Objective:To investigate the clinicopathological characteristics, immunophenotype and differential diagnosis of atypical forms of microglandular hyperplasia of the cervix (AMGH).Methods:A total of 29 cases of AMGH diagnosed at the Obstetrics and Gynecology Hospital of Fudan University, Shanghai, China from January 2010 to December 2024 were analyzed. Relevant clinical and pathological data of the patients were collected using the electronic medical record system and medical records copied from the outside hospitals. The patients were followed up.Results:Among the 29 cases, 28 were consultation cases, 22 (79%) of the 28 cases were considered as glandular neoplastic lesions by the original institutions. The nature of the lesion was uncertain in 1 case, the diagnosis was suspicious for AMGH in another 1 case, and only 4 cases were clearly diagnosed as AMGH. The median age of the 29 patients was 44 (43, 48) years. Eighteen (62%) of the 29 cases presented as cervical polyp. Twelve of the 16 tested cases were negative for human papillomavirus. The pathological presentation was complex and diverse, including solid, trabecular, cribriform, and papillary patterns, forming pseudo-invasive structures. The glandular epithelium and proliferating reserve cells had diverse morphologies, which presented with abundant eosinophilic cytoplasm or clear cytoplasm. Signet-ring or hobnail cells were also seen. The nuclear atypia was mild, with 0-7 mitotic figures per 10 HPF. Immature squamous metaplasia was noted. The stroma showed edema, myxoid change and hyaline degeneration, accompanied by infiltration of acute and chronic inflammatory cells. Immunohistochemistry demonstrated that p16 was negative in 8/16 of the cases or patchy positive in the other 8/16, Ki-67 positive rate was less than 10% in all 16 cases, p53 was wild phenotype (9/9), and carcinoembryonic antigen was negative in 4/5 cases and focally positive in 1/5 cases, while p63 was positive in 6/9 of the tested cases.Conclusions:AMGH is a benign non-neoplastic lesion of the cervical glands. Half of the cases occur in perimenopausal or postmenopausal women, often presenting as polypoid hyperplasia or localized cervical thickening/elevation with a friable, fragile texture. Microscopically, it may show a pseudoinvasive pattern, making it prone to misdiagnosis as a malignant lesion. Thus, differentiation from cervical adenocarcinoma, clear cell carcinoma and microglandular endometrioid carcinoma is required. Integration of clinical history, immunohistochemistry and molecular testing may aid in the differential diagnosis.

Objective:To investigate the effect of Ureaplasma spp.lipid-associated membrane proteins(LAMPs)on macro-phage M1 polarization,and clarify how LAMPs resist to ferroptosis by regulating macrophage M1 polarization.The overall goal is to contribute to a better understanding of pathogenesis of non-gonococcal urethritis caused by Ureaplasma spp.Methods:ELISA,qPCR and flow cytometry were used to analyze the effect of LAMPs on M1 polarization of macrophages;Western blot to detect how LAMPs stimulates NF-κB pathway during M1 polarization of macrophages;gene interference technology was used to elucidate how LAMPs affects macrophage M1 polarization through regulating NF-κB pathway;CCK8 and Western blot were used to find out how LAMPs affects macrophage ferroptosis by regulating NF-κB pathway.Results:ELISA,qPCR and flow cytometry results showed that LAMPs promoted macrophage polarization to M1;Western blot results showed that LAMPs increased levels of p-p65 in macrophages;interfering with p65 expression could inhibit M1 polarization process of macrophages that mediated by LAMPs,and inhibition of macrophage ferroptosis that mediated by LAMPs could be significantly reversed.Conclusion:LAMPs inhibits ferroptosis by accelerating macrophage M1 po-larization,which is one of the important pathogenic factors of non-gonococcal urethritis caused by Ureaplasma spp.

Objective:To investigate the clinicopathological, molecular pathological features, and family genetic pedigree of small cell carcinoma of the ovary, hypercalcemic type (SCCOHT).Methods:A total of 16 cases of SCCOHT diagnosed in Obstetrics and Gynecology Hospital of Fudan University from January 2013 to January 2023 were collected. The clinicopathologic features, SMARCA4/2/B1 protein expression, outcomes and SMARCA4 gene detection were reported. A follow-up study was also carried out.Results:The average age at diagnosis was 28.7 years (range 17-38 years). The preoperative calcium level was evaluated in 3 of 6 patients. The tumor was unilateral in all 16 cases, ranged from 8 to 26 cm (average 15.8 cm) in the greatest dimension. Extraovarian spread was present in 7 cases. In 10 cases, the tumors were initially misinterpreted as other ovarian neoplasms. BRG1 and BRM expression by immunohistochemistry were all lost in detected cases, while INI1 exhibited retained nuclear expression. All BRM-negative SCCOHTs also lacked BRG1 protein,but retained INI1 expression. SCCOHTs were only focally positive for EMA, CKpan, Calretinin, SALL4, and diffusely positive for WT1. Two of nine cases exhibited mutation-type p53 immunoreactivity. Ki-67 index was 58% on an average. ER, PR, FOXL2, α-inhibin, chromogranin A and LCA were negative in all the cases. SMARCA4 sequencing was available in 8 cases of SCCOHT, which revealed a germline SMARCA4 mutation in one patient, and others carried somatic mutation. Furthermore, two daughters, mother and an aunt of a patient with germline mutation were reported to be SMARCA4 mutation carriers. Follow-up was available for 15 patients, and the 6-month, 1-year and 2-year survival rate was 65.8%, 45.1%, and 22.6%, respectively. For patients in FIGO stages Ⅱ+Ⅲ, 6-month, 1-year survival rate was 53.6% and 35.7% respectively, compared to 80% (6-month) and 60% (1-year) in patients of staged I ( P=0.358). Conclusions:With dismal prognosis of SCCOHT, accurate diagnosis is necessary. The typical age distribution, a panel of various staining results, especially concomitant loss of BRG1 and BRM may be of diagnostic aid and can be used to distinguish SCCOHT from its histological mimics. After the diagnosis of SCCOHT, genetic testing and genetic counseling are recommended.

