Background:
Rosacea is a chronic inflammatory skin disorder characterized by erythema, papules, ocular symptoms, and heightened sensitivity. Patients with neurogenic symptoms such as burning or stinging remain particularly difficult to manage. Current guidelines often underrepresent energy-based devices (EBDs), pigmentary sequelae, psychosocial burden, and ocular comorbidities.
Objective:
To examine Korean dermatologists’ expert perspectives on rosacea management, focusing on skin sensitivity, neurogenic symptoms, pigmentary changes, psychosocial impact, ocular involvement, and EBD use.
Methods:
A web-based, 29-item survey was administered to 25 board-certified Korean dermatologists (May–June 2025). Quantitative and qualitative responses were analyzed.
Results:
Erythematotelangiectatic and papulopustular phenotypes with sensitivity skin predominated. EBDs (pulsed dye laser, intense pulsed light) were frequently used but limited by cost and sensitivity issues. Neurogenic symptoms were recognized but rarely treated with neuromodulators. Post-inflammatory hyperpigmentation was infrequent, yet monitoring was inconsistent.Psychosocial and ocular aspects were acknowledged but seldomly systematically addressed.Respondents expressed interest in emerging adjunctive treatments such as cold plasma, skin boosters, and holistic care approaches.
Conclusion
Korean dermatologists adopt individualized strategies for rosacea, yet practice gaps remain regarding neurogenic symptoms, pigmentary complications, and psychosocial and ocular comorbidities. Findings support the need for updated multidisciplinary, phenotype-driven guidelines aligned with real-world practice.
2.Correlation between Portable Photopic Electroretinography and Postoperative Best-Corrected Visual Acuity in Patients with Diabetic Vitreous Hemorrhage

Purpose:
To determine the correlation between electroretinography (ERG) performed using a handheld ERG device equipped with skin electrodes and postoperative best-corrected visual acuity (BCVA) in patients with vitreous hemorrhage (VH) due to proliferative diabetic retinopathy (PDR).
Methods:
The medical records of patients who underwent vitrectomy for diabetic VH caused by PDR at our institution between July 1, 2017, and June 30, 2023, were reviewed retrospectively. We analyzed the correlation between preoperative ERG obtained using a handheld ERG device (RETeval), RETeval skin electrodes (Sensor Strips), and postoperative BCVA.
Results:
We evaluated the medical records of 70 eyes of 70 patients. The BCVA, which was measured in logMAR units, improved from 1.910 ± 0.504 preoperatively to 0.287 ± 0.225 postoperatively (p < 0.001). In the group that underwent panretinal photocoagulation before VH, the amplitudes of the b-wave in photopic ERG and 30-Hz flicker ERG were negatively correlated with the postoperative BCVA. The implicit times of the b-wave in photopic ERG and 30-Hz flicker ERG were positively correlated with the postoperative BCVA (p < 0.05).
Conclusions
Photopic ERG recorded using a handheld retinal ERG device equipped with skin electrodes can aid in predicting the visual prognosis in patients with severe VH caused by diabetes mellitus.
3.Comparison of Clinical Outcomes of Intravitreal 0.625% Povidone-Iodine versus Antibiotic Injection for Postoperative Endophthalmitis

