1.Study on preoperative bedside ultrasonography-assessed perioperative reflux and aspiration risk in surgical colon cancer patients with incomplete intestinal obstruction

Objective:To investigate the correlation between preoperative incomplete intestinal obstruction and the risk of perioperative reflux and aspiration assessed by preoperative bedside ultrasonography in patients with colon cancer.Methods:A prospective case series study was conducted. A total of 300 patients with colon cancer who underwent elective surgery from March 2020 to January 2022 in the First Hospital of Shanxi Medical University were prospectively selected for the study. According to whether there was incomplete intestinal obstruction before operation, the patients were divided into obstruction group and non-obstruction group. All patients underwent preoperative ultrasound examination of the gastric antrum, measuring the cross-sectional area of the gastric antrum in the right lateral position, and calculating the gastric volume and unit body mass gastric volume (GV/W), which were all expressed as M ( Q1, Q3). The differences in cross-sectional area of the gastric antrum, gastric volume and GV/W in the right lateral position between the two groups were compared, as well as the risk of reflux and aspiration (the risk of reflux and aspiration was extremely low at GV/W<0.8 ml/kg, the risk was low at GV/W 0.8-1.5 ml/kg, and the risk was high at GV/W >1.5 ml/kg). Multivariate logistic regression was used to analyze the influencing factors of high risk of reflux and aspiration. Results:Of 300 patients, 7 patients were excluded because of blurred images on ultrasonography, and a total of 293 patients were finally included. The age [ M ( Q1, Q3)] of obstruction group (146 cases) was 58.0 years old (55.0 years old, 67.0 years old), including 80 males and 66 females; the age of non-obstruction group (147 cases) was 55.0 years old (53.5 years old, 64.0 years old), including 64 males and 83 females. The age and body mass index (BMI) of the patients in the obstructin group were higher than those in the non-obstruction group, and the differences were statistically significant (both P < 0.05), and there were no significant differences in gender, duration of dietary abstinence, comorbid hypertension, comorbid diabetes mellitus, smoking status, sedentariness, and alcoholism between the two groups (all P > 0.05). Compared with the non-obstruction group, patients in the obstruction group had a higher cross-sectional area of the gastric antrum [5.83 cm 2 (5.25 cm 2, 6.70 cm 2) vs. 5.13 cm 2 (4.43 cm 2, 5.79 cm 2), P < 0.001], gastric volume [43.00 ml (37.07 ml, 52.74 ml) vs. 32.78 ml (25.52 ml, 39.85 ml), P < 0.001] and GV/W [0.70 ml/kg (0.65 ml/kg, 0.82 ml/kg) vs. 0.55 ml/kg (0.46 ml/kg, 0.62 ml/kg), P < 0.001] in the right lateral position. The proportion of patients at high risk of reflux and aspiration in the obstruction group was higher than that in the non-obstruction group [22.6% (33/146) vs. 12.2% (18/147), χ2 = 4.59, P = 0.032]. Multivariate logistic regression analysis showed that combination of incomplete intestinal obstruction (combined vs. uncombined, OR = 2.145, 95% CI: 1.096-4.198, P = 0.026), and males (females vs. males, OR = 0.415, 95% CI: 0.199-0.867, P = 0.019) were the independent risk factors for a high risk of perioperative reflux and aspiration assessed by preoperative bedside ultrasonography. Conclusions:Colon cancer patients combined with incomplete intestinal obstruction have significantly larger preoperative cross-sectional area of the gastric antrum, gastric volume and GV/W in the right lateral position, and have gastric retention, which may contribute to a higher risk of perioperative reflux and aspiration.
2.Development and validation of an Exercise Behavior Perception Scale for Lung Cancer Patients

Objective To develop an Exercise Behavior Perception Scale for Lung Cancer Patients in China,and to test its reliability and validity.Methods According to the health belief model,the first draft of the scale was formed through literature review,semi-structured interview,research group discussion,2 rounds of expert correspondence and pre-survey.A total of 842 patients with lung cancer who received treatment in a tertiary A hospital in Zhengzhou City from October 2022 to April 2023 were selected for investigation,and item analysis,validity analysis,and reliability analysis were applied to adjust the items to form a formal scale.Results The Exercise Behavior Perception Scale for Lung Cancer Patients contained 24 items,and exploratory factor analysis generated 6 common factors,including physical rehabilitation benefits,emotional and mental benefits,information disorders,disease symptom disorders,condition disorders,and psychological disorders.The cumulative variance contribution rate reached 73.916%,and the results of confirmatory factor analysis showed that the model fit was good,the x2/df was 2.371,the root mean square error of approximation was 0.057,the normed fit index was 0.924,and the comparative fit index was 0.954,the scale score was moderately positively correlated with the total score of the Frenchay activities index(r=0.529,P<0.001).The Cronbach's α,coefficient,half reliability,and retest reliability of the scale were 0.884,0.741,and 0.889,respectively.Conclusion The Exercise Behavior Perception Scale for Lung Cancer Patients had good reliability and validity,and it can be used to effectively evaluate the exercise behavior perception status of lung cancer patients.

