of subchondral cystic lesion. The horse underwent surgery under general anesthesia. Acortical screw was located on serial radiographs. After placing the screw in the medial condyle, arthroscopy was performed to identify lesions in the femorotibial joint; however, no significant findings were observed. The horse recovered smoothly from anesthesia with assistance and was discharged 1 day later.The owner reported the horse did not show lameness; it returned to training and started racing. This report describes transcondylar screw fixation in a horse with a subchondral bone cyst in South Korea, and suggests that bone screw fixation is an effective treatment for subchondral cystic lesions of the femoral condyle.