Purpose:
Postoperative endophthalmitis is a sight-threatening intraocular infection requiring prompt treatment. This study aimed to compare the clinical outcomes of intravitreal 0.625% povidone-iodine (PI) injection with intravitreal antibiotic injection for the treatment of postoperative endophthalmitis.
Methods:
A retrospective review of 40 eyes diagnosed with postoperative endophthalmitis was conducted. Patients were divided into two groups: group 1 received intravitreal 0.625% PI injection (0.1 mL), while group 2 received intravitreal antibiotic injection. Clinical data, including best-corrected visual acuity (BCVA), intraocular pressure, the number of intravitreal injections, duration of anterior chamber hypopyon, and the need for pars plana vitrectomy (PPV), were analyzed.
Results:
Group 1 (24 eyes) showed significant improvements in BCVA posttreatment (mean, 1.04 ± 0.95 logMAR; p < 0.001) compared to group 2 (16 eyes; mean, 1.26 ± 0.99 logMAR; p = 0.092). Severe visual impairment (BCVA < 10 / 100) was significantly less frequent in group 1 (25.0%) than in group 2 (62.5%) (p = 0.025). The mean duration of hypopyon was shorter in group 1 (2.6 ± 2.2 days) compared to group 2 (6.4 ± 7.3 days, p = 0.048). Fewer intravitreal injections (p = 0.002) and PPV (p = 0.002) were required in group 1.
Conclusions
Intravitreal 0.625% PI injection was associated with fewer intravitreal injections, shorter hypopyon duration, and more favorable visual outcome–related measures, including significant visual improvement from baseline and a lower frequency of severe visual impairment, compared with intravitreal antibiotic treatment. These findings suggest that PI may be considered a safe and effective initial treatment option for postoperative endophthalmitis. Further prospective studies are needed to validate these results.
4.A New Methoxyflavonoid Rutinoside and PTP1B-Inhibitory Phenolic Compounds from the Water Extract of Areca catechu L.

Phytochemical investigation of the water extract and alkaloid fraction of Areca catechu L. led to theisolation of one new methoxyflavonoid rutinoside (1), together with eleven known compounds (2−12), five of which (8−12) belong to the pyridine alkaloid class. The structure of the new compound was elucidated through extensive spectroscopic analyses, including 1D and 2D NMR experiments and high-resolution mass spectrometry.Notably, rhamnazin-3-O-rutinoside (2) is reported here for the first time from this species. The isolated compounds were evaluated for their inhibitory activity against protein tyrosine phosphatase 1B (PTP1B) using a p-nitrophenyl phosphate assay. Flavonoids (3 and 4) and stilbene (5) exhibited notable inhibitory activity, with IC50 values of 13.30 ± 0.71, 13.44 ± 0.84, and 8.68 ± 0.12 μM, respectively. In contrast, methoxyflavonoid rutinosides (1 and 2) and pyridine alkaloids (8−12) did not show significant inhibitory activity against PTP1B.These findings provide additional chemical insights into A. catechu and its constituents in relation to PTP1B inhibition.
5.Efficacy and safety of metabolic bariatric surgery in patients aged ≥55 years: a multicenter retrospective cohort study in East Asians

Purpose:
Metabolic bariatric surgery (MBS) efficacy and safety is established for older patients, but East Asian data are limited. This study aimed to evaluate the safety and efficacy of MBS by comparing older (≥55 years) and younger (<55 years) East Asian patients with obesity.
Methods:
This multicenter, retrospective review included 410 patients undergoing MBS from January to December 2019.Patients were stratified into the older group (OG, age ≥55 years; n = 39) and the younger group (YG, age <55 years; n = 371). We compared surgical safety, weight parameters, and comorbidity resolution rates.
Results:
The OG had lower mean body weight (97.9 ± 16.4 kg vs. 113.2 ± 23.1 kg, P < 0.001) but more comorbidities and lower average ABCD score for type 2 diabetes mellitus (T2DM) remission. Postoperative complication rate (12.8% vs. 7.5%, P = 0.400) and postoperative hospital stay (4.1 ± 1.8 days vs. 4.0 ± 8.9 days, P = 0.773) showed no significant differences.At 12 months, the percentage of total weight loss was significantly lower in the OG (23.7 ± 6.9% vs. 27.8 ± 8.4%, P = 0.014).Remission rates for T2DM (47.6% vs. 80.5%, P < 0.001), hypertension (34.6% vs. 57.5%, P = 0.073), and dyslipidemia (12.5% vs. 44.4%, P = 0.012) were also lower in the OG, yet still demonstrated clinically meaningful metabolic improvement.
Conclusion
MBS is a safe and effective treatment for older East Asians with obesity, offering substantial comorbidity resolution despite achieving a lower weight loss compared to the younger patients.
6.Association of Physical Activity with Dementia Risk in Cancer Survivors: A Korean Nationwide Cohort Study