Objective:To investigate the current status of knowledge, attitude and practice regarding perioperative lung rehabilitation for lung cancer among nurses of Thoracic Surgery Department, and explore its influencing factors, so as to provide a basis for promoting clinical lung rehabilitation training.Methods:This study was a cross-sectional survey. From May to August 2022, 513 nurses of Thoracic Surgery Department from 10 ClassⅢ hospitals in Henan Province were selected as survey subjects using convenience sampling. Nurses were surveyed using a self-designed Thoracic Surgery Nurses' Perioperative Lung Rehabilitation Knowledge, Attitude, and Practice Questionnaire for Lung Cancer. Multiple linear regression was used to analyze the influencing factors of nurses' knowledge, attitude, and practice. A total of 485 valid questionnaires were collected, with an effective response rate of 94.54% (485/513) .Results:The total score, knowledge dimension score, attitude dimension score, and practice dimension score of knowledge, attitude, and practice of perioperative lung rehabilitation for lung cancer among 485 nurses of Thoracic Surgery Department was (186.25±33.55), (76.36±16.44), (43.27±6.39), and (66.62±15.71), respectively. The results of multiple linear regression analysis showed that the influencing factors of the total score of knowledge, attitude, and practice of perioperative lung rehabilitation for lung cancer among nurses of Thoracic Surgery Department were their post, whether they understood the content of lung rehabilitation, the times they participated in lung rehabilitation training, and whether they implemented lung rehabilitation for patients ( P<0.05) . Conclusions:Thoracic Surgery Department nurses have a positive attitude in perioperative lung rehabilitation for lung cancer, but their knowledge and behavior urgently need to be improved. Nursing managers should actively carry out perioperative lung rehabilitation training, encourage Thoracic Surgery Department nurses to update their self-knowledge system, strengthen their lung rehabilitation execution ability, and further promote the development of lung rehabilitation related work.
6.Analysis of invasion characteristics and prognostic factors of patients with Masaoka-Koga stage Ⅲ thymoma

Objective:To analyze the invasion characteristics and prognostic factors of patients with Masaoka-Koga stage Ⅲ thymoma.Methods:The tumor invasion characteristics of 179 patients who were diagnosed with Masaoka-Koga stage Ⅲ thymoma and treated in Affiliated Cancer Hospital of Zhengzhou University from January 2000 to June 2018 were analyzed retrospectively. According to the treatment methods, all patients were divided into the radical operation group ( n=94), palliative operation group ( n=39) and simple biopsy group ( n=46). The χ2 test was used to compare the classified variables, Kaplan- Meier method was utilized to calculate the cumulative survival rate, log-rank method was used for group comparison and univariate analysis, and Cox’s regression model was used for multivariate analysis. Results:Mediastinal pleural invasion (86.0%) was the most common site, followed by pericardium (50.8%), great vessel (40.8%) and lung (36.3%). The proportion of macrovascular invasion in the radical operation group was 14.9%, significantly lower than 79.5% and 60.9% in the palliative surgery group and biopsy group (both P<0.001). Multivariate analysis showed that the nature of operation ( P<0.001), age ( P=0.011), radiotherapy ( P=0.020) were the independent factors affecting overall survival (OS), while nature of operation ( P<0.001), age ( P=0.004), radiotherapy ( P=0.020), number of invasive organs ( P=0.023) and pathological type ( P=0.016) were the independent factors affecting progress-free survival (PFS). Conclusions:For patients with Masaoka-Koga stage Ⅲ thymoma, mediastinal pleura is the most common site of invasion, pericardium, lung and great vessels are also commonly invaded. The invasion of mediastinal pleura, pericardium and lung exerts slight effect on surgical resectability, whereas great vessel involvement can significantly affect surgical resectability. OS and PFS in patients undergoing radical resection are significantly better than those in patients treated with palliative resection and biopsy. Radical resection is the most important factor affecting prognosis.
7.Current status of knowledge, attitude and practice of nurses in Thoracic Surgery Department on thoracic drainage tube nursing after pneumonectomy

