1.Predictive Value of Residual Quantitative Flow Ratio for Long-term Vessel-oriented Composite Endpoints

Objectives:To explore the predictive value of residual Murray's law-based quantitative flow ratio(μQFR)on long-term vessel-oriented composite endpoints(VoCE).Methods:This retrospective study included 3 510 patients from the FAVOR Ⅲ China trial.Offline residual μQFR analysis was performed on all vessels(diameter≥2.5 mm)with 50%-90%stenotic lesions.Patients were stratified into high-,intermediate-,and low-risk groups based on residual μQFR tertiles.The primary endpoint was 3-year VoCE,defined as a composite of cardiac death related to the target vessel,target vessel-related spontaneous myocardial infarction,and ischemia-driven target vessel revascularization.Results:Offline analysis was performed on 5 256 vessels from 3 510 patients.The mean residual μQFR was 0.92±0.75.The high-risk group(residual μQFR≤0.91)with 1 554 patients(1 958 vessels);the intermediate-risk group(residual μQFR 0.92-0.96)with 1 211 patients(1 906 vessels);and the low-risk group(residual μQFR>0.96)with 745 patients(1 392 vessels).Over 3-year follow-up,VoCE occurred in 227 vessels(4.3%).The 3-year VoCE incidence was significantly higher in the high-risk group compared to the intermediate-and low-risk groups(6.2%vs.4.1%vs.2.5%,log-rank P<0.001),primarily driven by ischemia-driven target vessel revascularization(5.0%vs.3.0%vs.1.6%,log-rank P<0.001).Hypertension(OR=0.83,95%CI:0.72-0.96),hypercholesterolemia(OR=0.84,95%CI:0.73-0.97),bifurcation lesions(OR=0.72,95%CI:0.63-0.83),moderate/severe calcification(OR=0.70,95%CI:0.57-0.84),and tandem lesions(OR=0.59,95%CI:0.47-0.75)were independent predictors of lower residual μQFR values.Conclusions:Lower residual μQFR is significantly associated with increased VoCE risk during the 3-year follow up period.
2.Protective effect of achyranthes bidentata against doxorubicin-induced spermatogenic disorder in mice:An investigation based on the glycolytic metabolic pathway

Objective:To investigate the protective effect of achyranthes bidentata(AB)on sperm quality in mice with sper-matogenic disorder through the glycolytic metabolic pathway and its action mechanism.Methods:We equally randomized 40 Kun-ming mice into a normal control,a model control,a low-dose AB(3.5 g/kg)and a high-dose AB group(7.0 g/kg),and established the model of spermatogenic disorder in the latter three groups of mice by intraperitoneal injection of doxorubicin(30 mg/kg).Two days after modeling,we collected the testis and kidney tissues and blood samples from the mice for observation of the pathological changes in the testis tissue by HE staining,detection of perm motility with the sperm quality analyzer,examination of the apoptosis of testis cells by flow cytometry,measurement of the levels of testosterone(T),malondialdehyde(MDA),superoxide dismutase(SOD)and cata-lase(CAT)in the serum and testis tissue by ELISA,and determination of expressions of the key enzymes of glycolysis hexokinase Ⅱ(HK2),pyruvate kinase M2(PKM2),platelet phosphofructokinase(PFKP),lactate dehydrogenase A(LDHA)and the meiosis pro-teins REC8 and SCP3 by Western blot,and the mRNA expressions of glycolytic phosphofructokinase 1(PFK1),phosphoglycerate ki-nase 1(PGK1),tumor necrosis factor-α(TNF-α)and interleukin-1 β(IL-1β)by fluorescence quantitative PCR(FQ-PCR).Results:Compared with the model controls,the mice in the AB groups showed significant increases in the testis coefficient,kidney in-dex,sperm concentration,sperm motility,spermatogonia,primary spermatocytes,spermatids,sperm count and the serum T level(P<0.05 orP<0.01),but dramatic decreases in the apoptosis of testis cells and percentage of morphologically abnormal sperm(P<0.01).Achyranthes bidentata also significantly elevated the levels of SOD and CAT,and down-regulated the mRNA expressions of MDA,TNF-α and IL-1β(P<0.05 or P<0.01),and up-regulated the protein expressions of HK2,PKM2,PFKP,LDHA,REC8 and SCP3,and expressions of the glycolysis key genes Pfk1 and Pgk1(P<0.05 orP<0.01).Conclusion:Achyranthes bidentata ameliorates doxorubicin-induced spermatogenic disorder in mice by regulating the glycolytic pathway and reducing oxidative stress and the expressions of inflammatory factors.

