1.Short-term efficacy evaluation of single-session microwave ablation for benign thyroid nodules

OBJECTIVE To evaluate the short-term efficacy of single-session microwave ablation for benign thyroid nodule.METHODS Patients with benign thyroid nodules treated by microwave ablation between June 2019 and December 2022 at the Department of Otolaryngology Head and Neck Surgery,the First Affiliated Hospital of Xi'an Jiaotong University,were included for analysis.Thyroid function was tested 1 month after treatment,and ultrasound and thyroid function were performed 3 months,6 months and 1 year after treatment.Volume reduction rates(VRR)of nodules were calculated.Data of the last follow-up within 1 year were included for analysis.Variables including gender,age,whether Hashimoto's thyroiditis was present,longitudinal diameter of nodules,solid volume of nodules were included for univariate and multivariate analysis.RESULTS A total of 151 patients with 163 nodules were included.The perioperative complication rate was 1.99%(3/151).The VRR at half year after treatment was(79.58±17.70)%,and the success rate at half year after treatment was 93.43%(128/137).The VRR of at 1year after treatment was(81.24±24.29)%.The 1-year treatment success rate was 92.77%(77/83).Univariate and multivariate analysis showed that nodular solid volume and age were independent factors affecting VRR after ablation.Regression coefficient of age and solid volume was 0.34(P<0.05)and-0.47(P<0.05),respectively.For every 1 cm3 increase in solid volume,1-year VRR(%)decreased by 0.47.Regression equation:1-year VRR=68.92+0.34×age-0.47×solid volume.Serum FT4 gradually decreased and the thyroid stimulating hormone(TSH)gradually increased within 6 months after ablation,and the differences were statistically significant(P<0.05).After 6 months,serum FT4 gradually recovered to the normal level and TSH gradually recovered.However,TSH still did not reach the preoperative level one year after ablation.FT3 decreased gradually after treatment,but there was no significant difference between the values at each time point(P=0.40).After the ablation of thyroid nodule,the mean value of thyroid function index fluctuated,but all of them were within the normal reference range.CONCLUSION Microwave ablation is a safe and effective treatment method for benign thyroid nodules,with an overall success rate of over 90%.Solid nodule volume and age are independent factors affecting the microwave ablation effect of benign thyroid nodules.
3.Differences in the infertility stress, subjective well-being and fertility quality of life among infertile couples and mediating role of psychological resilience

Objective:To explore the differences in the infertility stress, subjective well-being and fertility quality of life among infertile couples and mediating role of psychological resilience.Methods:A cross-sectional study and convenient sampling method were used to recruit the new diagnosed as infertility couples from April 2017 to April 2018 in the Reproductive Center of the First Affiliated Hospital of Zhengzhou University. The Resilience Scale (CD-RISC), the Fertility Problem Inventory (FPI), the Subjective Well-being Scale (SWBS) and the Fertility Quality of Life Questionnaire (FertiQoL) were used to investigate the 230 infertile couples.Results:Infertility couples had statistical differences in psychological resilience ( P=0.034), infertility stress ( P<0.001), and fertility quality of life ( P=0.005), and there was no statistically significant difference in subjective well-being. The fitting degree of structural equation model of infertility stress and subjective well-being and fertility quality of life among infertility women was relatively ideal [χ 2/ df=1.437, goodness of fit index (GIF)=0.936, adjusted goodness of fit index (AGFI)=0.909, root mean square error of approximation (RMSEA)=0.043], and the mediating effects of psychological resilience on subjective well-being and fertility quality of life was significant. The fitting degree of structural equation model of infertility stress and subjective well-being and fertility quality of life among infertility men was relatively ideal [χ 2/ df=2.011, GFI=0.93, incremental fit index (IFI)=0.918, comparative fit index (CFI)=0.917, RMSEA=0.067], and psychological resilience had a mediating effect on fertility quality of life, but had no mediating effect on subjective well-being. Conclusion:There are differences in infertility stress and fertility quality of life among infertile couples. psychological resilience has a mediating effect on the infertility stress and fertility quality of life among infertile couples.
4.Relationship between saliva alpha-amylase and infertility stress among infertile women

Objective:To explore the relationship between salivary alpha-amylase (SAA) and infertility stress in infertile women.Methods:A convenient sampling method was used to investigate the infertile women in the Reproductive Medicine Center of the First Affiliated Hospital of Zhengzhou University from April 2017 to December 2017. A total of 458 infertile women were examined using the SAA and the Fertility Problem Inventory (FPI). SAA level was measured using an enzyme-linked immunosorbent assay (ELISA) kit.Results:The total infertility stress score was 155.86±24.54. The family monthly income, the cause of infertility and the type of infertility were statistically different between different groups ( t=4.123, P=0.041; F=4.079, P<0.001; t=4.491, P=0.035). Pearson correlation analysis showed that SAA and social concern, relationship concern, need for parenthood, rejection of child-free life, sexual concern and total infertility stress were significantly correlated ( r=0.360, r=0.348, r=0.222, r=0.166, r=0.203, r=0.426, all P<0.001). Multiple linear regression results showed that infertility stress, infertility causes and types of infertility were factors influencing SAA. Conclusion:SAA was associated with infertility stress. Combined with SAA and FPI may assess the psychological stress more objectively for infertile women.

