1.Mechanisms of neuroprotective effects of the endocannabinoid system following traumatic brain injury: a review

Traumatic brain injury (TBI) involves both primary mechanical damage and refractory secondary injuries, resulting in high disability rate and poor prognosis. Current therapeutic strategies for TBI include surgical intervention, neuroprotective agents, moderate hypothermia therapy and spinal cord stimulation. However, most of these therapeutic approaches primarily address wound surface management rather than targeting specific pathogenic mechanisms underlying post-injury inflammation and neurodegenerative diseases, resulting in suboptimal efficacy. Consequently, novel therapeutic strategies targeting TBI pathological mechanisms are urgently needed. The endogenous cannabinoid system (ECS) exerts multifaceted neuroprotective effects in TBI by modulating neuroinflammation, inhibiting glutamate excitotoxicity and activating pathways such as phosphatidylinositol 3-kinase/protein kinase B (PI3K/Akt). Investigating the characteristics of ECS components and their related signaling pathways may yield new approaches in the development of neuroprotective drugs for TBI. Nevertheless, few ESC-targeting drugs for TBI treatment have advanced beyond preclinical or clinical trial phases. Breakthroughs in this field depend on a deeper understanding of ECS and its mechanisms in TBI. To this end, the authors reviewed researches on the composition and functions of ECS, as well as the mechanisms underlying its neuroprotective effects following TBI, aiming to provide references for the development of ECS-targeting therapies.
2.Influence of juxtapapillary duodenal diverticula on bile microecology in patients with primary common bile duct stones

ObjectiveTo investigate the microbiological characteristics of bile in patients with common bile duct stones alone or comorbid with juxtapapillary duodenal diverticula (JPDD). MethodsA prospective study was conducted among 30 patients with common bile duct stones who were admitted to Department of Gastroenterology, Hebei General Hospital, from January to May 2024, and according to the presence or absence of JPDD, they were divided into JPDD group and simple common bile duct stones group (CBD group), with 15 patients in each group. Bile samples were collected during endoscopic retrograde cholangiopancreatography, and 16S rRNA microbial sequencing was performed to compare the differences in microbial composition, diversity, and metabolic pathways between the two groups. The independent-samples t test was used for comparison of normally distributed continuous data between two groups, and the Mann-Whitney U rank sum test was used for comparison of non-normally distributed continuous data between two groups; the chi-square test or the Fisher’s exact test was used for comparison of categorical data between two groups. Pearson correlation analysis or Spearman correlation analysis was used to analyze the correlation between clinical indicators and microbial species abundance. ResultsClinical data showed that compared with the CBD group, the JPDD group had significantly greater maximum diameter of stones (10.87±3.42 mm vs 6.80±2.08 mm, t=3.94, P0.01) and common bile duct diameter (14.73±3.95 mm vs 9.67±2.64 mm, t=4.13, P0.01). The microbiological analysis of the bile showed that Proteobacteria, Bacteroidetes, Firmicutes, and Actinobacteria were the most common phyla in both groups, and Proteobacteria was the dominate phylum in the JPDD group. At the genus and species levels, the JPDD group had higher relative abundances of Escherichia-Shigella, Enterococcus, and Escherichia coli. Alpha diversity was similar between the two groups, and there was a significant difference in beta diversity between the two groups (Adonis test, P0.05). The LEfSe analysis identified 25 differentially expressed species (LDA2) between the two groups, and the JPDD group had enrichment of 7 flora such as Enterobacter, Enterococcaceae, and Klebsiella, while the CBD group had significant enrichment of 18 flora such as Peptococcaceae, Roseburia, and Alistipes (P0.05). The correlation analysis showed that Enterococcaceae and Enterococcus significantly enriched in the JPDD group were positively correlated with the diameter of the common bile duct and the maximum diameter of stones (P0.01), whereas Peptococcaceae, Acinetobacter, and Alistipes with reductions in expression were negatively correlated with the diameter of the common bile duct and the maximum diameter of stones (P0.05). The enrichment analysis of biliary microbial metabolic pathways showed that there were significant differences between the two groups in 10 metabolic pathways such as cell growth and death, transportation and decomposition, nervous system, biosynthesis of valine, leucine, and isoleucine, and histidine metabolism (P0.05). ConclusionThe presence of JPDD may lead to alterations in bile microbiota, such as an increase in Enterococcus and a reduction in Roseburia, and specific flora and metabolism can promote the formation of common bile duct stones.
3.Predictive value of inflammatory markers for paroxysmal sympathetic hyperactivity after traumatic brain injury:a single-center retrospective case-control study