Recent studies have shown that the physiological homeostasis of oral mucosal cells is regulated by the circadian clock.Dis-ruption or dysfunction of the circadian clock is closely associated with the development of oral squamous cell carcinoma(OSCC).Research based on the circadian clock offers a novel perspective on the pathogenesis and therapeutic strategies for OSCC.However,there is current-ly limited research on this topic,and people generally have insufficient understanding and recognition of the circadian clock.Given the complexity and challenges of circadian clock which is the fourth dimension of medical research,we organize relevant experts based on summarizing the current research results of circadian clock in the pathogenesis and precision diagnosis and treatment of OSCC,combining the scientific principles of the circadian clock's role and their long-term research experience,then summarizes and recommends the con-sensus opinions for the research of circadian clock in the pathogenesis mechanism and precision diagnosis and treatment of human OSCC,with the hope of providing guidance for the basic research and clinical application of circadian clock or circadian rhythm in the pathogene-sis mechanism and precision diagnosis and treatment of oral and maxillofacial squamous cell carcinoma.
4.Dynamic changes of serum cytokines in mice following gastrointestinal infection with porcine-derived Proteus mirabilis

Proteus mirabilis is a significant foodborne pathogen that frequently causes diseases in humans and animals.Understanding the cytokine profile alterations following gastrointestinal in-fection with P.mirabilis is crucial for developing effective treatment strategies.This study em-ployed an intragastric infection model in mice to observe clinical symptoms and pathological chan-ges.Blood samples were collected at various time points post-infection,and the concentrations of 21 cytokines in the serum of both infected and control mice were quantitatively analyzed using the Bio-Plex suspension array system.The results of the infection experiment indicated that gastroin-testinal infection with P.mirabilis in mice led to splenic hemorrhage and significant splenomegaly.A notable difference in spleen index between the infected and control groups was observed at 24 hours post-infection(P=0.001 688),with the greatest difference occurring at 96 hours post-infec-tion(P=0.000 074).Cytokine analysis revealed significant elevations in IL-2,IL-17A,KC,Eotax-in,MCP-1,RANTES,MIP-1α,MIP-1β,IFN-γ,G-CSF,and IL-10 levels in the infected group.A-mong these,the pro-inflammatory cytokines IL-17A and G-CSF exhibited the most substantial changes,positively correlating with the splenomegaly,and peaking at 96 hours post-infection,with concentrations of(104.74±3.91)and(5 184.08±280.22)ng/L,respectively,which were 26.79 and 36.25 times higher than those in the control group.These findings indicate that IL-17A and G-CSF play significant roles in the pro-inflammatory response following gastrointestinal infection with P.mirabilis.

6.Efficacy comparison of laparoscopic novel anti-reflux surgery and traditional fundoplication in the treatment of gastroesophageal reflux disease complicated with hiatal hernia

