1.Observation on the clinical efficacy of electronic bronchoscope in difficult cases of gastric intubation

Objective To investigate the clinical efficacy of electronic bronchoscope in difficult cases of gastric intubation.Methods One hundred and sixty patients who were hospitalized from March,2016 to March,2017 due to difficulty in routine nasogastric intubation were selected as research subjects.During treatment,all the patients received gastric intubation by electronic bronchoscope.Blood pressure,heart rate,respiration rate and blood oxygen saturation were closely observed before,during and after intubation and the obtained data were compared between them.Results Of the 160 patients with gastric intubation under electronic bronehoseope,134 patients had first-try success,and 26 patients failed in first-try intubation and resorted to electronic bronchoscope-guided gastric intubation.Obvious changes in blood pressure,heart rate,respiratory rate,blood oxygen saturation could be observed during intubation,as compared with those before intubation,and statistical significance could be noted,when comparisons were made between them(P <0.05).Various data after intubation were all decreased,as compared with those before intubation,and there was no statistical significance,as compared with those before intubation (P > 0.05).Conclusion For those patients with refractory gastric intubation,gastric intubation under electronic bronchoscope could achieve high first-try success rate,with good safety and no obvious complications,which was worth further clinical extension.
2.Discussion on the standardized construction and management of medium and small contingency medical teams in military hospitals

The paper analyzed the present status and the problems existed in medium and small contingency medical teams in military hospitals, made recommendations that standardized construction of medium and small contingency medical teams could be im -plemented through the approaches of consolidated leadership , proper support and performance evaluation .The paper also explored in detail the ways to realize the standardized management of contingency medical teams , so that the target of overall planning and manage-ment could be reached and the management level of contingency medical teams in military hospitals be increased .
3.Research on the differences in clinical diagnosis of coronary heart disease by using coronary angiography gold standards and conventional diagnosis techniques

Objective To evaluate the reliability of conventional clinical diagnosis of coronary heart disease (CHD),as com-pared with the coronary angiography gold standards,and also to analyze reasons for CHD misdiagnosis.Methods A total of 3355 pa-tients clinically confirmed or suspected of CHD underwent coronary angiography,and the results obtained were compared with the clini-cal data.Results Generally,the positive rate of coronary angiography was 75.26%(2525 /3355).The clinically diagnostic accuracies in patients with myocardial infarction,unstable angina pectoris,stable angina pectoris and suspected CHD were 93.85%(397 /423), 79.88%(536 /671),74.89%(1366 /1824)and 51.72%(226 /437)respectively.With regard to the rate of diagnostic conformity,the patients with myocardial infarction ranked the first in diagnostic rate,and statistical significance could be seen,when comparisons were made with the other 3 groups(P <0.05 or P <0.01).Then came the patients with unstable angina pectoris and stable angina pectoris, and statistical significance could also be found,when compared with the patients with suspected CHD(P <0.05).Conclusion Coro-nary heart disease (CHD),especially suspected CHD,had relatively high rate of misdiagnosis clinically.For this reason,it was advis-able to perform coronary angiography and the said disease could be identified in time.
