1.Application of marginal donor livers in liver transplantation for patients with acute-on-chronic liver failure

Objective:To analyze the value of marginal donor livers in liver transplantation for patients with acute-on-chronic liver failure (ACLF).Methods:Clinical data of 58 patients with ACLF undergoing liver transplantation at the First Affiliated Hospital of Guangxi Medical University from January 2020 to June 2023 were retrospectively analyzed, including 33 males and 25 females, aged (40.4±14.4) years. According to the source of donor (marginal or standard), recipients were divided into the marginal group ( n=28), and standard group ( n=30). The preoperative model for end-stage liver disease (MELD) score, cold/warm ischemia time, intraoperative blood loss, postoperative tracheal intubation time, intensive care unit (ICU) stay, liver function, renal function, coagulation function, postoperative complications, and survival situation were compared between the groups. Results:The MELD score, cold/warm ischemia time, intraoperative blood loss, postoperative tracheal intubation time, length of ICU stay, alanine transaminase, aspartate transaminase, total bilirubin, serum creatinine, blood urea nitrogen, estimated glomerular filtration rate, fibrinogen, postoperative infection, primary graft nonfunction, biliary complications, and vascular complications were compared between the groups (all P>0.05). The incidence of delayed graft function (DGF) recovery was 28.6%(8/28) in marginal group, higher than that in standard group 6.7%(2/30) ( χ2=5.13, P=0.038). The one-year cumulative survival rates were 89.3% and 93.3% in marginal group and standard group, respectively ( P=0.580). Conclusion:The therapeutic effect of marginal donor liver in ACLF recipients is comparable to that of standard donor liver. The incidence of DGF is higher in recipients with marginal donor liver.
2.Correlation between the change track of metabolic syndrome index and thyroid nodules in individuals receiving health examinations

Objective:To analyze the correlation between metabolic syndrome indexes and thyroid nodules in individuals receiving health examinations.Methods:It′s a retrospective cohort study. A total of 2 678 individuals who received health examinations in the Health Management Center of the First Hospital of Shanxi Medical University for four consecutive years and met the admission criteria were selected as the research objects. According to the metabolic syndrome index data of health examination, according to the different duration of metabolic syndrome during the observation period, the group-based trajectory model, Bayesian information criterion and average posterior grouping probability were used to determine the best trajectory groups, and the objects were divided into three different metabolic syndrome index trajectory groups: normal, abnormal and recovery group. During the physical examination in 2022 and 2023, the detection of thyroid nodules in each group was followed up, and the difference of detection rate of thyroid nodules in different metabolic syndrome trajectory groups was compared by Log-rank test, and the correlation between different metabolic syndrome index change trajectories and thyroid nodules was analyzed by logistic regression model.Results:The cumulative detection rate of thyroid nodules in normal group, abnormal group and recovery group was 18.8% (77/410), 27.5% (327/1 190) and 24.7% (266/1 078), respectively ( χ2=19.482, P<0.001). In model 4, after adjusting for age, gender, smoking, drinking, staying up late, insomnia, physical activity, family history and other confounding factors, the risk of thyroid nodules in abnormal group and recovery group was still 2.011 times (95% CI: 1.457-2.776) and 2.006 times (95% CI: 1.389-2.897) of that in normal group. Conclusion:There is a positive correlation between metabolic syndrome index and thyroid nodules in individuals receiving health examinations, and metabolic syndrome index can be used as a predictive index of thyroid nodules.
3.Effect and Mechanism of Lobetyolin on Cholesterol Metabolism in HepG2 Cells

Objective To investigate the mechanism of lobetyolin's intervention in HepG2 cells abnormal cholesterol metabolism.Methods This study used oleic acid(OA)stimulation of HepG2 cells as a model.MTT assay,oil red O staining,biochemical kit assay,qRT-PCR assay,Western blot assay and NBD labeled cholesterol effection assay were used to study the effect of lobetyolin on cholesterol metabolism in HepG2 cells.Results The results showed that lobetyolin could reduce the content of lipid drops,the levels of triglyceride(TG)and total cholesterol(TC)in HepG2 cells stimulated by OA,increase cholesterol effection rate,and up-regulate cytochrome 7A1(CYP7A1),liver x receptor α(LXRα),ATP-binding cassette transporter A1(ABCA1),peroxisome proliferator-activated receptor(PPARγ)and other mRNA or protein expression levels.However,combined intervention with PPARγ antagonist Mifobate(SR-202)can significantly inhibit the promoting effect of lobetyolin on cholesterol metabolism in HepG2 cells.Conclusion This study revealed that lobetyolin can improve the cholesterol effection rate of HepG2 cells stimulated by OA and promote cholesterol catabolism,and the mechanism of action may be related to the regulation of PPARγ/CYP7A1 pathway.
4.The parallel mediating effects of anxiety and depression states between life events and behavior problems in adolescents

