Purpose: To investigate the recent prescription patterns among South Korean pediatric ophthalmologists to control myopia progression. Methods: An online survey was distributed to the members of the Korean Association of Pediatric Ophthalmology and Strabismus (KAPOS) between August 22 and 31, 2023. Respondents selected treatment modalities that they prescribed to slow myopia progression. Multiple responses were allowed, and prescribing patterns were analyzed by respondents’ characteristics, including years since board certification, practice type, practice location, and average number of pediatric myopia patients seen per week. Results: Of 167 active members, 119 (71.3%) completed the survey, and 118 provided full responses. Most respondents practiced in Seoul or metropolitan areas (75.4%), worked in hospitals (73.7%), had >10 years of clinical experience (68.9%), and treated >20 pediatric myopia patients weekly (62.2%). With multiple responses allowed, 0.125% atropine eye drops (75.6%) were most commonly prescribed, followed by orthokeratology lenses (57.1%). When grouped into mutually exclusive categories (topical atropine only, optical interventions only, both and neither), the largest proportion of respondents (63.0%) selected both pharmacological and optical treatments. Treatment patterns did not differ by experience or location, but significant differences were observed by practice type and weekly pediatric myopia patient volume. Private practitioners more often prescribed orthokeratology, dual-focus soft contact lenses, and selected both pharmacologic and optical treatments, whereas hospital practitioners more commonly selected topical atropine alone. Those treating a higher volume of pediatric myopia patients showed a greater tendency to select both pharmacologic and optical treatments. Conclusions In this survey of KAPOS members, topical atropine, particularly the 0.125% commercial formulation, was the most commonly prescribed single modality for myopia control. Topical atropine and optical interventions emerged as the most common treatment strategies, particularly in private clinics. These findings indicate a trend towards more proactive and multimodal approaches to myopia control compared with earlier practice patterns in South Korea.

Human chemically derived hepatic progenitors (hCdHs) reprogrammed using three chemicals—HGF, A83-01, and CHIR99021 (collectively denoted as “HAC”)—have been suggested as a novel therapeutic for patients with severe liver diseases in our previous study. Despite its high proliferation and re-differentiation ability into functional hepatocytes, the reprogramming mechanism of hCdHs remained unknown. Recently, it has been reported that autophagy, a self-degradation process, is responsible for stem cell metabolism. In this study, we investigated whether autophagy regulates the generation mechanism of CdHs, mainly using hepatocytes from C57BL/6 mice, with additional analysis using human hepatocytes. As a result, we found that autophagy flux is inhibited during the generation of mouse CdHs (mCdHs) by A83-01, which is compensated by CHIR99021. Moreover, the suppression of autophagy by bafilomycin A1 enhanced the proliferation ability of mCdHs during the generation process. hCdHs also showed a similar autophagy inhibition pattern to mCdHs during the generation process. Taken together, our study indicates that autophagy is downregulated during the generation of CdHs, promoting their proliferation. This may contribute to the production of hCdHs with stable productivity, which may serve as a therapeutic for severe liver diseases.

BACKGROUND/OBJECTIVES: Cultural and environmental factors may affect dietary habits and intake, regardless of age. As recent assessments of dietary habits of adults 95 yrs and older are absent, we aimed to determine dietary habits, diet quality, and intake of adults 95 yrs and older and test if they vary by region. SUBJECTS/METHODS: Adults 95 yrs and older residing in rural areas (Gurye-gun, Goksung-gun, and Sunchang-gun [GuGokSun]; n = 46), a near-city area (Hwasun-gun, and Damyang-gun [HwaDam]; n = 77), and a metropolitan city (Gwangju Metropolitan City [Gwangju]; n = 32) were surveyed. Dietary habits and quality were surveyed using the Nutrition Quotient for the Elderly (NQ-E). Participants (n = 20) recorded videos of their meals, which were subsequently analyzed for food and nutrient intake and compared with intakes of participants of the Korea National Health and Nutrition Examination Survey (KNHANES) aged ≥ 80 yrs (n = 1,769), which were assessed via 24-h recall. RESULTS: Most participants (85.2%) consumed similar amounts of food at meals; however, only 65.1% ingested meals at regular times. The mean NQ-E score was 48.0 ± 11.9 and did not differ among regions. In Gwangju participants, subjective income was positively associated with diet quality. The mean energy and grain intakes per meal were lower, whereas the percent energy intake from protein and intakes of seaweed, meat and poultry, and iron were higher in GuGokSun and HwaDam participants with meal recordings than in KNHANES participants. CONCLUSION Among Korean adults aged ≥ 95 yrs, few regional variations exist in dietary quality and habits, although associations with diet quality vary within regions. Adults 95 yrs and older exhibit lower energy intake but higher intakes of seaweed, meat and poultry, and iron than adults aged ≥ 80 yrs. Notwithstanding, further longitudinal studies on centenarians are warranted.

