Objective:To investigate the mechanism by which lycopene prevents and alleviates ionizing radiation-induced intestinal injury. Methods:Forty specific pathogen-free male C57BL/6 mice were randomly divided into 4 groups: control group (gavage with an equal volume of corn oil), radiation group (gavage with an equal volume of corn oil and exposed to a single dose of 12 Gy X-ray irradiation on day 3), lycopene group (gavage with lycopene), and lycopene + radiation group (gavage with lycopene and irradiated with 12 Gy X-ray on day 3). All gavage treatments continued for 7 days after irradiation. On day 7 post-irradiation, intestinal tissues were collected. Hematoxylin and eosin (HE), Masson's trichrome, and Sirius red staining were used to assess intestinal injury and fibrosis. Immunohistochemistry was used to detect the expression of α-smooth muscle actin (α-SMA). Immunofluorescence staining was performed to assess the expression of transforming growth factor-β (TGF-β) and connective tissue growth factor (CTGF) in intestinal tissues. Serum levels of malondialdehyde (MDA), total superoxide dismutase (T-SOD), total antioxidant capacity (T-AOC), and glutathione peroxidase (GSH-PX) were measured to evaluate oxidative stress. mRNA expression of tumor necrosis factor-α (TNF-α), interleukin (IL)-6, IL-1β, and CD68 in intestinal tissue was quantified using real-time quantitative PCR (RT-qPCR). Western blot was used to detect the expression of apoptosis-related proteins. One-way ANOVA was used to compare differences among groups. Results:Compared with the radiation group, the lycopene + radiation group showed significantly reduced radiation-induced intestinal injury index, fibrosis, and expression of α-SMA, TGF-β, and CTGF (all P<0.05). Oxidative stress assays showed that serum MDA levels were decreased, while T-SOD, T-AOC, and GSH-PX activities were significantly increased in the lycopene + radiation group (all P<0.05). The mRNA expression of inflammatory cytokines such as TNF-α and IL-6 was significantly downregulated (all P<0.05), while pro-apoptotic protein expression was reduced and anti-apoptotic protein expression was upregulated (all P < 0.05) in the lycopene + radiation group. Conclusions:Lycopene significantly alleviates ionizing radiation-induced intestinal injury through its antioxidant, anti-inflammatory, and anti-apoptotic effects, making it a promising agent for the prevention and treatment of radiation-induced intestinal injury.

Turning to critical illness is a common stage of various diseases and injuries before death. Patients usually have complex health conditions, while the treatment process involves a wide range of content, along with high requirements for doctor′s professionalism and multi-specialty teamwork, as well as a great demand for time-sensitive treatments. However, this is not matched with critical care professionals and the current state of medical care in China. Telemedicine, which shortens the distance of medical professionals and the gap of disease diagnosis and treatments in various regions through electronic information, can effectively solve the current problem. Therefore, there is an urgent need to develop a standardized, high-quality visualization telemedicine round system .Therefore, experts have been organized to search domestic and foreign literature on telemedicine round for critically ill patients and to form this consensus based on clinical experiences so as to further improve the level of critical care treatments in regions.

Chinese Journal of Hematology 2024;45(11):1022-1027
Objectives:This study aimed to analyze the clinical and molecular characteristics of carbapenem-resistant Enterobacteriaceae (CRE) bloodstream infection (BSI) in patients with hematological diseases and to explore prognostic risk factors. Methods:This retrospective study included patients with hematologic diseases with CRE BSI at the Institute of Hematology and Blood Diseases Hospital from January 2015 to December 2022. The clinical features, carbapenemase test results, antimicrobial treatments, and outcomes were analyzed. Results:A total of 120 patients developed CRE BSI. Escherichia coli (58/120, 48.3%) was the most prevalent Enterobacteriaceae, followed by Klebsiella pneumoniae (52/120, 43.3%). A total of 93 CRE strains were tested for carbapenemase, of which 75 strains produced carbapenemase (metalloenzyme: 51 strains; serine enzyme: 24 strains). The 30-day mortality rate after BSI was 24.2% (29/120). Univariate analysis revealed significantly lower mortality in patients treated with the ceftazidime-avibactam-containing regimen than in those treated with other antibiotics (7.8% vs 36.2%, P<0.001). Moreover, initiating active therapy within 24 h of BSI onset significantly reduced mortality (15.0% vs 33.3%, P=0.019). The proportion of patients with CRE colonization receiving active therapy within 12 and 24 h was significantly higher compared with patients without colonization (12 h: 14.5% vs 34.1%, P=0.012; 24 h: 40.8% vs 65.9%, P=0.008). Multivariate analysis revealed that septic shock ( HR=24.436, 95% CI 4.148 - 143.966, P<0.001) and pulmonary infection ( HR=9.346, 95% CI 2.718-32.140, P<0.001) were independent risk factors for death within 30 days. Appropriate therapy was initiated within 24 h ( HR=0.225, 95% CI 0.059 - 0.851, P=0.028), and treatment with the ceftazidime-avibactam-containing regimen ( HR=0.082, 95% CI 0.018-0.362, P=0.001) significantly reduced mortality. Conclusion:The prognosis of CRE BSI in patients with hematological diseases is poor. Timely, appropriate therapy and receipt of a ceftazidime-avibactam-containing regimen can improve survival and prognosis.

