1.Efficacy and safety of a facilitated percutaneous coronary intervention with half-dose recombinant staphylokinase in ST-segment elevation myocardial infarction

Objective To investigate the clinical efficacy and safety of facilitated percutaneous coronary intervention(PCI)with half-dose recombinant staphylokinase(r-SAK)in patients with ST-segment elevation myocardial infarction(STEMI)who are expected to undergo PCI within 120 minutes.Methods From October 2021 to August 2022,a total of 200 STEMI patients in eight centers were included and randomly assigned in a 1﹕1 ratio to either r-SAK group or control group.Patients received loading doses of aspirin and ticagrelor and intravenous heparin and were randomized to receive an intravenous bolus of either 5 mg r-SAK or normal saline prior to PCI.The outcomes were set as ST-segment resolution(STR)at 60-90 minutes after PCI,the proportion and transition of pathological Q waves on the 5th day after PCI,and the proportion of high-sensitivity cardiac troponin T(hs-cTnT)peaking within 12 hours of onset.The safety outcome was major bleeding events defined as Bleeding Academic Research Consortium(BARC)≥type 3 bleeding during hospitalization.Results Compared with the control group,the r-SAK group had a higher proportion of STR≥70%within 60-90 minutes after PCI(58.3%vs.40.3%,P=0.009);a lower proportion of pathological Q waves(59.1%vs.74.1%,P=0.040);a lower rate of Q wave progression(14.8%vs.43.2%,P<0.001);a higher rate of Q wave disappearance(12.5%vs.3.7%,P=0.027);and a higher proportion of hs-cTnT peaking within 12 hours of symptom onset[31/40(77.5%)vs.17/33(51.5%),P=0.027].Regarding the safety outcome,no significant difference in BARC≥type 3 bleeding was found between the two groups during hospitalization(P>0.05).Conclusions For STEMI patients who were expected to undergo primary PCI within 120 minutes of symptom onset,the facilitated PCI with half-dose r-SAK significantly increased the proportion of STR≥70%at 60-90 minutes after PCI,reduced the formation of pathological Q waves,and shortened the time to peak hs-cTnT,without increasing the risk of bleeding,which should be an alternative reperfusion strategy worthy of further study.
2.Efficacy of phacoemulsification with intraocular lens implantation combined with limbal relaxing incision in treatment of cataract patients with corneal astigmatism

Objective To investigate the corrective effect of phacoemulsification with intraocular lens implantation combined with limbal relaxing incision(LRI)on cataract patients with corneal astigmatism.Methods A total of 39 patients(41 eyes)with cataract complicated by corneal astigmatism(the astigmatism was-2.0 to-0.5 D)who visited Department of Ophthalmology of The First Affiliated Hospital of Naval Medical University from Sep.2023 to May 2024 were enrolled.During phacoemulsification with intraocular lens implantation,LRI was performed to correct corneal astigmatism.Preoperative corneal astigmatism was measured using corneal topography and IOLMaster.Corneal astigmatism was re-examined 1 and 3 months after surgery,and Alpins method was used to analyze the changes of corneal astigmation before surgery and 1 month after surgery.The primary vector parameters,such as target induced astigmatism vector(TIA),surgically induced astigmatism vector(SIA),difference vector(DV),correction index and flattening index(FI),were observed.Results The preoperative corneal astigmatism was(-1.07+0.27)D,which was decreased to(-0.53+0.23)D 1 month postoperatively and(-0.55+0.24)D 3 months postoperatively(both P<0.05).Vector analysis of corneal astigmatism showed that the TIA was(1.07+0.27)D,SIA was(0.99+0.42)D,DV was(0.53+0.24)D;correction index was 0.93+0.36(the ideal value is 1),suggesting slight undercorrection;the median of the angle of error had an absolute value of 18.15°,and the interquartile range was 15.19°;and the flattening effect was(0.70+0.45)D,the FI was 0.65+0.42,and the success rate was(50.42+19.29)%.Conclusion The combination of LRI with cataract surgery for correcting corneal astigmatism has demonstrated favorable corrective outcomes,especially for the correction of low to moderate corneal astigmatism.
3.Automated fiber quantification for analyzing relationships of abnormal white matter microstructures and executive dysfunction in children with attention deficit hyperactivity disorder

