1.Literature review of one case of follicular lymphoma complicated with iliac bone Cryptococcus neoformans infection

OBJECTIVE To conduct a literature review of one case of follicular lymphoma complicated with iliac bone Cryptococcus neoformans infection so as to raise the understanding of such pathogen in the field of HIV-negative Hodgkin's/non-Hodgkin's lymphoma.METHODS The clinical data were collected from one case of follicular lymphoma patient complicated with C.neoformans infection who was treated in Xinrui Hospital of Xin-wu District,Wuxi City on Feb.24,2023 and retrospectively analyzed.All of relevant literatures regarding to the subject were retrieved in Pubmed,CNKI,Wanfang and VIP databases,and the clinical data of the patients with HIV-negative Hodgkin's/non-Hodgkin's lymphoma with C.neoformans infection were screened out and summa-rized.RESULTS The case was a 28-year-old male and had the underlying disease of follicular lymphoma.The C.neoformans was detected by iliac bone histopathology and metagenome next generation sequencing(mNGS).The condition of the patient improved after the treatment with amphotericin B cholesterol sulphate com-pound and fluconazole.A total of 28 patients,with this case included,were involved in the literature review,23 of whom were male,and 5 were female,and the age ranged between 16 and 79 years old.With respect to major in-fection sites,there were 15(53.57%)cases of cerebral infection,9(32.14%)cases of blood infections,7(25.00%)cases of pulmonary infection,4(14.285%)cases of skin infections,2(7.14%)cases of muscle tis-sue infections,2(7.14%)cases of pleural effusion infections,2(7.14%)cases of bone infections and 1(3.57%)case of bone marrow infection.Totally 11 patients had disseminated Cryptococcus infection,accounting for 39.28%.CONCLUSIONS The C.neoformans infection is seldom detected in the patients with HIV-negative Hodgkin's/non-Hodgkin's lymphoma.The brain is the major infection site with the high probability of dissemina-ted infection.It is necessary for the hospital to deepen the understanding of the pathogen in the field of HIV-negative Hodgkin's/non-Hodgkin's lymphoma.
2.Qualitative study on the experiences and needs of ward nurses caring for elderly patients with postoperative delirium

Objective:To deeply understand the psychological experiences and needs of ward nurses caring for elderly patients with postoperative delirium, providing insights for improving their care capabilities.Methods:A descriptive qualitative study was conducted using purposive sampling to select nurses from the surgical wards of the Second Affiliated Hospital of Zhejiang University School of Medicine who had cared for elderly patients with postoperative delirium between January and March 2024. Semi-structured interviews were conducted, and traditional content analysis was used to extract themes.Results:Sixteen surgical ward nurses, aged 26-48 years old and all female, were interviewed. Four themes and nine sub-themes were identified: internal stress in caring for elderly patients with postoperative delirium (concerns about failing to recognize changes in condition, leading to delayed treatment; physical and psychological stress overwhelming nursing work); external challenges in caring for elderly patients with postoperative delirium (uncontrollable patient behavior; lack of close collaboration between medical staff; lack of awareness among patient families about postoperative delirium); lack of knowledge about postoperative delirium (difficulty in identifying subtypes of postoperative delirium; lack of knowledge about postoperative delirium management); and a desire for systematic professional support (urgent need for operational guidelines on postoperative delirium management in elderly patients; expectation for the establishment of a multidisciplinary team for postoperative delirium management in elderly patients).Conclusions:Nursing managers should pay attention to the physical and mental health of ward nurses, provide multifaceted support, strengthen their knowledge about postoperative delirium in elderly patients, optimize postoperative delirium management, and improve their care capabilities.
3.Network analysis of core symptoms and bridge symptoms of patients after hepatectomy

Objective:To construct the symptom network of patients after hepatectomy, analyze the core symptoms and bridge symptoms in the network, and provide evidence for symptom management of patients after hepatectomy.Methods:A cross-sectional study was conducted to select the patients after hepatectomy in the Second Affiliated Hospital of Zhejiang University from January to July 2024 by convenient sampling method. M.D. Anderson Sympotom Inventory and the Symptom Module Specific to Primary Liver Cancer were used to investigate the composition of symptoms. Symptom clusters were extracted by exploratory factor analysis, and simultaneous network layout was constructed by R language to describe the relationship between symptoms and analyze the central indicators.Results:A total of 228 questionnaires were distributed and 220 valid questionnaires were collected. There were 174 males and 46 females, aged (60.31 ± 10.69) years old. The four symptom groups were neuroaffective symptom group, gastrointestinal symptom group, fatigue-related symptom group and pain-related symptom group, and the cumulative variance contribution rate was 54.592%.In the concurrent symptom network analysis, distress was the core symptom of node centrality index, and the top 3 bridge symptoms were pain (r bs=2.40), decreased appetite (r bs=2.10) and nausea (r bs=2.00), the correlation stability coefficients of network strength centrality, compact centrality and expected influence values were 0.595, 0.595 and 0.673, the symptom group network was stable. Conclusions:Distress is the core symptom of patients after hepatectomy, and pain, decreased appetite and nausea are bridge symptoms. It is recommended that medical staff construct intervention programs based on core symptoms and bridge symptoms, implement precise symptom management, and improve the outcome of symptom management of patients.

