1.Current situation in regulatory focus on nurses in operating room and the influencing factors and corresponding nursing strategies

Objective To investigate the current status of regulatory focus among the nurses in operating room,analyse the influencing factors and summarise nursing strategies to provide a reference for development of the programs in targeted nursing management.Methods A total of 681 operating room nurses from 18 hospitals of Grade ⅡA and above in Jiangsu Province were selected by convenience sampling method from Arprial to July 2023.The nurses who participated in the study were surveyed with a general information questionnaire,the nurse regulatory focus scale(NRFQ),spiritual climate scale(SCS),calling scale-12(CS-12),and perceived stress scale(CPSS).Results All 681 operating room nurses completed the study.The promotive focus score was lower than the defensive focus(23.57±2.64 vs.24.06±2.72).It was found that the promotive focus and defensive focus were positively correlated with spiritual climate and occupational call,and negatively correlated with perceived stress(P<0.01).Spiritual climate,occupational call and perceived stress were the influencing factors that affected the promotion focus,jointly explaining 28.00%of the variance(all P<0.05).While occupational call and perceived stress among the operating room nurses were the influencing factors to the defensive focus,jointly explaining 23.60%of the variance(both P<0.05).Conclusion This study has identified that spiritual climate of nurses in operating room positively influences promotive focus in the regulatory focus.The regulatory focus as a whole is positively regulated by occupational call and negatively influenced by perceived stress.Therefore,nursing administrators should create a favourable spiritual climate to enhance the sense of professional call,reduce the perceived stress and increase the promotive and defensive focus among the nurses in operating room,hence to increase the nursing efficiency,thereby improving the quality of nursing care.

3.Construction of a knowledge base for hierarchical prevention care of neonatal hypoglycemia based on risk prediction model

Objective:To build the standardized knowledge base for hierarchical prevention care of neonatal hypoglycemia based on the risk prediction model of neonatal hypoglycemia, and to provide a decision-making basis for risk management to achieve predicitive neonatal hypoglycemia.Methods:Based on the best evidence summarized in strategies for the prevention and management of neonatal hypoglycemia published in 2020, evidence on the prevention and management of neonatal hypoglycemia was searched from BMJ Best Practice, UpToDate, Registered Nurses Association of Ontario, CNKI and other domestic and foreign databases and professional association websites. The retrieval period was from September 1, 2019 to August 31, 2022. The quality of newly included literature was evaluated, new evidence was extracted, and the best evidence in the prevention and management strategy of neonatal hypoglycemia published in 2020 was summarized and combined to form the first draft of the knowledge base. Experts in the field of neonatal nursing were invited to revise and discuss each item of the knowledge base, and the final draft of the knowledge base was formed. The final draft of the knowledge base was coded using the 2.5 version of the Clinical care classification system as the standardized language.Results:The risk prediction model of neonatal hypoglycemia was used as a grading tool, the final draft of the knowledge included 1 nursing diagnosis, 6 modules and 18 specific preventive nursing measures.Conclusions:The knowledge base for hierarchical prevention care of neonatal hypoglycemia based on risk prediction model can realize the prospective hierarchical nursing of neonatal hypoglycemia, which is scientific and practical, and is the basis to assist nurses to make clinical decisions.
4.Study on the status quo of neonatal hypoglycemia prevention cognition and management among neonatal and obstetric nurses in Jiangsu Province

Objective:To explore the status quo of neonatal hypoglycemia (NH) prevention cognition and management among neonatal and obstetric nurses in Jiangsu Province and provide a reference for NH prevention and nursing.Methods:In this cross-sectional study, between May and June 2020, convenience sampling and a self-designed questionnaire were used to investigate neonatal and obstetric nurses from 21 hospitals in 13 cities across Jiangsu Province. The questionnaire encompassed general information, nurses' cognition of NH prevention and care, and differences between nurses' cognition and implementation of departmental NH preventive management system. Totally 331 questionnaires were distributed and 318 were collected, accounting for an effective recovery rate of 91.1%.Results:Among the 318 neonatal and obstetric nurses, the NH diagnosis and intervention thresholds in the hospitals where the neonatal and obstetric nurses worked were 2.2-2.9 mmol/L. Most nurses considered blood glucose levels≤ 2.2 and≤ 2.6 mmol/L as diagnostic and intervention thresholds, accounting for 59.75% (190/318) and 58.18% (185/318) , respectively. Among the open-ended questions, tremor, feeding difficulties, and hypotonia were the most common clinical manifestations of NH mentioned by nurses, and more than 60% of nurese knew that gestational diabetes and macrosomia were high-risk factors for NH.Neonatology and obstetric nurses' approval rate for the department's NH management system was 72.96%-87.11%, and the implementation rate was 46.77%-84.48%. In all projects, the approval rate was higher than the implementation rate. In terms of establishing NH risk assessment, formulating NH screening population table, organizing preventive health education for NH, and regularly analyzing the causes of NH, there were statistically significant differences between nurses' cognition and implementation rate ( P<0.05) . Conclusions:There is a lack of uniform standards for NH management in hospitals in Jiangsu Province. Nurses' cognition of NH assessment, screening and monitoring is quite different, and the recognition rate of NH management systems is higher than the implementation rate. It is necessary to establish a standardized NH prevention and management system and strengthen the implementation of specific systems within the department.
5.The application of the cloning and expression of SMCY gender-specific fusion antigens with the corresponding polyclonal antibodies

