Objective To observe the effect of rehabilitation nursing pathway in transjugular intrahepatic portosystemic shunt(TIPS).Methods From January 2022 to December 2024,160 patients with cirrhotic esopha-geal and gastric varices bleeding and intractable hydrothorax and ascites who underwent TIPS surgery were selected and randomly divided into path group and control group,with 80 cases in each group.The clinical treatment schemes of the two groups were consistent.The control group was given routine nursing,and the path group was intervened by rehabilitation nursing path model.Six-minute walking test(6 MWT)distance,Activity of Daily Living,ADL)scores,self-rating anxiety scale(SAS)and self-rating depression scale(SDS)scores,as well as the incidence of postoperative complications(hepatic encephalopathy and abdominal hemorrhage),hospitalization days,hospitalization expenses and nursing satisfaction scores were compared between the two groups.Results The incidence of postoperative complications in the pathway group was 6.25%(5/80),which was lower than that in the control group(16.25%,13/80).The 6MWT distance,ADL score,SAS score and SDS score in the path group were better than those in the control group,and the difference was statistically significant(P<0.05).The hospital-ization days in the path group were(10.50±2.50)days,which were shorter than those in the control group(12.70±3.50)days.The hospitalization expense was(8.31±0.71)million yuan,which was lower than that of the control group(8.92±0.53)million yuan.The nursing satisfaction score was(93.90±3.31),which was higher than that of the control group(89.50±4.21),and the difference was statistically significant(P<0.05).Conclusion TIPS rehabilitation nursing path can effectively promote patients'rehabilitation,reduce medical costs and improve patients'satisfaction.

OBJECTIVE To explore the effect of bundled intervention measures on early infections in patients with malignant biliary obstruction(MBO)after intrabiliary stent implantation under the percutaneous transhepatic cholangial drainage(PTCD)route.METHOD Through convenience sampling method,37 patients with MBO ad-mitted to the First Affiliated Hospital of Fujian Medical University from Jan.2020 to Dec.2021 who received in-trabiliary stent implantation under the PTCD route were selected as the control group,43 patients admitted to the hospital from Jan.2022 to Dec.2023 who received the same procedure were selected as the study group,the control group received conventional infection prevention measures during the perioperative period,the study group added bundled inter-vention measures on this basis,and the postoperative infection status,white blood cell counts,C-reactive protein,and lowering levels of the two groups of patients.procalcitonin,blood amylase,blood lipase levels,hospitalization days and hospitalization costs between the two groups were compared.RESULTS The infection rate of the study group was 9.30％(4/43),lower than that of the control group(P=0.005).White blood cell counts,C-reactive protein,procalcitonin,blood amylase and blood lipase were elevated in both groups of patients on the first postoperative day,while the above in-dicators decreased in both groups of patients on the third postoperative day,with the study group lower than the control group.The hospitalization days in the study group was(7.33±2.55)days,lower than that in the control group(P＜0.001),the hospitalization cost of the study group was(20.32±3.70)thousand yuan,lower than that of the control group(P＜0.001).CONCLUSION Perioperative bundled measures can effectively reduce the risk of early infections in the patients with MBO after intrabiliary stent implantation under the PTCD route,decrease the occurrence of complications,shorten the length of hospital stay and reduce hospitalization costs.

Objective:The purpose of this study was to analyze the pharmacoeconomic research progress on nutrition therapy for cancer patients in recent ten years and explore the research hotspots in this field.Method:Bibliometric analysis was conducted based on Web of Science core database using CiteSpace, VOSviewer, Gephi and Scimago Graphics.Results:A total of 647 researches in the field of pharmacoeconomics about nutrition therapy for cancer patients published from 2013 to 2022 were included, with the annual publication output peaking in 2022. A total of 286 authors from 94 countries or regions have conducted relevant research. Among the 306 institutions involved in our study, the World Health Organization was with the most citations, and the journal with the most citations was Clinical nutrition. The United States, China and the United Kingdom were the top three countries concerning publication outputs. Harvard University was the most important research institution in this field, and the authors with the highest publication volume was Carli Francesco. Keyword analysis identified colorectal cancer as the most important research topic. Keyword clustering resulted in 16 groups, among which enteral nutrition was the largest, and the burst keyword was enhanced recovery after surgery. Conclusions:This study found that research hotspots in nutrition therapy for cancer patients have mainly focused on colorectal cancer and enhanced recovery after surgery in the past decade, and enteral nutrition is also an important research topic in this field. With the emerging precision medicine, based on big data and individual patient conditions (especially patients with colorectal cancer), tailored whole-course perioperative nutrition management program with superior cost-effectiveness may represent the future research trend.

HuR (human antigen R), an mRNA-binding protein responsible for poor prognosis in nearly all kinds of malignancies, is a potential anti-tumor target for drug development. While screening HuR inhibitors with a fluorescence polarization (FP) based high-throughput screening (HTS) system, the clinically used drug eltrombopag was identified. Activity of eltrombopag on molecular level was verified with FP, electrophoretic mobility shift assay (EMSA), simulation docking and surface plasmon resonance (SPR). Further, we showed that eltrombopag inhibited cell proliferation of multiple cancer cell lines and macrophages, and the anti-tumor activity was also demonstrated in a 4T1 tumor-bearing mouse model. The data showed that eltrombopag was efficient in reducing microvessels in tumor tissues. We then confirmed the HuR-dependent anti-angiogenesis effect of eltrombopag in 4T1 cells and RAW264.7 macrophages with qRT-PCR, HuR-overexpression and HuR-silencing assays, RNA stability assays, RNA immunoprecipitation and luciferase assays. Finally, we analyzed the anti-angiogenesis effect of eltrombopag on human umbilical vein endothelial cells (HUVECs) mediated by macrophages with cell scratch assay and Matrigel angiogenesis assay. With these data, we revealed the HuR-dependent anti-angiogenesis effect of eltrombopag in breast tumor, suggesting that the existing drug eltrombopag may be used as an anti-cancer drug.