Objective To observe the value of automated fiber quantification(AFQ)for analyzing relationships of abnormal white matter microstructures and executive dysfunction in children with attention deficit hyperactivity disorder(ADHD).Methods Brain diffusion tensor imaging(DTI)and white matter structure AFQ analysis were prospectively performed on 83 children with ADHD(ADHD group)and 52 healthy children(healthy control[HC]group).The axial diffusivity(AD),radial diffusivity(RD),mean diffusivity(MD),and fractional anisotropy(FA)of complete fiber bundles and respective segments were obtained and compared between groups.Partial correlation analysis was conducted of fiber bundle diffusion parameters and the scores of Swanson Nolan and Pelham-Ⅳ(SNAP-Ⅳ)and behavior rating inventory of executive function(BRIEF)in ADHD group.Receiver operating characteristic curves were applied to assess the efficacy of each diffusion parameter for evaluating ADHD.Results AD and MD of right anterior thalamic radiation(ATR),and AD of right cingulum bundle(CB)in ADHD group were all higher than those in HC group(FWE corrected,all P<0.000 7).Significant differences of AD of the 54-78 segments of right ATR,MD of the 13-24 and 53-77 segments of right ATR,AD of the 24-26,53-56 and 93-97 segments of right CB,as well as MD of the 72-85 segments of left inferior longitudinal fasciculus(ILF)were found between groups(FWE corrected,all P<0.001).In ADHD group,AD of 54-78 segments of right ATR were negatively correlated with the attention deficit scores,while of 24-26 segments of right CB were negatively correlated with the emotional control scores.MD of right ATR and some segments,AD of right CB and some segments in ADHD group were positively correlated with the scores of task initiation,planning,organization,monitoring and metacognitive index(all P<0.05).The area under the curve(AUC)of segmental fiber bundle diffusion parameter and intact fiber bundle diffusion parameter for evaluating ADHD was 0.881 and 0.808,respectively,the former was higher than the latter(Z=2.435,P<0.05).Conclusion AFQ could be used to analyze relationships of abnormal white matter microstructures and executive dysfunction in ADHD,which could be effectively evaluated based on specific fiber bundle segments.

Ultrasonography(US)is the first-line imaging modality recommended by domestic and international guidelines for liver tumor screening,owing to its non-invasive nature,real-time dynamic imaging capability,cost-effectiveness,and operational convenience. In recent years,the integration of artificial intelligence(AI)and medical imaging has emerged as a major research focus. By leveraging large-scale data training,AI models can automatically recognize and analyze input imaging data and generate predictive outcomes. Notably,AI-based ultrasound imaging technology has achieved breakthrough advancements in the diagnosis and treatment of liver tumors. These innovations significantly improve diagnostic accuracy,optimize treatment strategies,predict disease progression and prognosis,and monitor therapeutic response. This article provides a comprehensive review of the latest applications and research progress of AI in ultrasound-based diagnosis and treatment of liver tumors.

8.Mechanism of traditional Chinese medicine Bai Zhi in preventing microgravity induced bone loss based on network pharmacology and transcriptomics

Objective In this study,we investigated the active ingredients,targets and molecular mechanisms of Bai zhi based on network pharmacology and mRNA transcriptome sequencing technology for the treatment of microgravity induced bone loss,with a view to providing new therapeutic strategies for microgravity induced bone loss diseases.Methods Investigating the antagonistic effect of the traditional Chinese medicine Bai zhi on microgravity induced bone loss by constructing a hindlimb unloading mice model.18 healthy male mice were randomly divided into Control,HLU and HLU+BZ groups,6 mice in each group,and the effects of Bai zhi on the bone mineral density of the model mice were evaluated after 28 d of continuous gavage.Screening the active ingredients and targets of Bai zhi through TCMSP,Genecards,OMIM,TTD,DisGeNET and other network pharmacology databases.A hindlimb tail suspension unloading animal(HLU)model was established,and the differentially expressed genes(DEGs)responding to mechanical unloading were analyzed using transcriptome sequencing methods,and the targets of Bai zhi active ingredients intersected with the targets of the differentially expressed genes responding to mechanical unloading,so that the core gene targets of Bai zhi for intervening in weightlessness bone loss could be obtained;Construction of Bai zhi component-target protein interaction network(PPI)using String database and Cytoscape software,and enrichment and analysis of key signaling pathways of Bai zhi for treating microgravity induced bone loss using R Studio software.Results Bai zhi effectively restored bone loss in hindlimb tail suspension mice.By quantitative analysis of bone mineral density scanning of hindlimb tail suspension mice,the results showed that Bai zhi significantly restored the loss of bone mineral density in the model mice(P?0.001),Bai zhi has obvious improvement effect on mirogravity induced mice bone loss.10 kinds of Bai zhi effective active ingredients were obtained through the network pharmacology database screening,306 components of the action target,1751 osteoporosis-related disease targets,and 33 disease drug common targets were obtained by drawing the intersection of Wayne's diagram calculation.Through target intersection with transcriptome sequencing differentially expressed genes under simulated microgravity conditions,32 core genes of Bai zhi for intervening in microgravity induced bone loss were obtained,including nuclear transcription factor(JUN),Toll-like receptor 4(TLR4),capsaicin receptor(TRPV1),and so on;Enrichment analysis of the 32 core genes was performed,and the results of GO functional enrichment analysis showed that a total of 194 GO terms were obtained,including 123 biological process(BP)terms,which mainly included insulin-like growth factor receptor,glucose homeostasis in vivo,and negative regulation of gene expression;28 Molecular Function(MF)terms,including RNA polymerase II transcription factor activity,ligand-activated sequence-specific DNA binding,and transcription factor binding;there were 43 cellular component(CC)terms,including cytoplasmic membrane,receptor complex,euchromatin,etc.The results of KEGG enrichment analysis indicated that Bai zhi may be involved in the regulation of insulin-like factor,HIF-1 hypoxic stress,AMPK and other signaling pathways to play a therapeutic function for microgravity induced bone loss.Conclusion under simulated microgravity conditions,the effective active ingredients of Bai zhi may inhibit the accumulation of glycosylation end products(AGEs),oxidative stress damage,and the production of inflammatory factors induced by microgravity stress by regulating the expression of differential genes in response to mechanical unloading,and then participate in osteoblast differentiation in the bone microenvironment to alleviate the occurrence of microgravity induced bone loss diseases.
9.Characterization and Comparison Analysis of WHO Monographs on Selected Medicinal Plants and ISO Standards of Chinese Materia Medica.

