An assay was established for detection of toxigenic Clostridioides difficile by combining microfluidic chip analysis with immunochromatography, and its performance was evaluated and compared with those of the Xpert C.difficile/Epi and VIDAS CD AB tests.Primer pairs were designed according to the tcdB and tpi genes in C.difficile.The specificity, limit of detection, reproducibility, and stability were evaluated.A total of 215 stool samples from patients with diarrhea were collected and tested in parallel with the Xpert C.difficile/Epi,VIDAS CDAB,and our assay.C.difficile was isolated from samples, and the tcdB gene was identified when discrepant results were obtained from the three above assays. Our assay showed no cross-reaction with other diarrhea-associated pathogens. Its reproducibility was 100%in testing of two standard plasmids containing tcdB and tpi genes at two concentrations(105 and 102 copies/μL).Two standard plasmids were detected after the PCR and immunochromatography reagents had been stored for 3,6,9,and 12 months, and all the results were posi-tive.The limit of detection was 10 copies/μL for toxigenic C.difficile.Testing of 33 samples positive for C.difficile with our assay(33/215,15.3%)yielded findings statistically coherent with those of the Xpert C.difficile/Epi test(kappa value=0.965).The sensitivity, specificity, positive predictive value, and negative predictive value of our assay, with respect to Xpert C.difficile/Epi as the standard, were 94.3%,100.0%,100.0%,and 98.9%;these values were significantly higher than those of VIDAS CDAB(60.0%,98.9%,91.3%,and 92.7%)(Kappa=0.714,OR=157.50,95%CI:62.03-847.28,P=0.013).In conclusion, our newly developed assay is specific, stable, and reproducible, and may be used for rapid and accu-rate detection of toxigenic C.difficile.The assay could be used for C.difficile infection screening in outpatient and emergen-cy,community medical service center, and epidemiological settings.

Older critically ill patients with tuberculosis may have multiple organ involvement and rapid disease progression, and often have high mortality.Consequently,high-risk patients must be identified early to receive aggressive treatment. The purpose of this study was to investigate the correlation between serum inflammatory factors and prognosis in older critically ill patients with tuberculo-sis,to provide a diagnosis and treatment basis for clinicians, and to facilitate early intervention for patients with poor prognosis.SPSS software was used to analyze data for 172 patients included in the study(82 patients in the survival group and 90 patients in the death group).Univariate analysis indicated significantly higher levels of IL-6,IL-1β,TNF-α,IL-8,IL-17,IL-12P70,IFN-γ,and IL-10 in the death group than the survival group(P<0.05).Multivariate logistic regression analysis demonstrated that IL-6(OR=1.002,95%CI:1.000-1.003,P=0.021),TNF-α(OR=1.175,95%CI:1.034-1.335,P=0.013),and IFN-γ(OR=1.008,95%CI:1.001-1.015,P=0.036)were independent risk factors, and the AUC of the three combined factors in predicting critical and severe tuberculo-sis prognosis in older patients was 0.771.Serum levels of IL-6,TNF-α,and IFN-γ were closely associated with the occurrence and development of tuberculosis in critically ill older patients. The combined detection of the three factors had a high clinical value for the assessment of prognosis of critically ill older patients with tuberculosis, and might be used to predict death in this patient group.

