1.Relationship between serum a proliferation-inducing ligand and C-X-C motif-chemokine ligand 14 expression and disease outcome in children with severe mycoplasma pneumoniae pneumonia

Objective:To investigate the relationship between the expression of serum a proliferation inducing ligand (APRIL), C-X-C motif-chemokine ligand 14(CXCL14) and disease regression in children with severe Mycoplasma pneumoniae pneumonia (SMPP).Methods:A total of 112 children with SMPP admitted to Changzhou Maternal and Child Health Hospital from January 2021 to December 2022 were selected as study subjects. After 28 d of treatment, the children were divided into the good prognosis group ( n=87) and the poor prognosis group ( n=25) according to their disease regression, and 100 children with Mycoplasma pneumoniae pneumoniae (MPP) admitted to our hospital during the same period were selected as the control group. Serum APRIL and CXCL14 expression were measured by enzyme-linked immunosorbent assay in all children. The predictive value of serum APRIL and CXCL14 for disease regression in children with SMPP was assessed by receiver operating characteristic (ROC) curve, and the factors influencing disease regression in children with SMPP were analyzed by multifactorial Logistic stepwise regression. Results:Serum APRIL and CXCL14 levels in the study group were higher than those in the control group(all P<0.05). Serum APRIL and CXCL14 levels of children with SMPP in the poor prognosis were higher than those in the good prognosis group(all P<0.05). The area under the curve (AUC)(95% CI)of serum APRIL and CXCL14 for predicting the prognosis of SMPP children were 0.783(0.721-0.835)and 0.835(0.789-0.882),respectively,and the cut-off values were 34.47 ng/mL and 289.32 pg/mL,respectively. The specificity was 53.61%,65.43%,and the sensitivity was 93.20%,93.20%,respectively. The AUC(95% CI)of the combined detection of both was 0.913(0.872-0.954),with a specificity of 85.59% and a sensitivity of 86.35%. The proportions of children in the poor prognosis group with fever days> 7 d and admission National Institute of Health Stroke Scale(NIHSS) score> 30 points were higher than those in the good prognosis group( P<0.05). Multifactorial Logistic stepwise regression analysis showed that the fever days> 7 d( OR=2.273,95% CI 1.403-3.681),admission NIHSS score> 30 points( OR=2.088,95% CI 1.327-3.283),APRIL≥34.47 ng/mL( OR=3.093,95% CI 1.711-5.590),and CXCL14≥289.32 pg/mL( OR=5.013,95% CI 2.079-12.086)were risk factors for death in children with SMPP( P<0.05). Conclusion:Serum APRIL and CXCL14 are highly expressed in children with SMPP and are associated with disease regression, which may serve as potential markers for predicting the prognosis of children with SMPP.

3.Bone transport with unilateral external fixation for treatment of massive tibial bone defects

Objective:To analyze the clinical efficacy of bone transport with unilateral external fixation in the treatment of massive tibial bone defects.Methods:A retrospective study was conducted to review the 21 patients with massive tibial bone defects who had been treated by bone transport with unilateral external fixation from February 2017 to January 2022 at Department of Trauma Orthopedics, Ganzhou People's Hospital. There were 14 males and 7 females with a mean age of (46.3 ± 11.3) years. Causes for bone defects: trauma ( n=5), resection of bone non-union ( n=9), resection of infected bone ( n=6) and resection of bone tumor ( n=1). The mean bone defect length was (8.3 ± 1.7) cm. Bone transport started from 10 to 12 days after operation, with a speed of 1 mm/d, and was completed in 4 times. X-ray films were reviewed every 2 weeks. The bone union time, external fixation time (EFT), external fixation index (EFI), docking site situation and complications were recorded. The clinical efficacy was assessed by Paley score. Results:All patients were followed-up for a mean time of (13.5 ± 5.5) months. The mineralization of regenerated bone was good. The bone union time was (9.6 ± 2.2) months, the EFT (10.3 ± 4.0) months, and the EFI (1.3 ± 0.4) months/cm. All docking sites got united. The docking sites were cleaned in 14 patients, of whom simple compression with external fixation was performed in 5 and bone grafts at the docking sites in 9. Postoperative nail tract infection was observed in 6 cases, tibial alignment deviation in 1 case, foot drop deformity in 5 cases, horseshoe varus foot deformity in 1 case, toe flexion deformity in 3 cases, and refracture after removing the external fixation in 1 case. By the Paley score, the bony outcomes were rated as excellent in 16 and as good in 5 cases. The functional outcomes were excellent in 10, good in 7, and acceptable in 4.Conclusion:Bone transport with unilateral external fixation is an effective treatment for massive tibial bone defects, showing advantages of easy operation and convenient carry.
4.Texture Analysis of Three-Dimensional MRI Images May Differentiate Borderline and Malignant Epithelial Ovarian Tumors

