1.The role of SENP-1 in chronic intermittent hypoxia induced myocardial injury in rats

Objective :
To investigate the effects and mechanism of small ubiquitin-like modifier ( SUMO) specific proteinase-1 (SENP-1) on chronic intermittent hypoxia ( CIH) induced myocardial injury in rats.
Methods :
32 male SD rats were randomly divided into : control group,CIH group,negative control adeno-associated virus interven- tion group (AAV-shNC) and SENP-1 shRNA adeno-associated virus intervention group (AAV-shSENP-1) ,with 8 rats in each group.After 6 weeks of CIH induction,echocardiography was performed.The levels of cardiac troponin I (cTNI) ,creatine kinase MB isoenzyme ( CKMB) ,myoglobin (Mb) ,lactate dehydrogenase (LDH) in serum and malondialdehyde (MDA) ,uperoxide dismutases ( SOD) ,glutathione ( GSH) ,interleukin( IL) -1 β , IL-6 and tumor necrosis factor-α(TNF-α) in myocardial tissue were detected by ELISA.The pathological changes of myocardial tis- sue was observed by HE staining.The reactive oxygen species ( ROS) level in myocardial tissue was detected by DCFH-DA fluorescence probe labeling.The small ubiquitin-like modifier (SUMO) level of hypoxia inducible factor- 1 α (HIF-1 α) protein in myocardial tissue was detected by kit.The mRNA and protein levels of SENP-1 and HIF- 1 α in myocardial tissue were detected by qRT-PCR and Western blot.
Results :
Compared with the control group, the pathological damage of myocardial tissue in CIH group was serious,the levels of left ventricular end diastolic diameter (LVEDD) ,left ventricular end systolic dimension (LVESD) and serum cTNI,CKMB,Mb and LDH signif- icantly increased (P<0. 05) ,and the levels of ROS,MDA,IL-1 β , IL-6,TNF-α and the mRNA and protein levels of SENP-1 and HIF-1α in myocardial tissue also significantly increased (P <0. 05 ) ,while the levels of LVEF, LVFS,serum GSH and SOD significantly decreased (P <0. 05) ,and the SUMOylates level of HIF-1α protein in myocardial tissue also significantly decreased (P <0. 05 ) .Compared with CIH group,AAV-shSENP-1 group had less myocardial pathological damage,the levels of LVEDD,LVESD and serum cTNI,CKMB,Mb and LDH signifi- cantly decreased (P<0. 05) ,and the levels of ROS,MDA,IL-1 β, IL-6,TNF-α and the mRNA and protein levels of SENP-1 and HIF-1α in myocardial tissue also significantly decreased (P<0. 05) ,the levels of LVEF,LVFS,serum GSH and SOD significantly increased (P<0. 05) ,and the SUMOylates level of HIF-1α protein in myocardial tissue also significantly decreased (P<0. 05) .
Conclusion
Inhibition of SENP-1 expression can alleviate CIH induced myocarditis and oxidative stress in rats,improve myocardial injury and cardiac dysfunction,and its mechanism may be related to the improvement of HIF-1α SUMOylates level,thus inhibiting HIF-1α expression.
2.NAT10 promotes cell proliferation by acetylating CEP170 mRNA to enhance translation efficiency in multiple myeloma.

Multiple myeloma (MM) is still an incurable hematologic malignancy, which is eagerly to the discovery of novel therapeutic targets and methods. N-acetyltransferase 10 (NAT10) is the first reported regulator of mRNA acetylation that is activated in many cancers. However, the function of NAT10 in MM remains unclear. We found significant upregulation of NAT10 in MM patients compared to normal plasma cells, which was also highly correlated with MM poor outcome. Further enforced NAT10 expression promoted MM growth in vitro and in vivo, while knockdown of NAT10 reversed those effects. The correlation analysis of acetylated RNA immunoprecipitation sequencing (acRIP-seq) and ribosome profiling sequencing (Ribo-seq) combined with RIP-PCR tests identified centrosomal protein 170 (CEP170) as an important downstream target of NAT10. Interfering CEP170 expression in NAT10-OE cells attenuated the acceleration of cellular growth caused by elevated NAT10. Moreover, CEP170 overexpression promoted cellular proliferation and chromosomal instability (CIN) in MM. Intriguingly, remodelin, a selective NAT10 inhibitor, suppressed MM cellular growth, induced cellular apoptosis in vitro and prolonged the survival of 5TMM3VT mice in vivo. Collectively, our data indicate that NAT10 acetylates CEP1 70 mRNA to enhance CEP170 translation efficiency, which suggests that NAT10 may serve as a promising therapeutic target in MM.
3. Role of histological evaluation of periprosthetic tissue in diagnosis of periprosthetic joint infection

