1.Role of intensive insulin pump therapy in type 2 diabetes mellitus patients with diabetic kidney disease

Objective:To investigate the effect of short-term intensive insulin pump therapy on blood glucose control and renal function in type 2 diabetes mellitus (T2DM) patients with diabetic kidney disease (DKD).Methods:The clinical data of 360 T2DM patients with DKD in Shanxi Bethune Hospital from January 2022 to October 2024 were retrospectively analyzed. These patients were divided into an observation group ( n=192; treated with continuous subcutaneous insulin infusion) and a control group ( n=168; treated with conventional hypoglycemic therapy) according to the different treatment modalities; subsequently, 103 pairs of patients entered final analysis by using the propensity score matching method. The differences in glycemic control and improvement of renal function were compared between these two groups. Results:Before the treatment, the blood glucose-, blood glucose fluctuation-, and kidney function-related indicators showed no significant differences between these two groups (all P>0.05). After the treatment, fasting blood glucose, two-hour postprandial blood glucose, and average blood glucose significantly decreased in both groups and were significantly lower in the observation group than in the control group (all P<0.05). In addition, the amplitude of postprandial blood glucose fluctuation, standard deviation of intraday multi-point blood glucose, and difference between the maximum and minimum intraday blood glucose values significantly declined after treatment and were also significantly lower in the observation group than in the control group (all P<0.05). The observation group had a significantly higher total glycemic control rate and a significantly lowr incidence of hypoglycemia (both P<0.05). Finally, the 24-hour urine protein level, serum creatinine level, and urine albumin/creatinine ratio of the observation group significantly dropped after the treatment, and these results were significantly lower than those in the observation group (all P<0.05). Conclusions:Intensive insulin pump therapy benefits T2DM patients with DKD by improving glycemic control and enhancing renal function.
2.Psychological safety experience of new nurses in Neurosurgery Department: a qualitative study

Objective:To gain an in-depth understanding of the psychological safety experiences and changes of new nurses in Neurosurgery Department, providing insights for improving their psychological safety.Methods:A purposive sampling method was used to select new nurses from the Neurosurgery Department at Zhejiang Enze Medical Center Enze Hospital in July to August 2023. Semi-structured interviews were conducted, and Colaizzi's 7-step analysis method was applied to organize and analyze the interview data, summarizing and refining the themes.Results:A total of eleven new nurses from the Neurosurgery Department were included in the study. A total of five themes were identified: insufficient awareness of psychological safety, self-doubt, lack of interpersonal safety, fear and worry, and coping and adaptation.Conclusions:New nurses in Neurosurgery Department often experience a lack of psychological safety during their development. It is crucial to focus on their stage-specific characteristics, enhance new nurse training, create a safe environment, assist them in adopting correct coping strategies, and improve their mental health.

5.Analysis of prevalence of dental caries and associated factors among preschool children with disabilities in Bengbu

Objective:
To understand the oral health status and associated factors of preschool children with disabilities in Bengbu City, so as to provide evidence for preventing dental caries in children with disabilities.
Methods:
From September 2021 to March 2022, a total of 405 preschool children with disabilities from two rehabilitation institutions in Bengbu were randomly selected for oral health examination, questionnaire survey and physical examination. The caries prevalence rate among disabled children was analyzed. After grouping based on childrens BMI, the correlation between body mass index (BMI) and dental caries among children was explored. Multivariate binary Logistic regression analysis was employed to investigate the factors associated with dental caries occurrence in children.
Results:
The prevalence of dental caries in preschool children with disabilities was 74.07%, with rates of 71.01% for boys and 77.27% for girls. There was a significant difference in caries prevalence between boys and girls aged 5(66.67%,88.24%) (χ2=7.53, P<0.05). There were significant differences in the dmft index among different BMI groups (underweight: 240, normal weight: 606, overweight:30,obese:60,H=35.66,P<0.05). BMI was negatively correlated with dmft(r=-0.50,P<0.01). Frequent tooth brushing (2-3 times daily), the use of fluoride toothpaste, limited intake of sugary foods (< 2 times/d) in the past six months, and exclusively breastfeeding within first six months were negatively correlated with the occurrence of dental caries in disabled children (OR=0.09,0.41,0.24, P<0.05). Sleep forward to eat,parental education level of junior high school or lower, and parental education of vocational school or high school were positively correlated with dental caries (OR=3.18,5.95,3.99,66.95,7.75,P<0.05).
Conclusions
The caries prevalence rate of disabled children in Bengbu City is high and is influenced by multiple factors. It is time to strengthen the oral health training for parents and teachers in educational institutions, pay attention to childrens oral health care, and help disabled children improve their quality of life.

