Objective:To investigate the accuracy of resting energy expenditure(REE)prediction formulas and to develop a new REE prediction formula suitable for hospitalized older adults.Methods:Older adults hospitalized in the Department of Geriatrics from October 1, 2022, to November 31, 2022, were included in the study.The predicted values of REE(pREE)were estimated using 13 commonly employed formulas that incorporate parameters related to resting energy expenditure, such as gender, age, body mass index(BMI), and body weight.Indirect calorimetry measurements(mREE)served as the gold standard for comparison.Group differences between pREE and mREE, the coefficient of concordance( ICC), and accuracy(defined as±10% of the mREE values)were utilized to evaluate the performance of the formulas.The ten-fold cross-validation method was employed to identify valid variables and to construct a new prediction formula.The performance of this new formula was compared to mREE, the Harris-Benedict formula, the European Society of Clinical Nutrition and Metabolism(ESPEN)formula, and the Chinese Society of Clinical Nutrition and Metabolism(CSPEN)formula. Results:A total of 223 hospitalized participants aged 60 to 98 years(mean age 79.5±8.2 years)were included in the study.Among these participants, 49.3%(110 cases)were male, and the prevalence of frailty was approximately 84.3%(188 cases).The median difference between pREE and mREE ranged from 9.1 to 232.1 kcal/d.The predictions from the Harris-Benedict, ESPEN, and CSPEN equations differed significantly from mREE(all P<0.05), with respective accuracies of 30.9%, 31.4%, and 24.7%.A new equation was developed: pREE=794.847+ 8.661×body weight -7.976 × age+ 14.757 ×grip strength+ 5.037 × heart rate, with an ICC of 0.6(95% CI: 0.5-0.7), and the accuracy reached 56.3%. Conclusions:Existing equations demonstrate low accuracy in predicting REE among hospitalized older adults.The newly developed equation shows improved performance compared to previous models and can serve as a reference method for predicting REE in this demographic.

Objective To observe the clinical,pathological features and prognosis of IgA nephropathy in children with crescent formation.Methods A total of 38 cases of children with IgA nephropathy diagnosed by renal biopsy at Qilu Hospital of Shandong University were retrospectively analyzed.According to whether there were crescent formation,they were divided into crescents formation group (crescent formation≥10％,n=18) and control group consisting of children with complete clinical data and matching age,gender and course of disease (no crescent formation,n=20).The clinical,pathological features and prognosis of the two groups were compared.Results Fourteen cases (77.78％) of crescents formation group were associated with nausea hematuria,and 12 cases (66.67％) had hypertension,8 patients in control group (40％) had gross hematuria,and 6 cases (30％)had high blood pressure.The differences were statistically significant (P ＜ 0.05).In crescents formation group,Alb and eGFR were significantly lower than those of control group.24 h urine protein and Scr were significantly higher than those of control group and there were significant statistically differences between two groups (P ＜ 0.05).Nephropathological changes in crescent formation group were mainly in grade Ⅱ and grade Ⅲ and were usually combined with tubular interstitial damages.In addition to IgA immunoglobulin deposition,they often accompanied by IgG and IgM immunoglobulin deposition.In control group,nephropathological changes were mainly in grade Ⅰ and grade Ⅱ and tubular interstitial damages were rare.IgA was the main form of deposition in control group.After 12 weeks steroid treatment and (or) immunosuppressive therapy,urinary protein turned negative in 7 cases in crescent formation group,24 h urinary protein level declined by half than before in 7 cases and no obvious change in 4 cases.In control group,urinary protein turned negative in 12 cases,6 cases showed a more than 50％ decline in urinary protein level and 2 cases had no obvious change.During the 1-3 years of follow-up,renal function of 14 cases and 19 cases was normal in the crescent formation group and control group respectively,serum creatinine of 3 cases and 1 case increased mildly in the crescent formation group and control group respectively,1 case needed dialysis treatment in the crescent formation group and the one in the control group who had slightly increasement of Scr did not reach the standard of dialysis treatment.Conclusions The clinical and pathological manifestations of children with IgA nephropathy with crescent formation are more severe and more likely to manifest renal insufficiency,suggesting that crescent formation is one of the indicators of poor prognosis.