Objective To investigate the clinical efficacy and safety of facilitated percutaneous coronary intervention(PCI)with half-dose recombinant staphylokinase(r-SAK)in patients with ST-segment elevation myocardial infarction(STEMI)who are expected to undergo PCI within 120 minutes. Methods From October 2021 to August 2022,a total of 200 STEMI patients in eight centers were included and randomly assigned in a 1﹕1 ratio to either r-SAK group or control group.Patients received loading doses of aspirin and ticagrelor and intravenous heparin and were randomized to receive an intravenous bolus of either 5 mg r-SAK or normal saline prior to PCI.The outcomes were set as ST-segment resolution(STR)at 60-90 minutes after PCI,the proportion and transition of pathological Q waves on the 5th day after PCI,and the proportion of high-sensitivity cardiac troponin T(hs-cTnT)peaking within 12 hours of onset. The safety outcome was major bleeding events defined as Bleeding Academic Research Consortium(BARC)≥type 3 bleeding during hospitalization. Results Compared with the control group, the r-SAK group had a higher proportion of STR≥70%within 60-90 minutes after PCI(58.3%vs.40.3%,P=0.009);a lower proportion of pathological Q waves(59.1%vs.74.1%,P=0.040);a lower rate of Q wave progression(14.8%vs.43.2%,P<0.001);a higher rate of Q wave disappearance(12.5%vs.3.7%,P=0.027);and a higher proportion of hs-cTnT peaking within 12 hours of symptom onset[31/40(77.5%)vs.17/33(51.5%),P=0.027].Regarding the safety outcome,no significant difference in BARC≥type 3 bleeding was found between the two groups during hospitalization(P>0.05).Conclusions For STEMI patients who were expected to undergo primary PCI within 120 minutes of symptom onset, the facilitated PCI with half-dose r-SAK significantly increased the proportion of STR≥70%at 60-90 minutes after PCI,reduced the formation of pathological Q waves, and shortened the time to peak hs-cTnT,without increasing the risk of bleeding, which should be an alternative reperfusion strategy worthy of further study.

Aim To investigate the mechanism by which sufentanil(Suf)improved hypoxia-reoxygen-ation(H/R)-induced myocardial cell injury by regula-ting hypoxia inducible factor-1α(HIF-1α)and KC-NQ1 opposite strand/antisense transcript 1(Kcnq1ot1).Methods Bioinformatics analysis was conducted to predict the interaction between HIF-1αand Kcnq1ot1.Subsequently,H9c2 cells were divided into multiple treatment groups:Ctrl group,H/R group, and Suf group.Further grouping was based on different transfection conditions, including oe-HIF-1α group,oe-HIF-1α+Suf group,sh-HIF-1α group, and sh-HIF-1α+Kcnq1ot1 group.Cell viability was detected u-sing the MTT assay, cell apoptosis was detected using the TUNEL assay, and the concentrations of CK-MB and HBDH in cell supernatants were measured using ELISA.HIF-1α protein expression in cellswas deter-mined by Western blot, and the mRNA expression level of Kcnq1ot1 was measured by reverse transcription quantitative PCR(RT-qPCR).Additionally,a rat model of myocardial is chemia reperfusion was con-structed to evaluate the therapeutic potential of Suf for myocardial ischemia reperfusion injury in vivo. Results The results of bioinformatics analysis showed a direct interaction between HIF-1α and Kcnq1ot1.Compared with the Ctrl group, the H/R group showed significantly reduced H9c2 cell viability, increased cell apoptosis, and significantly upregulated concentrations of CK-MB and HBDH,along with significantly enhanced expres-sion of HIF-1α and Kcnq1ot1(all P<0.05).When H9c2 cells were transfected with oe-HIF-1 α,cell via-bility further decreased, apoptosis was worsened, and CK-MB and HBDH concentrations further increased(all P<0.05);however,these adverse effects were significantly inhibited when combined with Suf inter-vention(all P<0.05).Additionally,compared with the H/R group, the sh-HIF-1α group showed signifi-cantly improved cell viability, reduced apoptosis and decreased CK-MB and HBDH concentrations(all P<0.05);however,these improvements were partially re-versed upon transfection with Kcnq1ot1(all P<0.05).Animal experiments confirmed that Suf could improve myocardial ischemia-reperfusion injury in myo-cardial ischemia-reperfusion injury rats. Conclusions Suf improves myocardial H/R injury by inhibiting the HIF-1α-Kcnq1ot1.