Purpose:
This study aimed to investigate the impact of physical activity on dementia risk among cancer survivors in South Korea.
Materials and Methods:
This retrospective, population-based cohort study included 344,152 cancer survivors identified from the National Health Insurance Service database in South Korea. The mean follow-up time was 5.81 years. Different levels of physical activity post-cancer diagnosis, ranging from inactive to highly active, were assessed. The primary outcome was the incidence of overall dementia, Alzheimer’s disease, and vascular dementia. Secondary outcomes included dementia risk stratified by cancer type and treatment (chemotherapy and radiation).
Results:
Of the total participants, 24,363 (7.08%) developed dementia. The risk of overall dementia decreased sequentially across the exercise groups compared to the inactive group: insufficiently active (adjusted hazard ratio [HR], 0.89; 95% confidence interval [CI], 0.86 to 0.92), active (adjusted HR, 0.85; 95% CI, 0.83 to 0.88), and highly active (adjusted HR, 0.79; 95% CI, 0.76 to 0.82). This inverse relationship between exercise and dementia risk was statistically significant across various cancer types and was consistent regardless of age, comorbidities, and whether or not excluding the first 1, 2 years.
Conclusion
Among cancer survivors in South Korea, increased physical activity post-diagnosis was associated with a significantly lower risk of dementia. These findings underscore the importance of promoting physical activity in cancer survivors for cognitive health.

Purpose:
The aim of this study is to compare the detection ability of quality of life (QoL) changes and responsiveness of the KOrean QUality of life in Stomach cancer patients Study group (KOQUSS)-40 and European Organization for Research and Treatment of Cancer Quality of Life Questionnaire (EORTC QLQ).
Materials and Methods:
A multicenter prospective observational study was conducted to evaluate QoL changes after various gastrectomies between January 2021 and April 2022. Participants were instructed to complete the KOQUSS-40 and EORTC QLQ-C30/STO22 preoperatively and at 1, 3, 6, and 12 months postoperatively. QoL changes over time and QoL responsiveness were assessed for each questionnaire.
Results:
Data from 491 patients who underwent curative gastrectomy for gastric cancer at 22 institutions were analyzed. The summary scores of the KOQUSS-40 and EORTC QLQ-STO22 showed significant differences between the total and proximal gastrectomy groups (p=0.044 and p=0.038, respectively), but no difference was observed for the EORTC QLQ-C30. Dysphagia on the KOQUSS-40 was significantly different between the total and proximal gastrectomy groups (p=0.031); however, dysphagia on the EORTC QLQ-STO22 did not differ. The responsiveness of the KOQUSS-40 was similar to that of the EORTC QLQ in patients who experienced ≥ 10% body weight loss, but approximately 10% less in patients receiving adjuvant chemotherapy than the EORTC QLQ.
Conclusion
KOQUSS-40 has several advantages over EORTC QLQ-C30/STO22 when comparing QoL between the total and proximal gastrectomy groups. The findings provide information for researchers investigating the QoL of patients who have undergone curative gastrectomy for gastric cancer.
8.Combined Transarterial Chemoembolization and External Beam Radiotherapy for Identifying Surgical Candidates for Hepatocellular Carcinoma with Macroscopic Vascular Invasion: A Propensity Score–Weighted Analysis

