1.Association of blood selenium exposure with sex hormones among men aged 18-79 years in China

Objective:To investigate the association between blood selenium levels and sex hormones in Chinese men aged 18-79 years.Methods:Data were derived from the China National Human Biomonitoring survey conducted in 2017-2018, with a final sample size of 5 414 men. General demographic characteristics, behavioral habits, and dietary frequency were collected through questionnaires and physical examinations. Fasting blood samples were collected to measure blood lead, serum testosterone, and estradiol levels. Complex sampling linear regression models were used to analyze the associations between blood selenium levels and testosterone, estradiol, and the testosterone/estradiol ratio, adjusting for confounding factors including age, education level, marital status, smoking status, alcohol consumption, seafood intake, soy product intake, protein supplement intake, BMI, and diabetes status.Results:The mean age of the 5 414 participants was (46.85±27.91) years; 4 774 (91.65%) were of Han ethnicity and 4 505 (86.68%) were married. The median ( Q1, Q3) blood selenium concentration in men was 97.80 (80.64, 116.99) μg/L. After adjusting for confounding factors, the complex sampling linear regression model revealed negative associations between blood selenium levels and both testosterone levels and the testosterone/estradiol ratio, with a significant linear trend ( Ptrend<0.05). Compared with the Q1 group, the β (95% CI) values for testosterone in the Q2, Q3, and Q4 groups were -0.02 (-0.06 to 0.02), -0.03 (-0.08 to 0.01), and -0.06 (-0.09 to -0.02), respectively. Similarly, the β (95% CI) values for the testosterone/estradiol ratio in the Q2, Q3, and Q4 groups were -0.01 (-0.03 to 0.02), -0.01 (-0.04 to 0.04), and -0.03 (-0.06 to -0.01), respectively. Subgroup analysis indicated stronger associations between blood selenium levels and testosterone/estradiol levels in non-smoking and obese men (BMI≥28 kg/m2). Conclusion:Blood selenium levels are negatively associated with testosterone levels and the testosterone/estradiol ratio in Chinese adult males.

JMJD1C (Jumonji Domain Containing 1C), a member of the lysine demethylase 3 (KDM3) family, is universally required for the survival of several types of acute myeloid leukemia (AML) cells with different genetic mutations, representing a therapeutic opportunity with broad application. Yet how JMJD1C regulates the leukemic programs of various AML cells is largely unexplored. Here we show that JMJD1C interacts with the master hematopoietic transcription factor RUNX1, which thereby recruits JMJD1C to the genome to facilitate a RUNX1-driven transcriptional program that supports leukemic cell survival. The underlying mechanism hinges on the long N-terminal disordered region of JMJD1C, which harbors two inseparable abilities: condensate formation and direct interaction with RUNX1. This dual capability of JMJD1C may influence enhancer-promoter contacts crucial for the expression of key leukemic genes regulated by RUNX1. Our findings demonstrate a previously unappreciated role for the non-catalytic function of JMJD1C in transcriptional regulation, underlying a mechanism shared by different types of leukemias.

3.Signal mining and analysis of adverse events of basiliximab used in solid organ transplantation based on FAERS database

Objective Based on the US Food and Drug Administration(FDA)adverse event Reporting System(FAERS),to explore the signal of adverse events of basiliximab in solid organ transplantation,and to provide reference for its clinical safety.Methods The adverse drug event(ADE)signals of basiliximab in solid organ transplantation from the first quarter of 2004 to the second quarter of 2024 were retrospectively analyzed in the FAERS database of the FDA.The reporting odds ratio(ROR)method and Bayesian confidence propagation neural network(BCPNN)method were used for signal mining.Results A total of 1,581 ADE reports related to the use of basiliximab were obtained,involving 27 system/organ classifications.A total of 460 ADE signals were identified.The top 5 PT signals in terms of report frequency were mainly elevated serum creatinine,fever,cytomegalovirus infection,decreased hemoglobin,and decreased lymphocyte count.The top 5 PT signals in terms of signal intensity were mainly cytomegalovirus infection,elevated serum creatinine,decreased lymphocyte count,increased neutrophil count,and thrombotic microangiopathy.New ADE signals identified included decreased lymphocyte count,thrombotic microangiopathy,eye organ diseases,Kaposi's sarcoma,elevated blood lactate dehydrogenase,elevated transaminases,and shock.Conclusion Data mining using FAERS database is helpful to find new ADE signals of basiliximab in solid organ transplantation.Clinicians should focus on new severe ADE signals such as coagulation function,ocular organ diseases,elevated transaminase,shock and so on.