Chinese Journal of Modern Nursing 2023;29(33):4522-4529
Objective:To explore the subtypes of professional quality of life for Operating Room nurses from an "individual centered" perspective, so as to compare the differences in negative emotions and turnover intentions among different subtypes of nurses. Methods:This article was a cross-sectional survey study. Using the convenient sampling method, a total of 409 Operating Room nurses from three tertiary hospitals in Shandong Province were selected as the research objects in November 2021. The survey was conducted using General Information Questionnaire, Professional Quality of Life Scale, Depression Anxiety Stress Self-assessment Scale and single item of resignation intention. The latent profile analysis was used to explore the potential categories of professional quality of life for operating room nurses. Results:Finally, 350 valid questionnaires were collected, and the effective recovery rate was 85.57% (350/409). 350 operating room nurses scored (50.35±9.82) in the dimension of compassionate satisfaction, (49.01±10.43) in the dimension of burnout, and (49.18±9.53) in the dimension of secondary trauma, all of which were at a moderate level. The professional quality of life of operating room nurses was divided into three potential categories, namely good type (25.14%, n=88), contradictory type (17.43%, n=61) and low satisfaction type (57.43%, n=201). The scores of depression dimension were (3.89±7.69), (14.39±11.13) and (13.18±9.50) in good, contradictory and low-satisfaction class nurses, respectively, and the differences were statistically significant ( P<0.01). The scores of anxiety were (5.82±8.45), (16.92±10.87) and (14.45±9.29), respectively, and the difference was statistically significant ( P< 0.01). The scores of stress dimension were (5.64±8.27), (17.18±11.36) and (15.01±9.10), respectively, and the difference was statistically significant ( P<0.01). The scores of turnover intention were (1.45±0.52), (2.07±1.09), and (2.42±1.06), respectively, and the differences were statistically significant ( P<0.01) . Conclusions:There are different characteristic groups of professional quality of life in operating room nurses, and the population distribution is closely related to negative emotion and turnover intention. Nursing managers should pay attention to the identification and intervention of contradictory and low-satisfaction operating room nurses and give accurate and classified policies.

Objective:To investigate the changes in cognitive and behavioral function in patients with cerebral small vessel disease and their correlation with serum homocysteine (Hcy) and cystatin C (CysC) levels.Methods:Ninety-four patients with cerebral small vascular disease who received treatment in Zhejiang Xin'an International Hospital from March 2019 to March 2021 were included in the observation group. An additional 80 healthy controls who concurrently received physical examination were included in the control group. The Mini-Mental State Scale (MMSE) and the Montreal Assessment Scale (MoCA) were used to evaluate cognitive function. The activity of daily living (ADL) was used to evaluate behavioral function. Enzyme-linked immunosorbent assay was used to measure serum Hcy and CysC levels. MMSE score, MoCA score, ADL score, serum Hcy and CysC levels were compared between the two groups. The receiver operating characteristic (ROC) curve was used to analyze the sensitivity and specificity of serum Hcy and CysC levels in the diagnosis of cerebral small vessel disease. Pearson correlation analysis was performed to analyze the correlation between serum Hcy and CysC levels and cognitive behavioral function. Results:MMSE score, MoCA score, and ADL score in the observation group were (21.21 ± 2.87) points, (20.18 ± 3.25) points, and (34.32 ± 5.45) points, which were significantly lower than (28.23 ± 1.02) points, (28.45 ± 0.89) points and (76.35 ± 8.29) points in the control group ( t = 20.78, 22.05, 40.03, all P < 0.05). Serum Hcy and CysC in the observation group were (16.28 ± 2.97) μmol/L and (1.21 ± 0.23) mg/L, which were significantly higher than (8.35 ± 1.76) μmol/L and (0.56 ± 0.12) mg/L in the control group ( t = 20.95, 22.77, both P < 0.05). ROC curve analysis showed that the sensitivity and specificity of serum Hcy in diagnosing cerebral small vessel disease were 85.29% and 53.85%, respectively; the sensitivity and specificity of serum CysC in diagnosing cerebral small vessel disease were 81.54% and 41.38%, respectively. Pearson correlation analysis revealed that serum Hcy level was linearly negatively correlated with MMSE score, MoCA score, and ADL score ( r = -0.79, -0.68, -0.71, all P < 0.05); serum CysC level was linearly negatively correlated with MMSE score, MoCA score, and ADL score ( r = -0.67, -0.75, -0.78, P < 0.05). Conclusion:The cognitive and behavioral function of patients with cerebral small vessel disease are decreased, and serum Hcy and CysC levels are increased. Serum Hcy and CysC levels are closely related to the cognitive and behavioral function of patients with cerebral small vessel disease.

Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection can cause acute respiratory distress syndrome, hypercoagulability, hypertension, and multiorgan dysfunction. Effective antivirals with safe clinical profile are urgently needed to improve the overall prognosis. In an analysis of a randomly collected cohort of 124 patients with COVID-19, we found that hypercoagulability as indicated by elevated concentrations of D-dimers was associated with disease severity. By virtual screening of a U.S. FDA approved drug library, we identified an anticoagulation agent dipyridamole (DIP) , which suppressed SARS-CoV-2 replication . In a proof-of-concept trial involving 31 patients with COVID-19, DIP supplementation was associated with significantly decreased concentrations of D-dimers ( < 0.05), increased lymphocyte and platelet recovery in the circulation, and markedly improved clinical outcomes in comparison to the control patients. In particular, all 8 of the DIP-treated severely ill patients showed remarkable improvement: 7 patients (87.5%) achieved clinical cure and were discharged from the hospitals while the remaining 1 patient (12.5%) was in clinical remission.
