1.Beyond the chain of survival: a scoping review of artificial intelligence applications in cardiac arrest

BACKGROUND: To provide a comprehensive analysis of the landscape of artificial intelligence (AI) applications in cardiac arrest (CA).
METHODS: Comprehensive searches were conducted in PubMed, the Cochrane Library, Web of Science, and EMBASE from database inception through 10 June 2025. Studies that applied AI in both in-hospital cardiac arrest (IHCA) and out-of-hospital cardiac arrest (OHCA) populations across the following domains were included: prediction of cardiac arrest occurrence, prognostication of CA outcomes, applications of large language models (LLMs), and evaluation of cardiopulmonary resuscitation (CPR) and other AI-driven interventions related to CA.
RESULTS: The scoping review included 114 studies, encompassing data from 9,574,462 patients in total. AI was most commonly applied to the prediction of CA (overall, n=40; IHCA, n=30; OHCA, n=4; and both, n=6), CPR-related decision support during CA (n=16), and post-arrest prognosis and rehabilitation outcomes (overall, n=38; OHCA, n=21; IHCA, n=3; and both, n=14). Additional application areas included LLM-based applications (n=8), emergency call handling (n=4), wearable device-based detection (n=3), heart rhythm identification (n=2), education (n=2), and extracorporeal cardiopulmonary resuscitation (ECPR) candidate identification (n=1). Across all application scenarios, the highest area under the receiver operating characteristic curve (AUROC) value for pre-arrest CA prediction in IHCA patients was 0.998 using a multilayer perceptron (MLP) model, whereas the optimal AUROC for pre-arrest CA prediction in OHCA patients was 0.950 using extreme gradient boosting (XGBoost) or random forest (RF) models. For CPR-related decision support during CA, the highest AUROC achieved was 0.990 with a convolutional neural network (CNN) model. In prognostic prediction, the optimal AUROC for IHCA patients was 0.960 using XGBoost, while for OHCA patients it reached 0.976 using an MLP model.
CONCLUSION: This review shows that AI is most commonly used for the prediction of CA and CPR-related support, as well as post-arrest and rehabilitation outcomes. Future research directions include drug discovery, post-resuscitation management, neurorehabilitation, and clinical trial innovation. Further studies should prioritize multicenter clinical trials to evaluate AI models in real-world settings and validate their effectiveness across diverse patient populations. Overall, AI has significant potential to improve clinical practice, and its role in CA application is increasingly important.

3.Differential Diagnosis of Prostate Cancer from Benign Prostatic Hyperplasia Using 5.0T Multiparametric MRI with Histogram Analysis

Purpose To evaluate the efficacy of ultra-high field 5.0T MRI combined with histogram analysis for diagnosing prostate cancer and benign prostatic hyperplasia.Materials and Methods This retrospective analysis included data from 63 patients with prostatic diseases at the Chongqing University Three Gorges Hospital from January to May 2024,comprising 31 cases of prostate cancer and 32 cases of benign prostatic hyperplasia.MRI sequences included T2WI,T1WI,diffusion-weighted imaging,intravoxel incoherent motion and T2 mapping.Histogram data of apparent diffusion coefficient,true diffusion coefficient,pseudo-diffusion coefficient,perfusion fraction and T2 relaxation time were calculated,and diagnostic efficacy was assessed using the area under the receiver operating characteristic curve.Results In prostate cancer,the 10th percentile,the 90th percentile,mean,median,and minimum values of histogram parameters from apparent diffusion coefficient,true diffusion coefficient,pseudo-diffusion coefficient,perfusion fraction and T2 mapping were significantly lower than those of benign prostatic hyperplasia(Z=-6.036--3.368,all P<0.05).Notably,the combined model of apparent diffusion coefficient,intravoxel incoherent motion and T2 mapping parameters achieved an the area under the curve of 0.987,with sensitivity and specificity of 96.77%and 96.87%,respectively.Conclusion This study confirms that 5.0T MRI histogram analysis technique demonstrates significant diagnostic efficacy in differentiating prostate cancer from benign prostatic hyperplasia.

5.Application of multi-dimensional nursing based on empowerment theory in patients undergoing thoracoscopic and laparoscopic esophagectomy