Objective:To evaluate the efficacy of different anti-reflux surgery combined with laparoscopic hiatal hernia repair in the treatment of gastroesophageal reflux disease (GERD) complicated with hiatal hernia.Methods:A retrospective analysis was conducted on 405 patients with GERD and hiatal hernia undergoing laparoscopic hiatal hernia repair combined with different anti-reflux surgery at the First Affiliated Hospital of Zhengzhou University from Jan 2013 to Jun 2019. The patients were divided into four groups based on the anti-reflux surgery: 38 patients underwent Nissen fundoplication (Nissen group), 67 patients underwent Toupet fundoplication (Toupet group), 101 patients underwent Dor fundoplication (Dor group), and 199 patients underwent His angle plasty (His group). The improvement of symptoms, the use of proton pump inhibitor (PPI), surgical outcomes, and postoperative complications were compared among the four groups at 5 years after surgery.Results:Compared with preoperative scores, the scores of acid regurgitation and heartburn in all four groups decreased significantly after surgery (Nissen group: Z=-4.974, -4.905; Toupet group: Z=-9.245, -9.413; Dor group: Z=-8.080, -8.225; His group: Z=-11.705, -11.619, all P<0.05), and there was no significant difference in postoperative symptom scores among the four groups (acid regurgitation: 2.2 vs. 2.0 vs. 1.9 vs. 1.9; heartburn: 2.0 vs. 1.7 vs. 1.7 vs. 1.8, all P>0.05). There was no significant difference in the total effective rate of treatment among the four groups (86.8% vs. 92.5% vs. 87.1% vs. 91.5%, χ2=2.314, P=0.510). Additionally, there was no significant difference in the PPI independence among the four groups (63.2% vs. 61.2% vs. 60.4% vs. 60.3%, χ2=0.120 P=0.989). The incidence of severe dysphagia and abdominal distension was higher in the three fundoplication groups (Nissen, Toupet, and Dor groups) than in the His group (all P<0.05). Conclusions:Laparoscopic hiatal hernia repair combined with different anti-reflux surgery is safe and effective in the long-term treatment of GERD complicated with hiatal hernia. His angle plasty has the advantages of simple surgical operation and low incidence of postoperative dysphagia and abdominal distension, and is a recommended new anti-reflux surgery.

8.Conceptual analysis of family avoidance of communication about cancer

Objective:To systematically analyze the definition, development, conceptual application, characteristic attributes, extension, and similar concepts of family avoidance of communication about cancer through the conceptual analysis method, so as to clarify the conceptual connotation of family avoidance of communication about cancer.Methods:Literature on family avoidance of communication about cancer was searched in WanFang Data, VIP, China National Knowledge Infrastructure, China Biomedical Database, CINAHL, Embase, PubMed, PsycINFO and so on. The search period was from database establishment to December 31, 2023. Walker and Avant's conceptual analysis method was used for analysis.Results:Through a compilation and analysis of relevant literature, the conceptual characteristics of family avoidance of communication about cancer were summarized, including bidirectional influence (both the cancer patient and family members tend to conceal their self-emotions and influence each other), information concealment (lack of communicative behaviors on both sides) and the desire to protection (aiming to hopefully protect each other) .Conclusions:The conceptual attributes of family avoidance of communication about cancer compiled can provide a basis for the proper understanding of related concepts within the subject area and provide conceptual support for the practice and research of family avoidance of communication about cancer.

10.Mechanistic study of Yigan Fupi decoction-mediated repair of the intestinal barrier and reduction of IBS sensitivity through regulation of the PKA/PKC-CREB pathway

Objective To study the therapeutic effects of Yigan Fupi decoction(YGFP)on irritable bowel syndrome(IBS)and its mechanism of action in repairing the intestinal barrier and reducing IBS sensitivity through the PKA/PKC-CREB pathway.Methods Baby rats separated from their mother were randomly divided into a model control(M)and a YGFP group,while baby rats without maternal separation were used as a normal control(N)group.The YGFP group was given YGFP for 4 weeks.Abdominal withdrawal reflux was used to evaluate intestinal sensitivity.Liquid chromatography tandem mass spectrometry and ELISA were used to detect bile acid metabolite concentrations and serum levels of interleukin(IL)-6 and CXCL1,respectively.HE staining was used to observe pathological changes in the colon,and Western Blot and immunohistochemistry were used to analyze the relative protein expression levels of PKA,PKC,CREB,5HT2AR,5-HT7R,ZO-1,and Claudin 1.Results Compared with the normal control group,the M group showed a significantly decreased visceral pain threshold,significantly increased levels of total bile acid metabolites,IL-6,and CXCL1,significantly increased relative expression of PKA,PKC,CREB,5HT2AR,and 5-HT7R,and significantly decreased relative expression of ZO-1 and Claudin 1.Compared with the M group,the YGFP group showed a significantly increased visceral pain threshold,significantly reduced levels of total bile acid metabolites,IL-6,and CXCL1,significantly reduced relative expression of PKA,PKC,CREB,5HT2AR,and 5-HT7R,and increased relative expression of ZO-1 and Claudin 1.Conclusions YGFP effectively improved IBS through a mechanism that may involve repair of the intestinal barrier and reduced sensitivity through the PKA/PKC-CREB pathway.
