1.Serum β2-MG, sCHE, and PSGL-1 Expression in Patients with Esophageal Cancer and Their Association with Postoperative Lung Infection After Mediastinoscopy

Objective To investigate serum β2-MG, sCHE, and PSGL-1 expression in patients with esophageal cancer and their relationship to lung infection after mediastinoscopy. Methods A total of 118 patients with esophageal cancer were selected and divided into infected and uninfected groups according to whether they developed lung infection after surgery. An automatic microbiological identification system was used to detect the pathogenic bacteria of lung infection. ELISA was used to detect the levels of β2-MG, sCHE, and PSGL-1. Multivariate logistic regression was used to analyze the influencing factors of postoperative lung infection in patients with esophageal cancer. ROC curves were plotted to analyze the assessment value of serum β2-MG, sCHE, and PSGL-1 on postoperative lung infection. Results Fifty-two strains of bacteria were isolated from the sputum of 38 patients with postoperative lung infections, and these included 35 (67.31%) Gram-negative, 14 (26.92%) Gram-positive, and 3 (5.77%) fungal strains. The difference in long-term smoking history between the infected and uninfected groups was statistically significant (P<0.05). Serum β2-MG and PSGL-1 levels were significantly higher and sCHE levels were significantly lower in the infected group than in the uninfected group (P<0.05). Serum β2-MG and PSGL-1 levels were sequentially higher (P<0.05) and sCHE levels were sequentially lower (P<0.05) in the mild, moderate, and severe lung infection groups. Long-term smoking history, β2-MG, and PSGL-1 were risk factors affecting postoperative lung infection in patients with esophageal cancer (P<0.05), and sCHE was a protective factor (P<0.05). The AUCs of serum β2-MG, sCHE, and PSGL-1 for assessing postoperative lung infections were 0.807, 0.845, and 0.800, respectively, and the AUC of the three combined factors for assessing postoperative lung infections was 0.954, which was superior to that assessed individually (Zcombination vs. β2-MG=2.576, Zcombination vs. sCHE=2.623, Zcombination vs. PSGL-1=2.574, all P<0.05). Conclusion The serum levels of β2-MG and PSGL-1 increase and the sCHE level decreases in patients with esophageal cancer and postoperative pulmonary infection, which are also related with lung infection. Combined testing can improve the evaluation value of postoperative pulmonary infection in patients.
2.Analysis of the associated factors and cumulative effects of cardiometabolic multimorbidity among residents in southern Xinjiang

Objective:To analyze the associated factors and cumulative effects of cardiometabolic multimorbidity (CMM) among residents in southern Xinjiang.Methods:A stratified random cluster sampling method was used to conduct questionnaire surveys, physical examinations and laboratory tests among the personnel of the 51st Brigade, 3rd Division, Xinjiang, in 2016. The multivariate logistic regression, multivariate linear regression, restricted cubic spline, and network analysis methods were used to study the association of lifestyle (smoking, alcohol consumption and physical activity), socioeconomic (occupation, education and marital status) and clinical factors (waist circumference, body mass index and family history) with CMM.Results:A total of 12 773 study subjects were included. The prevalence of cardiovascular metabolic diseases among residents in southern Xinjiang was 52.49%. Specifically, the prevalence rates of dyslipidemia, hypertension, coronary heart disease, diabetes, and stroke were 31.14%, 29.95%, 6.78%, 6.26%, and 2.47%, respectively, and the prevalence of CMM was 19.06%. Multivariate logistic regression analysis revealed that the associations between clinical and socioeconomic factors and CMM significantly increased with higher scores. Specifically, the OR rose from 1.75 (clinical factors) and 1.07 (socioeconomic factors) on a score of 1 to 4.41 and 1.93 on a score of 3, respectively. The association between lifestyle factors and CMM was only observed at higher scores ( OR=1.26, 95% CI:1.07~1.62). The trend test using the scores of each group as continuous variables in the model showed that the risk of disease increased with the accumulation of clinical, socioeconomic and lifestyle factors (all P<0.05). Restricted cubic spline analysis demonstrated a non-linear relationship between the total number of associated factors and CMM ( Poverall<0.05 and Pnon-linear<0.05). Network analysis identified hypertension (strength=0.42) as the “core node” among the five diseases. When analyzing the three types of influencing factors, hypertension (strength=0.68), dyslipidemia (strength=0.47), coronary heart disease (strength=0.37), and clinical factors (strength=0.53) emerged as “core nodes”. In the network of nine associated factors, abnormal waist circumference and BMI (strength=0.90 and 0.84) were identified as “key factors”, while hypertension (strength=0.68) and dyslipidemia (strength=0.52) were identified as “key diseases”. Conclusion:The prevalence of CMM among residents in southern Xinjiang is high, and there is a cumulative effect of multiple factors. Hypertension and dyslipidemia are key diseases in the multimorbidity network, while abnormal BMI and waist circumference are key associated factors.