Objective:To explore the effects of multi-dimensional nursing based on empowerment theory in patients undergoing thoracoscopic and laparoscopic esophagectomy.Methods:A convenience sampling method was used to select 110 esophageal cancer patients who were admitted to Henan Provincial People's Hospital from August 2022 to April 2024. Patients were randomly divided into two groups using a random number table: the control group and the observation group, with 55 patients in each group. The control group received routine care, while the observation group received multi-dimensional nursing based on empowerment theory in addition to routine care. The two groups were compared in terms of stress responses (heart rate, systolic blood pressure, diastolic blood pressure) , self-care ability, nutritional status (hemoglobin, albumin) , and nursing satisfaction.Results:After the intervention, the observation group had lower heart rate, systolic blood pressure, and diastolic blood pressure compared to the control group, with statistically significant differences ( P<0.01) . The self-care ability score in the control group was lower than that in the observation group, with a statistically significant difference ( P<0.01) . The observation group also had higher hemoglobin and albumin levels compared to the control group, with statistically significant differences ( P<0.01) . The nursing satisfaction rate in the observation group was higher than that in the control group, with a statistically significant difference ( P<0.05) . Conclusions:Multi-dimensional nursing based on empowerment theory can effectively reduce stress responses, improve self-care ability, enhance nutritional status, and increase nursing satisfaction in esophageal cancer patients.

Dental treatments for children and adolescents have unique clinical characteristics that differ from dental care for adults in terms of children's physiology, psychology, and behavior. These differences impose specific requirements on the application of local anesthesia in pediatric dental procedures. This article presents expert consensus on the principles of local anesthesia techniques in pediatric dental therapies, including the use of common anesthetic drugs and dosage control, safety and efficacy evaluation, and prevention and management of complications. The aim is to improve the safety and quality of pediatric dental treatments and offer guidance for clinical application by dentists.

8.Mechanism of L-perilla alcohol in intervening hypoxic pulmonary hypertension based on network pharmacology and experimental verification.

The mechanism of L-perilla alcohol(L-POH) in intervening hypoxic pulmonary hypertension(HPAH) was discussed based on network pharmacology, and experimental verification. The active components and potential targets of the volatile oil of Rhodiola tangutica(VORA) in the intervention of HPAH were screened by network pharmacology. The biological process of Gene Ontology(GO) and the signaling pathway enrichment of Kyoto Encyclopedia of Genes and Genomes(KEGG) were analyzed for the core targets, and a "component-common target-disease" network was constructed. Four active components were screened from VORA: L-POH, linalool, geraniol, and(-)-myrtenol. The core targets for treating HPAH were HSP90AA1, AKT1, ESR1, PIK3CA, EP300, EGFR, and JAK2. GO enrichment analysis mainly involved biological processes such as reaction to hypoxia, heme binding, and steroid binding. KEGG enrichment analysis mainly involved hypoxia-inducing factor 1(HIF-1) signaling pathway, phosphatidylinositol 3-kinase/protein kinase B(PI3K/AKT) signaling pathway, and Janus kinase/activator of signal transduction and transcription(JAK/STAT) signaling pathway. The vasodilation effects of the four active components were screened by perfusion experiment of extracorporeal vascular rings, and the mechanism of the main active component L-POH was studied by channel blockers. The inhibitory effects of the four active components on the proliferation of pulmonary artery smooth muscle cells(PASMCs) induced by hypoxia were screened by cell proliferation experiment, and the mechanism of the main active component L-POH was studied by flow cytometry, cell cycle experiment, and Western blot. The results showed that L-POH could directly act on vascular smooth muscle to relax pulmonary arterioles, induce ATP-sensitive potassium channels to open, and inhibit extracellular Ca~(2+) influx through voltage-gated calcium channels to relax blood vessels. In addition, L-POH could inhibit the abnormal proliferation of PASMCs induced by hypoxia and promote its apoptosis, and its mechanism may be related to the increase in Bax protein expression and the decrease in p-JAK2, p-STAT3, Bcl-2, and cyclinA2 protein expression. In summary, L-POH can interfere with HPAH by relaxing pulmonary arterioles and inhibiting the proliferation of smooth muscle cells.

9.Mechanism of matrine against senescence in human umbilical vein endothelial cells based on network pharmacology and experimental verification.

