1.Microcirculatory Dysfunction After Percutaneous Coronary Intervention Predicts the Early Prognosis of Patients With ST-segment Elevation Myocardial Infarction

Objectives:To explore the predictive value of angiography-derived index of microcirculatory resistance(Angio-IMR)for early prognosis in patients with acute ST-segment elevation myocardial infarction(STEMI)after percutaneous coronary intervention(PCI).Methods:This multicenter study enrolled 1 629 consecutive STEMI patients who underwent successful PCI at three grade A tertiary hospitals(The Second Affiliated Hospital,Zhejiang University School of Medicine;Shengjing Hospital of China Medical University;Renji Hospital,Shanghai Jiao Tong University School of Medicine)from June 1,2017,to May 31,2020.According to postoperative Angio-IMR,patients was stratified into two groups:the Angio-IMR>40 group(n=508)and the Angio-IMR≤40 group(n=1 121).The incidence of major adverse cardiovascular events(MACE;defined as a composite endpoint including cardiac death,heart failure rehospitalization,cardiogenic shock,malignant arrhythmia,cardiopulmonary resuscitation and stent thrombosis)within 1-month post-PCI was compared between the two groups.Results:The median Angio-IMR after PCI was 32.4(22.3,42.6).The cumulative incidence of early-term MACE was significantly higher in patients with Angio-IMR>40,compared to those with Angio-IMR≤40(5.5%vs.2.3%,log-rank P<0.001).Multivariate Cox regression analysis showed that Angio-IMR>40 was an independent predictor of early-term MACE(HR=2.07,95%CI:1.20-3.58,P=0.009).The addition of Angio-IMR enhanced the predicting performance of the clinical risk model to predict early adverse outcomes(AUC:0.820 vs.0.794,P=0.043).Conclusions:In patients with STEMI after PCI,Angio-IMR can predict the occurrence of early-term MACE.The incorporation of Angio-IMR to clinical models significantly improves the model ability to predict early adverse outcomes in these patients.
2.Effect of salidroside combined with rosavin on ischemic brain injury in rats

Aim To study the mechanism of salidro-side combined with rosavin in rats with ischemic stroke.Methods The MCAO rats was established by using thread-embolic method.The rats were divided into the sham group,MCAO group,salidroside com-bined with rosavin group,and positive control group;the drug was given continuously for seven days.Western blot was used to detect apoptosis indicators.Proteomics was used to analyse differential proteins(DEPs).STEP receptor inhibitor was injected into the lateral ventricles,the rats were administered for seven days,then the apoptosis indicators were detected.Re-sults Salidroside combined with rosavin could reduce neurological function scores in MCAO rats and inhibit cell apoptosis.Quantitative proteomics identified 496 DEPs in brain tissue and discovered core proteins STEP,p38,and CRTC1.Salidroside combined with rosavin could promote the STEP and CRTC1 while in-hibiting p38 protein.After treatment with STEP inhibi-tor,those effects were reversed.Conclusion Salidro-side combined with rosavin can inhibit cell apoptosis in MCAO rats,which is closely related to the regulation of the STEP/p38/CRTC1 signaling pathway.

4.Effect of laparoscopic surgery on incarcerated inguinal hernia in children

Objective To evaluate the effect of laparoscopic surgery on incarcerated inguinal hernia(IIH)in children.Methods A total of 81 IIH children treated at Jinhua Maternal & Child Health Hospital from September 2018 to December 2023 were selected as subjects.The children were divided into laparoscopic surgery group(n=45)and open surgery group(n=36).Comparative analysis was conducted on admission age,gender,duration of incarceration,surgical timing,intraoperative hernia contents,and postoperative complications between two groups.Results Operative duration,intraoperative blood loss,postoperative bowel function recovery time,number of occult hernias,and hospitalization duration in laparoscopic surgery group were statistically better than those in open surgery group(P<0.05).There were fewer postoperative complications in laparoscopic group compared to open surgery group,there was significant difference between two groups(P<0.05).Conclusion Compared with traditional open surgery,laparoscopic surgery for IIH in children has the advantages of less trauma,shorter operation time and hospitalization time,and less intraoperative bleeding.

This consensus was developed by the Orthodontic Society of the Chinese Stomatological Association to provide a systematic, scientific, and practical guideline for informed consent in orthodontic care. Orthodontic treatment is typically lengthy, highly individualized, and involves multiple factors such as growth and development, occlusal function, and facial esthetics. Rapid technological advances and diverse risk profiles make the traditional reliance on orthodontist experience or institutional templates insufficient to ensure patients′ full understanding and autonomous decision-making. To address this, the expert panel conducted extensive reviews of domestic and international guidelines, analyzed representative dispute cases, and performed multicenter patient-clinician surveys. Using a multi-round Delphi method, the group established a standardized informed consent framework covering the initial consultation, treatment, and retention phases. The consensus emphasizes that informed consent is not only a fundamental legal and ethical requirement but also a key step in building trust, improving patient compliance, and enhancing treatment satisfaction. Orthodontists should clearly and comprehensively explain treatment plans, potential risks, uncertainties, and associated costs, while respecting the autonomy of patients or guardians, and maintain continuous communication and dynamic evaluation throughout the treatment process. The release of this consensus provides unified and authoritative guidance for clinical orthodontics, helping to standardize informed consent, enhance its transparency, safeguard patient rights, reduce medical risks, and promote high-quality, sustainable development of orthodontic practice.
7.Mechanism of action of D-limonene on steatosis in primary hepatocytes based on AMPK/ACC/CPT1A signaling pathway