Objective:
To evaluate the role of histologicalpathology in the diagnosis of periprosthetic joint infection.
Methods:
A total of 145 cases of joint arthroplasty during October 2017 and October 2018 from Beijing Jishuitan Hospital were collected. There were 23 cases of infection, including knee joint arthroplasty (12 cases) and hip arthroplasty (11 cases). There were 17 females and 6 males. Patients′ age ranged from 39 to 76 years (mean 63 years). The infection was diagnosed if there were >5 neutrophils per high power field in at least 5 high power field. The permanent sections were examined twice separately by two pathologists, and the interval time of histologic examination was at least two weeks. Sensitivity (SE), specificity (SP), positive predictivevalue (PPV), and negative predictive value (NPV) were calculated. The consistency evaluation of histologic examination of two pathologists was calculated by Kappa analysis.
Results:
The neutrophil cells could locate scattered or focally in the synovium tissue of periprosthetic joint infection. Somewhere, the infiltration of vessel and the perivascular distribution could also exist. Opportunity coincidence rate between two pathologists was 91.3% (Kappa=0.817). The results showed that SE was 60.9%, SP was 100.0%, NPV was 93.1%, PPV was 100.0%.
Conclusions
The presence of polymorphonuclear cells in histologic examination is correlated with infection. There was high consistency between histologic examination and clinical diagnosis of joint arthroplasty.
4. Clinicopathological and ALVAL score analysis of pseudotumor-like tissue around aseptic joint arthroplasty

Objective:
To analyze the clinicopathological features of pseudotumor-like tissue around aseptic joint arthroplasty and aseptic lymphocytic vasculitis-associated lesions (ALVAL) scores. The characters of wear granules were observed.
Methods:
Total 122 cases were retrieved from the surgical pathology files between May 2015 and August 2018 in the department of pathology in Beijing Jishuitan Hospital, which included the knee joint arthroplasty (10 cases) and hip arthroplasty (112 cases). There were 62 females and 60 males. Patients′ age ranged from 29 to 86 years (mean 56 years). The pseudotumor-like tissue around aseptic joint arthroplasty were stained with HE and analyzed by two ALVAL score systems. The characters of wear granules were observed by light microscope and polarized light.
Results:
The cohort included 62 females and 60 males. Patients′ age ranged from 29 to 86 years (mean 56 years). Compbell-ALVAL system includes synovial lining,inflammatory infiltrate and tissue organization. The scores were: low (0-4): 18cases; moderate (5-8): 101 cases; high (9-10): 3 cases. Oxford-ALVAL system only evaluated the inflammatory infiltrate,and the scores were:0 grade:56 cases; 1 grade:51 cases; 2 grade: 12 cases; 3 grade:3 cases. Cases with high score in the Compbell-ALVAL system were concordant with the 3 grade of the Oxford-ALVAL system. Under light microscope,the metal particles were small black granules; the polyethylene fibers were needle-like and easily visible in polarized light. The polymethylmethacrylate showed clear spaces because of particle melting.
Conclusions
The Compbell-ALVAL scoring system is based on the histologic analysis of pseudotumor-like tissue around aseptic joint arthroplasty, and the Oxford-ALVAL scoring systems is based on lymphocytic response. The wear particles could be differentiated by the features in the light microscope.

6.Exploring the Correlation between Left Coronary Bifurcation Angle and Coronary Plaques Forming in Left Anterior Descending by Dual-source CT Angiography

Objective To explore the potential relationship between left coronary bifurcation angle and plaques forming in left anterior descending artery by dual-source CT angiography.Methods One hundred patients who underwent dual-source CT angiography in our hospital from January to May during 2016 and had plaques in left anterior descending artery were retrospectively studied,while 100 patients with normal image were as negative controls.There were 57 male and 43 female at the mean age of (57.54 ±-8.38) years in negative controls,and 64 male and 36 female at the mean age of (62.08 ± 13.94) years in patients with left anterior descending artery plaques.The left coronary bifurcation angle was measured by multi-planar reconstruction (MPR) and maximum density projection (MIP) techniques.Results The left coronary bifurcation angle in negative control group were range from 41.25°to 112.14°,at the mean age of 69.45 ° ± 18.71 °,while range from 54.14 ° to 128.12 o,at the mean age of 85.65 °-± 15.96 ° in plaque group.The bifurcation angle in plaque group was larger than those in negative control group(P < 0.05).Conclusion Dual-source CTA could objectively evaluate left coronary bifurcation angle and plaque character.The left coronary bifurcation angle had an important influence on the formation of left anterior descending artery plaque.
7.The relationship between the muscles edema and skeleton involvement surrounding sacroiliac joint of axial seronegative spondyloarthropathy patients with functional magnetic resonance imaging

