1.Effects of senior-friendly foods on health, nutritional status, and dietary intake among rural elderly women in Korea: a quasi-experimental study

Objectives:
We evaluated the impacts on health, nutritional status, and dietary intake of providing senior-friendly foods to community-dwelling elderly women in a rural area in Korea.
Methods:
A pretest–posttest nonequivalent control group design with repeated measures was conducted among 71 rural-dwelling elderly women. Changes in health indicators, nutritional status, and dietary intake were assessed at three time points: baseline, post-intervention, and two months after intervention.
Results:
Immediately after a three month intervention, significant differences were observed between the intervention and control groups in frailty score, Dysphagia Handicap Index, Mini Nutritional Assessment, social isolation, resilience, quality of life, and depression (P < 0.05). Significant group-by-time interaction effects were found for muscle mass, hemoglobin A1c, and energy, protein, and micronutrient intake, all of which showed significant improvements in the intervention group (P < 0.05).
Conclusion
Providing senior-friendly foods effectively improved physical and physiological health and emotional well-being among rural older adults. This intervention also contributed broadly to improved dietary intake. These findings provide empirical evidence to support the development of community-based integrated care models and tailored nutrition intervention programs for rural elderly populations in Korea.Trial Registration: Clinical Research Information Service Identifier: KCT0011666.
2.Factors Associated with Functional Outcomes Following Combined Pelvic Ring and Acetabular Fractures: A 2-Year Retrospective Analysis

Background:
Combined pelvic ring and acetabular fractures are complex injuries associated with high morbidity. Despite advancements in surgical techniques, limited data are available on the long-term functional outcomes and associated factors. This study aimed to identify the factors associated with functional outcomes in surgically treated patients with combined pelvic ring and acetabular fractures.
Methods:
This retrospective study included 25 adult patients who underwent definitive surgical fixation for radiologically confirmed combined pelvic ring and acetabular fractures at a Level I trauma center between 2005 and 2021, with a minimum clinical and radiological follow-up of 24 months. Surgical approaches included the Kocher–Langenbeck or modified Stoppa methods, tailored to fracture morphology. Matta–Saucedo and Matta criteria assessed the pelvic ring and acetabular reduction quality, respectively. Functional outcomes at 2 years were evaluated using the Rommens–Hessmann criteria. Neurological recovery was assessed clinically and by electromyography where indicated. The association between the injury characteristics and functional recovery was evaluated using t-tests and Fisher’s exact test.
Results:
The mean patient age was 40.4 years, and 72% were male. Transverse acetabular fractures (58.6%) and anteroposterior compression pelvic ring injuries (64%) were the most common injuries. Anatomic or congruent acetabular reduction (within 2–3 mm) was achieved in 89.6% of fractures and was significantly associated with superior functional outcomes (p = 0.002). Conversely, the pelvic ring reduction quality did not significantly correlate with functional outcomes (p = 0.314). Transverse acetabular fractures (p = 0.046) and initial neurological deficits (p = 0.032) were associated with poorer recovery. Among the patients with neurological injury, 60% achieved partial or full recovery. Overall, bony union occurred in 96% of cases, with a mean time to union of 14.3 weeks. Reported complications included neurological deficits (40%) and infections (8%).
Conclusions
Functional outcomes following the surgical fixation of combined pelvic and acetabular fractures are primarily influenced by the acetabular reduction quality and neurological status at presentation. Transverse fracture patterns are complex and associated with nerve injuries, posing additional challenges. These findings emphasize the importance of precise joint reduction and early neuroassessment. Further multicenter, prospective studies are warranted to optimize the management of these complex injuries.
3.Health Effects of Sugar-Sweetened and Artificially Sweetened Beverages: Umbrella Review and Evidence-Based Consensus Statement of the Korean Diabetes Association and the Korean Nutrition Society

Background:
Excess intake of added sugars contributes to obesity, type 2 diabetes mellitus (T2DM), cardiovascular disease (CVD), and premature mortality. Sugar-sweetened beverages (SSBs), the main source of added sugars, are consistently linked to adverse outcomes. Artificially sweetened beverages (ASBs) have been suggested as short-term substitutes, but evidence regarding benefits and harms remains inconclusive, and guidance is lacking.
Methods:
This consensus statement draws on a structured evidence review combining two approaches: an updated meta-analysis of randomized controlled trials (RCTs) assessing short- to intermediate-term effects of replacing SSBs with ASBs on weight and metabolic outcomes; and an umbrella review of systematic reviews of cohort studies evaluating long-term associations of SSBs and ASBs with major outcomes, including mortality, CVD, and T2DM.
Results:
In 14 RCTs (3–76 weeks), replacing SSBs with ASBs produced modest reductions in body weight (–0.73 kg) and body fat (–0.72%), with inconsistent effects on glycemic and cardiometabolic markers. Evidence from 20 systematic reviews of cohorts (up to 34 years follow-up) showed that higher intake of both SSBs and ASBs was associated with increased risks of T2DM, CVD, and mortality, with relative risks for ASBs similar to those for SSBs.
Conclusion
ASBs may serve as a short-term substitution for individuals with high SSB intake, particularly those at elevated metabolic risk. However, regular or long-term use is not recommended due to uncertain safety and potential reinforcement of sweet preference. Public health strategies should emphasize reducing both SSBs and ASBs, prioritizing water and unsweetened beverages as the ultimate goal.
4.Long-term bladder and renal outcomes after cutaneous vesicostomy closure in pediatric patients with non-neurogenic bladder