Purpose:
This study aimed to evaluate the role of hepatic resection in patients with objective responses after combined transarterial chemoembolization (TACE) and radiotherapy (RT) for hepatocellular carcinoma (HCC) with macroscopic vascular invasion (MVI).
Materials and Methods:
We retrospectively reviewed the patients treated with combined TACE and RT for HCC with MVI between 2010 and 2015. Some of the patients with objective responses underwent hepatic resection or liver transplantation; to investigate the impact of surgery, patients with objective responses who did not undergo surgery were selected as the control group. Survival outcomes were compared using a propensity score–based stabilized inverse probability of treatment weighting method.
Results:
Out of the 170 patients with objective responses after combined TACE and RT, 41 patients underwent surgery, including eight liver transplantations. The unweighted surgery group was younger and had a higher proportion of solitary tumors and unilateral vascular involvement. After adjustment, the 3-year overall survival (OS) rates were 61.0% and 28.6% in the surgery and non-surgery groups, respectively. The most important prognostic factor for OS was surgery (adjusted Cox hazard ratio [HR], 0.28; 95% confidence interval [CI], 0.17 to 0.46; p < 0.001). Complete response after TACE and RT (vs. partial response) was also a significant prognostic factor for OS (adjusted HR, 0.41; 95% CI, 0.27 to 0.61; p < 0.001). There was no surgical mortality. Four patients (9.8%) required additional surgery due to bleeding or graft failure.
Conclusion
Hepatic resection was significantly associated with improved OS in patients who showed objective responses after receiving combined TACE and RT for HCC with MVI.
9.Evaluating the Performance of Basic Local Government Initiatives in Suicide Prevention: Insights From Korea

Background:
Korea’s national suicide prevention strategies emphasize the role of local authorities. Evaluating local government performance helps determine whether suicide reduction goals are being met. This study assesses the performance of basic local government suicide prevention activities and explores their implications.
Methods:
Data were drawn from the 2023 Survey on Suicide Prevention Activities. Seventeen indicators, covering both process and outcome measures, were used to evaluate performance.A composite score (0–100) was calculated by weighting and summing the indicators, with weights based on average scores for each indicator. The 229 local governments were grouped into four categories by urbanization and population size: large Si (≥ 300,000 population), small Si (< 300,000), Gun, and Gu. The maximum–minimum (max–min) value ratio was used to examine composite score variation.
Results:
Among the 229 local governments, 29 were large Si areas, 49 small Si, 82 Gun, and 69 Gu. Significant disparities were found across several indicators, such as the operation of community suicide prevention centers, availability of mental health professionals, and allocated budgets. Composite scores were highest in large Si areas (59.8), followed by small Si (59.3), and Gu (57.9). The greatest variation was in Gun areas (max–min ratio: 2.6), suggesting inconsistent performance.
Conclusion
There are notable disparities in suicide prevention performance among Korea’s basic local governments. Customized strategies that reflect local capacities and contexts are essential to reduce these gaps and improve overall effectiveness.
10.Comparative survival outcomes of surgical resection versus radiotherapy after FOLFIRINOX in borderline resectable and locally advanced pancreatic cancer

Purpose:
This study evaluated the clinical outcomes and prognostic factors in patients with borderline resectable pancreatic cancer (BRPC) and locally advanced pancreatic cancer (LAPC) treated with upfront FOLFIRINOX followed by local-regional therapy (LRT), surgical resection (SR), and radiotherapy (RT). We aimed to identify specific patient subgroups for which RT may serve as a reasonable alternative to SR for local tumor control.
Methods:
We retrospectively analyzed 116 patients (SR group, n= 70; RT group, n= 46) at a single center between 2015 and 2020. Survival outcomes were compared based on LRT modalities, focusing on identifying subgroups in which RT provided an efficacy comparable to that of SR.
Results:
Among 116 patients, the SR group achieved a significantly higher 5-year overall survival (OS) than the RT group (27.1% vs. 8.7%, P< 0.0001), despite similar progression-free survival (P= 0.23). Significant prognostic factors for OS included carbohydrate antigen 19-9 (CA19-9) response in BRPC (P= 0.02) and radiologic partial response in LAPC (P= 0.05). Subgroup analysis revealed that, while SR provided a survival advantage in CA19-9 responders, no significant difference in OS was observed between SR and RT in CA19-9 non-responders (P= 0.37).
Conclusion
Although surgery remains the gold standard, RT may be considered a justifiable local alternative for CA19-9 non-responders and surgically ineligible patients with LAPC, yielding comparable outcomes in these specific, biologically unfavorable subgroups.
