1.Mechanism of Intervening with Diarrhea-predominant Irritable Bowel Syndrome in Rats with Spleen Deficiency by Xingpi Capsules Through Regulating 5-HT-RhoA/ROCK2 Pathway

ObjectiveTo investigate the efficacy of Xingpi capsules (XPC) in treating diarrhea-predominant irritable bowel syndrome (IBS-D) with spleen deficiency and elucidate its potential molecular mechanisms. MethodsA rat model of IBS-D with spleen deficiency was established by administering senna leaf in combination with restrained stress and swimming fatigue for 14 d. Ten specific pathogen free (SPF)-grade healthy rats were used as the normal control group. After successful modeling, SPF-grade rats were randomly divided into a model group, a pinaverium bromide group (1.5 mg·kg-1), and low- and high-dose XPC groups (0.135 and 0.54 g·kg-1), with 10 rats in each group. Rats in the normal control group and the model group were given distilled water by gavage, while the remaining groups were administered corresponding drug solutions by gavage once a day for 14 consecutive days. The rat body weights and fecal condition were observed every day, and the Bristol score was recorded. Enzyme-linked immunosorbent assay (ELISA) was used to determine the levels of 5-hydroxytryptamine (5-HT) in serum and colon tissue. Transmission electron microscopy was used to observe the microvilli and tight junctions in the colon. The integrity of the colonic barrier, intestinal motility, and expression of related pathway proteins were evaluated by hematoxylin-eosin (HE) staining, immunohistochemistry, and Western blot. ResultsCompared with those in the normal control group, rats in the model group showed a significantly decreased body weight and increased diarrhea rate, diarrhea grade, and Bristol score (P<0.01). HE staining revealed incomplete colonic mucosa in the model group, with evident congestion and edema observed. Electron microscopy results indicated decreased density and integrity of the colonic barrier, shedding and disappearance of microvilli, and significant widening of tight junctions. The expression levels of colonic tight junction proteins Occludin and Claudin-5 were downregulated (P<0.01), and the levels of 5-HT in serum and colon tissue were elevated (P<0.01). The small intestine propulsion rate significantly increased (P<0.01), and the expression of contractile proteins Ras homolog family member A (RhoA) and Rho-associated coiled-coil containing protein kinase 2 (ROCK2) in colon and phosphorylation of myosin light chain (MLC20) were upregulated (P<0.01). Compared with the model group, the treatment groups showed alleviated diarrhea, diarrhea-associated symptoms, and pathological manifestations of colon tissue to varying degrees. Specifically, high-dose XPC exhibited effectively relieved diarrhea, promoted recovery of colonic mucosal structure, significantly reduced congestion and edema, upregulated expression of Occludin and Claudin-5 (P<0.01), decreased levels of 5-HT in serum and colon tissue (P<0.05,P<0.01), significantly slowed small intestine propulsion rate (P<0.01), and significantly downregulated expression of contractile proteins RhoA and ROCK2 in colon and phosphorylation of MLC20 (P<0.05,P<0.01). ConclusionXPC effectively alleviates symptoms of spleen deficiency and diarrhea and regulates the secretion of brain-gut peptide. The characteristics of XPC are mainly manifested in alleviating IBS-D with spleen deficiency from the aspects of protecting intestinal mucosa and inhibiting smooth muscle contraction, and the mechanism is closely related to the regulation of the 5-HT-RhoA/ROCK2 pathway expression.
2.Phenotypic screening uncovered anti-myocardial fibrosis candidates using a novel 3D myocardial tissue under hypoxia.