Utilizing network pharmacology, molecular docking, and cellular experimental validation, this study delved into the therapeutic efficacy and underlying mechanisms of matrine in combating senescence. Databases were utilized to predict targets related to the anti-senescence effects of matrine, resulting in the identification of 81 intersecting targets for matrine in the treatment of senescence. A protein-protein interaction(PPI) network was constructed, and key targets were screened based on degree values. Gene Ontology(GO) function and Kyoto Encyclopedia of Genes and Genomes(KEGG) pathway enrichment analyses were performed on the key targets to elucidate the critical pathways involved in the anti-senescence effects of matrine. Molecular docking was conducted between matrine and key targets. A senescence model was established using human umbilical vein endothelial cells(HUVECs) induced with hydrogen peroxide(H_2O_2). Following treatment with varying concentrations of matrine(0.5, 1, and 2 mmol·L~(-1)), cell viability was assessed by using the CCK-8. SA-β-galactosidase staining was employed to observe the positive rate of senescent cells. Flow cytometry was utilized to measure the apoptosis rate. Real-time quantitative PCR(RT-PCR) was utilized to measure the mRNA expression of apoptosis-related cysteine peptidase 3(CASP3), albumin(ALB), glycogen synthase kinase 3β(GSK3B), CD44 molecule(CD44), and tumor necrosis factor-α(TNF-α). Western blot was performed to detect the protein expression of tumor protein p53(p53), cyclin-dependent kinase inhibitor 1A(p21), cyclin-dependent kinase inhibitor 2A(p16), and retinoblastoma tumor suppressor protein(pRb) in the senescence signaling pathway, p38 protein kinase(p38), c-Jun N-terminal kinase(JNK), and extracellular regulated protein kinases(ERK) in the mitogen-activated protein kinase(MAPK) pathway, and phosphatidylinositol 3-kinase(PI3K) and protein kinase B(Akt) in the PI3K/Akt signaling pathway. The experimental results revealed that matrine significantly increased the viability of HUVECs(P<0.05), decreased the positive rate of senescent cells and the apoptosis rate(P<0.05), and reduced the mRNA expression levels of CASP3, ALB, GSK3B, CD44, and TNF-α(P<0.05). It also inhibited the protein expression of p53, p21, p16 and pRb in the senescence signaling pathway(P<0.05), upregulated the protein expression of p-PI3K/PI3K and p-Akt/Akt(P<0.05), and downregulated the protein expression of p-p38/p38, p-JNK/JNK, and p-ERK/ERK(P<0.05). Collectively, these findings suggest that matrine exerts an inhibitory effect on HUVECs senescence, and its mechanism involves the modulation of the senescence signaling pathway, MAPK pathway, and PI3K/Akt signaling pathway to suppress cell apoptosis and inflammation.

10.Treatment of extensive wound of Degree IV burns in limbs with free anterolateral thigh perforator flap: a report of 9 cases

Objective:To explore the clinical efficacy of anterolateral thigh perforator flap (ALTPF) in treatment of extensive wound of Degree IV burns in limbs.Methods:A retrospective analysis was conducted on 9 patients who had extensive wound of Degree IV burns in limbs caused by stove burns admitted to Department of Burns and Plastic Surgery, the 988th Hospital of the Joint Logistics Support Force of the Chinese PLA between January 2017 and January 2024. Among the patients, there were 8 males and 1 female, aged between 36 and 63 years. Three patients had the wound from anterior leg to dorsal foot, 3 from leg down to ankle, 1 from forearm to hand and 2 from arm to forearm. Area of burns ranged from 20 cm × 15 cm to 30 cm × 25 cm, and all patients were treated by free ALTPF. According to whether the main artery at the recipient site was feasible for direct anastomosis with the vessels carried in flap, 4 patients were treated by bilateral parallel ALTPFs, and 5 were treated by unilateral ultra-long internally supercharged ALTPF. A total of 13 ALTPFs were harvested, with individual flap size at 20 cm × 8 cm to 46 cm × 12 cm. Donor sites were directly sutured. Time for flap harvesting, flap survival and wound healing time were records. Scheduled postoperative follow-up was conducted at outpatient clinic and via telephone interviews to evaluate functional recovery. Follow-up assessments included evaluation of flap condition, two-point discrimination (TPD), recovery of joint function at recipient sites, flap appearance and donor site recovery.Results:The time for flap harvest was 1.0 to 4.5 hours. All 13 ALTPFs successfully survived. The time from surgery to healing of recipient sites was 18 to 72 days, and all donor sites healed. Over the postoperative follow-up that lasted for 6 to 34 months, the recipient sites had found with good cosmetic outcomes, without osteomyelitis or deep tissue infection. Four ALTPFs in 2 patients were found swelling, which were revised at 6 months after surgery. Four ALTPFs in other 2 patients had pigment deposition at edges. One ALTPF was scalded, which healed after dressing changes but left with patchy scars. The remaining ALTPFs were soft, elastic, free from pain and well-perfused, with regained protective sensation at S 3. However, all of the ALTPFs failed to detect TPD. Six patients with lower limb injuries were evaluated using the American Orthopedic Foot and Ankle Society (AOFAS) ankle hindfoot scoring system: 2 patients were rated as excellent and 4 were rated as good. Three patients with upper limb injuries were evaluated using Evaluation Trial Standards of Upper Limb Partial Functional of Hand Surgery of Chinese Medical Association: 1 patient was rated as excellent and 2 were rated as good. Only linear scars left at the 13 donor sites, with normal blood supply to the distal limbs, and without restrictions in range of motion of knee joints nor muscle strength of quadriceps. Conclusion:The ALTPF offers advantages such as anatomical consistency, reliable blood supply and flexible combination in treatment of extensive wound of Degree Ⅳ burns in limbs. It is an ideal surgical procedure for treatment of large soft tissue defects of extremities.