Objective:To explore the relationship between life events, anxiety, depression, and behavior problems in adolescents.Methods:From September to October 2022, the cluster sampling method was used to select 5 341 adolescents from 4 middle schools in Xinxiang urban area.The subjects and their parents were investigated by the adolescent self-rating life events check list (ASLEC), generalized anxiety disorder scale (GAD-7), patient health questionnaire (PHQ-9), and child behavior checklist (CBCL). SPSS 27.0 software was used for Spearman correlation analysis, and AMOS 28.0 software was used to construct the structural equation model.Results:The scores of anxiety, depression, and behavioral problems were 1 (0, 4), 1 (0, 4), and 3 (0, 10). The total score of life events was 5 (1, 13), and the dimensions scored as follows: interpersonal conflict 1 (0, 4), academic pressure 2 (0, 5), punishment 0 (0, 2), loss 0 (0, 0), health and adaptation problem 0 (0, 1), and others 0 (0, 2). There were positive correlations between life events and its dimensions, depression, anxiety and behavioral problems ( r=0.28-0.69, all P<0.01). In the overall population, anxiety and depression played parallel mediating roles in the impact of life events on behavior problems. Life events could positively predict anxiety ( β=0.68, P<0.01), and anxiety could positively predict behavior problems ( β=0.04, P=0.02). Life events could positively predict depression ( β=0.77, P<0.01), and depression could positively predict behavior problems ( β=0.18, P<0.01). The standardized total effect size of the impact of life events on behavioral problems was 0.622 (95% CI=0.564-0.675). The standardized direct effect size and indirect effect size were 0.460 (95% CI=0.374-0.539) and 0.162 (95% CI=0.108-0.218), accounting for 74.0% and 26.0%of the total effect, respectively. After stratification by gender, the results for male adolescents were consistent with the overall population, while the mediating effect of anxiety was not significant in the female adolescents. Conclusion:Life events can lead to anxiety and depression in adolescents, thereby increasing the risk of behavior problems.

9.Spleen Stiffness in Predicting High Risk Esophageal and Gastric Varices in Cirrhosis via Shear Wave Elastography

Purpose To evaluate the value of shear wave elastic imaging of liver and spleen in predicting high risk esophageal and gastric varices in cirrhosis.Materials and Methods Sixty-four patients with cirrhosis in the First Hospital of Lanzhou University from November 2019 to April 2022 were enrolled prospectively,and underwent gastroscopy,elastography and serological examination.According to gastroscopy,patients were divided into high-risk varices(HRV,30 cases)and non-HRV group(34 cases),the differences of each clinical index and shear wave elastic imaging parameter between the groups were compared.The diagnostic efficacy of liver and spleen stiffness measurement for HRV was analyzed.And the diagnostic models were established for exploring the diagnostic effectiveness of different models with respect to excluding HRV in compensated liver cirrhosis.Results There were significant differences in platelet[(74±24)×109/L vs.(124±50)×109/L],liver stiffness[(16.1±5.6)kPa vs.(13.6±5.5)kPa]and spleen stiffness[(41.7±8.9)kPa vs.(28.1±8.6)kPa]between HRV and non-HRV groups(t/Z/χ2=5.136,-1.832,-6.206,all P<0.05).The areas under the curve of liver stiffness and spleen stiffness for predicting HRV were 0.660 and 0.858,respectively.The Baveno Ⅵ combined with the spleen stiffness model had the best performance(30.0%)for excluding HRV.Conclusion The measurement of liver and spleen stiffness by shear wave elastography has great diagnostic value for predicting HRV in cirrhosis,and spleen stiffness is better than liver stiffness.
10.Current situation of intelligent health management service of examination (management) institutions in Shanxi Province

Objective:To analyze the status quo of intelligent health management services in health examination institutions in Shanxi Province.Methods:This study is a cross-sectional study. In October 2023, 292 health examination institutions in 11 cities of Shanxi Province were surveyed by convenient sampling with the help of “Questionnaire star” to investigate the health examination (management) institutions to carry out smart health management services from January to December 2022. It was mainly divided into three aspects: pre-inspection, in-inspection and post-inspection, including 6 dimensions, with a total of 21 items. A total of 300 questionnaires were distributed in this survey, and 292 were effectively collected, with an effective recovery rate of 97.33%. The 292 questionnaires were divided and compared according to four regions: the north of Shanxi Province, the middle of Shanxi Province, the south of Shanxi Province and the southeast of Shanxi Province.Results:The health examination (management) institutions were distributed in 11 prefectures and cities in Shanxi Province, and there were 56 (19.2%) tertiary health examination (management) institutions. There were 178 government organizations (61.0%) and 114 social organizations (39.0%). There were 256 (87.7%) that carried out smart health management services; among them, 229 (78.4%) had smart management service systems, and 58 (19.9%) had physical examination centers with smart health management clinics. A total of 108 institutions (36.99%) carried out follow-up visits after intelligent screening. The prevalence of intelligent referrals were 63.3%, 59.6%, 78.0% and 85.2% in the north of Shanxi Province, the middle of Shanxi Province, the south of Shanxi Province and the southeast of Shanxi Province, respectively ( χ2=14.02, P<0.05). The prevalence of self-service in the north of Shanxi Province, the middle of Shanxi Province, the south of Shanxi Province and the southeast of Shanxi Province were 39.2%, 52.3%, 62.0% and 37.0%, respectively ( χ2=9.75, P<0.05). Conclusions:The development of intelligent health management in health examination (management) institutions in Shanxi Province is good, and the service scale continues to grow, but the problem of uneven development among regions is severe. In addition, the proportion of institutions providing services through information technology is not high, and the rate of intelligent management outpatient service, intelligent post-examination follow-up and intelligent physical examination service satisfaction is low.