Objective:
To explore the value of magnetic resonance imaging (MRI)-based whole tumor texture analysis in differentiating borderline epithelial ovarian tumors (BEOTs) from FIGO stage I/II malignant epithelial ovarian tumors (MEOTs).
Materials and Methods:
A total of 88 patients with histopathologically confirmed ovarian epithelial tumors after surgical resection, including 30 BEOT and 58 MEOT patients, were divided into a training group (n = 62) and a test group (n = 26).The clinical and conventional MRI features were retrospectively reviewed. The texture features of tumors, based on T2-weighted imaging, diffusion-weighted imaging, and contrast-enhanced T1-weighted imaging, were extracted using MaZda software and the three top weighted texture features were selected by using the Random Forest algorithm. A non-texture logistic regression model in the training group was built to include those clinical and conventional MRI variables with p value < 0.10. Subsequently, a combined model integrating non-texture information and texture features was built for the training group. The model, evaluated using patients in the training group, was then applied to patients in the test group. Finally, receiver operating characteristic (ROC) curves were used to assess the diagnostic performance of the models.
Results:
The combined model showed superior performance in categorizing BEOTs and MEOTs (sensitivity, 92.5%; specificity, 86.4%; accuracy, 90.3%; area under the ROC curve [AUC], 0.962) than the non-texture model (sensitivity, 78.3%; specificity, 84.6%; accuracy, 82.3%; AUC, 0.818). The AUCs were statistically different (p value = 0.038). In the test group, the AUCs, sensitivity, specificity, and accuracy were 0.840, 73.3%, 90.1%, and 80.8% when the non-texture model was used and 0.896, 75.0%, 94.0%, and 88.5% when the combined model was used.
Conclusion
MRI-based texture features combined with clinical and conventional MRI features may assist in differentitating between BEOT and FIGO stage I/II MEOT patients.
5.Study on the Isolation and Purification of Baicalin Metabolites from Rat Bile and Its Effect on the Proliferation of Liver Cancer HepG 2 Cells

OBJECTIVE:To isolate and purify baicalin metabolites from rat bile ,and to study its effect on the proliferation of liver cancer HepG 2 cells. METHODS :Totally of 6 SD rats were collected ,anesthetized and then intubated with bile ducts. They were given baicalin (168 mg/kg)intragastrically after the rats were awake as well as the bile sample 1 was collected during 24 h after intragastric administration. After processed ,bile sample 1 was preliminarily analyzed by HPLC. Another 5 SD rats were anesthetized and intubated in the same way as above ,and then given baicalin intragastrically (400 mg/kg). The bile sample 2 was collected during 48 h after intragastric administration. After processed ,the bile sample 2 was isolated and purified by semi-preparative HPLC. The isolated metabolites were identified by using UV ,IR,MS and NMR ,as well as based on physical and chemical properties. MTT method combined with high content cell imaging analysis system was used to study the effect of the metabolites on the proliferation of HepG 2 cells. RESULTS :Baicalin was mainly metabolized into metabolite 1 and metabolite 2 through bile. After isolation and purification , they were identified as oroxylin A- 7-O-β-D-glucuronide and 2726719792@qq.com baicalein 6-O-β-D-glucuronide. The results of MTT assay showed that the metabolite 2 had a significant inhibitory effect 分析。E-mail:gaoxl@gmc.edu.cn on the proliferation of HepG 2 cells,and its IC 50 was 90 µg/mL;the results of high content cell imaging analysis showed that at a certain concentration metabolite 2 may inhibit proliferation by changing the mitochondrial distribution and membrane permeability of HepG 2 cells(studies on the pharmacological activities of metabolism 1 had been reported ,it was skipped in this study ). CONCLUSIONS :In this study ,two baicalin bile metabolites were successfully isolated and identified ,of which baicalein 6-O-β-D-glucuronide has a significant inhibitory effect on the proliferation of HepG 2 cells.
6. Application of in vitro porcine gastric model of submucosal eminence lesion in endoscopic submucosal dissection

