2.Clinical significance of serum antioxidant markers in cephalofacial herpes zoster

Objective To investigate the expression change of serum antioxidant markers in cranofacial herpes zoster and its relationship with clinical severity and postherpetic neuralgia(PHN).Methods Totally 109 cases of cranofacial herpes zoster(cranofacial group)and 169 cases of non-cranofacial herpes zoster(non-cranofacial grlup)hospitalized in the Department of Dermatology in the Second Affiliated Hospital of Nanjing Medical University and 200 cases of health check-ups without underlying diseases(healthy control group)from January 2022 to December 2023 were selected,and the serum antioxidant markers including uric acid(UA),total bilirubin(TBIL),and albumin(ALB)of the patients in the three groups group were compared.Meanwhile,the relationship between these markers and clinical severity and PHN of cranofacial herpes zoster was assessed.Results The serum levels of UA,TBIL and ALB in the cranofacial group and non-cranofacial were significantly lower than those in the healthy control group(P<0.05).Serum UA,TBIL and ALB levels in cranofacial group were significantly lower than those in non-cranofacial group(P<0.05),and were nega-tively correlated with disease severity(P<0.05).Serum UA,TBIL and ALB levels in cranofacial group were independent influencing factors for the occurrence of PHN(P<0.05),and had predictive value for PHN in cranofacial herpes zoster(P<0.05).Conclusion Reactivation of cranofacial herpes zoster virus,acute nerve injury and PHN may all be related to the low antioxidant status of the body,and the antioxidant biomarkers UA,TBIL and ALB may be protective factors for herpes zoster,but more studies are needed to clarify the un-derlying mechanisms.
3.Application of cup-stacking task based on the dyadic coping model in patients with ischemic stroke

Objective:To evaluate the effects of cup-stacking task based on the dyadic coping model in patients with ischemic stroke, in order to provide reference for clinical nurses to effectively rehabilitate these patients and improve their disease coping ability.Methods:This was a quasi-experimental study, a total of 90 patients with upper extremity motor function disorder after ischemic stroke admitted to the Neurology of the First Affiliated Hospital of Zhengzhou University were selected as participants from October 2022 to March 2023. Among them, 45 patients admitted from October 2022 to December 2022 were selected as the control group, 45 patients admitted from January 2023 to March 2023 were selected as the experimental group. Patients in the control group received routine care and patients in the experimental group received cup-stacking task based on the dyadic coping intervention model. The dyadic coping between the two groups and their spouses were compared before intervention and after intervention(3 months after discharge), the upper extremity motor function, activities of daily living, anxiety, rehabilitation self-efficacy between the two groups were compared before the intervention and after intervention.Results:A total of 5 patients were dropped and 43 patients of the control group and 42 patients of the experimental group completed the study at last. In the control group, there were 28 males, 17 females, aged (61.84 ± 7.13) years old; while their spouses were 17 males, 26 females, aged (61.02 ± 6.79) years. In the experimental group, there were 28 males, 14 females, aged (62.36 ± 7.03) years old; while their spouses were 14 males, 28 females, aged (60.95 ± 6.81) years. After the intervention, the scores of the dyadic coping between the experimental group patients and their spouses were (135.05 ± 8.52), (139.24 ± 9.67) points, which were higher than (119.26 ± 12.23), (120.02 ± 12.34) points of the control group, the differences were statistically significant ( t=6.92, 8.00, both P<0.05); the scores of the upper extremity motor function, activities of daily living, rehabilitation self-efficacy of the experimental group patients were (55.48 ± 4.78), (79.55 ± 6.83), (83.64 ± 10.30) points, which were higher than (51.44 ± 6.20), (72.74 ± 8.93), (70.28 ± 13.13) points of the control group, the differences were statistically significant ( t=3.36, 3.94, 5.21, all P<0.05); the score of anxiety was (13.26 ± 2.96) points, which was lower than (18.53 ± 3.35) in the control group, and the difference was statistically significant ( t=-7.69, P<0.05). Conclusions:The cup-stacking task based on the dyadic coping model can effectively improve the dyadic coping level of patients after ischemic stroke and their spouses, improve the patients′ upper extremity motor function and rehabilitation self-efficacy, so as to facilitate disease recovery and improve the quality of life, which is worthy of clinical promotion.
4.High-throughput screening of novel TFEB agonists in protecting against acetaminophen-induced liver injury in mice.

