Objective To explore the effects of intravenous thrombolysis combined with Solitaire FR stent thrombectomy on vascular recanalization,neurologic function and prognosis in patients with acute cerebral infarction(ACI)due to large artery occlusion(LAO).Methods A total of 172 patients with ACI-LAO treated in our department between October 2020 and March 2023 were retrospectively enrolled.According to treatment regimens,they were assigned into control group(86 cases,alteplase intravenous thrombolysis)and study group(86 cases,alteplase intravenous thrombolysis combined with Solitaire FR stent thrombectomy).Vascular recanalization,neurolog-ic function,cerebral perfusion and occurrence of adverse events were compared between the two groups.After 90 d of follow-up,their prognosis was evaluated with modified Rankin scale.Results There was no significant difference in success rate of vascular recanalization between the two group(P＞0.05),but complete recanalization rate was statistically higher in the study group than the control group(68.60％vs 50.00％,P＜0.05).The study group had obviously lower NHISS scores at 7 and 14 d after treatment,higher cerebral blood volume and cerebral blood flow,but shorter mean transit time when compared with the control group(P＜0.05,P＜0.01).No notable difference was observed in the total incidence of adverse events between them(P＞0.05).After 90 d of follow-up,the proportion of good prognosis was higher in the study group than the control group(80.23％vs 63.95％,P＜0.05).Conclusion Intravenous thrombolysis combined with Soli-taire FR stent thrombectomy shows better efficacy in ACI-LAO patients,with better vascular re-canalization and great improvements in neurologic function and prognosis.

Objective:To explore the effects of hyperbaric oxygen combined with task-oriented mirror therapy on the upper limb motor function (ULMF) recovery and motor evoked potentials (MEPs) in patients with cerebral infarction.Methods:A total of 80 convalescent patients with cerebral infarction admitted to Nanjing Lishui People’s Hospital from January 2017 to June 2020 were selected and divided into control group and observation group according to the random number table method, with 40 cases in each group. The control group was given task-oriented mirror therapy, and the observation group was treated with hyperbaric oxygen therapy on the basis of the treatment in the control group. The ULFM, the cortex latency (CL) of MEPs, the central motor conduction time (CMCT), and patients’ daily functioning before and after treatment were compared between the two groups.Results:Before treatment, there was no significant difference in ULMF score, CL of MEPs, CMCT, and daily functioning between the two groups ( P>0.05). After treatment, the ULMF score in the observation group (41.51±4.28) was higher than that in the control group (34.87±3.44), with a statistically significant difference ( P<0.05); the CL and CMCT in the observation group [(21.89±0.97) ms and (9.24±0.70) ms, respectively] were all shorter than those in the control group [(22.91±0.92) ms and (9.96±0.83) ms, respectively], with statistically significant differences ( P<0.05); the daily functioning score in observation group (65.42±7.90) was higher than that in the control group (58.79±8.32), with a statistically significant difference ( P<0.05). Conclusion:Hyperbaric oxygen combined with task-oriented mirror therapy can improve the central nerve conduction of the convalescent patients with cerebral infarction, thus promoting their recovery of ULMF and daily functioning.

Objective To investigate the prevalence and prognostic value of hyponatremia on admission in hospitalized patients for heart failure (HF) from the first HF management center in China.Methods Consecutive adult (age of 18 years or older) symptomatic HF patients (NYHA functional class Ⅱ-Ⅳ) admitted between March 2009 and March 2012 in our center were included in the present analysis.Hyponatremia was defined as a serum sodium level ＜ 135 mmol/L.Association between hyponatremia on admission and in-hospital mortality as well as all-cause death and heart failure death during 1-year follow-up after discharge was analyzed.Results A total of 1 048 hospitalized patients for HF with complete baseline data were enrolled and the prevalence of hyponatremia on admission was 9.2％ (96/1 048).Blood pressure was significantly lower while NYHA functional class and N-terminal pro-B type natriuretic peptide levels were significantly higher in hyponatremic patients than non-hyponatremic patients (all P ＜ 0.05).Kaplan-Meier survival analysis showed that patients with hyponatremia on admission had significant higher in-hospital mortality (P ＜ 0.01), all-cause death rate (P ＜ 0.01) and HF death rate (P ＜ 0.01) during 1-year follow-up post discharge compared with non-hyponatremic patients with.Multiple Cox proportional hazard analysis showed that hyponatremia on admission remained as independent predictor for all-cause death (hazard risk (HR) =2.105, 95％ confidence interval (CI) 1.460-3.036, P ＜ 0.01) and HF death (HR =2.458, 95％CI: 1.704-3.545, P ＜0.01) after adjustment for other covariates.Conclusion Hyponatremia is relatively common in patients hospitalized with HF in China and hyponatremia on admission is associated with higher in-hospital mortality and all-cause death and HF death one year after discharge.