Objective:
To establish the in vitro porcine gastric model of submucosal eminence lesion and to evaluate its application to endoscopic submucosal dissection(ESD).
Methods:
Silicone rubber impression materials and steel balls with diameters of 1 cm, 2 cm, and 3 cm were used to make three pairs of spherical cavities. And then raw ground beef was put into spherical cavities and boiled for 20 minutes to make spherical mass models. Six isolated porcine stomach with esophagus and duodenum were selected. The mass models with diameters of 1 cm, 2 cm and 3 cm were imbedded respectively into the submucosa of fundus, body, and antrum of porcine stomach through the incision on serosal layer. The submucosal masses were observed by endoscopy and endoscopic ultrasonography and ESD was performed.
Results:
A total of 18 mass models were constructed in 6 porcine stomachs, of which 17 models were successfully established and 1 failed. Typical endoscopic characteristics of gastric submucosal eminence lesions were found in 17 models. Endoscopic ultrasonography showed that these models originated from submucosal layer and demonstrated mixed echo. There were no significant differences between mucosa of lesions and that of surrounding areas. ESD was successfully performed in the porcine gastric models of submucosal eminence lesions, and all models were not broken or detached.
Conclusion
The in vitro porcine gastric model of submucosal eminence lesions can well replicate disease status and provide a suitable model for study on endoscopic therapy of submucosal eminence lesion and training of endoscopists.
7.Value of heart murmurs and transcutaneous oxygen saturation in screening for neonatal congenital heart disea-ses

Objective To compare the importance of heart murmurs and transcutaneous oxygen saturation (SpO2)in screening neonatal congenital heart diseases(CHD),and to explore a reliable and easy-to-use method for the screening of CHD. Methods All infants (16 070 cases) born in Changzhou Maternal and Child Health Hospital from May 1 st 2011 to April 30 th 2013 were screened of CHD for heart murmurs and SpO2 . The screen-positive infants were divided into 3 groups:heart murmurs group,abnormal SpO2 group and mixed group. All screen-positive infants re-ceived cardiac color ultrasound to make a definite diagnosis of CHD,so as to compare the values of heart murmurs and SpO2 . Results Eight thousand seven hundred and ninety-nine boys and 7 271 girls were screened,in which the ratio was 1. 21:1. 00. One hundred and eighteen infants were screened positive,including 76 murmurs of which 45 were confirmed as CHD,28 with abnormal SpO2 of which 18 were confirmed and 14 mixed of which 13 were confirmed,so the confirmation rates were 59. 21%,64. 29% and 92. 86%,respectively. The diagnostic rate of CHD in mixed group was higher than other 2 groups,which had statistical significance(P=0. 045). The top three formation types of CHD were ventricular septal defect (VSD) (29/76 cases,38. 16%),patent ductus arteriosus (PDA) (13/76 cases, 17. 11%) and atrial septal defect (ASD) (11/76 cases,14. 48%). Conclusions The combination of heart murmurs and SpO2 was useful in increasing detection and decreasing misdiagnosis of CHD.
8.The prognostic value of arterial blood lactate for severe neonatal asphyxia