Myocardial fibrosis (MF) is a common pathological hallmark of cardiovascular diseases, reflecting shared mechanisms in their progression. However, the lack of reliable MF models that accurately mimic its pathogenesis has hindered drug discovery, highlighting the urgent need for more effective therapeutic agents. Herein, a novel contractile three-dimensional (3D) myocardial tissue model integrating cardiomyocytes, cardiac-fibroblasts, and bone marrow-derived macrophages in collagen hydrogel was developed to simulate the fibrotic changes of cardiovascular disease, and facilitate the screening of anti-MF compounds. The 3D myocardial tissue model exhibited precise, visualizable, and quantifiable contractile characteristics under hypoxia and drug interventions. 76 compounds extracted from the resins of Toxicodendron vernicifluum, a traditional Chinese medicine with clear clinical benefits for fibrotic diseases, were screened for anti-fibrotic activity. Using an in vitro 3D oxygen-glucose deprivation (OGD)-treated myocardial tissue model instead of a two-dimensional transforming growth factor-β treated cardiac-fibroblasts model, two candidates including LQ-40 and SQ-3 exert impressive anti-MF activity, which was further validated in left anterior descending coronary artery ligation-induced MF mouse model. The current results demonstrate the feasibility and advantage of the novel contractile 3D tissue model with multi-cell types in discovering candidates for MF, further stressing the great potential of regulating macrophages in the treatment of MF.
3.Influence of hyperuricemia on deep vein thrombosis of lower extremities after artificial joint replacement

Objective To investigate the impact of hyperuricemia on risk of deep vein thrombo-sis after joint replacement surgery.Methods A total of 953 patients undergoing joint replacement surgery were enrolled in this study.Among them,662 patients did not develop thrombosis postopera-tively(postoperative non-thrombosis group),while 291 patients developed thrombosis(postoperative thrombosis group).Data including gender,age,surgical procedure,body mass index,hypertension,coronary heart disease,diabetes,preoperative D-dimer positivity(≥0.5 mg/L),prothrombin time,prothrombin activity,international normalized ratio,activated partial thromboplastin time,fibrinogen,thrombin time,white blood cell count,red blood cell count,hemoglobin,hematocrit and platelet count were analyzed.Logistic regression analysis was used to identify independent risk factors for postoperative deep vein thrombosis.Results Univariate analysis showed that age,gender,surgical procedure(total knee arthroplasty),hypertension,diabetes,hyperuricemia,preoperative red blood cell count,preoperative hematocrit and preoperative D-dimer positivity were influencing factors for deep vein thrombosis after joint replacement surgery(P<0.05).Logistic regression analysis re-vealedthat age(>60 years),surgical procedure(total knee arthroplasty)and hyperuricemia were independent risk factors for postoperative deep vein thrombosis(P<0.05).Conclusion Hyperuri-cemia is an independent risk factor for deep vein thrombosis after joint replacement surgery.
4.Stress effect of femoral prosthesis misalignment on structure of lateral compartment during medial unicompartmental knee arthroplasty in patients with osteoporosis

BACKGROUND:Prosthesis misalignment and patient bone condition are important factors affecting the prognosis of unicompartmental knee arthroplasty,and postoperative osteoarthritic progression of the lateral compartment is a major complication leading to its high revision rate.Therefore,this will become a hot research topic in the future.OBJECTIVE:To analyze the effect of different femoral prosthesis tilt angles on the stress on the structure of the lateral compartment after unicompartmental knee arthroplasty in patients with normal bone and osteoporosis,and to investigate the correlation between osteoporosis and osteoarthritis of the lateral compartment in the postoperative period.METHODS:Using a validated finite element model of the knee,normal bone(M1)and osteoporotic(M2)unicompartmental knee arthroplasty were modeled.The femoral prosthesis tilt models(normal bone group:varus angles of 3°,6°,9°,12°,and 0°,valgus angles of 3°,6°,9°,12°;osteoporosis group:varus angles of 3°,6°,9°,12°,0°,valgus angles of 3°,6°,9°,12°)were established respectively,totaling 18 working conditions.The effects of different femoral prosthesis tilt angles on stress magnitude and distribution in the meniscus,tibial cartilage,and femoral cartilage of interstitial compartment were evaluated.RESULTS AND CONCLUSION:(1)The high stress values on the meniscus surface and tibial cartilage surface of the healthy lateral compartment of the two models increased with the increase of the prosthesis varus angle,and decreased with the increase of the prosthesis valgus angle.Under the same working conditions,the peak stress on the meniscus surface of the osteoporotic group was greater than that of the normal bone group,but that of the tibial cartilage surface of the osteoporotic group was less than that of the normal bone group.(2)The high stress values on the femoral cartilage of the lateral compartment of the two models increased with the increase of the prosthesis varus and valgus angles.Under the same working conditions,the peak stress on the femoral cartilage of the osteoporotic group was greater than that of the normal bone group.(3)It is indicated that when the fixed-bearing femoral prosthesis is varus,the stresses on the lateral compartment structures after medial unicompartmental knee arthroplasty in normal bone and osteoporotic knees will increase,and the increase in the stresses on the lateral intercondylar structures in osteoporotic knees will be more pronounced.Therefore,special attention should be paid to the placement of the prosthesis during unicompartmental knee arthroplasty to avoid varus and valgus of the prosthesis as much as possible.At the same time,the findings confirm that osteoporosis may exacerbate the progression of lateral compartment osteoarthritis after medial unicompartmental knee arthroplasty.