Objective To explore the value of inflammatory markers in predicting paroxysmal sympathetic hyperactivity(PSH)after traumatic brain injury(TBI).Methods A total of 84 TBI patients who were admitted to The Second Affiliated Hospital of Naval Medical University(Second Military Medical University)from Dec.2016 to Nov.2020 were retrospectively analyzed.They were classified into PSH group(n=41)and non-PSH group(n=43)according to whether PSH occurred during hospitalization.The baseline data and laboratory results of the 2 groups were collected and compared.Kendall correlation analysis was used to analyze the correlation between inflammatory markers and the occurrence of PSH after TBI,and receiver operating characteristic(ROC)curve was used to analyze the predictive value of inflammatory markers to PSH.Results There were no significant differences in baseline data,including age,gender,or Glasgow coma scale score,between the 2 groups(all P>0.05).Compared with patients in the non-PSH group,the neutrophil to lymphocyte ratio(NLR),platelet to lymphocyte ratio(PLR),systemic immune-inflammation index(SII),neutrophils and leukocytes in the PSH group were significantly increased(all P<0.05).NLR,SII and neutrophil were positively correlated with PSH(r=0.360,0.308,0.289;all P<0.01),with the corresponding ROC area under curve values being 0.752,0.716 and 0.702,respectively.Conclusion NLR,SII and neutrophils have a value in predicting the occurrence of PSH after TBI.
4.Flipped classroom and traditional lecture-based classes in pediatrics clerkship: analysis of teaching effectiveness feedback and optimization strategies

Objective:To investigate feedback on teaching effectiveness and optimization strategies for flipped classrooms in pediatric probation of eight-year clinical medicine programs, and to provide insights for enhancing the educational quality of pediatric probation.Methods:From June to September 2022, eight-year clinical medicine students from the class of 2018 undergoing pediatric probation were included in the study using purposive sampling. The subjects participated in one-on-one, offline, semi-structured, in-depth interviews to provide feedback on traditional lectures and flipped classroom teaching. The sample size was determined based on the principle of data saturation, with nine students participating in-depth interviews. NVivo 12 software was used for coding and classifying the interview data, which were then analyzed using thematic framework analysis to identify key themes.Results:Analysis of the interview data revealed three themes and ten sub-themes. Specifically, theme 1: The flipped classroom enhanced learning effectiveness but increased students' workload; theme 2: The current pediatric probation was scientifically designed but could be improved; theme 3: Several strategies exist for optimizing the flipped classroom teaching in pediatric probation.Conclusions:The flipped classroom in pediatric probation is in its early stages. While offering some benefits, further exploration and optimization are needed in curriculum design, student motivation, and supervision and evaluation methods.
5.Value of multiple diffusion weighted imaging models in predicting the efficacy of neoadjuvant-treated locally advanced gastric cancer