Objective To investigate the prognostic value of arterial blood lactate(Lac) and lactate clearance rate(LCR) for severe neonatal asphyxia.Methods One hundred and fifteen cases of severe neo natal asphyxia admitted in NICU of our hospital from January 2011 to October 2014 were retrospectively analyzed.Lac and LCR values were measured at multiple time points and were compared between those died (27 cases,the death group) and those survived (88 cases, the survival group).The correlation between prognosis and Lac as well asLCR was investigated.Receiver operating characteristic(ROC) curves were drawn to evaluate the prognostic value of Lac and LCR at different time points.Results No significant differences in initial lactate levels and neonatal critical illness score (NCIS) were detected between the two groups (P >0.05), while Lac and LCR values at 1 h,2 h,and 6 h showed significant differences between the two groups (P < 0.05).The low-LCR group showed a higher rate of multiple organ dysfunction syndrome and mortality rates than the high-LCR group (44.64% vs.15.25 %, P =0.001;32.14% vs.15.25 %, P =0.033, respectively).Post-treatment Lac level was positively correlated with prognosis, while LCR at 1 h,2 h, and 6 h were negatively correlated with prognosis.Areas under curve of Lac ROC at 1 h,2 h, and 6 h were 0.625,0.719,and 0.835 respectively, while areas under curve of LCR ROC at 1 h, 2 h, and 6 h were 0.676,0.784, and 0.898 respectively.Conclusion Low LCR for severe neonatal asphyxia predicts poor prognosis.Lac level and LCR at 6 h after emergency treatment is a prognostic indicator for severe neonatal asphyxia.
9.Feasibility of intentional occlusion of the left subclavian artery in endovascular aortic repair of thoracic aortic disease

Objective To investigate the feasibility and effect of intentional occlusion of the left subclavian artery (LSA) in endovascular aortic repair EVAR of thoracic aortic disease.Methods The cases,who needed occlusion of the LSA in EVAR of thoracic aortic disease,were examined cerebral circulation,carotid artery,vertebral basilar artery and circle of Willis.If the vertebral artery had a good blood pathway,and carotid artery and circle of Willis had no stenotic,we occluded the LSA directly in EVAR,and observed the brain and upper limb ischemia complications postoperation.Results Among 40 cases who were occluded the LSA directly in EVAR,28 cases were successful with no neurological complications and left upper limb ischemia symptoms postoperation,12 patients with mild symptoms of subclavian steal syndrome and neurdogical complications,but none required and secondary surgical intervention.Conclusions Prior to intentional LSA occlusion,attention must be paid to potential supraaortic variants and pathologies.Only in this way,it was safe and effective to occlude the LSA in EVAR of thoracic aortic disease.
10.Chemical components of essential oils from the herb of Sabina pingii var. wilsonii.

Aim: To provide the foundation for reasonable utilization by analyzing the essential oils of Sabina pingii var. wilsonii.. Methods: The essential oils were extracted using steam distillation and separated with GC capillary column. The components were quantitatively determined with normalization method, and were preliminarily identi-fied by GC-MS. Results: 79 Components were identified, which took up 93. 78% of the essential oils. Conclusion: The main components of essential oils were a-terpineol( 16. 70%); α-fenchene( 14. 98%); 2,6,10-dodecatrien-1-ol, 3,7, 11-trimethyl-( 6.49%); 1, 4-cyclohexadiene, 1-methyl-4-[1-methylethyl]-( 4. 74%); kaura-5, 16-dien-18 [orl9]-ol(3. 62%); naphthalenemethanol, 1, 2, 3, 4, 4a, 5, 6, 7-octahydro-α, α, 4a, 8-tetramethyl-, [2R-cis]-(3. 04%); [ + ]-4-carene ( 2. 81%); D-limonene ( 2. 71%); β-myrcene ( 2. 59%); cedrol (2.40%); a-pinene (2. 36%); 7-isopropyl-1, 1, 4a-trimethyl-1 ,2,3,4,4a, 9,10,10a-octahydrophenanthrene(2. 32%).