Objective:To explore the current status and influencing factors of knowledge, attitude and practice of nurses in Thoracic Surgery Department on thoracic drainage tube nursing after pneumonectomy, aiming to provide a basis for the training of clinical thoracic surgery specialist nursing personnel.Methods:Using the convenient sampling method, a total of 319 nurses in Henan Province were selected as research objects from August 13 to November 13, 2020, and self-designed questionnaire on knowledge, attitude and practice of thoracic drainage tube nursing after pneumonectomy was used to investigate. A total of 319 questionnaires were issued and 300 valid questionnaires were returned, with an effective recovery rate of 94.04%.Results:The total score of knowledge, attitude and practice of 300 Thoracic Surgery Department nurses on thoracic drainage tube nursing after pneumonectomy and scores of knowledge, attitude and behavior dimensions were respectively (72.09±6.65) , (7.96±2.51) , (26.04±2.24) and (38.09±4.78) . The results of multiple linear regression analysis showed that main influencing factors of nursing knowledge of Thoracic Surgery Department nurses on thoracic drainage tube after pneumonectomy were education level, professional title and position ( P<0.05) . The influential factor of attitude was working years ( P<0.05) . The influencing factors of behavior were knowledge, attitude, position and whether participated a thoracic drainage training or not ( P<0.05) . The influencing factor of total score of knowledge, attitude and practice was professional title ( P<0.05) . Conclusions:Thoracic Surgery Department nurses have a positive attitude towards thoracic drainage tube nursing after pneumonectomy, but their knowledge and behavior level need to be improved. Nursing leaders should carry out thoracic drainage tube nursing training according to the different characteristics of nurses. At the same time, they need to encourage nurses to actively learn new knowledge and constantly update their own knowledge system, so as to change nursing behavior and improve the quality of thoracic surgery.

When applying deep learning to the automatic segmentation of organs at risk in medical images, we combine two network models of Dense Net and V-Net to develop a Dense V-network for automatic segmentation of three-dimensional computed tomography (CT) images, in order to solve the problems of degradation and gradient disappearance of three-dimensional convolutional neural networks optimization as training samples are insufficient. This algorithm is applied to the delineation of pelvic endangered organs and we take three representative evaluation parameters to quantitatively evaluate the segmentation effect. The clinical result showed that the Dice similarity coefficient values of the bladder, small intestine, rectum, femoral head and spinal cord were all above 0.87 (average was 0.9); Jaccard distance of these were within 2.3 (average was 0.18). Except for the small intestine, the Hausdorff distance of other organs were less than 0.9 cm (average was 0.62 cm). The Dense V-Network has been proven to achieve the accurate segmentation of pelvic endangered organs.

9.Metabonomic analysis of plasma biochemical changes in pyrexia rats after treatment with Gegenqinlian decoction, aspirin and itraconazole by UHPLC-FT-ICR-MS

A metabonomic approach involving an ultrahigh-performance liquid chromatography combined with Fourier transform ion cyclotron resonance mass spectrometry (UHPLC-FT-ICR-MS) was used to investi-gate the changes in the endogenous metabolites in the plasma of rats with yeast-induced pyrexia treated with Gegenqinlian decoction (GQLD), aspirin and itraconazole. The differences in the small molecule profiles of treatment using traditional Chinese medicine, etiological treatment and symptomatic treat-ment were elucidated. Thirty-six plasma metabolites were identified or putatively identified, and the effects of the three medicines on the thirty-six metabolites were studied. Their metabolic pathways indicated that GQLD, aspirin and itraconazole ameliorated the rats with yeast-induced pyrexia pre-dominantly by regulating the metabolisms of phospholipid, sphingolipid, fatty acid oxidation, fatty acid amides, amino acid and glycerolipid in vivo. The pharmacodynamics and metabonomic results showed that the three medicines exhibited the therapeutic effects on pyrexia by regulating the perturbations of multiple metabolisms. The study provided a scientific basis for an in-depth understanding of the ther-apeutic effects of GQLD, aspirin and itraconazole on rats with yeast-induced pyrexia.
10. Development and psychometric test of oncology missed nursing care self-rating scale

Objective:
To develop a scientific oncology missed nursing care self-rating scale suitable for hospital culture background in China, and to test its validity and reliability.
Methods:
The initial scale was formulated by literature review, qualitative interview and delphi expert consultation. A total of 388 clinical nurses from Shanxi Province were selected by convenience sampling. Item analysis was used to filtrate items. The validity was evaluated by using evaluation scale of construction and content validity, and the reliability was tested by using internal consistency, split-half reliability and retest reliability.
Results:
Oncology missed nursing care self-rating scale consisted of 33 items and 4 dimensions including nursing assessment, nursing plan, basic nursing and nursing intervention. Exploratory factor analysis (EFA) revealed that 4 factors accounted for 63.664% of the accumulated variances. The scale-level content validity index (S-CVI) was 0.904. The Cronbach α coefficient was 0.948, the spilt-half reliability coefficient was 0.786, and the retest reliability coefficient was 0.833.
Conclusion
Oncology missed nursing care self-rating scale has good validity and reliability, which can be used as a measuring tool for oncology missed nursing care.
