1.Incidence of active tuberculosis in Korean patients with rheumatoid arthritis: a comparison between tumor necrosis factor inhibitors and tofacitinib

Objective:
This study aims to compare the incidence of active tuberculosis (TB) among Korean patients with rheumatoid arthritis (RA) initiating treatment with tumor necrosis factor inhibitors (TNFi) or tofacitinib.
Methods:
Using the Korean National Health Insurance database, we conducted a nationwide, retrospective cohort study of RA patients who started TNFi or tofacitinib therapy between 2015 and 2018. We calculated the incidence rates of active TB based on the treatment type and results from latent tuberculosis infection (LTBI) screening tests. A multivariable Cox proportional hazards model was employed to evaluate the risk of active TB in RA patients beginning TNFi or tofacitinib treatment.
Results:
Among 3,382 RA patients (596 on tofacitinib and 2,786 on TNFi), LTBI screening was predominantly conducted using the IGRA (interferon-gamma release assays) test. Of these patients, 624 (18.5%) with a positive LTBI test received prophylactic treatment. No cases of active TB occurred in tofacitinib users, while 32 cases were observed in TNFi users. The adjusted hazard ratio for active TB in LTBI-positive patients was 5.47 (95% confidence interval 2.74 to 10.92) compared to LTBI-negative patients.In subgroup analyses, TB incidence was significantly higher among individuals aged over 65 years.
Conclusion
Despite LTBI treatment, active TB remains prevalent among RA patients on TNFi therapy and in those with positive LTBI tests.
2.Disproportionality Analysis on Adverse Reactions Following Immune Checkpoint Inhibitors Using a Nationwide Spontaneous Adverse Event Reporting Database

Background:
The use of immune checkpoint inhibitors (ICIs) for advanced cancer is increasing, particularly in combination withchemotherapy. However, previous studies have focused on immune-related adverse events (irAEs) in specific organs, with limited re-search on combination therapy.
Objective:
We aimed to detect signals of adverse drug reaction (ADR) in both ICIs plus chemotherapy and ICIs alone compared to chemotherapy.
Methods:
We investigated individual case safety reports (ICSRs) between 2014 and 2022using Korea Institute of Drug Safety and Risk Management Korean Adverse Event Reporting System database (KIDS KAERS DB(2304A0076)). We defined adverse events (AEs) and drugs using MedDRA and the national drug code directory provided by the Min-istry of Food and Drug Safety. To detect signals, we performed disproportionality analysis using indices of reporting odds ratio (ROR) and information component (IC).
Results:
We identified 206,959 ICSRs in the KIDS KAERS DB, of which 5,154 contained ICIs and 201,805 were for chemotherapy. Compared to chemotherapy, radiation pneumonitis (ROR, 2313.6; IC, 5.4), hyperthyroidism (ROR, 53.4; IC, 4.2), and eczema (ROR,18.3; IC, 3.4) were detected as signals for ICIs. For ICIs with chemotherapy, additional signals including heart failure (ROR 9.65; IC 2.45), proteinuria (ROR 28.67; IC 4.16), and urinary tract infections (ROR 3.46; IC 1.03) were detected.
Conclusion
The study highlights an essential signal of ADRs associated with ICIs, both alone and in combination therapy, with conditions such as hyperthyroidism, hypothyroidism, and pneumonitis. These AEs should be monitored in clinical settings.
3.Designing a user-centered mobile health application to empower older adults in managing oral health lifestyle

Objectives:
This study aimed to develop an age-friendly oral health lifestyle service application to enhance older adults’ self-management and promote a prevention-oriented approach to oral health. The application integrates user-centered design principles and the Capability, Opportunity, Motivation, and Behavior (COM-B) behavioral change theory to mitigate oral function decline and the digital divide.
Methods:
The application was developed based on three principles: (1) user-centered design, reflecting the physical and cognitive characteristics of older adults with large fonts, high-contrast colors, intuitive icons, and simplified navigation; (2) behavioral change theory (COM-B model), enhancing capability, opportunity, and motivation through educational content, mobile accessibility, and feedback using self-monitoring and goal setting; and (3) a prevention-oriented approach, aligned with the WHO Global Oral Health Action Plan. The system included four modules: (1) a 16-item self-assessment across five oral health domains, (2) a three-level risk classification with visual feedback (green/yellow/red), (3) a personalized education module with multimedia content, and (4) a habit-formation tool that supported self-monitoring and reminders.
Results:
An Android-based prototype was developed with four menus: self-assessment, feedback, education, and habit formation. The self-assessment tool classified users into low-, moderate-, and high-risk groups and provided motivational feedback. Based on the results, stage-specific educational videos and motivational cues were delivered as alerts. Senior-friendly UI/UX features such as large text, high contrast, intuitive icons, and tactile feedback enhanced accessibility and usability. Overall, the prototype integrated oral frailty assessment with theory-based interventions, fostering self-efficacy and engagement in preventive oral healthcare.
Conclusions
The application provides an accessible theory-based digital solution that empowers older adults to manage their oral health and prevent frailty. This presents a promising model for shifting oral health care to self-care and prevention. Future studies will use surveys and log data to examine the relationships among usage, usefulness, usability, acceptance, and behavior change. A randomized controlled trial is required to verify the effectiveness of this application.

