Objective:
To identify early clinical predictors associated with dysphagia and delayed swallowing recovery in patients with traumatic brain injury (TBI).
Methods:
In this retrospective study, we enrolled adult TBI patients admitted to the rehabilitation unit of a tertiary medical center between June 2019 and June 2023. Data on baseline characteristics, neurological status, imaging findings, and rehabilitation-related variables were collected. Swallowing function was assessed using two indicators: (1) nasogastric (NG) tube retention and (2) the Functional Oral Intake Scale (FOIS) scores at 1, 4, and 12 weeks post-injury. Regression analyses were conducted to identify predictors associated with dysphagia and swallowing recovery.
Results:
A total of 160 patients were included. At 1 week post-injury, longer intensive care unit (ICU) stay, poor initial sitting balance and use of sedative medication in ICU were associated with NG tube retention. At 4 weeks, lower initial Rancho Los Amigos Scale (RLAS) scores, immobility-related complications, longer hospitalization, and temporal lobe hematomas were associated with persistent NG tube dependence. By 12 weeks, older age, delayed ability to follow commands, and poor initial sitting balance remained associated with NG tube retention. FOIS outcomes were also associated with older age, delayed time to follow commands, impaired initial sitting balance, prolonged ICU stay, temporal lobe hematomas, lower initial RLAS scores, immobility-related complications, prolonged endotracheal tube placement and extended hospital stays.
Conclusion
Impaired cognitive status, poor physical function, immobility-related complications, and temporal lobe hematomas were key factors associated with dysphagia and delayed oral intake in individuals with TBI.

Objective:To investigate the incidence, clinical characteristics, and complications of Torque teno virus(TTV)in children after hematopoietic stem cell transplantation(HSCT).Methods:A total of 40 children with hematological diseases who underwent HSCT were selected, and metagenomic next-generation sequencing(mNGS)technology was used to detect the gene sequences of pathogenic microorganisms in the blood.Combined with clinical data, the characteristics of TTV infection were analyzed. Results:Among the 40 pediatric patients post-HSCT,the TTV positive rate was 42.5%(17/40).There were no statistically significant differences between the TTV-positive group and the TTV-negative group in sex, age, white blood cell count(WBC),red blood cell count(RBC),hemoglobin, platelet count, neutrophil count, lymphocyte count, and high-sensitivity C-reactive protein(all P>0.05).The incidence of TTV infection was significantly higher in children who underwent haploidentical HSCT and in those with bone marrow stem cells(BMSC)as the transplant source(P<0.05).However,there were no significant differences in the TTV infection rate among patients with different disease types, different HLA matching statuses,or different engraftment times of neutrophils and platelets(all P>0.05).Among 17 children infected with TTV,13(76.5%)had co-infections with other viruses, mainly including cytomegalovirus(58.8%,10/17),human polyomavirus(41.2%,7/17),and Epstein-Barr virus(17.6%,3/17).In children with TTV infection, the most common complications were sepsis(82.4%),graft-versus-host disease(GVHD)(70.6%),pulmonary infection(41.2%),and hemorrhagic cystitis(17.6%).The incidence of GVHD in the TTV-positive group was significantly higher than that in the TTV-negative group(P<0.05).Conclusion:TTV infection is common in children undergoing HSCT,and it is prone to be complicated with cytomegalovirus infection and GVHD,which has an important influence on the clinical outcomes.

Objective To analyse the clinical efficacy and safety of acupuncture combined with intermittent catheterisation in treating postoperative urinary retention after radical surgery for cervical cancer, and provide reference for the clinical treatment of postoperative urinary retention. Methods A retrospective analysis was conducted based on the clinical data of patients who underwent radical hysterectomy for cervical cancer at Obstetrics and Gynecology Hospital,Fudan University, from Sept 2019 to Dec 2021 and subsequently developed urinary retention. The patients were treated with a combination of acupuncture and intermittent catheterization. We observed residual urine volume, duration of acupuncture treatment, and urinary tract infection status to evaluate the effectiveness and safety of the acupuncture combined with intermittent catheterization regimen. We examined the relationship between treatment duration and various factors such as age, pathological type,surgical-pathological staging, surgical method,pre-treatment catheter indwelling time,pre-treatment residual urine volume, presence or absence of postoperative radiotherapy and chemotherapy,pre-existing urinary tract infection, and whether ureteral stenting was performed,in order to analyze the factors affecting the treatment duration. Results A total of 156 patients were reviewed. After treatment,132 cases had a post-treatment residual urine volume<100 mL,7 cases had a volume of 100-200 mL,and 15 cases had a volume>200 mL.Two cases still had great difficulty in urination after treatment, resulting in an overall effectiveness rate of 89.1%.No significant adverse events occurred in the patients. The treatment duration ranged from 2 to 17 days, with an average treatment time of 6.2 days. The treatment duration was a correlated with the preoperative indwelling catheter duration, residual urine volume and postoperative radiotherapy. Conclusion Acupuncture combined with intermittent catheterisation in treating urinary retention after radical surgery for cervical cancer has good efficacy, short treatment time and no obvious adverse effects, and can be an option for treating postoperative urinary retention.

Ventricular assist devices(VADs)have emerged as a pivotal therapeutic option for end-stage heart failure, undergoing significant technological advancements from pneumatic membrane pumps to continuous-flow centrifugal pumps. This review provides a detailed overview of several VAD models, including HeartMate Ⅲ,EVAHEART 2,and Heart Con.The initial generation of VADs established a foundation for subsequent developments, despite the occurrence of frequent complications. The second generation of VADs demonstrated a notable improvement in patient prognosis but carried the risk of ventricular collapse. The third generation of VADs represents a further reduction in size, incorporating magnetic levitation technology to enhance durability and blood compatibility. In the future, the development of VADs will focus on better emulating the physiological function of the native heart, improving pulsatility, and extending device longevity. As technology advances,VADs are expected to offer heart failure patients with a greater range of treatment options and a higher quality of life.

Following extensive interdisciplinary research and development over several years, mechanical circulatory support devices(MCSD),including ventricular assist device(VAD)and total artificial heart(TAH),are now established as vital treatment options for patients with advanced heart failure. These devices have proven to be crucial in assisting or replacing a failing heart, offering patients a new lease of life and improving their quality of life.Currently, mechanical circulatory support(MCS)has become a well-recognised,long-term treatment option for patients who are unable to undergo heart transplantation due to donor organ shortages or contraindications.Given their continuous availability independent of donor organ limitations, these devices are poised to play an increasingly vital role in the future of medicine. This article aims to summarize the evolution, clinical applications, categorization, and potential complications of MCSD.

Objective To develop a novel type of transparent simulation manikin as a surgical training model to meet the surgical treatment demand on the medical train. Methods A transparent manikin was developed with the steps of basic data collection, motherboard design and manufacture and module production and assembly.Firstly, basic data collection was carried out with reference to standardized human anatomy and parameters.Secondly, some software such as UG NX7.5 was used to construct the motherboard of the manikin. Finally, module production and assembly were performed with the materials of acrylic, transparent rubber, silicone and hydrogel and the technology of silicone infusion. Results The transparent manikin developed had its anatomy structure close to that of the real body and high visuality for its internal and external components, which simulated a variety of war wounds and thus could be integrated with the surgical training scenarios on the medical train effectively. Conclusion The transparent manikin developed is characterized by high visuality, modularity and blood flow, and meets the demands for surgical training on the medical train.[Chinese Medical Equipment Journal,2025,46(6):111-115]