Tuberous sclerosis complex(TSC)is a rare genetic disease that can lead to benign dysplasia in multiple organs such as the skin, brain, eyes, oral cavity, heart, lungs, kidneys, liver, and bones. Its main symptoms include epilepsy, intellectual disabilities, skin depigmentation, and facial angiofibromas, whilst incidence is approximately 1 in 10 000 to 1 in 6000 newborns. This case presents a middle-aged woman who initially manifested with epilepsy and nodular depigmentation. Later, she developed a lower abdominal mass, elevated creatinine, and severe anemia. Based on clinical features and whole exome sequencing, the primary diagnosis was confirmed as TSC. Laboratory and imaging examinations revealed that the lower abdominal mass originated from the uterus. CT-guided biopsy pathology and surgical pathology suggested a combination of leiomyoma and abscess. With the involvement of multiple organs and various complications beyond the main diagnosis, the diagnostic and therapeutic process for this patient highlights the importance of rigorous clinical thinking and multidisciplinary collaboration in the diagnosis and treatment of rare and challenging diseases.
8.Incidence and risk factors of early transient intraocular pressure elevation after canaloplasty for primary open-angle glaucoma.

Glaucoma is one of the most common optic neuropathies, featuring progressive retinal ganglion cell damage and visual field loss (Tham et al., 2014; Xu et al., 2020). Currently, the only effective treatment for this condition is the reduction of intraocular pressure (IOP) (Palmberg, 2001; Heijl et al., 2002). Canaloplasty is a proven bleb-independent surgery with good efficacy and safety profiles in primary open-angle glaucoma (POAG) (Gołaszewska et al., 2021). However, early transient postoperative IOP elevation has been reported in up to 30% of cases (Riva et al., 2019), similar to that commonly observed in other internal drainage glaucoma surgeries such as implantation using iStent (0%-21.0%), CyPass (10.8%), and Hydrus (4.8%-6.5%) (Lavia et al., 2017). This complication may be a predictor of poor reserve in the outflow system and is potentially associated with surgical failure. Nonetheless, the exact pathophysiology of glaucoma remains unknown, and studies clarifying the risk factors for postoperative IOP elevation have been scarce.

9.Assessment of left ventricular systolic function in patients with persistent atrial fibrillation using the single beat method based on the ratio of preceding R‐R intervals approaching to 1

To observe the effect of different R‐R interval on left ventricular systolic function measured by echocardiography in patients with persistent atrial fibrillation ( AF ) ,and assess the clinical feasibility of the use of an index that is based on the RR1/RR2 ratio to accurately estimate left ventricular systolic function during AF . Methods T wenty‐one patients with persistent AF received echocardiographic examination ,left ventricular ejection fraction ( LVEF ) and stroke volume ( SV ) were recorded at least 10-20 consecutive cardiac cycles for each patient . T he effects of different R‐R intervals on LVEF and SV determined by echocardiography were analyzed . Bland‐Altman analysis was used to assess the correlation and agreement between the systolic parameters of a single beat method based on the ratio of preceding R‐R intervals approaching to 1 and the average value of multiple cardiac cycles as the golden standard . Results In the same patient with AF ,LVEF and SV measured in different cardiac cycles were unstable with great variation . LVEF and SV were not only correlated with heart rate ,showing significantly positive correlations with RR interval ( all r > 0 .4 , P < 0 .05 ) ; w hich were also affected by heart rate variability ,w hen the heart rate variability was high ( heart rate standard deviation ≥ 30 beats/min ) ,the measured SV was better correlated with RR 1/RR2 ( r :0 .581 vs 0 .835 , P = 0 .009 ) . Bland‐Altman analysis showed excellent correlation and agreement between the LVEF and SV of a single beat with identical RR1and RR2 intervals and measured average values over all cardiac cycles ( r =0 .897 ,0 .918 ; all P < 0 .001 ) . Conclusions LV systolic function is significantly correlated with RR interval and is also affected by heat rate variability . Single beat based on ratio of RR 1/RR2 to assess LV systolic function in patients with AF is as accurate as the time‐consuming method of averaging multiple cardiac cycles .
10.Efficacy and safety of higenamine hydrochloride stress echocardiography for diagnosis of coronary artery disease

Objective To evaluate the efficacy and safety of higenamine hydrochloride(HG) stress echocardiography for diagnosis of coronary artery disease (CAD). Methods The study was designed as prospective,randomized,open-labled,positively controlled and crossover phase II multi-center clinical research.Ninety subjects who were suspected to have CAD were enrolled.HG dosage was titrated at 0.5,1, 2,4 μg.kg -1.min-1every 3 min.Adenosine was injected 140 μg.kg -1.min-1for 6 min with total dosage 0.8 mg/kg.Visual assessment of the left ventricle wall motionand 17-segment model were used for analysis of stress echocardiography.CAD was defined as identifying >50% diameter stenosis in at least one major coronary artery by coronary angiogram.All adverse reaction were recorded. Results For HG group,the sensitivity,specificity,accuracy,positive predictive value and negative predictive value were 28.9%,89.7%, 57.1%,76.5% and 47.8%,respectively;for adenosine group,they were 26.7%,94.9%,58.3%,85.7%and 47.1%,respectively. There was no significant difference between the two groups( P > 0.05). The diagnostic sensitivities of HG and adenosine echocardiography for single vessel stenosis were 11.1% and 5.6%,respectively( P >0.05).Both HG and adenosine echocardiography have the same sensitivity with 37.5% for double vessel stenosis and 44.4% for triple vessel stenosis.Advers reaction rate was 84.4% in HG group and 92.2% in adenosine group( P >0.05).Conclusions HG stress echocardiography for CAD diagnosis has high specificity,good safety and low sensitivity,which are similar to adenosine echo.