6.Analysis of global research trends and hotspots on the correlation between hyperuricemia and cardiovascular disease

Objective To analyze the latest research hotspots and development trends of hyperuricemia and cardiovascular diseases in the past 20 years by using CiteSpace software and VOSviewer software.Methods The Web of Science database was searched from January 2005 to July 2024 for the literature on hyperuricemia and cardiovascular diseases,and the keywords,authors,institutions,and countries were analyzed by CiteSpace software and VOSviewer software,and the network cooperation map was constructed.Results A total of 2344 articles written by 5844 authors from 75 countries and 1713 institutions were analyzed.The journal Nutrition Metabolism and Cardiovascular Diseases had published the most papers in this field,with a total of 36 articles.Scholar Johnson has published 19 papers in this field,ranking first.A total of 2678 keywords were included,and the keywords that identified the research frontier and continued through 2024 were"asymptomatic hyperuricemia""xanthine oxidoreductase""adults".Conclusion The literature published in the field of hyperuricemia and cardiovascular disease correlation from 2005 to 2024,are comprehensively summarize,and sort out the development trends and research hotspots in this field in the past 20 years,through bibliometrics and visual analysis,which is expected to provide a reference direction for researchers in this field for subsequent research.
7.Impact of prosthesis malposition on internal structural strain in unicompartmental knee arthroplasty for osteoporotic patients

Objective To investigate the impact of femoral prosthesis malposition on internal structural strain in unicompartmental knee arthroplasty for osteoporotic knees. Methods A normal bone knee joint model was constructed based on imaging studies (CT, magnetic resonance imaging) of healthy volunteers, and an osteoporotic knee joint model was constructed by altering the elastic modulus. Finite element models of femoral prosthesis malposition in unicompartmental knee arthroplasty were then established for both normal bone and osteoporotic groups. A vertical static load of 1, 000 N was applied to both models, and the peak strain values and distributions were observed in the medial compartment structures (polyethylene gasket, cancellous bone under tibial prosthesis) and lateral compartment structures (meniscus, tibial cartilage) at various malposition angles of the femoral prosthesis. Results The peak strain values of the polyethylene gasket, cancellous bone under tibial prosthesis, meniscus, and tibial cartilage in the osteoporotic group were higher than those in the normal bone group at different varus and valgus angles. In the osteoporotic group, the peak strain values of the polyethylene gasket and cancellous bone under tibial prosthesis increased with the increase in femoral prosthesis malposition angle, with a greater increase at valgus angles than at varus angles. Similarly, the peak strain values of the meniscus and tibial cartilage increased with the increase in femoral prosthesis malposition angle, but the increase was greater at varus angles than at valgus angles. The strain concentration area of the polyethylene gasket was located in a circular-like area where the femoral prosthesis contacted the gasket, the strain concentration area of the cancellous bone was mainly located in the posterolateral region, the strain concentration area of the meniscus was located near the anterior horn, and the strain concentration area of the tibial cartilage was mainly located in the middle near the intercondylar eminence. Conclusion Osteoporosis may adversely affect the strain on internal structures of the knee joint after unicompartmental knee arthroplasty. Varus malposition of the femoral prosthesis may exacerbate the progression of osteoarthritis in the contralateral compartment (lateral compartment), while valgus malposition may increase the risk of prosthesis loosening and revision surgery.
8.Mechanism of turnover intention of junior nurses as second victims after experiencing adverse events