Aim To explore the effects of D-limonene on the steatosis of primary mouse hepatocytes and its potential mechanism of action.Methods Oleic acid-induced steatosis in primary mouse hepatocytes was used as a model to observe the effects of D-limonene on cell viability,cellular lipid content,and intracellular expression of proteins such as AMP-activated protein kinase(AMPK),acetyl-coenzyme A carboxylase 1(ACC1),and carnitine palmitoyl transferase 1A(CPT1A).Results It was found that a low dose of D-limonene could effectively enhance the viability of primary mouse hepatocytes.When oleic acid at a con-centration of 300 μmol·L-1 successfully induced steatosis in primary mouse hepatocytes,D-limonene re-duced the lipid content of the cells,and D-limonene up-regulated the cellular AMPK expression level,down-regulated the cellular ACC1 and fatty acid synthetase(FAS)expression levels,which in turn promoted the overexpression of CPT1A.Conclusions D-limonene has the effect of reducing lipid deposition in primary mouse hepatocytes,and the mechanisms may be related to the activation of AMPK,the inhibitions of ACC1 and FAS,and the up-regulation of CPT1A protein expres-sion level.
8.The value of the monocyte to lymphocyte ratio in predicting the prognosis of patients with liver cirrhosis complicated with esophageal and gastric varices

Objective:To explore the application value of the new inflammatory index in predicting the prognosis of patients with liver cirrhosis complicated with esophageal and gastric varices.Methods:This study was a single-center cross-sectional study. Patients who were hospitalized in the Department of Gastroenterology of Minhang Hospital, Fudan University due to liver cirrhosis combined with esophageal and gastric varices from January 2019 to February 2024 were included. All patients were diagnosed clearly based on clinical data, laboratory tests and imaging examination results. According to the results of the first fasting blood routine test after the patient′s admission, the monocyte/lymphocyte ratio (MLR), neutrophil/lymphocyte ratio (NLR), platelet/lymphocyte ratio (PLR), and systemic immune inflammation index (SII) were calculated. Multivariate Cox regression analysis was used to screen the independent risk factors affecting the prognosis of patients with liver cirrhosis complicated with esophagogastric varices. Receiver operating characteristic (ROC) curve analysis was used to determine the optimal cutoff value of MLR on survival. Kaplan-Meier survival analysis was used to compare the survival differences between patients with MLR>0.33 and those with MLR≤0.33.Results:A total of 177 patients were included, among which 155 were in the survival group and 22 were in the death group. The results of multivariate Cox regression analysis indicated that MLR was an independent risk factor for the prognosis of patients with liver cirrhosis complicated with esophageal and gastric varices [ HR=5.16(95% CI: 1.39-19.15), P=0.014]. The Kaplan-Meier survival curve analysis results showed that the survival rate of patients in the MLR>0.33 group was significantly lower than that in the MLR≤0.33 group (80.90% vs 94.32%, P=0.012). Stratified analysis showed that in patients who did not receive endoscopic intervention, MLR was closely related to prognosis. Conclusions:MLR can be used as an independent risk factor for the prognosis of patients with liver cirrhosis complicated with esophageal and gastric varices, which is helpful for clinicians to conduct risk assessment and thereby formulate personalized treatment strategies.
9.Conceptual analysis of family avoidance of communication about cancer

Objective:To systematically analyze the definition, development, conceptual application, characteristic attributes, extension, and similar concepts of family avoidance of communication about cancer through the conceptual analysis method, so as to clarify the conceptual connotation of family avoidance of communication about cancer.Methods:Literature on family avoidance of communication about cancer was searched in WanFang Data, VIP, China National Knowledge Infrastructure, China Biomedical Database, CINAHL, Embase, PubMed, PsycINFO and so on. The search period was from database establishment to December 31, 2023. Walker and Avant's conceptual analysis method was used for analysis.Results:Through a compilation and analysis of relevant literature, the conceptual characteristics of family avoidance of communication about cancer were summarized, including bidirectional influence (both the cancer patient and family members tend to conceal their self-emotions and influence each other), information concealment (lack of communicative behaviors on both sides) and the desire to protection (aiming to hopefully protect each other) .Conclusions:The conceptual attributes of family avoidance of communication about cancer compiled can provide a basis for the proper understanding of related concepts within the subject area and provide conceptual support for the practice and research of family avoidance of communication about cancer.