Macroautophagy (referred to as autophagy hereafter) is a major intracellular lysosomal degradation pathway that is responsible for the degradation of misfolded/damaged proteins and organelles. Previous studies showed that autophagy protects against acetaminophen (APAP)-induced injury (AILI) via selective removal of damaged mitochondria and APAP protein adducts. The lysosome is a critical organelle sitting at the end stage of autophagy for autophagic degradation via fusion with autophagosomes. In the present study, we showed that transcription factor EB (TFEB), a master transcription factor for lysosomal biogenesis, was impaired by APAP resulting in decreased lysosomal biogenesis in mouse livers. Genetic loss-of and gain-of function of hepatic TFEB exacerbated or protected against AILI, respectively. Mechanistically, overexpression of TFEB increased clearance of APAP protein adducts and mitochondria biogenesis as well as SQSTM1/p62-dependent non-canonical nuclear factor erythroid 2-related factor 2 (NRF2) activation to protect against AILI. We also performed an unbiased cell-based imaging high-throughput chemical screening on TFEB and identified a group of TFEB agonists. Among these agonists, salinomycin, an anticoccidial and antibacterial agent, activated TFEB and protected against AILI in mice. In conclusion, genetic and pharmacological activating TFEB may be a promising approach for protecting against AILI.
5.A preliminary clinical study of polyester spacer-enhanced pancreatico-intestinal anastomosis in pancreaticoduodenectomy

Objective:To investigate the safety and utility of using polyester spacers in conventional pancreatico-enteric anastomosis in pancreaticoduodenectomy to achieve prevention of postoperative pancreatic fistula.Methods:The clinical data of 82 patients with pancreaticoduodenectomy completed by the same physician in Anyang People's Hospital from August 2018 to August 2023 were retrospectively analyzed, including 52 males and 30 females, aged (62.21±9.75) years. They were divided into two groups, test group ( n=40) and control group ( n=42) according to whether polyester spacers were used in pancreatico-intestinal anastomosis, the perioperative data of the two groups were compared, and Logistic analysis was used to analyze the risk factors affecting postoperative pancreatic fistula. Results:There was no statistically significant difference between the two groups in terms of biliary fistula, intestinal fistula, abdominal infection, abdominal bleeding, and postoperative mortality rate (all P>0.05). The incidence of postoperative pancreatic fistula in the test group was 20.0% (8/40) which was lower than that in the control group 45.2% (19/42), and the difference was statistically significant ( P=0.015). Soft pancreas texture ( OR=16.595, 95% CI: 1.891~145.657) was an independent risk factor for postoperative pancreatic fistula, while improved pancreatic enterostomy with polyester spacers ( OR=0.332, 95% CI: 0.114~0.969) could reduce the risk of postoperative pancreatic fistula. Conclusion:Use of polyester spacers to reinforce the pancreatico-enteric anastomosis during pancreaticoduodenectomy reduces the incidence of postoperative pancreatic fistulae with good safety and practicality.
6.Effects of attribution training in stroke patients

