1.Study on the efficacy of Schroth PSSE combined with 3D printing brace in the treatment of adolescent id-iopathic scoliosis

Objective To evaluate the efficacy of Schroth PSSE combined with 3D printing braces in the treatment of adolescent idiopathic scoliosis.Method Forty patients were included and divided into a support group(3D printing support group)and a support+exercise therapy group(3D printing support+Schroth PSSE group).Excluding outliers and lost follow-up data,32 patients were ultimately included,with 16 patients in each group.Wearing time≥18 h for both groups of 3D printing supports.The intervention period of Schroth PSSE is 12 weeks,and exercise is maintained after 12 weeks until the end of follow-up.The longest follow-up time for the brace group was 25 months,with an average follow-up time of(15.00±1.29)months.The longest follow-up time for the brace+exercise therapy group was 24 months,with an average follow-up time of(16.59±1.01)months.Relevant indicators were evaluated after follow-up.Results The support and exercise therapy group showed better improvement in Cobb angle and trunk rotation angle(ATR)than the support group,with statistically significant differences(P<0.05);The distance between the midpoint of the C7 vertebral body and the midline of the sacrum(C7-CSVL)in the support and exercise therapy group showed a statistically significant difference before and after treatment(P<0.05),while there was a significant difference before and after Apical Vertebral Translation(AVT)treatment(P<0.01).There was no statistically significant difference compared to the support group(P<0.05);In terms of SRS-22 score,the brace+exercise therapy group can comprehensively improve the SRS-22 score.Conclusion The combination of Schroth PSSE and 3D printing braces has a better effect on improving Cobb angle and torso rotation angle,improving coronary imbalance,increasing patient satisfaction,and improving quality of life compared to using 3D printing braces alone.
2.Analysis of medium and long term efficacy of ossicle chain bypass technique in treatment of tympanosclerosis.

Objective:For tympanosclerosis patients with ossicular chain fixation, we use ossicular chain bypass technique and evaluate its long-term effects. Methods:From June 2017 to June 2019, 147 patients with tympanosclerosis who underwent middle ear surgery with otoscopy in Yinchuan First People's Hospital were reviewed. The subjects were divided into three groups according to the implemented operation plan, 51 cases in the ossicular chain mobilization group(OCM), 56 cases in the ossicular chain bypass reconstruction group(OCB), and 40 cases in the malleus-incus complex resection reconstruction group(MICR). Through a three-year follow-up, the medium and long-term effects of different operation plans were compared and analyzed. Results:There was no significant difference among the three groups in the incidence of tympanic membrane perforation, delayed facial nerve palsy, and the dispatch and displacement of PORP. The incidence of tympanic membrane retraction pocket or cholesteatoma after operation in OCB group(0) was significantly lower than that in OCM group(11.76%) and MICR group(7.5%)(P<0.05). At 12 months after operation, ΔABG of OCB group and MICR group were better than that in the OCM group(P<0.05). At 36 months after operation, ΔABG of OCB group was better than that in the OCM group(P<0.05), and there was no significant difference between OCB group and MICR group. The audiological performance of patients with epitympanic sclerosis(ETS) at 12, 24 and 36 months after operation was better than that of patients with posterior tympanosclerosis(PTS) and total tympanosclerosis(TTS)(P<0.05). Conclusion:Compared with patients undergoing ossicular chain mobilization and malleus-incus complex resection for ossicular chain reconstruction, patients with tympanosclerosis undergoing bypass technique have better and stable hearing prognosis in medium and long term. This technique can effectively prevent the formation of retracted pocket and cholesteatoma in patients with tympanosclerosis after operation.

3.Early outcomes of arthroscopic repair of obsolete traumatic subscapularis tears