From January 2010 to January 2021, a retrospective study was conducted on a total of 63 patients who underwent the MIPO technique for the treatment of humeral shaft fractures. We divided these patients into 2 groups: in group I, patients underwent MIPO without a positional screw; in group II, patients underwent MIPO with a positional screw. We compared functional outcomes including the American Shoulder and Elbow Surgeons score, University of California at Los Angeles score, Simple Shoulder Test, range of motion before and after surgery, operation time, blood loss, and complications. And we compared radiologic outcomes including pre- and postoperative anteroposterior (AP) and lateral displacement of the fracture and union time on plain radiographs.
Results:
The average patient age was 64.6 ± 15.1 years (range, 25–88 years). Group I consisted of 30 patients (10 men and 20 women), and group II consisted of 33 patients (11 men and 22 women). Between the 2 groups, there was no statistically significant difference in sex, body mass index, functional scores, AP and lateral displacement of the fracture on postoperative x-ray, operation time, and blood loss. In group II, a faster bony union was obtained than that in group I (4.6 vs. 6.4 months). Complications included 2 cases of transient radial nerve palsy in both groups and metallic failures (2 in group I and 1 in group II).
Conclusions
When performing MIPO for humeral shaft fractures, adding a positional screw could be more stabilizing than bridge plating without a positional screw, leading to faster bony union. A positional screw might help control interfragmentary movement without inhibiting essential interfragmentary movement for fracture healing.

8.Nationwide Study of the Epidemiology, Diagnosis, and Treatment of Hypersensitivity Pneumonitis in Korea

Background:
Hypersensitivity pneumonitis (HP) is a condition with an uncertain global incidence, and information on its diagnosis and management is limited. This study aimed to address these knowledge gaps.
Methods:
This study utilized customized claims data from the Health Insurance Review and Assessment Service (HIRA) in South Korea from January 2010, to December 2021. Patients with HP were identified based on the diagnosis code (International Classification of Diseases, 10th Revision, J67) between 2011 and 2020. Incident HP cases were defined as new HP claims, excluding those with claims in the previous year. The study examined various factors such as age, sex, comorbidities, diagnostic methods, and treatment patterns. Additionally, multivariate logistic regression analysis was performed to identify risk factors associated with treatment initiation.
Results:
A total of 8,678 HP incident cases were confirmed, with age- and sex-adjusted annual incidence rates ranging from 1.14/100,000 in 2020 to 2.16/100,000 in 2012. The mean age of patients with incident HP was 52 years, with a higher incidence observed among males. Additionally, the most common comorbidity was asthma. Bronchoscopy was performed on 16.9% of patients, and 25.4% of patients did not receive treatment within 1 year of diagnosis. Among those who received treatment, prednisone was the most used systemic steroid, and azathioprine was the most commonly used second-line immunosuppressant.Factors associated with treatment initiation included the female sex, having asthma or gastroesophageal reflux disease (GERD), and undergoing bronchoscopy.
Conclusion
This study provides valuable insights into the incidence, diagnosis, and treatment patterns of HP in South Korea using nationwide medical claims data.

Background:
Daily usage of facial masks during coronavirus disease 2019 pandemic influenced on facial dermatoses.
Objective:
This study investigated the impact of mask-wearing habits on facial dermatoses.
Methods:
A nationwide, observational, questionnaire-based survey was conducted from July through August 2021, involving 20 hospitals in Korea.
Results:
Among 1,958 facial dermatoses, 75.9% of patients experienced aggravation or development of new-onset facial dermatoses after wearing masks. In aggravated or newly developed acne patients (543 out of 743), associated factors were healthcare provider, female gender, and a long duration of mask-wearing. Irritating symptoms, xerosis, and hyperpigmentation were more frequently observed in this group. Aggravated or newly developed rosacea patients (515 out of 660) were likely to be female, young, and have a long duration of mask-wearing per day. Seborrheic dermatitis patients who experienced aggravation or de novo development (132 out of 184) were younger, and they more frequently involved the chin and jaw in addition to the nasolabial folds and both cheeks. Contact dermatitis patients (132 out of 147) with aggravation or de novo development tended to be female, involve both cheeks, and complain of pruritus. Aggravated or newly developed atopic dermatitis patients (165 out of 224) were more likely to be female, and had a higher baseline investigator global assessment score before mask-wearing.
Conclusion
Clinical features and factors related to aggravation were different according to the types of facial dermatoses.

Background:
More than half of acne patients have truncal acne on their chest, back, and shoulders. However, since most studies on acne have focused on the face, data on clinical characteristics and proper management for truncal acne are insufficient.
Objective:
To establish a Korean Acne Rosacea Society (KARS) consensus for experts’ perception and treatment patterns of truncal acne.
Methods:
We conducted two rounds of the Dephi technique to gather expert opinion and reach a consensus on truncal acne. The first round comprised 48 questionnaires focusing on various aspects such as epidemiology, clinical features, diagnosis, treatment, prognosis and more, while second rounds consisted of 26 questionnaires.
Results:
A total of 36 dermatologists (36/38 KARS members, 94.7%) completed this survey. In the first-round survey, consensus was reached on 20 out of the 48 questions (41.7%). In the secondround questionnaire, consensus was achieved on 9 of the 26 questions (34.6%). The most unresponsive lesion to truncal acne treatment was scars (atrophic/hypertrophic). The most commonly used treatments for each non-inflammatory and inflammatory truncal acne lesions were selected to use topical retinoids (78.1% of the responders) and oral antibiotics (93.8% of the responders).
Conclusion
Our study has yielded valuable insights into the epidemiology, clinical manifestations, diagnosis, treatment, and quality of life of patients with truncal acne. We anticipate that this study will inspire further comprehensive research for individuals with truncal acne.