Objective To investigate the value of quantitative parameters of diffusion weighted imaging(DWI)based on mono-expo-nential model(MEM),diffusion kurtosis imaging(DKI)model,and stretched-exponential model(SEM)in predicting the efficacy of neoadjuvant therapy in locally advanced gastric cancer(LAGC).Methods Forty LAGC patients who underwent MRI examinations before neoadjuvant therapy and before radical surgery were prospectively enrolled.A radiologist delineated lesions on DWI images and acquired quantitative parameters before and after treatment,including lesion volume,apparent diffusion coefficient(ADC)of MEM,mean diffusivity(MD)and mean kurtosis(MK)of DKI model,distribution diffusion coefficient(DDC),and α of SEM.According to pathological tumor regression grade(TRG),the patients were stratified into good response group(TRG 0-1)and poor response group(TRG 2-3).The pre-treatment parameters and Δ of pre-and post-treatment parameters were compared between the two groups with Mann-Whitney U test;multivariate analysis was performed with binary logistic regression.Multiple DWI models and the combined model were established,and the prediction efficiency of each model was calculated.Results There was no significant differ-ence in each parameter before neoadjuvant therapy between the two groups(P>0.05).The delta of volume,ΔADC,ΔMD,and ΔDDC pre-and post-treatment were all statistically different between the two groups(P<0.05).The area under the curve(AUC)of ΔADC,ΔMD,and ΔDDC in predicting good response for LAGC were 0.900,0.806,and 0.762,respectively.The AUC of the combined model was 0.946.Conclusion Quantitative parameters of MEM,DKI model,and SEM can help predict the efficacy of neoadju-vant-treated LAGC patients.

9.Optimisation of fixed daily dose regimens for amikacin based on PPK auxiliary system of JPKD

AIM:To examine the predictive perfor-mance of the PPK software JPKD for the steady-state concentrations of amikacin and recommend the applicable conditions under fixed daily dosage of 400 mg and 600 mg.METHODS:Inpatients using amikacin in the First Affiliated Hospital of Bengbu Medical University from July 2022 to February 2024 were enrolled,and the measured concentra-tions of amikacin were detected by LC-MS/MS;Ver-ified the predictive performance of JPKD software for peak and trough concentrations of amikacin;JP-KD software was applied to predict the steady-state concentrations of amikacin in the patients at the infusion time of 0.5,1.0,1.5,2.0,2.5,and 3.0 h,and then compared the variability of steady-state concentrations with different levels of renal function at optimal infusion time,then the C max/MIC values were measured.RESULTS:A total of 69 patients were enrolled,including 18 patients with steady state trough concentrations and 17 patients with steady state peak concentrations.It was found that JPKD had a poor predictive ability for steady state trough concentrations but a good predictive ability for peak concentrations,the WRES<10%be-tween predictive and measured concentrations,and a strong correlation existed between them(r=0.806).With the shortening infusion time,the high-er peak concentrations.The predicted peak concen-trations at 0.5 h and 1.0 h infusion time groups were(34.81±6.87)μg/mL and(32.51±6.07)μg/mL,respectively.With the decline of the renal function,the peak concentrations showed a increasing trend.On the same level of renal function,the peak concentrations in the 600 mg group was high-er than that of the 400 mg group.When MIC ≤2 μg/mL,400 mg daily dose was chosen;when MIC=4 μg/mL,400 mg daily dose could be used for CKD3b stage patients,and 600 mg daily dose could be used for CKD1,CKD2,and CKD3a stage patients;when MIC=8 μg/mL,it was predicted that a higher dose was needed to achieve the expected target.CONCLUSION:Amikacin is preferably administered intravenously for 0.5 to 1.0 h,fixed daily doses of 400 mg and 600 mg are indicated for some pa-tients according to the target bacterial MIC and re-nal function.
10.Progress in irisin and its upstream and downstream antidepressants

Depression is a major cause of disability and has adverse effects.Despite the many types of anti-depressants,clinical treatments of depression remain poor.Therefore,novel anti-depressant mechanisms need to be explored.The beneficial effects of irisin on the nervous system are gradually being elucidated,and studies have found that irisin has an anti-depressant effect,which may become a new treatment for depression.This study explored the mechanism of irisin and its upstream and downstream anti-depressants by reviewing the existing studies explaining the link between irisin and depression,and proposes that SIRT1/PGC-1α may mediate FNDC5/irisin to regulate BDNF to promote neurogenesis and improve depression,which provides a new idea to study irisin and its upstream and downstream anti-depressants.