Objective:To study the early effect of arthroscopy in the treatment of obsolete traumatic subscapularis tears.Methods:A retrospective case series study was used to analyze the clinical data of 28 patients with obsolete traumatic subscapularis tears admitted to Second People 's Hospital of Changzhou City from July 2017 to July 2020,including 16 males and 12 females,aged 47-72 years[(60.5±6.3)years]. According to Lafosse classification,the injury size was upper one third tear in 9 patients,upper half tear in 18 and complete tear in 1. According to Patte classification of the degree of retraction,4 patients were with no retraction,7 with retraction to the level of the lesser tuberosity,14 with retraction to the level of the humeral head,and 3 with retraction to the level of the glenoid. According to Goutallier classification,the level of fatty infiltration was at grade 0-1 in 17 patients,at grade 2 in 10 and at grade 3-4 in 1. All patients were repaired by arthroscopic surgery. The wound healing was observed at 2 weeks postoperatively. The visual analogue score(VAS),University of California at Los Angeles(UCLA)shoulder rating score,shoulder active forward elevation(AFE),active external rotation(AER),active internal rotation(AIR)were assessed before operation and at 3 months and 6 months postoperatively. Six months after operation,reexamination of the front and side view of the shoulder and MRI of the shoulder were performed to assess the recovery of the subscapular tendon. Results:All patients were followed up for 6-12 months[(8.7±1.3)months]. All wounds healed 2 weeks after operation,with no infection occurred. The VAS was(3.5±0.7)points and(2.6±0.5)points at 3 months and 6 months postoperatively,lower than(6.3±1.2)points before operation( P<0.01). The UCLA score was(20.4±2.5)points and(25.6±6.2)points at 3 months and 6 months postoperatively,higher than(9.5±1.7)points before operation( P<0.01). The preoperative range of motion of the affected shoulder in AFE,AER and AIR was(80.2±12.6)°,(52.5±7.6)°,(3.7±1.2)°,respectively. Three months after operation,the range of motion was(113.2±7.5)°,(60.1±6.3)°,(6.8±1.6)°,respectively. Six months after operation,the range of motion was(122.3±15.2)°,(65.6±10.5)°,(7.8±2.3)°,respectively. The range of motion of the affected shoulder at 3 months and 6 months postoperativley was better than that before operation( P<0.01). Six months after operation,MRI showed good recovery in 27 patients,while a small re-tear in 1 patient(Goutallier classification grade 3). Conclusion:For patients with obsolete traumatic subscapularis tears,arthroscopic repair can significantly relieve the pain,and recover range of motion and function of the joint.
4.The prognosis evaluation of sICAM-1, KL-6 combined with EVLWI in severe pneumonia patients with acute respiratory distress syndrome

Objective:To evaluate the prognostic value of extravascular lung water index (EVLWI) , soluble intercellular adhesion molecule-1(sICAM-1) and Krebs yon den lungen-6 (KL-6) in severe pneumonia patients with Severe Acute Respiratory Syndrome (ARDS).Methods:A prospective study was conducted in Respiratory Intensive Care Unit of the Affiliated Zhengzhou Central Hospital of Zhengzhou University from October 2017 to February 2020. The study included 65 severe pneumonia patients with ARDS, who was performed by measurement of pulse index continuous cardiac output and survived more than 3days after admission. The Extravascular Lung Water Index (EVLWI) , sICAM-1, KL-6 and Oxygenation Index(OI) on 1st, 3rd and 5th day were detected. APACHEⅡ score, patient survival events (days) and survival outcome were recorded. Correlation analysis between EVLWI, sICAM-1, KL-6 and OI was performed on the 1st, 3rd and 5th day after admission. Independent risk factors of mortality in severe pneumonia patients with ARDS were analyzed by multiple logistic regression. Receiver operating characteristic curve was drawn, and the prognostic value of each parameter was assessed finally.Results:The PCT, EVLWI, sICAM-1, KL-6 and APACHEⅡ score in the death group were significantly higher than those in the survival group ( P<0.05) at RICU admission, and the length of RICU stay was significantly shorter than that in the survival group ( P<0.05), while differences in other clinical characteristics between two groups were not statistically significant ( P>0.05) . These parameters including levels of EVLWI, sICAM-1, KL-6, Procalcitonin and APACHE Ⅱscore in the death group were significantly higher than those in the survival group on the 1st, 3rd and 5th day ( P<0.05), whereas the OI was significantly lower than that of the survival group on the 3rd and 5th day ( P<0.05). Logistic regression analysis showed that EVLWI, sICAM-1, KL-6 level were significantly related with the mortality of these patients. The levels of sICAM-1, kl-6 and EVLWI on 1st, 3rd and 5th day after RICU admission showed a significant negative correlation with OI ( P<0.001). Whereas, The levels of sICAM-1, kL-6 on 1st, 3rd and 5th day showed a significant positive correlation with EVLWI ( P<0.001). The sensitivity and specificity of sICAM-1, KL-6 combined with EVLWI in prognosis evaluation on 1st, 3rd and 5th day were 75.0%, 84.4%, 85.0%, 66.7%, 80.0%, 86.7%, respectively. The AUC was 0.864, 0.881, 0.892 on 1st, 3rd and 5th day, respectively ( P<0.001), which had a better prognostic value than each of them. Conclusions:EVLWI, sICAM-1 and KL-6 were independent risk factors for the prognosis of severe pneumonia patients with ARDS. The combination of EVLWI, sICAM-1 and KL-6 might be important in early predicting the prognosis of the 28d mortality.
5.Analysis of the therapeutic effect and influencing factors of alkalized urine in patients with gout

