1.Research progress of airway epithelial cell senescence mechanism in chronic obstructive pulmonary disease

As the first defense of respiratory system,airway epi-thelial cells(AECs)play an important role in separating the re-spiratory internal and external environment.They are essential for the natural immune function.Small airway lesions are an im-portant early pathology of chronic obstructive pulmonary disease(COPD),when AECs are exposed to harmful particles or gases for a long time,the epithelial barrier is damaged,and the signa-ling pathways which involved in differentiation,repair,and in-flammatory are disordered,resulting in epithelial cell cycle stag-nation and accelerated aging.A number of studies have sugges-ted that AECs of COPD patients express high levels of aging markers,suggesting that senescence of AECs is closely related to COPD.This review discusses the potential mechanisms of AECs senescence in COPD,the impact of AECs senescence on the de-velopment and severity of the disease,and highlights potential targets for modulating cellular senescence in airway epithelium as a therapeutic approach in COPD.
2.Efficacy and safety of eculizumab on acute attacks in patients with aquaporin-4 antibody positive neuromyelitis optica spectrum disorder

Objective:To evaluate the efficacy and safety of eculizumab in treating acute attacks of aquaporin-4 antibody(AQP4-IgG)positive neuromyelitis optica spectrum disorder(NMOSD).Methods:Six patients with AQP4-IgG-positive NMOSD treated with eculizumab at the Affiliated Hospital of Qingdao University from August 2023 to April 2024 were included.The patients' clinical characteristics, changes in scores of neurological function, pain and spasm before and after eculizumab treatment, and adverse drug reactions during the treatment period were described, and the data were subjected to Friedman test using SPSS 25.0.Results:The median score of expanded disability status scale in the six patients decreased from 3.25 before treatment to 1.25 after treatment, and the difference was statistically significant ( F=44.77, P<0.01).The functional status score showed noticeable improvement as early as week 1, demonstrating greater sensitivity to treatment response.Three patients experienced moderate to severe pain before treatment, with a mean pain score of 6, which reduced to 4.67 and 1.33 at week 2 and 6, respectively( F=29.17, P<0.05).Two patients reported spasms before treatment, with a mean score of 3.00, which decreased to 2.00, 1.50, and 1.00 at week 2, 3, and 6 after treatment, respectively( F=18.00, P>0.05).No serious adverse reactions were reported during the treatment. Conclusion:Eculizumab can effectively improve the neurological function and alleviate pain in patients with AQP4-IgG-positive NMOSD during acute attacks, and demonstrate good safety.
3.Effects of key molecules in m6A methylation modification on the replication and proliferation of Japanese encephalitis virus

This study was aimed at investigating the effects of demethylase fat mass and obesity-associated protein(FTO)and methyltransferase methyltransferase like protein 3(METTL3),key molecules in N6-methyladenosine(m6A)modification,on the replication and proliferation of Japanese encephalitis virus(JEV).Recombinant lentiviruses were generated by packaging the FTO and green fluorescent protein into lentiviral vectors.Neuro2a cells,a mouse neuroblastoma cell line,were infected with the lentivirus,and stable FTO-expressing cell lines were obtained through puromycin selection.Successful overexpression of FTO was confirmed through fluorescence microscopy,real-time quantitative PCR,and western blot analysis.When Neuro2a cells overexpressing FTO were infected with JEV,the overexpression of FTO decreased JEV replication in the cells,and increased the expression of interferon(IFN)and related molecules.Additionally,treatment of JEV-infected Neuro2a cells with the METTL3-specific inhibitor STM2457 resulted in a dose-dependent decrease in JEV replication and viral protein expression.These findings suggested that lowering m6A methylation levels inhibits JEV replication,thus shedding light on the regulatory role of methylation modification in JEV replication.
4.Effect of pinocembrin on the malignant biological behavior of gastric can-cer cells by regulating the RhoA/ROCK signaling pathway

Objective:Exploring the effect of Pinocembrin(Pino)regulating the Ras homolog gene family member A/Rho associated with curly helix binding protein kinase(RhoA/ROCK)signaling pathway of Ras homologous gene family members on the malignant biological behavior of gastric cancer cells.Methods:Cultivate human gastric cancer cells MGC803 with different concentrations of Pino(0~240μmol/L),detect cell survival rate using CCK-8 method,and screen for the optimal drug concentration.MGC803 cells were rseparated into MGC803 group(Control group),Pino-L group,Pino-M group,Pino-H group,and Pino-H+RhoA agonist CN03 group.The clone formation experiment was applied to detect the number of clones formed of cells in each group.Assessment of cell apoptosis using flow cytometry.Tran-swell invasion and migration experiments were used to detect the number of cells undergoing migration and invasion in each group;Detection of RhoA/ROCK signaling pathway and expression of epithelial mesenchymal transition related proteins in MGC803 cells using Western blot method.Results:Compared with the MGC803 group,the cell survival rate,clone formation number,migration cell number,and invasion cell number were all reduced in the Pino-L group,Pino-M group,and Pino-H group,and RhoA was also present in the cells,ROCK2,The expression levels of vimentin and N-cadherin gradually decreased(P<0.05),while the apoptosis rate and E-cadherin expression level gradually in-creased(P<0.05).The Pino-H+CN03 group reversed the trend of changes in the above indicators).Conclusion:Pino can prevent malignant biological behavior of gastric cancer cells,which may be related to the inhibition of the RhoA/ROCK signaling pathway.
5.Research status of action of alkaloids in the treatment of rheumatoid arthritis