6.Application and effect observation of ADOPT model in parturients with gestational diabetes

Objective To analyze the application of ADOPT(attitude,definition,open mind,planning,try it out)model in parturients with gestational diabetes.Methods A total of 120 parturients with gestational diabetes admitted to Nantong Maternal and Child Health Hospital from January 2022 to February 2023 were selected as research objects.They were divided into control group and observation group according to the admission order,with 60 cases in each group.The mean age of parturients in the control group was(28.35±5.06)years(range,24-35 years).The mean age of parturients in the observation group was(28.29±5.33)years(range,23-36 years).The control group received conventional care,and the observation group were given ADOPT mode.The psychological resilience scale score,diabetes self-efficacy scale score,and blood glucose level before the intervention and at 7 d of intervention,as well as the incidence of adverse pregnancy outcomes were compared between the two groups.Results Before the intervention,there was no significant difference in the maternal psychological resilience scale score,diabetes self-efficacy scale score or blood glucose level between two groups(all P>0.05).At 7 d of intervention,the scores of maternal health knowledge rating scale and diabetes self-efficacy scale of the observation group were higher than those of the control group,and fasting blood glucose and 2-hour postprandial blood glucose in the observation group were lower than those in the control group(all P<0.05).After the intervention,the incidence of adverse pregnancy outcomes in the observation group(6.67%in parturients,5.00%in newborns)was lower than that in the control group(20.00%in parturients,16.67%in newborns)(P<0.05).Conclusion The ADOPT model can significantly improve the psychological resilience,strengthen self-efficacy,effectively control blood glucose level,and reduce the incidence of adverse pregnancy outcomes in parturients with gestational diabetes.
7.Pregnancy complicated with antithrombin Ⅲ deficiency: a case report and literature review

Objective:To report the diagnosis and treatment of a pregnant woman with antithrombin (AT) Ⅲ deficiency in pregnancy, and combine the diagnosis and treatment ideas with the literature.Methods:The clinical diagnosis and treatment process of one pregnant woman with AT-Ⅲ deficiency admitted to the Second Affiliated Hospital of Zhengzhou University from October 2023 to June 2024 were retrospectively analyzed. Combined with relevant literature, the effect of pregnancy with AT-Ⅲ deficiency on pregnancy outcome was summarized, and reference was provided for the formulation of anticoagulant regimen.Results:The patient had 3 adverse pregnancy histories and a deep venous thrombosis of the lower limbs. The AT activity fluctuated between 34% and 50% during this pregnancy. The patient received 3 transfusions of fresh frozen plasma during pregnancy and was administered low molecular weight heparin from pre-pregnancy until 24 hours before delivery. Warfarin, hydroxychloroquine, cyclosporine and methylprednisolone were added in the second trimester. The pregnancy was terminated at 35 weeks of gestation. The infant was born with a weight of 2,330 g and was transferred to the pediatric department for treatment. After 14 days, the infant was discharged. Follow-up to date has not revealed any abnormalities.Conclusion:Precise treatment of pregnancy with AT-Ⅲ deficiency can effectively improve pregnancy outcomes.

9.Clinical Efficacy of Compound Tripterygium wilfordii Preparation(Xinfeng Capsules) in Improving Immune Inflammation of Rheumatoid Arthritis Due to Spleen Deficiency and Dampness Exuberance Based on Apriori Algorithm and Random Walk Model