Objective:To investigate the therapeutic effect and influencing factors of alkalized urine in patients with gout, thus providing the basis for the clinical treatmeat.Methods:A total of 90 cases of gout patients in remission without alkalization of urine and lowering uric acid treatment were selected from January 2019 to June 2019. All patients were given a low purine diet (purine intake<200 mg/d). 90 patients were randomly divided into different drugs of alkalized urine groups according to rardom number table: sodium bicarbonate group, taking potassium sodium hydrogen citrate 1.0 g tid; the citrate granule group, taking potassium sodium hydrogen citrate 2.5 g tid; and control group, taking low purine diet only. On the 5th day of treatment, fasting urine in the morning was retained, breakfast was prescribed, and alkalized urine drugs were taken after the meal. Their urine pH was determined at 1 h, 2 h, 3 h, and 4 h after the medicine taken, and copmare the changes of urine pH in different groups. Taking urine pH 6.2 as cut-off point, patients receiving alkalized urine drugs were divided into the standard group (urine pH≥6.2) and the non-standard group (urine pH<6.2) according to their mean urine pH at 2 h, 3 h, and 4 h after taking medicine. Finally, the influencing factors of urine pH were compared between the groups.Results:There were no significant differences in fasting urine pH between the sodium bicarbonate group and the control group, but the urine pH of 2 h and 3 h after sodium bicarbonate taken were significantly higher than the control group ( P<0.05). The urine pH of the citrate granule group was higher than the control group and the sodium bicarbonate group on fasting and at any time after taking drugs ( P<0.05 or P<0.01). The peak pH value of urine in the sodium bicarbonate and citrate granule groups was 4 h after taking drugs, followed by 2 h and 3 h. There were statistically significant differences in body mass index, waist circumference, and blood pressure between the standard and non-standard groups ( P<0.05 or P<0.01). Conclusion:Fasting urine pH alone can not be used as an index for the efficacy of alkalized urine, it is recommended to refer to the pH value of 2-4 h after taking drugs. The efficacy of potassium sodium hydrogen citrate was better than that of sodium bicarbonate. Obesity or overweight seems to be an adverse factor affecting the alkalized urine.
6.Expression and clinical significance of miR-191 in cervical cancer tissues

[Abstract] Objective: To investigate the expression of miR-191 in cervical cancer tissues and its effect on the patients' prognosis.
Methods: One hundred and seven cervical cancer tissue specimens from patients who underwent surgical treatment and 46 normal
cervical tissue specimens from patients who underwent surgical resection of uterine fibroids (the control group) in Xinxiang Central
Hospital were collected from December 2012 to December 2014. The expression of miR-191 in cancer tissues was detected by qPCR.
All patients were followed up from the first day after surgery, and the follow-up deadline was December 31, 2019. All patients were
followed up for 5 years, with death as the end event. The survival time and 5-year survival rate of the patients were recorded.
Survival analysis was performed using Kaplan-Meier method and the factors affecting survival prognosis were analyzed using Cox
proportional hazard regression model. Results: The expression level of miR-191 in cervical cancer tissues was significantly higher
than that in the tissues from control group (P<0.01). There were significant differences in miR-191 expression among patients with
different high-risk HPV infection status, different pathological grades and FIGO stages, and different lymph node metastasis status
(all P<0.01). The 5-year survival rate of patients in the miR-191 low expression group was significantly higher than those patients in
the high expression group (81.48% vs 33.75%, χ 2
=16.905, P<0.01). Pathological grade, FIGO stage, lymph node metastasis and the
expression of miR-191 were risk factors affecting the prognosis of cervical cancer patients (HR=0.486, 3.065, 2.339 and 2.755, all
P<0.05). Conclusion: miR-191 is highly expressed in cervical cancer tissues, and its expression level increases with the progression of
malignancy. It is expected to become a potential biomarker for early diagnosis and prognosis evaluation of cervical cancer.
7.Pyramiding of 3-resistant-gene to improve rice blast resistance of a restorer line, Fuhui 673.