7.Analysis of the mastery of delirium-nursing-related knowledge in ICU nurses in Zhengzhou

Objective To investigate the mastery of knowledge about delirium nursing for mechanical ventilation patients in ICU nurses in Zhengzhou and analyze its influencing factors. Methods A total of 267 ICU nurses from 7 general hospitals in 5 urban areas of Zhengzhou city were selected as the study object by convenience sampling. The general information of these study subjects were collected by questionnaire, together with their knowledge of delirium nursing for patients with mechanical ventilation in intensive care units. Results In this study, the overall score of the study object's knowledge scale was (42.20 ± 5.51). The top 4 sources of knowledge related to ICU delirium were continuing education and training (73.1%), department lectures (64.2%), academic conferences (61.5%) and experience exchange among colleagues (58.5%); the top 4 items of delirium-related knowledge that ICU nurses wanted most were the analysis and application of scales (82.5%), treatment of delirium (76.4%), definition and classification of delirium (69.0%) and risk factors and etiology (60.3%). The univariate analysis showed that age, education, job title, age of care and related content training were the factors related to ICU nurses' scores in delirium-related knowledge (P<0.001). According to the multivariate linear regression analysis, academic qualifications, job title and nursing age were the factors affecting ICU nurses' scores in delirium-related knowledge (P<0.05). Conclusions ICU nurses in Zhengzhou have relatively inadequate knowledge of delirium nursing for ICU patients with mechanical ventilation. Therefore, emphasis should be placed on the role of continuing education and department lectures, focusing on training in the analysis and application of ICU delirium rating scales and handling of delirium, especially for those nurses with low academic qualifications, low professional titles and shorter nursing age.
8.Foxp3 overexpression in lung cancer cells suppresses immune activities of activa-ted CD4+T cells

Objective:To determine the effects of Foxp3-overexpressing lung cancer cells on activated CD4+T lymphocyte.Methods: Stable Foxp3-overexpressing lung cancer cells NCIH-1299,NCIH-hFoxp3,was generated by transfection of NCIH-1299 cells with plasmid pcDNA3-hFoxp3 mediated by Lipofectamine 2000 and by selection with G418,and validated by quantitative PCR and Western blot.The expression levels of IL-8 and IL-10 secreted by NCIH-hFoxp3 and NCIH-control were measured by ELISA.IL-2 secrection by activated human CD4+T lymphocyte which was tested after stimulation with 20% conditioned medium of NCIH-hFoxp3 and NCIH-control cells.The proliferation of activated human CD4+ T lymphocytes was assessed by MTT after coculture with NCIH-hFoxp3 cells.The adhesive ability of activated human CD4+ T lymphocytes was probed with NCIH-hFoxp3 cells by immunocytochemistry.Results: Compared with NCIH-control cells,NCIH-hFoxp3 secreted high level of IL-10 and low level of IL-8.NCIH-hFoxp3 with Foxp3 overexpression significantly suppressed the proliferation,adhesive potential and IL-2 expression by activated CD4+ T cells.Conclusion: Suppression of immune activities of activated CD4+ T cells by Foxp3 overexpression in lung cancer cells may correlate with cytokine IL-8 and IL-10,which can contribute lung cancer progression.
9.Enforced expression Foxp3 enhances proliferation and tumorigeneity of lung cancer cell

Objective:To determine the effects of enforced expression of Foxp3 in lung cancer cell with regards to proliferation and tumorgeneity. Methods: A stable subline NCIH-hFoxp3 was established by liopfectamin-mediated pcDNA plasmid transfection carrying exogenous hFoxp3. The growth curve and secrection of IL-8 and IL-10 of NCIH-hFoxp3 were evaluated using MTT and ELISA, respectively. The in vivo tumorigeneity was assessed as well by inoculation of NCIH-hFoxp3 subcutaneously in nude mice. Results:Lung cancer cell NCIH-hFoxp3 with enforced expression of Foxp3 proliferated slowly but exihited increased in vivo tumorgeneity compared with corresponding control subline. In addition,increased expression of hFoxp3 in NCIH-hFoxp3 augmented secretion and at-tenuated secretion of IL-8 and IL-10,respectively. Conclusion:Increased expression of Foxp3 may promote progression of lung cancer cell by change of cellular microenvironment and evasion of immune surveillance.
10.Application of the xenogenic acellular dermal matrix membrane application used in the postoperative tissue shortage repair.

OBJECTIVE:
To observe the short-term and long-term curative effect of the xenogenic acellular dermal matrix membrane (or joint muscle flap transfer) application used in the 82 cases postoperative tissue shortage repair that after the head neck carcinoma resection.
METHOD:
To held the 82 cases head neck carcinoma postoperative mucosa shortage repaired after resection by the xenogenic acellular dermal matrix membrane (or joint muscle flap transfer), 65 cases mucosa shortage wound be directly covered by the repair membrane and the other 17 cases mucosa shortage wound be repaired by the tranfered muscle tissue flap with the repair membrane covered; 53 cases underwent additional postoperative radiotherapy between 2-4 weeks and follow-up in 1, 3, 6, 12, 18, 24, 30, 36, 48, 60 months and observed the operation site repair process through the electronic laryngoscope, observed the patients respiration, swallow, phonation function.
RESULT:
Seventy-seven cases patients operation incision reached I phase healing standard, another 5 cases patients operation incision reached II phase healing standard because of the wound infection and fully-recovered through the local wound drainage,dressing process. All the patients tracheal cannula,the stomach tube be extubated successfully and without the local cicatricial constriction occurred. Seventy-eight cases follow up period reached 1 year including 53 cases who underwent postoperative radiotherapy, 49 cases follow up period reached 3 years including 32 cases who underwent postoperative radiotherapy, 14 cases follow up period reached 5 years including 12 cases who underwent postoperative radiotherapy. The patients with static local lesions discovered no reaction such as exclusion, allergy.
CONCLUSION
The application of xenogenic acellular dermal matrix membrane (or joint muscle flap transfer used in in the postoperative tissue shortage repair that after the head neck carcinoma resection have several advantage such as comparatively easily implementation, operation safety edge enough,well preserved organ function, comparatively low incidence about the laryngeal stenosis, the short-term and long-term repair effect are all exact.