Objective with the advantages of rapidity in detection protein, We selected the gender-specific amino acid sequence based on human SMCY and SMCX, cloned and expressed SMCY gender-specific fusion antigens. The rabbits were immunized with purified antigens to obtain the polyclonal antibodies. A new method was established for rapidly sex identification of forensic evidence samples by detecting SMCY antigens with the corresponding polyclonal antibodies. Methods We found three differential fragments by analyzing the sequence of human SMCY and SMCX. Then we cloned this three fragments and ligated as a new recombinant.This SMCY gender-specific fusion antigen gene was sub-cloned into pET-28a and expressed in Escherichia coli. The fusion antigen was purified by Ni-NTA column. The rabbits were immunized with purified antigen to produce the specific polyclonal antibodies.The reactivity of the polyclonal antibody was evaluated by ELISA and Western blotting. We developed a colloidal gold test strip for detecting the gender of human samples. Results We successfully selected gender-specific amino acid sequence, the SMCY gender-specific fusion antigen was expressed by prokaryotic expression and the polyclonal antibody was prepared by immunizing rabbit. The results of colloidal gold strip tests showed that there is a significant difference between male and female serums. Conclusion The results showed that the SMCY gender-specific fusion antigen could be recognized by the polyclonal antibody.The colloidal gold strip tests made by SMCY gender-specific fusion antigens and the corresponding polyclonal antibodies could be used for rapidly determining the gender of forensic evidence samples.
6.The complications comparison and detailed catheter management strategiesof PICC in the low birth weight infants

Objective To analyze the complications of PICC in the low birth weight infants (LBWI)and to explore the optimal catheter-indwelling approach so as to put forward the detailed management strategies on catheter.Methods The conditions 80 low birth weight infants with PICC were retrospectively analyzed.The stitch counts,tube plugging rate,phlebitis,catheter mis-placement and difficulties in injection pipelines of venipuncture in upper and lower limbs were com-pared.Results The vein catheterization complications in upper limbs were relatively lower than those in the lower limbs,but the catheter misplacement rate and tube feeding difficulty was more than lower limbs vein.The rate of phlebitis and tube plugged in lower limbs were higher than that in upper limbs.Conclusion Choosing the best puncture position,grasping the anatomical charac-teristics and skills of different vein and strengthening detailed managements can effectively reduce the complications of PICC,relieve pain and improve efficiency.

8.Determination of six lignin components in crude and different processed products of Schisandra chinensis.

OBJECTIVETo compare the contents of lignin in crude and processed fruits of Schisandrae chinensis, and to explore the processing mechanism of S. chinensis fruits.
METHODContents of schisandrin, schisandrol B, deoxyschisandrin, gomisin N, gamma-schizandrin and schisandrin C were determined by high performance liquid chromatography (HPLC).
RESULTExcept the content of Schisandrol B was higher or less in processed fruits than that in the crude, the other lignin contents of S. chinensis fruits in different processed products were higher than that in the crude. The alcohol-processed product had the highest content of lignin.
CONCLUSIONThe contents of lignin have changed by different processed methods, and alcohol-processed method may be the best processed method.

9.Development of the fingerprints for the quality evaluation of Viscum coloratum by high performance liquid chromatography

A high-performance liquid chromatography coupled ultraviolet (HPLC-UV) method was developed for a chemical fingerprint analysis of Viscum coloratura. Eighteen peaks were selected as the common peaks and Homoeriodictyol-7-O-β-D-apiosiyl-(1→2)-β-D-glucoside was used as a reference. The relative areas of common peaks were used for hierarchical clustering analysis and similarity calculation. Thirty-seven samples collected from different sources were classified into five groups. The similarities of 21 batches Viscum coloratura samples were beyond 0.90. The results obtained suggest that the chromatographic fingerprint can efficiently identify Viscum coloratum. Additionally, the fingerprints can then be used to evaluate the correlation between Viscum coloratura and hosts.