To improve the blast resistance of elite rice restorer line Fuhui 673, 3 blast resistance genes Pi-1, Pi-9 and Pi-kh were introduced into Fuhui 673 from a good-quality restorer line Jinhui 1059 through 3 successive backcrosses followed by one selfing using the technique of marker-assisted selection. Ten near-isogenic lines (NILs) of Fuhui 673 carrying the 3 introduced resistance genes were created. Genotype analysis using 68 SSR markers evenly distributed in the genome indicated that 92.96%-98.59% of the NILs' genetic background had been recovered to Fuhui 673. Both indoor and field resistance tests indicated that the NILs and their hybrids with sterile line Yixiang A were all resistant to rice blast, with resistance levels significantly higher than those of controls Fuhui 673 and hybrid Yiyou 673 (Yixiang A Fuhui 673). In addition, among the 10 hybrids between the NILs and Yixiang A, 2 showed significantly higher yield than and 4 displayed similar yield to that of control Yiyou 673, suggesting that most of the NILs retained the elite characteristics of Fuhui 673. Two new hybrid rice cultivars Liangyou 7283 and Jintaiyou 683 from NIL Line 9 showed high yield, good resistance to blast and moderate growth period in regional trial, suggesting that the NIL Line 9 has a good prospect for application.

8.The predictive value of sFlt-1 combined with extravascular lung water index in severe pneumonia patients with acute respiratory distress syndrome complicated with septic shock

Objective To evaluate the prognosis value of plasma soluble vascular endothelial growth factor receptor (sFlt-1) combined with extravascular lung water index (EVLWI) in acute respiratory distress syndrome (ARDS) complicated with septic shock caused by severe pneumonia. Methods A retrospective analysis was conducted in Respiratory Intensive Care Unit of the Affiliated Zhengzhou Central Hospital of Zhengzhou University from January 2015 to July 2017. The study included 52 severe pneumonia patients with ARDS complicated with septic shock, who was performed by measurement of pulse index continuous cardiac output (PICCO) and survived more than 3 days after admission. According to the 28-day mortality, these patients were divided into the survival group (31 cases) and the death group (21 cases). PICCO was used to record the EVLWI level. The plasma level of sFlt-1 was measured by enzyme-linked immunosorbent assay (ELISA). Acute physiology and chronic health evaluation II (APACHE II) score and sequential organ failure assessment (SOFA) score were calculated. Independent risk factors were analyzed by multiple logistic regression. Correlation analysis between plasma sFlt-1 and EVLWI and APACHE II values was performed on the 1st, 2nd and 3rd day after admission. Receiver operating characteristic curve (ROC) was calculated, and the prognostic value of each parameter was assessed. Results The blood lactate, APACHE II score and SOFA score in the death group were significantly higher than those in the survival group at RICU admission (P<0.05), and the length of RICU stay was significantly shorter than that in the survival group (P<0.05), while differences in other clinical characteristics between the two groups were not statistically significant. The levels of EVLWI, sFlt-1 and blood lactate, APACHE II score and SOFA score in the death group were significantly higher than those in the survival group on the 1st, 2nd and 3rd day (all P<0.05), whereas the PaO2/FiO2 was significantly lower than that of the survival group on the 2nd and 3rd day (all P<0.05). Logistic regression analysis showed that sflt-1 level and EVLWI were significantly related with the patient mortality. The levels of sFlt-1 on day 1, 2 and 3 after RICU admission were positively related to EVLWI and APACHE II score (all P<0.01). The sensitivity and specificity of sFlt-1 combined with EVLWI in prognosis evaluation were 89.7%, 78.2% and 86.3%, 75.7%, respectively. The AUC of sFlt-1 combined with EVLWI was 0.875 and 0.856 on the 1st and 3rd day, respectively (all P<0.01), which had a better prognostic value than each of them. Conclusions SFlt-1 could be used as a biomarker of mortality for severe pneumonia patients with ARDS complicated with septic shock. The combination of sFlt-1 and EVLWI might be important in early prediction of the prognosis of the 28-day mortality in patients with ARDS complicated with septic shock caused by severe pneumonia.
9.Effectiveness of Xuebijing in treatment of multiple organ dysfunction syndrome: a Meta analysis