Objective:To explore the effects of attribution training on coping style, stigma, quality of life and negative emotions of stroke patients.Methods:From June 2019 to June 2020, 85 patients with ischemic stroke admitted to the First Affiliated Hospital of Zhengzhou University were selected by simple random sampling. The patients were randomly divided into the control group (42 cases) and the observation group (43 cases) . The control group was given routine nursing, while the observation group received attribution training on the basis of the control group. Before and one and three months after the intervention, the patients in the two groups were evaluated with the Attributional Style Questionnaire (ASQ) , the Stigma Scale for Chronic Illness-8 (SSCI-8) , the World Health Organization Quality of Life-100 (WHOQOL-100) , the Self-Rating Anxiety Scale (SAS) , and the Self-Rating Depression Scale (SDS) .Results:There were statistical differences between the two groups in time main effect, intervention main effect, interaction between intervention and time of ASQ, SSCI-8, WHOQOL-100 scores ( P<0.05) . Before intervention, there was no statistical difference in SAS and SDS scores between the two groups ( P>0.05) . Three months after the intervention, the SAS and SDS scores of the two groups were lower than those before the intervention, and those of the observation group were lower than those of the control group, and the differences were statistically significant ( P<0.05) . Conclusions:Attribution training can effectively improve the coping style of stroke patients, improve their quality of life, reduce patients' stigma, anxiety and depression, which is worthy of clinical promotion.
7.Effects of hypoxic preconditioning on energy metabolism of mitochondria in mouse hippocampal HT22 cells

Objective :
To investigate the effect of hypoxic preconditioning (HPC) on mitochondrial energy metabolism in mouse hippocampal HT22 cells and its possible mechanism.
Methods :
In this paper, mouse hippocampal nerve cells HT22 were divided into control group, hypoxia group, HPC group, and the levels of adenosine triphosphate (ATP) and reactive oxygen species (ROS) in each group were measured for observing the effect of HPC on cell mitochondrial metabolism. Western blot was used to detect the expression of target of rapamycin ( mTOR), phosphorylated mTOR protein and autophagy substrate P62 protein; cellular immunofluorescence was used to detect phosphorylated mTOR, and LysoTrackerTM probe was used to detect lysosomes.
Results :
Compared with the control group, the ATP level was significantly decreased and the ROS level was increased in the hypoxia group. Exposed to HPC, the ATP level was increased and the ROS level was decreased. Compared with the control group, the expression of phosphorylated mTOR was down⁃regulated and the expression of autophagy substrate P62 was down⁃regulated in the HPC group.
Conclusion
HPC may affect the energy metabolism of HT22 cells through the mTOR/autophagy signaling pathway, thereby exerting a protective effect on the HT22 cells.
8.Inhibition of Long Noncoding RNA SNHG15 Ameliorates Hypoxia/Ischemia-Induced Neuronal Damage by Regulating miR-302a-3p/STAT1/NF-κB Axis

Purpose:
Ischemic brain injury results in high mortality and serious neurologic morbidity. Here, we explored the role of SNHG15 in modulating neuronal damage and microglial inflammation after ischemia stroke.
Materials and Methods:
The hypoxia/ischemia models were induced by middle cerebral artery occlusion in mice and oxygenglucose deprivation/reoxygenation (OGD/R) in vitro. Quantitative real-time PCR (qRT-PCR) and Western blot were conducted to determine the levels of SNHG15, miR-302a-3p, and STAT1/NF-κB. Moreover, gain- or loss-of functional assays of SNHG15 and miR-302a-3p were conducted. MTT assay was used to evaluate the viability of HT22 cells, and the apoptotic level was determined by flow cytometry. Furthermore, enzyme-linked immunosorbent assay was performed to detect oxidative stress and inflammatory mediators in the ischemia cortex and OGD/R-treated BV2 microglia.
Results:
The SNHG15 and STAT1/NF-κB pathways were both distinctly up-regulated, while miR-302a-3p was notably down-regulated in the ischemia cortex. Additionally, overexpressing SNHG15 dramatically enhanced OGD/R-mediated neuronal apoptosis as well as the expression of oxidative stress and inflammation factors from microglia. In contrast, knocking down SNHG15 or overexpressing miR-302a-3p relieved OGD/R-mediated neuronal apoptosis and microglial activation. Moreover, the rescue experiment testified that overexpressing miR-302a-3p also attenuated SNHG15 up-regulation-induced effects. In terms of the mechanisms, SNHG15 sponged miR-302a-3p and activated STAT1/NF-κB as a competitive endogenous RNA, while miR-302a-3p targeted STAT1 and negatively regulated the STAT1/NF-κB pathway.
Conclusion
SNHG15 was up-regulated in the hypoxia/ischemia mouse or cell model. The inhibition of SNHG15 ameliorates ischemia/hypoxia-induced neuronal damage and microglial inflammation by regulating the miR-302a-3p/STAT1/NF-κB pathway.
9.Research on prenatal genetic diagnosis of fetal renal cystic disease

