1.Clinical management of hypersensitivity reactions to antituberculosis drugs

Tuberculosis remains one of the leading causes of death from infectious diseases worldwide, and its effective treatment requires prolonged multidrug regimens. Hypersensitivity reactions to antituberculosis drugs (ATDs) can significantly compromise treatment outcomes. These reactions range from mild skin eruptions to severe cutaneous adverse reactions (SCARs), including Stevens-Johnson syndrome, toxic epidermal necrolysis, and drug reaction with eosinophilia and systemic symptoms. The most frequently implicated agents are pyrazinamide, rifampin, isoniazid, and ethambutol, which are often associated with multiple drug hypersensitivity.Diagnostic tools such as patch testing, intradermal testing, and lymphocyte transformation tests may aid in identifying the culprit drug, but their sensitivity and specificity are limited. In cases of SCARs, immediate discontinuation of the offending agent is essential, and systemic corticosteroids may be effective in moderate to severe cases. Reintroduction of ATDs may be attempted through drug rechallenge or desensitization. Although drug reintroduction was traditionally contraindicated in patients with SCARs, recent reports have demonstrated successful desensitization when performed cautiously with informed consent. Breakthrough reactions may occur during desensitization, but they are often manageable with dose or interval adjustments. The development of standardized desensitization protocols for ATD-induced hypersensitivity remains an important future objective. In addition, ATDs can also induce classical immediate-type reactions such as urticaria and angioedema. In rare cases, they may present as atypical non-IgE-mediated anaphylaxis, for which no definitive diagnostic tools are currently available. Severe reactions may occur upon re-administration and cannot be prevented by desensitization. Thus, careful differential diagnosis is essential and any form of re-exposure is strictly contraindicated.
2.Risk Factors for Suicide Among Adolescents with Migration Backgrounds: A Secondary Data Analysis from the 2024 Korea Youth Risk Behavior Survey

Purpose:
This study aimed to identify suicide risk factors among adolescents with a migration background.
Methods:
A descriptive, secondary data analysis was conducted using data from the 2024 Korea Youth Risk Behavior Survey, which included 1,782 adolescents with a migration background. Data analysis utilized a complex sample design, incorporating frequencies, descriptive statistics, x2 tests, t-tests, ANOVA, and logistic regression, performed with SPSS 26.0.
Results:
Logistic regression identified significant predictors of suicide risk, including age, feelings of sadness and hopelessness, stress, anxiety, perceived health status, history of injury, and use of liquid e-cigarettes. Together, these factors explained 33.5% of the variance in suicide risk.
Conclusion
The findings underscore the necessity for targeted mental health interventions for adolescents with a migration background, focusing on emotional regulation, stress management, and the prevention of risky behaviors. Early screening and culturally sensitive strategies are crucial for reducing suicide risk in this vulnerable population.

Purpose:
Since 1995, the Korean Gastric Cancer Association (KGCA) has been periodically conducting nationwide surveys on patients with surgically treated gastric cancer. This study details the results of the survey conducted in 2023.
Materials and Methods:
The survey was conducted from March to December 2024 using a standardized case report form. Data were collected on 86 items, including patient demographics, tumor characteristics, surgical procedures, and surgical outcomes. The results of the 2023 survey were compared with those of previous surveys.
Results:
Data from 12,751 cases were collected from 66 institutions. The mean patient age was 64.6 years, and the proportion of patients aged ≥71 years increased from 9.1% in 1995 to 31.7% in 2023. The proportion of upper-third tumors slightly decreased to 16.8% compared to 20.9% in 2019. Early gastric cancer accounted for 63.1% of cases in 2023.Regarding operative procedures, a totally laparoscopic approach was most frequently applied (63.2%) in 2023, while robotic gastrectomy steadily increased to 9.5% from 2.1% in 2014.The most common anastomotic method was the Billroth II procedure (48.8%) after distal gastrectomy and double-tract reconstruction (51.9%) after proximal gastrectomy in 2023.However, the proportion of esophago-gastrostomy with anti-reflux procedures increased to 30.9%. The rates of post-operative mortality and overall complications were 1.0% and 15.3%, respectively.
Conclusions
The results of the 2023 nationwide survey demonstrate the current status of gastric cancer treatment in Korea. This information will provide a basis for future gastric cancer research.

9.Scientific Publication Speed of Korean Medical Journals during the COVID-19 Era

Objectives:
This study compared the scientific publication speeds of Korean medical journals before and during the coronavirus disease 2019 (COVID-19) era.
Methods:
We analyzed 2,064 papers from 43 international Korean medical journals, selecting 12 papers annually from 2019 to 2022. We assessed publication speed indicators, including the time from submission to revision and from submission to publication. Additionally, we examined variations in publication speed based on journal and paper characteristics, including whether the studies were related to COVID-19.
Results:
Among the 43 journals analyzed, 39.5% disclosed the peer review duration from submission to the first decision, and 11.6% reported their acceptance rates. The average time from submission to acceptance was 127.0 days in 2019, 126.1 days in 2020, 124.6 days in 2021, and 126.4 days in 2022. For COVID-19-related studies, the average time from submission to revision was 61.4 days, compared to 105.1 days for non-COVID-19 studies; from submission to acceptance, it was 87.4 days for COVID-19-related studies and 127.1 days for non-COVID-19 studies. All indicators for COVID-19-related studies showed shorter durations than those for non-COVID-19 studies, and the proportion of studies accepted within 30 or 60 days was significantly higher for COVID-19-related studies.
Conclusions
This study investigated the publication speed of Korean international medical journals before and during the COVID-19 pandemic. The pandemic influenced journals’ review and publication processes, potentially impacting the quality of academic papers. These findings provide insights into publication speeds during the COVID-19 era, suggesting that journals should focus on maintaining the integrity of their publication and review processes.

Rare endocrine diseases are complex conditions that require lifelong specialized care due to their chronic nature and associated long-term complications. In Korea, a lack of nationwide data on clinical practice and outcomes has limited progress in patient care. Therefore, the Multicenter Networks for Ideal Outcomes of Pediatric Rare Endocrine and Metabolic Disease (OUTSPREAD) study was initiated. This study involves 30 centers across Korea. The study aims to improve the long-term prognosis of Korean patients with rare endocrine diseases by collecting comprehensive clinical data, biospecimens, and patient-reported outcomes to identify complications and unmet needs in patient care. Patients with childhood-onset pituitary, adrenal, or gonadal disorders, such as craniopharyngioma, congenital adrenal hyperplasia (CAH), and Turner syndrome were prioritized. The planned enrollment is 1,300 patients during the first study phase (2022–2024). Clinical, biochemical, and imaging data from diagnosis, treatment, and follow-up during 1980–2023 were retrospectively reviewed. For patients who agreed to participate in the prospective cohort, clinical data and biospecimens will be prospectively collected to discover ideal biomarkers that predict the effectiveness of disease control measures and prognosis. Patient-reported outcomes, including quality of life and depression scales, will be evaluated to assess psychosocial outcomes. Additionally, a substudy on CAH patients will develop a steroid hormone profiling method using liquid chromatography-tandem mass spectrometry to improve diagnosis and monitoring of treatment outcomes. This study will address unmet clinical needs by discovering ideal biomarkers, introducing evidence-based treatment guidelines, and ultimately improving long-term outcomes in the areas of rare endocrine and metabolic diseases.