Purpose:
To assess long-term outcomes of vesicostomy on bladder capacity (BC) and voiding function in non-neurogenic bladder, and explore the association between kidney ultrasonographic findings and renal function.
Materials and Methods:
Thirty-four patients under 2 years at the time of vesicostomy formation (2005–2020) with ≥3 years of follow-up were reviewed. Patients were further stratified based on neurogenic bladder status. Twenty-one patients were nonneurogenic. A subgroup analysis of 7 patients under 3 months with primary vesicoureteral reflux (VUR) and compromised renal function was conducted.
Results:
The median age at vesicostomy formation was 1.0 months (interquartile range [IQR] 0.0–3.5); the median duration of vesicostomy was 16.0 months (IQR 8.0–21.0). At a median age of 93.0 months (IQR 59.5–117.5), all patients achieved spontaneous micturition and continence. Eleven patients (52.4%) showed bell-shaped voiding patterns. Five patients showed interrupted (n=2) or plateau (n=3) patterns. With the exemption one patient, all patients with primary VUR showed bell-shaped curves. None initiated clean intermittent catheterization during follow-up. The median BC-to-estimated BC in patients with non-neurogenic bladder and primary VUR was 0.9 (IQR 0.7–1.1) and 0.9 (IQR 0.8–1.1), respectively. Three patients underwent revision due to prolapse. The glomerular filtration rate (GFR) was improved by 68.9% compared to the baseline (p=0.045). Parenchymal abnormalities on kidney ultrasonography were associated with decrease in GFR.
Conclusions
Vesicostomy in non-neurogenic bladder patients was associated with recovery of BC, preservation of continence, and improved renal function. Parenchymal abnormalities on ultrasonography predicted lower GFR.

Background:
Rosacea is a chronic inflammatory skin disorder characterized by erythema, papules, ocular symptoms, and heightened sensitivity. Patients with neurogenic symptoms such as burning or stinging remain particularly difficult to manage. Current guidelines often underrepresent energy-based devices (EBDs), pigmentary sequelae, psychosocial burden, and ocular comorbidities.
Objective:
To examine Korean dermatologists’ expert perspectives on rosacea management, focusing on skin sensitivity, neurogenic symptoms, pigmentary changes, psychosocial impact, ocular involvement, and EBD use.
Methods:
A web-based, 29-item survey was administered to 25 board-certified Korean dermatologists (May–June 2025). Quantitative and qualitative responses were analyzed.
Results:
Erythematotelangiectatic and papulopustular phenotypes with sensitivity skin predominated. EBDs (pulsed dye laser, intense pulsed light) were frequently used but limited by cost and sensitivity issues. Neurogenic symptoms were recognized but rarely treated with neuromodulators. Post-inflammatory hyperpigmentation was infrequent, yet monitoring was inconsistent.Psychosocial and ocular aspects were acknowledged but seldomly systematically addressed.Respondents expressed interest in emerging adjunctive treatments such as cold plasma, skin boosters, and holistic care approaches.
Conclusion
Korean dermatologists adopt individualized strategies for rosacea, yet practice gaps remain regarding neurogenic symptoms, pigmentary complications, and psychosocial and ocular comorbidities. Findings support the need for updated multidisciplinary, phenotype-driven guidelines aligned with real-world practice.
6.Technique and Outcomes of Transforaminal Endoscopic Lumbar Discectomy for Bilateral Disc Herniation

Objective:
This study aimed to describe the technique and outcomes of transforaminal endoscopic lumbar discectomy (TELD) for bilateral disc herniation.
Methods:
This retrospective review analyzed prospectively collected data. Patients who underwent single-level TELD for bilateral lumbar disc herniation with bilateral radiculopathy were included. The transforaminal entry approach utilized a 15° angle, compared to the conventional 30°. The disc space was approached from the side with the annular tear rather than the side with more severe symptoms. After introducing the working cannula and endoscope, the ipsilateral herniated disc was identified and removed. The approach angle was then adjusted to a more horizontal orientation to allow central decompression. The route to the contralateral herniated disc was identified, and the fragment was excised. Adequate decompression was confirmed by observing epidural pulsations, after which closure was performed. Operative time, length of hospital stay, patient-reported outcome measures, including the visual analogue scale (VAS) for back and leg pain and Oswestry Disability Index (ODI), and complications were analyzed.
Results:
Thirty-four patients were included, with a mean age of 41.6 years. The mean operative time was 58 minutes, and the mean hospital stay was 1.2 days. Significant postoperative improvement was observed in VAS back, VAS leg, and ODI scores. No intraoperative complications occurred. One patient (2.9%) developed postoperative dysesthesia, which resolved with conservative management. Another patient (2.9%) experienced reherniation, requiring revision endoscopic discectomy.
Conclusion
TELD for bilateral lumbar disc herniation with bilateral radiculopathy resulted in favorable clinical outcomes and minimal complications.
7.Unilateral Biportal Endoscopic Transforaminal Lumbar Interbody Fusion (TLIF) Using 3-Dimensional-Printed Titanium Cages Compared With Open TLIF: A Comparison of Clinical Outcomes and Fusion Rates