4.Application research of hospital track logistics system

Objective Based on running record of hospital track logistics system of a large comprehensive tertiary hospi-tal in Guangdong-Hong Kong-Macao Greater Bay Area,perform statistical analysis and discussion,evaluate the application effect and provide a reference for further optimization of the system operation.Methods Use a directed graph to describe the network topology of the system,establish a multi-commodity flow model,and then select a week during the run-in operation period,nor-mal operation period,and holiday operation period,export the operation log,perform descriptive statistics,X2 test and interval estimation.Results The number of transportation vehicles during the normal operation period increased by 23.94%compared with the running-in operation period.The average transportation time during the running-in operation period,normal operation pe-riod,and holiday operation period were 1 015.63 seconds,1 176.83 seconds,and 1 106.38 seconds respectively;The lowest ranking is afternoon,morning,evening,and early morning;divided by date,the number of transported vehicles on weekdays is higher than that on rest days;the nodes with the largest proportion of sent transported vehicles are the central pharmacy and the inpatient building,and the nodes with the largest proportion of received transported vehicles are the inpatient building and the la-boratory department;the main track carrying relatively heavy transportation tasks is closely related to the important nodes.Conclusion The operation data of the hospital track logistics system should be fully utilized,comprehensive and in-depth analy-sis should be carried out in time,and the application effect should be evaluated,so as to provide a basis for further strengthening the rational allocation and use of related resources.
5.Research on the security governance path and countermeasure of information-based medical equipment maintenance management data

Objective:To study the clinical application value of the data security governance path and countermeasures of information-based medical equipment maintenance management. Methods:The User Service Two Context (US2C) model was used to divide data governance indicators of medical equipment maintenance management,and the indicators of governance capability,governance effect and governance efficiency were evaluated from the conditional factors of users,services,internal environment and external environment,the necessity of antecedent factors and the coverage rate of combination conditions were analyzed by fuzzy set qualitative comparative analysis (fsQCA),and the security governance management path of security,data volume and system standardization was developed. The maintenance management data of 285 sets of medical equipment in clinical use in The Sixth Affiliated Hospital of South China University of Technology (Foshan Nanhai District People's Hospital) from 2020 to 2023 were selected,and the maintenance management data of 256 sets of equipment in the period from 2020 to 2021 adopted conventional management mode,and the maintenance management data of 268 sets (including 239 sets in use in conventional management) of medical equipment during 2022-2023 adopted the security governance model. A self-made questionnaire was used to investigate whether the amount of maintenance management data met the actual needs of 120 participants who used the equipment. The security,frequency of use and operation quality of medical equipment maintenance management data of the two management modes were compared. Results:The average risk rate of medical equipment maintenance management data using security governance mode was (1.333±0.741)%,which was lower than that of conventional management mode,the average accuracy and effectiveness were (93.925±3.432)%and (88.500±4.404)%,respectively,Which were higher those of the conventional management mode,the differences was statistically significant (t=2.298,4.784,P<0.05). The average satisfaction rate of the 120 participants who used the equipment was (94.367±3.093)% of the medical equipment maintenance management data using the safety governance model,which was higher than that of the conventional management mode,and the difference was statistically significant (t=4.365,P<0.05). The frequency of use of medical equipment failure data,maintenance item data and management input data of the security governance mode were (4.092±1.709) times/year,(4.341±1.397) times/year and (3.821±0.976) times/year,respectively,which were higher than those of the conventional management mode,the difference was statistically significant (t=2.101,4.400,6.386,P<0.05). The failure frequencies of medical electronic equipment,life support equipment,operating room equipment,medical laboratory equipment and other medical equipment using the security governance mode were (0.831±0.335) times/year,(0.927±0.293) times/year,(1.085±0.439) times/year,(0.702±0.047) times/year and (1.456±0.485) times/year,respectively 0.485),which were lower than those of the conventional management mode,and the difference was statistically significant (t=5.330,5.486,4.756,6.974,3.598,P<0.05). Conclusion:The data security governance path and management countermeasures of information-based medical equipment maintenance management can standardize the medical equipment maintenance management data collection process,improve data security performance,improve data application efficiency,and improve equipment operation quality.

