1.Exploration on the Mechanism of Intervention of Modified Sijunzi Decoction in Premature Ovarian Failure Based on PI3K/AKT Signaling Pathway Mediated by PTEN

Objective To explore the mechanism of modified Sijunzi Decoction in the intervention of premature ovarian failure(POF)rats through PTEN-mediated PI3K/AKT signaling pathway.Methods Totally 50 SPF-grade healthy female SD rats were divided into normal group(10 rats)and model group(40 rats).The model group was injected with cyclophosphamide to establish a POF model,and then randomly divided into model group and TCM low-,medium-and high-dosage group,with 10 rats in each group,TCM low-,medium-and high-dosage group were given modified Sijunzi Decoction 6.3,12.6,25.2 g/kg by gavage,the normal group and model group were given equal volume of normal saline,once a day for 28 days.The general condition and body mass of rats in each group were observed and recorded daily.After administration,the ovarian index was measured,HE staining was used to observe the morphology of ovarian tissue,ELISA was used to detect the contents of serum estradiol(E2)luteinizing hormone(LH),follicle-stimulating hormone(FSH),anti-Müllerian hormone(AMH),gonadotropin-releasing hormone(GnRH)and β-endorphin(β-EP),RT-PCR was used to detect the mRNA expression of PTEN,PI3K and AKT in ovarian tissue,Western blot was used to detect the protein expression of PTEN,PI3K,AKT,p-PI3K and p-AKT,immunohistochemistry was used to detect the positive expression of PTEN,PI3K and AKT.Results Compared with the normal group,the rats in the model group were depressed,the hair fell off,the diet and drinking water were reduced,the stools became thinner,the body mass was reduced(P<0.05),and the ovarian index was reduced(P<0.05),the ovarian tissue structure was disordered,the number of follicles decreased,and the contents of serum LH,FSH and GnRH increased(P<0.05),the contents of E2,AMH and β-EP decreased(P<0.05),the expression of PTEN mRNA and protein decreased(P<0.05),the expressions of PI3K and AKT mRNA and protein increased(P<0.05),and the expressions of p-PI3K and p-AKT protein increased(P<0.05).Compared with the model group,the general condition of rats in each dosage of TCM groups improved significantly,the body mass increased(P<0.05),ovarian index increased(P<0.05),the number of follicles in rat ovarian tissue increased,the contents of LH,FSH and GnRH decreased(P<0.05),the contents of E2,AMH and β-EP increased(P<0.05),the expression of PTEN mRNA and protein increased(P<0.05),the expression of PI3K and AKT mRNA and protein decreased(P<0.05),and the protein expression of p-PI3K and p-AKT decreased(P<0.05).Conclusion Modified Sijunzi Decoction may regulate the PI3K/AKT signaling pathway mediated by PTEN,repair the excessive consumption of follicles in ovarian tissue of POF rats,improve the ovarian function,so as to intervene the process of POF.

3.Analysis of drug therapy in a patient with pulmonary infection secondary to suppurative thrombophlebitis

Objective To explore the role of pharmaceutical care in the treatment of patients with pulmonary infection secondary to suppurative thrombophlebitis. Methods The treatment of a patient diagnosed with pulmonary metastatic infection secondary to suppurative thrombophlebitis and the whole process of clinical pharmacists participating in the monitoring were analyzed retrospectively. The use of antibiotics was evaluated, and the experience of coagulation management in suppurative thrombophlebitis was explored. Results Based on the infection site, characteristics of septic thrombus, monitoring of vancomycin blood concentration, pharmacokinetics and pharmacodynamics characteristics of antibiotics, clinical pharmacists provided comprehensive pharmaceutical services for clinicians and patients in terms of anti-infection scheme adjustment, optimization of vancomycin individualized treatment, anticoagulant timing. Patient’s systemic infection and septic thrombus can be effectively controlled and which promotes the treatment of patients with suppurative thrombophlebitis. Conclusion Clinical pharmacists can play an important role in the treatment team of severe patients to improve the rational use of antibiotics.

Objective To study the features,general patterns and predisposing factors of adverse drug reactions(ADR) occurred in our hospital and to provide the guidance for rational and safe use of drugs in clinic.Methods 302 cases of ADR were reported from September 2015 to September 2017 in our hospital.Age distribution,gender,route of administration,drug categories,systems or organs involved,clinical manifestations,adverse reaction types involved in ADRs were analyzed retro-spectively.Results Among the 302 ADR reports,137 cases are male patients(accounting for 45.4%)and 165 cases were fe-male patients(accounting for 54.6%).27.8% of ADR was related to anti-infective agents.70.2% of ADR came from oral ad-ministration and 24.8% ADR was due to intravenous administration.The main clinical manifestations of ADR were lesion of skin and its appendants(61.6%)and followed by the digestive system injury(15.9%).Conclusion The multiple factors may lead to high frequency of ADR.In order to promote rational drug use and reduce the occurrence of ADR,clinicians should pay attention to the ADR monitoring and strictly follow the clinical indications of medications.

Objective To explore the clinical significance of minimal residual disease (MRD) and WT1 in patients with acute myeloid leukemia (AML).Methods 349 bone marrow specimens of 42 patients with AML (except M3) before and after chemotherapy were collected.The expression level of MRD was determined by flow cytometry (FCM) and WT1 gene was detected by real time quantitative PCR (RQ-PCR).Results No correlation was observed between WT1 gene expression level of AML patients before chemotherapy with treatment response (x2 =0.166 3,P > 0.5).34 AML patients achieved morphological complete remission (CR) after the first course of inductive chemotherapy.The recurrence rate was significantly higher in patients with high MRD level than that in those with low MRD level (63.6 % vs 21.7 %) (x2 =5.729,P < 0.025),and significantly higher in patients with high WT1 expression than that in those with low WT1 expression (87.5 % vs 23.1%) (x2 =10.749,P < 0.005).WT1 expression level of patients with CR was significantly lower than that of patients without CR (t =4.669,P < 0.001).The recurrence rate of patients with both low level of MRD and WT1 was significantly lower than those of either high level of MRD and WT1 (15.0 % vs 64.3 %,P < 0.05).The level of WT1 in patients with CR had positive correlation with the level of MRD (r =0.835,P < 0.001).Conclusions MRD and WT1 have great importance in evaluating the therapeutic effects among patients with AML and increasing the positive rate of MRD.They also provide experimental basis for the individual and gradation treatment in the clinic.