Rheumatoid arthritis(RA)is an inflammatory disease mainly associated with polyarthritis.It is mainly due to the joint swelling and joint inflammation caused by immune cells.The alkaloids in traditional Chinese medicine can reduce the level of pro-inflammatory factors and increase the level of anti-inflammatory factors,reduce the joint synovial tissue hyperplasia,inflammatory cell infiltration and new blood vessel formation,inhibit the differentiation of osteoclasts,and then play an anti-RA role.Therefore,the review used the effects and mechanisms of alkaloids for RA to inform the development of new drugs for RA.
6.Pregnancy outcomes and prognosis after in vitro fertilization with fertility-sparing surgery for patients with borderline ovarian tumor

Objective To explore the pregnancy outcomes and prognosis following in vitro fertilization,and tumor prognosis of infertility patients with borderline ovarian tumor(BOT)after fertility-sparing surgery(FSS).Methods A retrospective study was conducted and 35 infertility patients with BOT after FSS from Shanghai Jiai Genetics and IVF Institute,Obstetrics and Gynecology Hospital,Fudan University were enrolled.The pregnancy outcomes and tumor prognosis were analyzed.Results Among the patients,serous BOT accounted for 62.9%.Twenty-five cases were International Federation of Gynecology and Obstetrics(FIGO)stage Ⅰ,2 stage Ⅱ,and 8 stage Ⅲ.Twelve patients experienced borderline tumor recurrence before assisted reproductive therapy,of which 5 patients were stage Ⅲ and 7 stage Ⅰ.After FSS,the 35 patients firstly underwent controlled ovarian stimulation(COS)at an average interval of(47.3±32.6)months.A total of 79 ovulation induction cycles were taken and mild stimulation protocol was adopted first(41.8%),followed by the natural cycle protocol(25.3%).Twenty-seven patients underwent embryo transfer in 37 cycles,resulting in 21 clinical pregnancies in 19 patients.The clinical pregnancy rate was 56.8%,with a cumulative pregnancy rate of 54.3%.The live birth rate was 43.2%,and the cumulative live birth rate was 42.9%.For patients with anti-Müllerian hormone levels<1.0 ng/mL(n=13),the pregnancy rate was 42.9%,and the cumulative clinical pregnancy rate was 23.1%.Among 30 patients,4(13.3%)patient with stage Ⅲ BOT experienced recurrence,and none had malignant transformation at the median follow-up period of 61 months after the first COS.Conclusion Assisted reproductive technology was safe and could improve pregnancy rates in infertile women with BOT.

10.Effects of stepped exercise rehabilitation training on cardiopulmonary function and quality of life in patients with ischemic heart disease

Objective:To explore the effects of stepped exercise rehabilitation training on cardiopulmonary function and quality of life in patients with ischemic heart disease.Methods:This randomized controlled study enrolled 110 patients with ischemic heart disease admitted to Jiangsu Province Hospital between January 2021 and December 2023.Patients were randomly divided into control group(n=53,routine intervention program)and intervention group(n=52,additional stepped exercise rehabilitation training).After 3-month intervention,cardiopulmonary function[stroke volume(SV),cardiac index(CI),forced expiratory volume in the first second(FEV1),maximum voluntary ventilation(MVV)],quality of life[China questionnaire of quality of life in patients with cardiovascular diseases(CQQC)score],exercise endurance[exercise duration(ED),peak oxygen intake(VO2peak),anaerobic threshold(AT)]and incidence of major adverse cardiovascular events(MACE)were compared between the two groups.Results:We included 53 patients[28 males(52.83%),age 40~69(56.25±7.76)years old]in control group compared to 52 patients[27 males(51.92%),age 40~71(55.25±2.40)years old]in the intervention group after 3-month intervention.Com-pared to patients in the control group,those in the intervention group had significantly higher SV[(65.46±3.58)ml vs.(61.69±3.78)ml],CI[(2.82±0.12)L·min-1·m-2 vs.(2.54±0.09)L·min-1·m-2],FEV1[(2.50±0.06)L vs.(2.31±0.06)L],MVV[(79.66±0.82)L/min vs.(77.16±1.09)L/min],CQQC score[(65.23±2.84)points vs.(47.98±3.25)points],ED[(382.62±31.19)s vs.(353.37±36.32)s],VQ2peak[(20.05±2.43)ml·kg-1·min-2 vs.(16.86±1.61)ml·kg-1·min-2]and AT[(13.34±0.83)ml·kg-1·min-2 vs.(11.46±0.89)ml·kg-1·min-2](P<0.001 all),and significantly lower total incidence of MACE(11.54%vs.32.08%,P=0.011).Conclu-sion:Stepped exercise rehabilitation training may improve cardiopulmonary function,quality of life,and exercise endurance in patients with ischemic heart disease during the rehabilitation.