8.Application of the bronchial priority treatment method in single-port thoracoscopic right upper lung lobectomy

Objective:To explore the safety, effectiveness, economy and surgical techniques of bronchial priority treatment in single-port thoracoscopic right upper lobectomy by comparing it with conventional single-port thoracoscopic right upper lobectomy.Methods:Clinical data of 72 patients who underwent single-port thoracoscopic right upper lobectomy from Mar. 2019 to Feb. 2022 were collected. According to different surgical treatment sequences, the patients were divided into observation group (bronchial priority treatment, 36 cases) and control group (conventional surgery, 36 cases). The general clinical characteristics, operation time, intraoperative blood loss, postoperative hospital stay, postoperative complications, postoperative pain score, and number of staplers used in the two groups were compared.Results:All operations were successfully completed without conversion to thoracotomy. There was no significant difference between the two groups in clinical characteristics, intraoperative blood loss [ (25.3±12.8) ml vs 32.5±14.2) ml, P>0.05], postoperative hospital stay[ (4.7±1.6) d vs (4.9±1.5) d, P>0.05], postoperative pain score [ (3.3±1.1), (4.8±1.4), (3.7±1.1) vs (3.5±1.2), (5.5±1.4), (4.1±1.4), P>0.05], number of lymph node dissection (9.1±1.8 vs 8.3±1.7, P>0.05), or postoperative complications (16.7% vs 27.8%, P>0.05). Compared with the control group, the observation group had significant advantages in the operation time [ (87.2±6.1) vs (106.4±21.8) min, P<0.05] and the number of staplers used (3.7±0.8 vs 5.8±1.3, P<0.05) . Conclusions:Single-port video-assisted thoracoscopic right upper lobe resection with bronchial priority treatment is safe and effective. It simplifies the surgical procedure, reduces the use of disposable consumables, does not increase the risk of perioperative period, and has clinical application prospects.
9.Evaluation of the effect of continuous spiral suture in cervical anastomosis of endoscopic esophageal cancer surgery

Objective:By comparing the advantages and disadvantages of different forms of purse suture, to explore how to minimize the incidence of anastomotic complications after cervical anastomosis of esophageal cancer.Methods:The clinical data of 45 patients with esophageal cancer who underwent mediastinal endoscopy combined with laparoscopic radical resection of esophageal cancer from Jan.2019 to Jun.2020 in Department of Thoracic Surgery, Henan Chest Hospital were selected. In the observation group, 22 cases were sutured with spiral packing at the esophageal stump, and in the control group, 23 cases were sutured with conventional loading forceps. The clinical effects of the two groups were objectively evaluated.Results:There was no significant difference between the two groups in operation time, intraoperative blood loss or hospitalization days ( P>0.05) . In terms of postoperative complications, the incidence of anastomotic leakage and anastomotic stenosis in the observation group (4.54%, 9.09%) was significantly lower than that in the control group (17.39%, 39.13%) , and there was significant difference in the incidence of anastomotic stenosis ( P<0.05) . Conclusion:The spiral continuous suture of esophageal stump can reduce the incidence of anastomotic fistula/anastomotic stenosis without increasing surgical trauma or prolonging operation time, which is worthy of clinical application.
10.Use of autologous fat grafting in prevention and treatment of expansion complications

Objective:To explore the effectiveness of autologous fat grafting in the treatment of undesirable skin expansion.Methods:Patients' data were reviewed from 2011 to 2016, including the expanded regions with early signs of skin complications in face and neck. The effects of fat grafting group and control group were evaluated by follow-up records of expansion volume, skin thickness, skin texture and local capillary reaction.Results:Fat grafting could increase the thickness of expanded skin. It also improved the texture of expanded skin, with 0.83± 0.71 points before treatment and 1.30±0.66 points after treatment ( P=0.04). The local capillary reaction was also improved from 1.06±0.54 points before treatment and 1.45±0.51 points after treatment ( P=0.03). The expansion in the fat grafting group was 2.21±0.57 times before treatment and 2.94±0.83 times after treatment. In the control group, the expansion was 2.19 times when it showed early signs, and no obvious changes were observed during the follow-up period. Conclusions:Autologous fat grafting can effectively treat complications of skin expansion, prolong expansion process and promote tissue regeneration.