Medicinal plants or Chinese materia medica (CMM) are now attracting worldwide attention as they have increasingly prominent advantages over chemical drugs in disease treatment and healthcare. Since the 1990s, World Health Organization (WHO) and International Organization for Standardization established the Technical Committee of Traditional Chinese Medicine (ISO/TC 249) have carried out the development of quality standards on medicinal plants or CMMs respectively, and a considerable number of monographs and international standards have been published. Since the two international organizations adhere to different principles, the standards they develop naturally have different emphasis. Driven by market demand and international trade, ISO mainly takes quality, efficacy and safety into consideration when developing standards, while WHO pays more attention to clinical practice, quality control and medication guidance. Up to now, there is a lack of comparative analysis on the records, background, principles, basic content, and main requirements of quality standards on medicinal plants or CMMs respectively published by WHO and ISO. Therefore, based on international standards of CMM developed by ISO/TC 249 platform and WHO Monographs on Selected Medicinal Plants, this paper systematically compares the purposes, selected principles, standard-developing process, basic content, and main quality requirements to summarize their similarities and differences, and find their merits, aiming to serve as a reference to the development of international standards for CMMs that helps them go global.

10.Catheter ablation versus medical therapy for atrial fibrillation with prior stroke history: a prospective propensity score-matched cohort study.

BACKGROUND:
Patients with atrial fibrillation (AF) and prior stroke history have a high risk of cardiovascular events despite anticoagulation therapy. It is unclear whether catheter ablation (CA) has further benefits in these patients.
METHODS:
AF patients with a previous history of stroke or systemic embolism (SE) from the prospective Chinese Atrial Fibrillation Registry study between August 2011 and December 2020 were included in the analysis. Patients were matched in a 1:1 ratio to CA or medical treatment (MT) based on propensity score. The primary outcome was a composite of all-cause death or ischemic stroke (IS)/SE.
RESULTS:
During a total of 4.1 ± 2.3 years of follow-up, the primary outcome occurred in 111 patients in the CA group (3.3 per 100 person-years) and in 229 patients in the MT group (5.7 per 100 person-years). The CA group had a lower risk of the primary outcome compared to the MT group [hazard ratio (HR) = 0.59, 95% CI: 0.47-0.74, P < 0.001]. There was a significant decreasing risk of all-cause mortality (HR = 0.43, 95% CI: 0.31-0.61, P < 0.001), IS/SE (HR = 0.73, 95% CI: 0.54-0.97, P = 0.033), cardiovascular mortality (HR = 0.32, 95% CI: 0.19-0.54, P < 0.001) and AF recurrence (HR = 0.33, 95% CI: 0.30-0.37, P < 0.001) in the CA group compared to that in the MT group. Sensitivity analysis generated consistent results when adjusting for time-dependent usage of anticoagulants.
CONCLUSIONS
In AF patients with a prior stroke history, CA was associated with a lower combined risk of all-cause death or IS/SE. Further clinical trials are warranted to confirm the benefits of CA in these patients.