Objective:To explore the mechanism of turnover intention of junior nurses after adverse events from three aspects: work pressure, leadership style and personal characteristics.Methods:This study was a cross-sectional survey, using the convenience sampling method, 289 nurses from Shanxi Bethune Hospital from December 2020 to March 2022 were selected as the research subjects, and the general data questionnaire, the Separation Tendency Scale, the Nurses′ Work Stressor Scale, the Service-oriented Leadership Style Scale, and the Prospective Personality Scale were used to survey them.Results:Structural equation model showed that nurses′ job stressor level positively predicted turnover intention, with a path coefficient of 0.74, P<0.01, while service-oriented leadership style and forward-looking personality negatively predicted turnover intention, with path coefficients of -0.60 and -0.37, respectively, both P<0.01. Hierarchical regression analysis showed that both service-oriented leadership style and forward-looking personality negatively moderated the promotion of job stressors on turnover intention, and the moderating effect regression coefficients were -0.146, -0.215, both P<0.01. Conclusions:Hospitals should improve the prospective personality traits of junior nurses and cultivate the service-oriented leadership style of nursing managers after adverse events, so as to reduce the turnover tendency of junior nurses as the second victim, promote the improvement of nursing quality, and stabilize nursing team development.
9.Exploring cognitive trajectories and their association with physical performance: evidence from the China Health and Retirement Longitudinal Study

OBJECTIVES:
The long-term trends of cognitive function and its associations with physical performance remain unclear, particularly in Asian populations. The study objectives were to determine cognitive trajectories in middle-aged and elderly Chinese individuals, as well as to examine differences in physical performance across cognitive trajectory groups.
METHODS:
Data were extracted from the China Health and Retirement Longitudinal Study. A total of 5,701 participants (47.7% male) with a mean age of 57.8 (standard deviation, 8.4) years at enrollment were included. A group-based trajectory model was used to identify cognitive trajectory groups for each sex. Grip strength, repeated chair stand, and standing balance tests were used to evaluate physical performance. An ordered logistic regression model was employed to analyze differences in physical performance across cognitive trajectory groups.
RESULTS:
Three cognitive trajectory groups were identified for each sex: low, middle, and high. For both sexes, higher cognitive trajectory groups exhibited smaller declines with age. In the fully adjusted model, relative to the low trajectory group, the odds ratios (ORs) of better physical performance in the middle cognitive group were 1.37 (95% confidence interval [CI], 1.17 to 1.59; p<0.001) during follow-up and 1.40 (95% CI, 1.20 to 1.64; p<0.001) at the endpoint. The ORs in the high trajectory group were 1.94 (95% CI, 1.61 to 2.32; p<0.001) during follow-up and 2.04 (95% CI, 1.69 to 2.45; p<0.001) at the endpoint.
CONCLUSIONS
Cognitive function was better preserved in male participants and individuals with higher baseline cognitive function. A higher cognitive trajectory was associated with better physical performance over time.
10.Cosmetic adverse reaction: a clinical analysis of 820 cases

Objective:To provide epidemiological data and clinical evidence for cosmetic adverse reactions.Methods:A retrospective clinical analysis was carried out on a total 820 outpatients (23 males and 797 females) suspected to be with cosmetic adverse reactions from January 2014 - October 2017, and average age of these patients was 7~75 (32.66±8.09) years. Suspicious cosmetics patch tests were performed in some patients. Suspicious cosmetics patch tests were performed in 687 patients.Results:Among 820 patients with cosmetic adverse reactions, women accounted for 97.20% and men accounted for 2.80%. Age distribution was most common among young people aged 21-40 years, accounting for 71.34%. The highest level of education was higher education, accounting for 59.69%. Occupational distribution was most commonly concentrated in employees and unemployed persons, accounting for 28.54% and 18.66%, respectively. A history of cosmetics allergies accounted for 17.28%. Cosmetic contact dermatitis was the most common clinical type of cosmetic adverse reactions, accounting for 92.70%. A total of 1682 suspected pathogenic cosmetics were involved. The positive rate of the cosmetic original patch test was 42.39%. Among the cosmetics with a positive patch test, moisturizing, anti-wrinkle and whitening freckle cosmetics accounted for the highest proportion, 31.59%, 15.09%, and 12.68%, respectively.Conclusions:Cosmetic contact dermatitis is the most common type of cosmetic adverse reaction. Patch testing is helpful in identifying the contact allergens in cosmetic adverse reaction.