Objective: To investigate the clinicopathological characteristics, histogenesis, immunophenotypes, molecular genetic characteristics, diagnosis and differential diagnosis of calcifying fibrous tumors (CFT). Methods: A total of 32 cases of CFT (22 cases from Henan Provincial People′s Hospital and 10 cases from PLA Army Medical Center) diagnosed between June 2009 and February 2019 were reviewed. The clinical and pathologic data were analyzed. Results: There were 12 male and 20 female patients, aged from 15 to 63 years (mean 40.8 years). Eleven cases occurred in stomach, four cases in retroperitoneum, four cases in ovary, two cases in scrotum, two cases in mediastinum, two cases in head and neck, one case each in thoracic cavity, lung, adrenal gland, kidney, sigmoid colon, epididymis and mesosalpinx. All the tumors were solid masses with clear boundaries. The maximal dimension of the tumors ranged from 0.6 to 10.0 cm. Microscopically, there was hypocellular stromal sclerosis and wavy storiform coarse collagen with superimposed scattered or patchy lymphocytes and plasma cells; calcification or gravel formation were also detected. Immunohistochemistry showed that spindle cells were positive for vimentin and some were positive for CD34; and they were negative for calponin, SMA, desmin, S-100 protein, SOX10, STAT6, β-catenin, ALK, CD117, DOG1, CKpan, and EMA. No ALK rearrangement was detected by FISH in all cases. No C-KIT and PDGFRA mutation was detected in all the tested 11 cases of stomach, four cases of retroperitoneal and one case of sigmoid colon CFT. MDM2 was not amplified by FISH in all four tested cases of retroperitoneal CFT. Conclusions CFT is a rare benign tumor of fibroblastic cell origin. The diagnosis mainly depends on histomorphologic analysis and immunophenotyping. CFT should be differentiated from other benign and malignant spindle cell mesenchymal tumors.

Objective To explore the value of 3.0T MRI in preoperative diagnosis and staging of elderly cervical carcinoma.Methods Totally 73 patients were enrolled in this trial.The MRI features of cervical carcinoma and invasion of adjacent tissue were evaluated,the correlation of MRI staging and pathological staging was studied.Results MRI showed lesions in 73 patients,the accuracy of diagnosis for uterine cervical carcinoma was 100.0 ％,for parametrial invasion was 94.2 ％.The results of MRI staging of 52 cases showed the accuracy was 88.5％ (46 cases) as compared with postoperative pathology.The correlation between the MRI and postoperative pathology in the stages of Ⅰ B,Ⅱ A and Ⅱ B uterine cervical cancer was very well(x2 ＝ 61.43,P＜0.01),the consistency of MRI in the diagnosis of uterine cervical cancer in stage Ⅲ and Ⅳ with postoperative pathology was extremely strong(100％).Conclusions The accuracy of diagnosis and staging for uterine cervical carcinoma is high in MRI imaging.

Background Rosuvastatin has been unanimously recognized as an very highly efficacious statin. Rosuvastatin significantly reduced microalbuminuria and its powerful anti-inflammatory effect confer anti-atherosclerosis in patients with essential hypertension. Objective To study the influence of rosuvastatin on microalbuminuria and arteriosclerosis plaque of carotid and coronary arteries in hypertensive patients. Methods Seventy-six hypertensive patients were randomly to receive conventional anti-hypertensive drugs (amlodipine 5 mg/d and telmisartan 80 mg/d,n=37) or rosuvastatin (10 mg QN) on the top of conventional anti-hypertensive drugs (n=39). The blood pressure,levels of microalbuminuria,echocardiography and helical computerized tomography were examined before and 8 months after treatment in all patients. Results After 8 months of treatment,SBP and DBP were decreased in both group [conventional treatment from (166.3?11.2)/(92.4?8.2) to (133.6?9.8)/(85.5?6.1)mmHg,combined group from (165.6?10.5)/(91.5?6.7) to (128.1?9.2)/(81.1?5.9)mmHg].
