1.Correlation of Multiple Clinical Indicators with Benign and Malignant Pulmonary Nodules

Objective To investigate the correlation between the benign and malignant pulmonary nodules and serum inflammatory factors,tumor markers,and imaging features.Methods A total of 209 patients with pulmonary nodules who underwent lung puncture biopsy at the Third People's Hospital of Kunming from January 2023 to January 2024 were enrolled.Based on pathological results,the patients were divided into benign pulmonary nodules group(n=106)and malignant pulmonary nodules group(n=103).General data and clinical indicators of all subjects were collected,and differences in various indicators between the two groups were analyzed.Results In terms of Serum inflammatory factors:there were statistically significant differences in LYMPH,NLR,LMR,IL-2,IL-6,IL-17,IFN-γ,HsCRP,SAA,and PCT between the two groups(P<0.05),while no significant differences were found in WBC,NETU,MONO,PLT,SII,IL-5,IFN-α,IL-1β,IL-10,IL-8,and IL-12P70(P>0.05).Regarding Serum tumor markers:there were statistically significant differences in CEA,CA125,CA199,CYFRA21-1,ProGRP,and NSE between the two groups(P<0.05),whereas no significant differences were found in TSGF,AFP,and CA153(P>0.05).In terms of Imaging features:there were statistically significant differences in the number,diameter,boundary,fissure sign,spiculated sign,and pleural retraction sign of nodules between the two groups(P<0.05),while no significant differences were found in nodular calcification,nodule density,and location(P>0.05).Binary logistic regression analysis(backward-Wald conditional method)identified patient age,fissure sign,CEA,NSE,ProGRP,and IL-6 were independent risk factors for malignant pulmonary nodules(P<0.05).The combined diagnostic model of these indicators for predicting malignant pulmonary nodules had an AUC of 0.965(P<0.05).Conclusion Malignant pulmonary nodules are more likely in older patients with lobulation on imaging and elevated serum levels of NSE,ProGRP,IL-6,and CEA.Combined detection of these indicators can predict the nature of pulmonary nodules,guiding early diagnosis of malignant nodules.
2.Construction of the teaching quality evaluation index system for clinical teachers of undergraduate nursing students based on teaching managers' perspective

Objective:To construct the teaching quality evaluation index system for clinical teachers of undergraduate nursing students based on teaching managers' perspective so as to provide a basis for clinical teaching management of undergraduate nursing students.Methods:From May 2018 to September 2019, the expert enquiry questionnaire on the teaching quality evaluation index system for clinical teachers of undergraduate nursing students based on teaching managers' perspective was developed by the literature review and expert interview. Delphi technique was used to select 20 nursing teaching management experts, 16 of them from 4 ClassⅢ Grade A teaching hospitals in Beijing, 4 of them from a nursing undergraduate university to carry out two rounds of expert enquiry.Results:The initial construction of the teaching quality evaluation index system for clinical teachers of undergraduate nursing students based on teaching managers' perspective included 5 first-level indexes and 30 second-level indexes. The valid recovery rates, expert authoritative coefficients, Kendall coordination coefficients of two rounds of expert enquiry questionnaire were 100%, 0.83 and 0.86, 0.395 and 0.554 respectively ( P<0.01) with the satisfied consistency test results and credible enquiry results. Conclusions:The construction of the teaching quality evaluation index system for clinical teachers of undergraduate nursing students based on teaching managers' perspective is scientifically rigorous. The system has definite evaluation indexes, rational administrative levels, objective and reliable system which could provide a basis for clinical teaching management of undergraduate nursing students.
3.Status quo and influencing factors of nurse-patient communication ability in undergraduate nursing students

Objective:To explore the status quo of nurse-patient communication ability in undergraduate nursing students in the early stage of clinical practice and its influencing factors, and provide a basis for clinical nursing education managers to formulate a training plan for communication skills of undergraduate nursing students in the clinical practice stage.Methods:From August to September 2019, totally 960 undergraduate nursing students engaging in clinical nursing practice for≤3 months were investigated. The survey content included demographic data, training content of nurse-patient communication ability, and clinical communication ability of nursing students. The status quo and influencing factors of communication ability of undergraduate nursing students in the early stage of clinical practice were analyzed.Results:The nurse-patient communication ability score of 960 undergraduate nursing students in the early stage of clinical practice was (2.18±0.21) , with the highest score in the dimension of acute listening (2.42±0.51) and the lowest score in the dimension of verification feeling (1.88±0.51) . Univariate analysis showed that there were statistically significant differences in the nurse-patient communication ability in the early stage of clinical practice between undergraduate nursing students with different gender, with or without training for nurse-patient communication in the early stage of clinical practice in the hospital and with different training forms ( P<0.05) . Pearson correlation analysis revealed that the nurse-patient communication ability score of undergraduate nursing students in the early stage of clinical practice was positively correlated with the number of nurse-patient communication training sessions in the hospital ( r=0.456, P<0.01) . According to multiple linear regression analysis, the number of training sessions and training forms in the hospital were main influencing factors to the nurse-patient communication ability of undergraduate nursing students in the early stage of clinical practice ( P<0.05) . Conclusions:The level of nurse-patient communication ability in undergraduate nursing students in the early stage of clinical practice is low and needs to be further improved. Clinical nursing teaching managers should formulate effective training programs based on its influencing factors to strengthen training accordingly.
4.Approach to the patient with Kallmann syndrome

Kallmann syndrome ( KS) is a rare disease and characteristic of an absence of puberty, infertility, and a defective sensation of smell (anosmia or hyposmia). Here, we analyze the features of a case of KS diagnosed clinically. In addition, the etiology, genetic features, clinical manifestations, diagnosis, and treatment of KS were reviewed.
5.Approach to the patient with Klinefelter syndrome combined with neuropsychological abnormality