Objective To study the differences between renal clear cell renal cell carcinoma (CCRCC)and angiomyolipoma (AML)with minimal fat,whose size were both smaller than 3cm and the lesions center were both located outside the renal contour. Methods 6 cases of CCRCC and 18 cases of AML with minimal fat were analyzed retrospectively.All the lesions were confirmed pathologically.The morphological characteristics,plain and enhanced CT values were obtained for compare.Results The lesion length diameter,morphology,the early excretory phase CT value and the pattern of enhancement were not significant different between the two groups (P>0.05).There was significant difference in the focal cystic and necrotic between CCRCC group and AML group (P<0.05).Plain scan density,enhanced uniformity,pseudo capsule sign,plain CT values,medullary junction CT value and the pattern of enhancement from medullary junction phase were statistically different between those two groups (P< 0.001).Conclusion The focal cystic and necrotic,plain scan density, enhanced uniformity,pseudo capsule signs,plain CT values,the cortex and medullary junction CT value and the enhanced amplitude were very important role in differential diagnosis of CCRCC and AML with minimal fat.

This study was directed to the effects of macrophage-colony stimulating factors (M-CSF) concentration, recerptor activator of nuclear factor kappaB ligand (RANKL) concentration and M-CSF preinduction on osteoclastogenesis and the related resorption function. Bone marrow mononuclear cells were isolated and were divided into 4 groups. Group A underwent osteoclastogenic induction with the use of 30 ng/ml M-CSF and 50 ng/ml RANKL, while Group B received 50 ng/ml M-CSF and 100 ng/ml RANKL treatment. Both C and D Group underwent preinduction with the use of 30 ng/ml M-CSF for 3days, and then they were treated with 30 ng/ml M-CSF and 50 ng/ml RANKL, 50 ng/ml M-CSF and 100 ng/ml RANKL, respectively. Osteoclastogenesis was examined by TRAP staining 6 days after induction, and dentin resorption lacunae were detected by Scanning Electron Microscope 9 days after induction. TRAP positive multinuclear cells were observed in all groups of cells, and resorption lacunae were formed in all of them. However, more TRAP positive multinuclear cells were observed and more large resorption lacunae were detected in groups B and D than in groups A and C, respectively. The number of TRAP positive cells, number of resorption lacunae and lacuna areas in groups C and D were also greater than those in groups A and B, respectively. Higher concentration of M-CSF and RANKL and preinduction with M-CSF may benefit osteoclastogenesis and increase resorption function of osteoclast.

ObjectiveTo explore the protective methods to the patients with insensitive feet caused by leprosy.Methods295 patients affected by leprosy (PAL) with insensitive feet did self-care and wore protective shoes which were bought in general market and met the requireing of with soft and elastic insole,hard sole higher instep, et al.All the patients had been followed up for 3 years.ResultsAll cases persisted on wearing those shoes, only 1 case occured fresh ulcer and no ulcers relapsed.Cracks and ulcers on foot declined by 96.15% and 32.81% respectively, and bone loss had no change.ConclusionsThe protective foot-wears can prevent the fresh ulcers happening and the healed ulcers reappearing for PALs with numb feet. It is important to educate the PALs how to choose the kind of foot-wears and how to do self-care.

Objective To investigate the influences of active immunotherapy on T helper cell (Th)1/Th2 type cytokines in women with unexplained habitual abortion(UHA) Methods A total of 55 patients with UHA were studied, including 30 cases after active immunotheraphy (AIT) and 25 cases without any therapy(NAIT). Fifteen cases of normal nonpregnant (NNP) women were selected as control group. Supernatants from trophoblast activated peripheral blood mononuclear cells (PBMC) of the three groups were tested by enzyme linked immunosorbent assay (ELISA) for interferon gamma (IFN ?), interleukin 2 (IL 2), IL 4, IL 10. Results (1) The levels of IL 2 and IFN ? in AIT group [(108?37) ng/L and (110 ?52) ng/L, respectively] were lower significantly than those in NAIT group[(223?85) ng/L and (326 ?92) ng/L, respectively]( P 0.05). (2) Twenty six women in AIT group got pregnant, but 8 women experienced pregnancy loss repeatedly whose IL 2?IFN ? levels were higher than those in other 18 women got successful pregnancy and IL 4,IL 10 levels lower than the latter. Conclusions UHA patients have Th1 type immunity to trophoblast and produce high level Th1 type cytokines which probably result in pregnancy loss. Active immunotheraphy could make a shift from Th1 to Th2 immunity, thus favoring the maintenance of pregnancy.