Objective:
Unilateral biportal endoscopic transforaminal lumbar interbody fusion (UBE-TLIF) has emerged as an alternative to open TLIF. However, limited evidence is available regarding the application of 3-dimensional (3D)-printed titanium cages in UBE-TLIF. We aimed to compare the clinical outcomes and fusion rates of UBE-TLIF and open TLIF using 3D-printed titanium cages.
Methods:
We retrospectively reviewed patients who underwent single-level TLIF with 3D-printed titanium cages between 2021 and 2023. The inclusion criterion was degenerative lumbar stenosis, while the exclusion criteria were trauma, infection, and multilevel surgery. Clinical and radiologic outcomes were compared between the UBE-TLIF and open TLIF groups.
Results:
Twenty-one patients underwent UBE-TLIF, and 21 underwent open TLIF. The visual analogue scale (VAS) for back (p=0.987) and leg pain (p=0.731) did not significantly differ between the groups at 1-year follow-up. However, VAS back pain at postoperative day 2 was significantly lower in the UBE-TLIF group than in the open TLIF group (p<0.001). Solid fusion was achieved in 21 patients (100%) in the open TLIF group and in 20 (95.2%) in the UBE-TLIF group (p=1.000). In multivariable logistic regression analysis, body mass index was the only factor that exhibited a significant relationship (odds ratio [OR], 0.70; 95% confidence interval [CI], 0.53–0.92; p=0.011) with interbody fusion. In contrast, the surgical method (UBE vs. open TLIF) was not a significant factor (OR, 0.47; 95% CI, 0.10–2.21; p=0.337).
Conclusion
Using a 3D-printed titanium cage for UBE-TLIF may yield comparable fusion rates to those of open TLIF.
8.Shifting the Paradigm of Medical Dispute Resolution: From Individual Punishment to System Improvement and Public Compensation

Legal risks and liability issues in medical practice serve as a primary catalyst for the current collapse of essential healthcare services in Korea. Currently, medical disputes in Korea are disproportionately focused on criminal prosecutions and high-damages civil litigation. This punitive approach fosters a culture of concealment, encourages defensive medicine, and accelerates the exodus of medical professionals from essential fields. Ultimately, this cycle deprives the system of opportunities for improvement and poses a significant threat to patient safety. In contrast, many advanced nations have adopted principles of “Just Culture” and “Safe Space,” prioritizing non-punitive reporting and systemic root-cause analysis over individual retribution. To address these issues, this paper proposes four key strategies: First, the establishment of an independent “Patient Safety Investigation Agency” to objectively investigate incidents and identify systemic flaws. Second, a transition from criminal punishment to licensing board-led management, focusing on re-education and counseling to maintain quality of care. Third, the enactment of “Apology Laws” to ensure that expressions of regret or apologies cannot be used as legal evidence of liability, thereby fostering trust and psychological recovery. Finally, the creation of a “Patient Safety Fund” to provide prompt and sufficient public compensation to victims regardless of proven negligence. In conclusion, it is imperative to shift the paradigm by defining medical accidents as “system failures” rather than individual faults. Strengthening the social safety net will encourage medical professionals to return to essential care and build a sustainable healthcare environment centered on patient safety.
9.Correlation between Portable Photopic Electroretinography and Postoperative Best-Corrected Visual Acuity in Patients with Diabetic Vitreous Hemorrhage

Purpose:
To determine the correlation between electroretinography (ERG) performed using a handheld ERG device equipped with skin electrodes and postoperative best-corrected visual acuity (BCVA) in patients with vitreous hemorrhage (VH) due to proliferative diabetic retinopathy (PDR).
Methods:
The medical records of patients who underwent vitrectomy for diabetic VH caused by PDR at our institution between July 1, 2017, and June 30, 2023, were reviewed retrospectively. We analyzed the correlation between preoperative ERG obtained using a handheld ERG device (RETeval), RETeval skin electrodes (Sensor Strips), and postoperative BCVA.
Results:
We evaluated the medical records of 70 eyes of 70 patients. The BCVA, which was measured in logMAR units, improved from 1.910 ± 0.504 preoperatively to 0.287 ± 0.225 postoperatively (p < 0.001). In the group that underwent panretinal photocoagulation before VH, the amplitudes of the b-wave in photopic ERG and 30-Hz flicker ERG were negatively correlated with the postoperative BCVA. The implicit times of the b-wave in photopic ERG and 30-Hz flicker ERG were positively correlated with the postoperative BCVA (p < 0.05).
Conclusions
Photopic ERG recorded using a handheld retinal ERG device equipped with skin electrodes can aid in predicting the visual prognosis in patients with severe VH caused by diabetes mellitus.