Klinefelter syndrome(KS) is the most common sex chromosome disorder in males,which is caused by the presence of the extra X chromosome that maybe inherited from mother or father. Approximately 80% karyotype of the cases is 47,XXY. KS is characterized by small firm testes, hypergonadotropic hypogonadism, infertility,gynaecomastia, increased height. However, cognitive disabilities and psychiatric disorders are rarely diagnosed in KS because they lack screening in related aspects. At the present, the pathogenesis of cognitive disabilities and increased risk of psychiatric diseases in KS have not been delineated. In this article,we report two cases of KS,and review their clinical manifestations,diagnosis,and treatments.
6.Quantitative evaluation of psychological characteristics and quality of life of patients with oral cancer during perioperative period

Objective To investigatethe quality of life of patients with oral cancer in different period in a hospital, and to explore the impact of patients' psychological status on their quality of life. Methods 50 cases of oral cancer patients in our hospital were investigated in this study by Convenience sampling method. The scale of the hospital anxiety and Depression Scale (HADS, the Japanese version), the quality of life of cancer patients were measured by the scale (FACT-G); the functional evaluation of tumor therapy- head and neck questionnaire (FACT-H&N). Results The comparison of HADS score between patients before and after surgery showed that the improvement of mental status was not obvious in the postoperative patients.The scores of HADS between patients before surgery and after hospital showed that the improvement of mental status was obvious (t=-2.809, P=0.01; t=-3.828, P=0.003). There were significant differences in HADS anxiety and depression-related factors between different periods (F=7.644,P=0.001;F=6.442,P=0.002);Before surgery,patients with low anxiety and depression had higher scores in both emotional and physical functional dimensions, and the difference between the two groups was statistically significant (t=7.882,5.847,6.870,7.262, P<0.05 or 0.01). The scores of the two groups were higher in the whole life quality dimension after surgery,and the difference between the two groups was statistically significant(t=3.640-7.931,P<0.01 or 0.01).There were significant differences in body function and psychological function scores between different periods(F=8.569, P=0.000; F=10.250, P=0.003). Conclusions Oral cancer patients have a higher level of anxiety and depression before and after surgery, and psychological status of patients with a certain impact on quality of life, it should be targeted for individual characteristics of patients with targeted care measures to improve the psychological status of patients,thereby improving the quality of life of patients.
7.Influence of plaque staining test on periodontal health of middle-aged patients

Objective To investigate the effects of plaque staining test on periodontal health of middle-aged and elderly patients with dental implants. Methods Ninety patients with dental implants were included in the study and divided into test group (n=45) and control group (n=45) by using random digit table. In the test group, the patients were all instructed with both routine health education and dental plaque dyeing education, while the control group received routine health education only. The plaque index (PLI) and gingival index (GI) were assessed at months 1 and 6 after implant restoration. Results One month after dental implantation, there were no significant differences in PLI and GI between the two groups (P>0.05), while 6 months after the implantation, the differences in PLI and GI between them were significantly different ( P<0 . 01 ) . Conclusion The plaque dyeing technology applied in the oral health education to elderly patients with dental implants can reduce plaque index and gingival index so that their periodontal health is enhanced.
8.Mechanical and physicochemical properties of xenogeneic bone scaffold materials A comparative study

BACKGROUND: The core of bone tissue engineering is to construct a scaffold that is similar to human bone tissue structure and features.OBJECTIVE: To compare pathochemical and mechanical characteristics between pig and human bone scaffold materials.DESIGN, TIME AND SETTING: Contrast study was performed at Clinical Anatomy Institute, South Medical University; Guangdong Province Key Laboratory of Tissue Construction and Detection from March to December 2006.MATERIALS: Four fresh health adult human cadavers were provided by South Medical University, Guangzhou Red Cross Society, and the relatives knew the fact. Ultra low temperature freezing 6-month iliac bones of 6 adult swines were also used in this study.METHODS: Pig iliac and healthy adults iliac bones were obtained to remove soft tissue, curettage periosteum and bone marrow. Bone sawing machine was used to cut cancellous bone into smaller bone sections around 5 mm×5 mm×40 mm, which underwent ultrasonic cleaning, H2O2 and alcohol soaking, freeze drying and radiation treatment; finally, xenogeneic bone scaffold and allogeneic bone scaffold were obtained.MAIN OUTCOME MEASURES: Xenogeneic bone scaffold material and human allograft bone scaffold were observed with scanning electron microscopy to compare porosity, contents of protein content, calcium and phosphorus, and mechanical properties.RESULTS: Xenogeneic bone scaffold and allogeneic bone scaffold both had intrinsical bone trabecula, trabecular spaces and bone cavity system. Both of them had unabridged natural three dimensional network structure. The 3D supporting frames of them were complete. The xenogeneic bone scaffold had more spaces than allogeneic bone scaffold. The size of both scaffolds was approximation, about 400 μm. The interval porosity of xenogeneic bone scaffold was higher than the allogeneic bone scaffold (P<0.05). And the protein of xenogeneic bone scaffold was not as many as it of allogeneic bone scaffold (P<0.05). The contents of Ca and P were similar (P>0.05), and there was no significant difference in Young's modulus of xenogeneic bone scaffold and allogeneic bone scaffold (P>0.05).CONCLUSION: Xenogeneic bone scaffold may completely meet the clinical demands for bone grafting or be the scaffold of bone tissue engineering in mechanical chemical properties.