Objective:To summarize the diagnosis features of the prenatal genetic diagnosis of fetal renal cystic disease and to explore the clinical feasibility and significance of prenatal genetic diagnosis of congenital cystic nephrosis.Methods:A total of 25 fetuses with congenital renal cystic disease were examined via invasive prenatal diagnosis in Henan Provincial People's Hospital from June 2017 to September 2019. Amniotic fluid samples were extracted by amniocentesis. Chromosomal microarray analysis (CMA) were performed in 17 cases. In addition to CMA, the other 8 cases were analyzed by G-band karyotype. Whole exome sequencing (WES) was performed in 6 cases which got normal results by CMA and karyotype, and highly suspected as hereditary disease.Results:Of the 25 fetuses assessed, 4 cases (16.0%) pathogenic copy number variation (pCNV) were found, including 2 cases of 17q12 deletion, 1 case of 10p15.1p14 deletion and 1 case of 4q21.28q22.1 deletion(including PKD2 gene). There were 8 cases without chromosome abnormality by karyotype analysis. Six clinical WES analysis found NPHS1 gene c.1440+1 G>A and c.925G > T mutations were related to Finnish type congenital nephrotic syndrome in 1 case, PKD1 gene c.6878C>T mutation was related to autosomal dominant polycystic kidney disease (ADPKD) in 1 case, and there was no definitive mutation in 4 cases. Conclusions:CMA and next generation sequencing are powerful tools for accurate diagnosis, treatment and genetic counseling of fetal congenital renal cystic diseases. For congenital cystic nephropathy, genetic detection is helpful to clarify the etiology, and provide more exactly informations for prognosis evaluation, treatment and family genetic counseling.
10.Effect of expressive writing in young and middle-aged patients with colorectal cancer

Objective:To explore the effect of expressive writing in young and middle-aged patients with colorectal cancer, so as to provide a basis for clinical intervention.Methods:From January to November 2019, convenience sampling was used to select 92 young and middle-aged colorectal cancer patients who underwent surgery in the First Affiliated Hospital of Guangxi Medical University as the research objects. Patients were divided into control group ( n=48) and observation group ( n=44) by the random method. Patients in control group received routine care during hospitalization for patients with colorectal cancer, and patients in observation group added expressive writing intervention on the basis of routine care. The Profile of Mood State-Short Form (POMS-SF) , Meaning in Life Questionnaire (MLQ) , Pittsburgh Sleep Quality Index (PSQI) were used to evaluation one to two days after admission and one day before discharge. Results:A total of 44 valid questionnaires were returned in control group and 43 valid questionnaires in observation group. After intervention, there were statistical differences in the dimension scores of tension-anxiety, depression-frustration, fatigue-indulgence, confusion-mull and energy-vigor in POMS-SF of patients between two groups ( P<0.05) . After intervention, the scores of the sense of meaning of life and searching for the meaning of life in the MLQ of observation group were higher than those of control group, and the differencs were statistically significant ( P<0.05) . There was no statistically significant difference in PSQI scores between two groups before and after intervention ( P>0.05) . Conclusions:Expressive writing has a positive effect on improving negative emotions and sense of meaning in life for young and middle-aged patients with colorectal cancer.