7.Evolutionary analysis of H9N2 subtype avian influenza virus in Shandong in 2020-2022

In order to understand the prevalence and genetic variation of H9N2 subtype avian influ-enza virus in Shandong,a total 492 tracheal and lung tissue samples collected from chicken farms with respiratory symptoms in partial areas in Shandong were detected by H9 subtype AIV real-time RT-PCR,and the positive samples were inoculated with chicken embryos for two generations.Whole genome sequences of the positive strains by applying Illumina Miaseq platform,and genetic evolution and mutation at positions associating with viral pathogenicity and transmissibility were analyzed.The results showed that there were 72 samples were positive for H9 subtype AIV among the 492 samples,with a positive rate of 14.63%.Thirty-four strains of H9 subtype AIV were ob-tained from the positive samples after passing through chicken embryo,meanwhile,the 34 isolates were all H9N2 subtype AIV by whole genome sequencing analysis.By analyzing the evolutionary tree of HA and NA genes,HA and NA genes of the 34 H9N2 AIV strains belonged to Y280-like branch and F/98-like branch,respectively.Meanwhile,based on above branches,there were obvious time node subbranch,which one was"isolates before 2013",another one was"isolates after 2013".The HA cleavage sites of thirty-four H9N2 strains were all 325PSRSSR↓GLF333,which met the se-quence characteristics of the lowly pathogenic avian influenza virus,and the HA receptor binding site 226 amino acid was leucine,which had the characteristics of blinding to a-2,6 mammalian sialic acid receptors.Among the internal amino acid sites that are key to mammalian adaptation,all strains had an I368V mutation in the PB1 gene that enhanced viral transmissibility in mammals and the PB2 genes of some strains were mutated to enhance the mammalian adaptation of I292 V and A588 V.The above results illustrated that the H9N2 subtype AIV gene segments in Shandong have different degrees of recombination and gene variation,so it is necessary to strengthen the monito-ring of virus variation.
8.A Meta-analysis of the association between adipokines and bone nutrition of senile osteoporosis in China

Objective:Through meta-analysis, the association of three common adipokines (leptin, adiponectin, and chemerin) with bone nutrition of senile osteoporosis (SOP) in China was systematically evaluated.Methods:CNKI, CBM, VIP, Wanfang, PubMed, Web of Science, Embase, Cochrane Library, and other databases were searched for articles published from the establishment of the database to July 30, 2022. After literature screening, data extraction, and quality evaluation of the included studies were independently conducted by two researchers, a meta-analysis was performed using RevMan5.4 and Stata17.0 softwares.Results:A total of 13 studies in the Chinese population were included, including 897 patients with SOP and 673 elderly with normal bone mineral density . The results of the meta-analysis showed that compared with the control group, the serum leptin levels were significantly lower ( MD -2.64, 95% CI -4.04 to -1.23, P < 0.001), chemerin levels were significantly higher ( MD 25.23, 95% CI 14.57 to 35.90, P < 0.001), and adiponectin levels were not significantly different ( MD -0.55, 95% CI -2.26 to 1.17, P > 0.05) in SOP patients. After subgroup analysis according to the measurement method, leptin levels remained lower in SOP patients than in the control group. Conclusions:Compared with the control group, leptin levels were lower and chemerin levels were higher in SOP patients. Therefore, dysregulation of adipokines may play an important role in the occurrence and development of SOP, and regulation of adipokine levels and functions may play a role in the treatment of SOP and the improvement of bone nutrition as a nutritional intervention.

10.Trends of female reproductive health status in Bangladesh from 1999 to 2018

Objective:To analyze the changes of relevant indicators in reproductive health status among Bangladeshi women from 1999 to 2018 and to assess whether the 2030 Sustainable Development Goals (SDGs) can be achieved.Methods:Data were obtained from both the Bangladesh Demographic and Health as well as from the Maternal Mortality and Health Care Surveys. The trends of SDGs indicators related to reproductive health from 1999 to 2018 were analyzed and compared, and the average annual rate of change was calculated. Development index was used to assess the difficulty of achieving the SDGs.Results:The maternal mortality rate increased first and then leveled off from 2001 to 2016. From 1999 to 2018, the coverage of reproductive health care services and the proportion of women who had the right to make the decision on their own health care service, were generally increasing. Proportion of the following areas as: "contraceptive needs, women who consider that partner violence is justified, the rate of early marriage, and the rate of early childbearing etc.", were declining at various degrees. Development index of the antenatal care coverage, rate of delivery in medical facilities, percentage of live births attended by medically trained providers and the rate of postnatal care etc., were less than 1. The development indices of the maternal mortality rates were close to 1, while all the other indicators were greater than 1. Conclusions:The reproductive health-related SDGs indicators in Bangladesh appeared somehow degrees of progress from 1999 to 2018. However, for most indicators, the average annual rate of change was still lower than the expected to achieve the 2030 target which called for acceleration in the next few years.