Objective To systematical evaluate the effect of Xuebijing injection in the treatment of multiple organ dysfunction syndrome (MODS).Methods With the keywords including Xuebijing, multiple organ dysfunction syndrome, multiple organ dysfunction and multiple organ failure, PubMed, the Cochrane Library, Embase, China National Knowledge Infrastructure (CNKI), China Biology Medicine (CBM), VIP and Wanfang Data from the database start until March 4th, 2018 were searched for relevant randomized controlled trials (RCTs) related to Xuebijing injection combined conventional treatment versus conventional treatment alone for MODS. The control group received conventional western medicine treatment, including etiological treatment, antibiotics, mechanical ventilation, nutritional support, and comprehensive treatment to maintain fluid, electrolyte, acid and alkali balance. The experimental group was given traditional western medicine combined with Xuebijing injection. The observation parameters included 7-day and 28-day mortality, acute physiology and chronic health evaluationⅡ (APACHEⅡ) and Marshall score, interleukin-6 (IL-6), tumor necrosis factor-α (TNF-α), the number of platelets, activated partial thromboplastin time (APTT) and prothrombin time (PT). According to the inclusion and exclusion criteria, two evaluators independently screened the literature, extracted data and evaluated the methodological quality of the included studies. RevMan 5.3 software was used for Meta analysis. Funnel plot was used to analyze publication bias.Results A total of 35 RCTs and 2131 patients were enrolled, including 1076 in the experimental group and 1055 in the control group. The results of Meta analysis showed that compared with control group, Xuebijing combined conventional treatment was in favor to decrease the mortality of patients with MODS [7-day mortality: odds ratio (OR) = 0.42, 99% confidence interval (99%CI) = 0.26-0.69,P < 0.00001; 28-day mortality:OR= 0.31, 99%CI= 0.21-0.45,P < 0.00001], also could obviously reduce critical condition degree of APACHE Ⅱ score and the organ function of Marshall score [APACHEⅡ: mean difference (MD) =3.24, 99%CI = 2.00-4.49,P < 0.00001; Marshall score:MD = 1.95, 99%CI = 0.50-3.40,P = 0.0005]. Meanwhile, the results of conventional western medicine combined with Xuebijing in the removal of IL-6 and TNF-α, platelets increase and improvement of PT were better than those of conventional western medicine (IL-6:MD = 5.56, 99%CI = 1.44-9.68, P = 0.0005; TNF-α:MD = 4.97, 99%CI = 3.44-6.50,P < 0.00001; platelets:MD = -50.79, 99%CI = -74.84 to -26.74, P < 0.0001; PT:MD = 4.55, 99%CI = 3.96-5.14,P < 0.00001), however, there was no obvious advantage in improving APTT (MD = 0.96, 99%CI = -5.08-7.00,P = 0.68). The analysis of funnel map showed that the effect points of various studies were mainly centered on the amount of combined effect, and the "inverted funnel" type was generally symmetrical distribution. However, because the number of the included studies was less, the literature bias could not be completely eliminated.Conclusion Xuebijing injection may through its strong cytokines clearance, platelet increase and blood coagulation improvement to protect the organ function in patients with MODS, so as to reduce the mortality and improve the prognosis.
10.Relationship Between ABO Blood Type and Spontaneous Re-canalization in Patients With Acute Myocardial Infarction

Objective: To study the relationship between ABO blood type and spontaneous re-canalization (SR) in patients with acute myocardial infarction (AMI). Methods: A total of 1209 consecutive AMI patients were enrolled. Based on TIMI grade, the patients were divided into 2 groups: Non-SR group, the patients with TIMI grade 0-1,n=442 and SR group, the patients with TIMI grade 2-3,n=767. The relationship between ABO blood type and SR was investigated. Results: Compared with Non-SR group, SR group had more patients with blood type O (32.3% vs 24.7%) and less blood type A (31.7% vs 24.9%). Meanwhile, we found that a lower cholesterol level was related to patients with O blood type and SR occurrence, bothP<0.05. Multi regression analysis indicated that with adjusted age, gender, BMI, hypertension, diabetes, smoking, LDL-C and C-reactive protein, ESR, fibrinogen, D-dimmer, endothelial cardiac function, blood type O may independently predict SR occurrence in AMI patients (OR=1.49, 95% CI 1.10-2.05), while blood type A may have disadvantage for SR (OR=0.65, 95% CI 0.48-0.80). Conclusion: ABO blood type has been related to SR in AMI patients, blood type O is in favor of SR, while blood type A has disadvantage for SR occurrence.