Purpose: This study assessed the impact of hepatic fibrosis on the diagnostic performance of the controlled attenuation parameter (CAP) in quantifying hepatic steatosis in patients with chronic hepatitis B (CHB). Methods: CHB patients who underwent liver stiffness measurement (LSM) and CAP assessment using transient elastography before liver resection between 2019 and 2022 were retrospectively evaluated. Clinical data included body mass index (BMI) and laboratory parameters. The histologically determined hepatic fat fraction (HFF) and fibrosis stages were reviewed by pathologists blinded to clinical and radiologic data. The Pearson correlation coefficient between CAP and HFF was calculated. The diagnostic performance of CAP for significant hepatic steatosis (HFF ≥10%) was assessed using areas under the receiver operating curve (AUCs), stratified by fibrosis stages (F0-1 vs. F2-4). Factors significantly associated with CAP were determined by univariable and multivariable linear regression analyses. Results: Among 399 CHB patients (median age 59 years; 306 men), 16.3% showed significant steatosis. HFF ranged from 0% to 60%. Of these patients, 9.8%, 19.8%, 29.3%, and 41.1% had fibrosis stages F0-1, F2, F3, and F4, respectively. CAP positively correlated with HFF (r=0.445, P<0.001). The AUC of CAP for diagnosing significant steatosis was 0.786 (95% confidence interval [CI], 0.726 to 0.845) overall, and significantly lower in F2-4 (0.772; 95% CI, 0.708 to 0.836) than in F0-1 (0.924; 95% CI, 0.835 to 1.000) (P=0.006). Multivariable analysis showed that BMI (P<0.001) and HFF (P<0.001) significantly affected CAP, whereas LSM and fibrosis stages did not. Conclusion CAP evaluations of significant hepatic steatosis are less reliable in CHB patients with significant or more advanced (F2-4) than with no or mild (F0-1) fibrosis.

Acute Respiratory Distress Syndrome (ARDS) is a severe condition characterized by extensive lung inflammation and increased alveolar-capillary permeability, often triggered by infections or systemic inflammatory responses. Mesenchymal stem cells (MSCs)-based therapy holds promise for treating ARDS, as MSCs manifest immunomodulatory and regenerative properties that mitigate inflammation and enhance tissue repair. Primed MSCs, modified to augment specific functionalities, demonstrate superior therapeutic efficacy in targeted therapies compared to naive MSCs. This study explored the immunomodulatory potential of MSCs using mixed lymphocyte reaction (MLR) assays and co-culture experiments with M1/M2 macrophages. Additionally, RNA sequencing was employed to identify alterations in immune and inflammation-related factors in primed MSCs. The therapeutic effects of primed MSCs were assessed in an LPS-induced ARDS mouse model, and the underlying mechanisms were investigated through spatial transcriptomics analysis. The study revealed that MSCs primed with IFN-γ and IL-1β significantly enhanced the suppression of T cell activity compared to naive MSCs, concurrently inhibiting TNF-α while increasing IL-10 production in macrophages. Notably, combined treatment with these two cytokines resulted in a significant upregulation of immune and inflammation-regulating factors. Furthermore, our analyses elucidated the mechanisms behind the therapeutic effects of primed MSCs, including the inhibition of inflammatory cell infiltration in lung tissue, modulation of immune and inflammatory responses, and enhancement of elastin fiber formation. Signaling pathway analysis confirmed that efficacy could be enhanced by modulating NFκB and TNF-α signaling. In conclusion, in early-phase ARDS, primed MSCs displayed enhanced homing capabilities, improved lung function, and reduced inflammation.

Background: and Purpose: This clinical practice guideline provides evidence-based recommendations for treatment of dementia, focusing on cholinesterase inhibitors and N-methyl-D-aspartate (NMDA) receptor antagonists for Alzheimer’s disease (AD) and other types of dementia. Methods: Using the Population, Intervention, Comparison, Outcomes (PICO) framework, we developed key clinical questions and conducted systematic literature reviews. A multidisciplinary panel of experts, organized by the Korean Dementia Association, evaluated randomized controlled trials and observational studies. Recommendations were graded for evidence quality and strength using Grading of Recommendations, Assessment, Development, and Evaluation (GRADE) methodology. Results: Three main recommendations are presented: (1) For AD, cholinesterase inhibitors (donepezil, rivastigmine, galantamine) are strongly recommended for improving cognition and daily function based on moderate evidence; (2) Cholinesterase inhibitors are conditionally recommended for vascular dementia and Parkinson’s disease dementia, with a strong recommendation for Lewy body dementia; (3) For moderate to severe AD, NMDA receptor antagonist (memantine) is strongly recommended, demonstrating significant cognitive and functional improvements. Both drug classes showed favorable safety profiles with manageable side effects. Conclusions This guideline offers standardized, evidence-based pharmacologic recommendations for dementia management, with specific guidance on cholinesterase inhibitors and NMDA receptor antagonists. It aims to support clinical decision-making and improve patient outcomes in dementia care. Further updates will address emerging treatments, including amyloid-targeting therapies, to reflect advances in dementia management.