The transient receptor potential( TRP) subfamilies be-long to the non-selective cation channels located at cell mem-branes. Calcium across the plasma membrane into the cell via most TRP channels may result in localized Ca2+ signals that con-tribute to cell proliferation, migration and metastasis. In recent years, many studies have focused on the role of TRP in cancer. This review intends to gather the latest progress concerning TRP channels on proliferation and migration of cancer.

Objective To investigate the influence of pinealectomy(Px)and melatonin (MLT) supplementation on CD4~+CD25~+Treg cells development and Foxp3 expression in rat thymus. Methods One hundred and twenty clean-grade male SD rats were divided into five groups: normal control group, sham pinealectomized group, pinealectomized group, Px +7.5 mg/(kg·d) MLT group, Px +15 mg/(kg·d) MLT group, and the thymus were taken out at the 4th week and the 8th week;Flow cytometric analysis was used to analyse the number of double positive cells in the thymus and peripheral blood;Semiquantitative RT-PCR and immunohistochemistry were applied to analyse Foxp3 expression of rat thymus. Results There was no difference of the number of CD4~+CD25~+ Treg cells in rat thymus between Px and normal/sham group, and the number increased dependently on time and dose after MLT supplementation. In rat peripheral blood the double positive cells significantly increased in models of Px at the 4th week, and then backed to normal level after MLT supplementation. The results showed no significant changes in all groups at the 8th weeek;At the 4th weeks, Foxp3 expression increased in the thymus of Px rats compared to nomal/sham group, which returned to normal level after MLT supplementation. On the other hand, Foxp3 expression showed no significant difference in all groups at the 8th weeek.Conclusion Px made no difference between the development of CD4~+CD25~+Treg cells in rat thymus, but resulted in significant increase of thymic output and Foxp3 expression. All the effects disappeared at the 8th week. MLT supplementation could reverse the abnormity.

In order to detect the relationships of human laryngeal squamous cell carcinoma (LSCC) with the expression of Vascular Endothelial Growth Factor C (VEGF-C) and Vascular Endothelial Growth Factor Receptor-3 (VEGFR-3), we examined the level of VEGF-C and VEGFR-3 gene expression in the samples from 30 cases of normal laryngeal mucosa tissue (NLM), primary laryngeal carcinoma (PLC) and cervical lymph node (CLN) by using reverse transcription polymerase chain reaction (RT-PCR). The protein levels of VEGF-C and VEGFR-3 expression were also determined by Western blotting in 10 cases of NLM, PLC, CLN,respectively. We found that in the samples from the same patient, the VEGF-C and VEGFR-3 expression level is different among normal laryngeal mucosa tissue, primary laryngeal carcinoma and cervical lymph node. In primary laryngeal carcinoma, the level of VEGF-C and VEGFR-3 expression was significantly higher in lymph node positive group than in lymph node negative group, and this difference was the associated with the histological grade of differentiation (P < 0.05). The expression levels of VEGF-C and VEGFR-3 were not associated with age, sex, site and T stage (P > 0.5). These results suggest that there is a close relationship of LSCC with VEGF-C/VEGFR-3; and VEGF-C/VEGFR-3 may have the potential for use as an indicator with prognostic significance in dealing with LSCC.

Objective To study and evaluate various therapeutic means for upper urinary tract calculi. Methods 3 218 cases of upper urinary tract calculi were reviewed.1 365 were pure kidney stones,651 complicated kidney stones,558 group Ⅰ ureteral stones,452 group Ⅱ ureteral stones and 192 group Ⅲ ureteral stones. Results ESWL (extracoporeal shock wave lithotripsy) has been the treatment of choice for pure kidney stones and for groupⅠ ureteral stones.Open surgery has been the best for complicated kidney stones and group Ⅱ,Ⅲ ureteral stones.1 164 patients underwent lithotomy and 1 923 ESWL.131 cases of group Ⅱ ureteral stones in the middle or lower segment of ureter were treated by ureteroscopic lithotripsy.Of the complicated kidney stones,residual stones were observed in 57 (8.8%).1.8%( n =12) of such cases underwent nephrectomy.The complete stone elimination rate in 3 months after ESWL was 81.6% whereas open operation was needed in 18.4%.Ure teroscopic lithotripsy yielded the poorest outcome,nephrectomy being undertaken afterwards in 0.8% and lithotomy in 6.1%. Conclusions ESWL,ureteroscopic lithotripsy,ballast lithotripsy or the combined use of 2 methods makes the surgical management of upper urinary tract calculi easier with less suffering to the patients.The use of double internal stent could reduce the surgical complications.Stone street after ESWL should be well taken care of and inflammation should be well controlled.Skilled technique is needed for ureteroscopic lithotripsy.