In this study,an ON/OFF type micro-valve with a sandwich(glass-silicon-glass)structure was designed and fabricated based on the micro-electro-mechanical system(MEMS)technique. The deformable membrane of this micro-valve was prepared on the silicon on insulator(SOI)substrate and sealed using Si-Si bonding and anodic bonding methods. The micro-valve had high-temperature stability and was suitable for integration with other gas chromatography components. The deformable membrane with a thickness of 10 μm was processed on the top silicon of the SOI substrate. The flow control of the micro-valve could be achieved by changing the driving pressure applied to the deformable membrane to deform it.Compared with polymer membranes, the deformable membrane prepared on the top layer silicon of SOI had better temperature stability and could be released using the deep reactive ion etching technique after silicon-silicon bonding, avoiding deformation during the preparation process. In addition, due to the small gap between the membrane and the inlet/outlet holes, the dead volume of the microvalve was very small. The test results indicated that the micro-valve achieved flow control and ON/OFF functions with good repeatability.

In this work, with tris(4-aminophenyl)amine(TAPA)and 1,3,5-tris(4-formylphenyl)benzene(TFPB)as monomers,an imine-type porous organic polymer,TAPA-TFPB,was synthesized using a simple method under the catalysis of acetic acid. The material TAPA-TFPB was used as solid-phase extraction adsorbent and combined with ultra-performance liquid chromatography/quadrupole time-of-flight-tandem mass spectrometry(UHPLC-QTOF-MS)to establish a detection method for four kinds of nitrofuran metabolites(NFMs)residues in meat samples. The parameters of the adsorbent dosage, the pH value and volume of sample, and the type and volume of washing and eluent solvents were optimized, respectively.Under the optimal extraction conditions, low detection limits(0.11-1.60 μg/kg)were achieved for four kinds of NFMs.At three different spiked levels, the intra-day and inter-day precisions(Relative standard deviations)were 2.8% -10.9% and 4.3% -16.2%,respectively,and the spiked recoveries were 72.0% -107.2%.The results showed that the method chould be used for efficient extraction and analysis of trace NFMs residues in meat samples, indicating that TAPA-TFPB was a kind of promising SPE adsorbent.

A monolithic integrated gas chromatography chip, consisting of a micro gas chromatography column(μGCC)and a micro helium discharge ionization detector(μHDID)was proposed. The chip was fabricated using micro electromechanical system(MEMS)technique, and its sensitivity was improved from two aspects. On one hand, open tubular column was selected as the separation device, and the auxiliary helium channel width of μHDID was modulated based on the microchannel width of the μGCC to match the flow rates of μHDID and μGCC.On the other hand, the electrode structure inside the μHDID collection zone was optimized,a bias electrode group around the collection electrode was constructed, and the ion collection efficiency was improved. After coating HKUST-1 as the stationary phase, the monolithic integrated gas chromatography chip could achieve baseline separation and detection of light hydrocarbon gas mixture(methane,ethane,propane,andn-butane),with a detection limit for propane as low as 25 pg.The chip could carried out test under temperature-programmed conditions, with a resolution of 9.24 for ethane and propane.

A micro gas chromatographic column with mesoporous surface micro column array prepared by anodization method was proposed.A porous support layer with a characteristic pore size of about 30 nm inside the chromatographic column was prepared in situ using anodization method, and a uniform alumina stationary phase was deposited on the mesoporous support layer using atom layer deposition(ALD)technique. The existence of a mesoporous support layer increased surface area of the chromatographic column, thereby increasing the total amount of stationary phase loading and enhancing column capacity, which facilitated chromatographic separation. The test results showed that the porous support layer significantly reduced the longitudinal molecular diffusion and mass transfer resistance of the micro gas chromatographic column, and significantly increased the number of theoretical plates(n-nonane increased by 290.2%).Furthermore,column efficiency of the chromatographic column was less affected by flow rate, which was conducive to rapid separation of heavy hydrocarbon mixtures.