Objective To observe the muscles and skeleton involvement surrounding sacroiliac joint (SIJ) of axial seronegative spondyloarthropathy (SPA) patients with magnetic resonance imaging (MRI),and to analyze the relationship between them.Methods A prospective study of 38 patients who meet the 2009 axial SpA diagnostic criteria was conducted.We carried out MRI of the SIJ for these patients to evaluate the muscles and skeleton involvement.Those cases were divided into muscles differences between the two groups of image scores,including Spondyloarthritis Research Consortium of Canada (SPARCC) scores and Spondyloarthritis Research Consortium of Canada MRI Sacroiliac Joint Structural Score (SPARCC SSS).The extent of muscles edema in patients with sacroiliac joints was ranked into twelve grades from 0-12,and we did Spearman rank correlation test of muscles edema scope and two indexes.Results We found that 28 cases (73.68%) of the 38 patients had significant muscle involvement by analyzing the STIR sequence,and found erector spinae in 22 cases (57.89 %)gluteal muscles in 13 cases (34.21 %),iliacus muscle in 11 cases (28.95 %),obturator muscle in 5 cases (13.16%),piriform muscle in 5 cases (13.16%) and other 4 cases (10.53%).SPARCC (t =2.28,P =0.03) and SPARCC SSS (t =3.37,P =0.00) were statistically different between the two groups.SPARCC (P =0.00,r =0.67) and SPARCC SSS (P-0.01,r =0.47) were positively correlated with the extent of muscles edema.Conclusions The muscles edema around sacroiliac joint is an important sign of axial SpA magnetic resonance imaging.Patients who had muscles edema tended to have more serious skeleton changes.
8.Application of multifunctional infusion support in early ambulation of patients receiving radical resection for esophageal cancer

Objective To explore the effects of early ambulation and exercise with multifunctional infusion support on improving the gastrointestinal function of patients who received radical resection for esophageal cancer.Methods Totally 83 patients who received radical resection for esophageal cancer and admitted in the Department of Thoracic Surgery of Nantong Tumor Hospital from October 2015 to October 2016 were randomly and equally divided into an observation group and a control group. Patients in the observation group, on the basis of conventional surgical nursing, started early ambulation and exercise with multifunctional infusion support 24 hours after surgery, while patients in the control group received conventional nursing for esophageal cancer and started ambulation 2-3 days after surgery. Then the patients in the two groups were compared in time of anal exhaust and such gastrointestinal intolerance symptoms as abdominal distention, diarrhea, nausea, vomiting and regurgitation 72 hours after being treated with enteral nutrition.Results The patients in the observation group showed a lower incidence of abdominal distention, diarrhea, nausea, vomiting and regurgitation than the patients in the control group (P<0.05).Conclusions Early ambulation and exercise with multifunctional infusion support can help to improve the gastrointestinal function of patients who received radical resection for esophageal cancer.
9.Application of fecal rotavirus antigen rapid detection in diagnosis of children patients with diarrhea

Objective To observe the application effect of fecal human rotavirus (HRV) antigen rapid detection in the diagnosis of children patients with diarrhea .Methods The stool specimens in 200 children cases of diarrhea from January 2014 to December 2015 were collected for detecting HRV antigen by using the gold standard immunochromatographic method .Results Among 200 cases of diarrhea ,129 cases were stool HRV positive with the positive rate of 64 .5% .Among 129 cases of HRV infection ,the ma‐jority were the children patients aged 6 months to 2 years old .The tools in the children patients with HRV infection were egg flow‐er soup like .Conclusion The fecal HRV antigen rapid detection has an important clinical significance for the early diagnosis and early treatment of HRV diarrhea ,so which is worth being promoted in primary hospitals .
10.Vacuum Sealing Drainage Combined with Autologous Free Skin Graft in the Treatment of Multiple Calf Venous Refractory Ulcer

Objective:To observe the clinical effect of vacuum sealing drainage (VSD)combined with autologous free skin grafting in the treatment of multiple calf venous refractory ulcer.Methods:A total of 7 patients (16 lesions)with lower limb varicose vein complicated by multiple calf venous refractory ulcer,who underwent vascular surgery for treatment of the primary disease,received wound closure with VSD technology after debridement.And 7-10 days later,wound was repaired by VSD combined with autologous free skin graft based on patients'status.Results:The ulcers in 7 cases were all healed.Postoperative pressure bandaging was done with elastic bandage.There was no recurrence in six months'follow-up.Conclusions:For multiple calf venous chronic ulcer wound caused by varix of lower limb,the timely wound repair by VSD combined with autologous free skin graft and pressure bandaging after cure can improve the cure rate and decrease the recurrence rate.