ObjectiveTo investigate the effect of Xinfeng capsules on immunoinflammatory indicators in patients with rheumatoid arthritis (RA) due to spleen deficiency and dampness exuberance. MethodA total of 102 patients were randomly divided into control group and observation group according to the random number table method, with 51 cases in each group. All patients were treated with methotrexate tablets, while those in the observation group received additional Xinfeng capsules. The course of treatment in both groups was 12 weeks. The 28-joint disease activity score (DAS28), visual analogue scale (VAS) scores, morning stiffness time, erythrocyte sedimentation rate (ESR), high-sensitivity C-reactive protein (hs-CRP), rheumatoid factor (RF), anti-cyclic citrullinated peptide (anti-CCP) antibody, vascular endothelial growth factor (VEGF), and serum amyloid A (SAA) of the two groups before and after treatment were compared. The efficacy and incidence of adverse events were compared between the two groups. The Apriori association rule model and random walk model were constructed to evaluate the effect of Xinfeng capsules in improving hs-CRP, ESR, RF, SAA, VEGF, and anti-CCP. ResultThere were no dropouts in this study. There was no statistical difference in the indicators between the two groups before treatment. After 12 weeks of treatment, the total effective rate in the observation group was 90.19% (46/51), which was higher than 74.51% (38/51) in the control group (χ2=4.320,P<0.05). DAS28, VAS score, and morning stiffness time in the observation group were improved compared with those in the control group (P<0.05). Apriori association rule model results showed that the application of Xinfeng capsules in the observation group had a strong correlation with the reduction of RF, ESR, hs-CRP, SAA, and VEGF. The results of the random walk model showed that the improvement coefficients of hs-CRP, ESR, RF, SAA, and VEGF in the observation group were all better than those of the control group, and the improvement coefficient of anti-CCP in the control group was better than that of the observation group. The improvement degree of hs-CRP, ESR, RF, SAA, and VEGF in the observation group was superior to that of the control group (P<0.05). The incidence of adverse reactions in the observation group was lower than in the control group (χ2=4.057,P<0.05). ConclusionOn the basis of the treatment with methotrexate tablets, Xinfeng capsules can effectively improve the immunoinflammatory level in RA due to spleen deficiency and dampness exuberance and reduce the incidence of adverse reactions.
10.To explore the mechanism of tripterygium wilfordii-white peony in rheumatoid arthritis based on network pharmacology and animal experiments

Objective Using network pharmacology to explore the mechanism of action of tripterygium wilfordii-white peony in the treatment of rheumatoid arthritis(RA)and validating it using an adjuvant arthritis rat model.Methods Obtain the effective ingredients of tripterygium wilfordii and white peony through TCMSP database and predict the action targets;Obtain the disease targets of RA using DisGeNET and GeneCards online database;Draw the network of"tripterygium wilfordii-white peony-active ingredient-RA"and screen the core effective components using the software Cytoscape 3.9.1;Construct the protein-protein interaction(PPI)network using String database and screen the core targets using the CytoNCA plug-in,using the Metascape database for gene enrichment analysis of common targets,and using Auto Dock Tools and Pymol software for molecular docking of core active ingredients and targets.Enzyme linked immunosorbent assay(ELISA)detection of serum tumor necrosis factor-α(TNF-α),interleukin(IL)-4,IL-6,and IL-17 levels in adjuvant arthritis rats,Western blot was used to detect the expression of phosphorylated phosphatidylinositol 3-hydroxy kinase(p-PI3K)and phosphorylated AKT protein(p-AKT),validate the results of network pharmacology analysis.Results There were a total of 61 effective ingredients in the treatment of RA with tripterygium wilfordii-white peony,with 116 targets and 191 signaling pathways involved.Animal experiments have shown that compared with the model group rats,the serum cytokine TNF-α,IL-6 and IL-17 of rats in tripterygium wilfordii-white peony group and tripterygium wilfordii extract group were significantly decreased(P<0.05),IL-4 significantly increased(P<0.05);The expression of p-PI3K and p-AKT in synovial tissue was significantly reduced(P<0.05);Compared with rats in tripterygium wilfordii extract group,the TNF-α,IL-6,IL-17,p-PI3K and p-AKT of rats in tripterygium wilfordii-white peony group were significantly reduced(P<0.05),while IL-4 was significantly increased(P<0.05).Conclusion Tripterygium wilfordii-white peony can inhibit the overexpression of phosphatidylinositol 3-hydroxy kinase(PI3K)/protein kinase B(AKT)signaling pathway,regulate cytokine release,exert anti-inflammatory effects,and ultimately treat RA through multiple active ingredients and targets.
