1.Research progress on traditional Chinese medicine in the treatment of sepsis by regulating helper T cell differen-tiation

epsis is a systemic inflammatory response syndrome triggered by infection, and its high mortality rate is closely associated with immune imbalance, particularly the imbalance in the differentiation of helper T cell (Th) cell subsets [Th1, Th2, Th17, regulatory T cell (Treg) ] . In recent years, traditional Chinese medicine (TCM), with its characteristics of multi-component and multi-target actions, has demonstrated unique advantages in regulating Th cell differentiation and function, as well as correcting immune imbalances in sepsis, offering new perspectives for immunotherapy of sepsis. This review summarizes relevant studies on the regulation of Th cell differentiation for sepsis treatment by TCM monomers and active ingredients (such as Astragalus membranaceus , Scutellaria baicalensis , Coptis chinensis , Rheum palmatum , Ganoderma lucidum , Ginkgo biloba , and Cistanche deserticola ), the alcohol extract of Dai Baijie, and TCM formulas and preparations categorized as blood-activating and stasis-removing, purgative and laxative, warming and tonifying yang, and tonifying qi and nourishing yin. The results indicate that these TCM monomers, active ingredients, extracts, formulas, and preparations can regulate the Th1/Th2 and Th17/Treg balance, target the differentiation balance of Th cell subsets, alleviate inflammatory responses, or improve immune suppression, thereby exerting therapeutic effects on sepsis.
2.Observation of the positive surgical margins after radical resection of pT3 prostate cancer using on-demand intermittent total androgen blockade therapy

Objective To explore the efficacy of the on-demand intermittent total androgen blockade regimen in treating cases with positive surgical margins after radical prostatectomy for pT3 stage prostate cancer. Methods A retrospective analysis was conducted on the clinical data of 21 patients with pT3 stage prostate cancer who underwent radical prostatectomy and had positive surgical margins confirmed by pathology in our hospital during Jan. 2013 and Dec. 2020. Prostate-specific antigen(PSA)was used as the indicator of observation. For those with biochemical recurrence(PSA>0.2 μg/L), at least 2 months of androgen deprivation therapy(ADT)combined with the maximum androgen receptor antagonist was administered for maximal androgen blockade endocrine therapy. If PSA still exceeded 0.1 μg/L after 2 months of maximal androgen blockade treatment, the maximal androgen blockade was maintained and monthly follow-up was conducted until PSA reached the discontinuation criteria. During the discontinuation period, the serum PSA and testosterone levels were checked every 4 to 6 weeks. The time of application of maximal androgen blockade and the subsequent interval of discontinuation were defined as one treatment cycle. Results Postoperative pathology confirmed that among the 21 patients, 17 were pT3a and 4 were pT3b. The PSA level 1 month after surgery was(0.23±0.45)μg/L. The follow-up lasted for 48 to 126 months, with an average of(68.86 ± 20.43)months. The interval between the first application of maximal androgen blockade and radical prostatectomy was 2 to 35 months, with an average of(12.48±8.83)months. One patient died of post-cerebral infarction complications 74 months after surgery, another progressed to hormone-resistant prostate cancer(CRPC)70 months after surgery and the treatment plan was immediately changed. Up to 90.5%(19/21)of the patients completed 5 to 12 cycles of combined endocrine therapy, with no tumor progression. Conclusion Based on the postoperative PSA level monitored, intermittent administration of the maximum androgen blockade endocrine therapy can effectively control the cancer progression of patients with positive surgical margins after radical prostatectomy, which can also reduce the adverse reactions.
3.A retrospective report on chiggers at a focus of scrub typhus on the China-Vietnam border in Yunnan of southwest China

Objective:
The present study made a retrospective analysis aiming to report chiggers on small mammals (hosts) in Jinping County on the China-Vietnam border, Yunnan of southwest China.
Methods:
The indexes of chigger infestation and chigger community were routinely calculated. The niche width and overlap were calculated for analyzing chiggers’ host selection. The sample completeness was determined through coverage index (CI).
Results:
A total of 7,718 chiggers from 274 small mammal hosts were identified as 52 species, 10 genera, three subfamilies and two families, including nine vector species. The CI of chigger species reached 99.84%. The genus Leptotrombidium accounted for 94.86% of 10 chigger genera. The majority of chiggers (constituent ratio [C r ] = 95.35%) came from rodents (Rodentia). The four dominant chigger species (C r = 89.88%, 6,937/7,718) included Leptotrombidium deliense, the most important vector of scrub typhus in China.
Conclusions
and Relevance: The abundant L. deliense in Jinping, together with other vector chigger species and their low host specificity, increases the potential transmission risk of scrub typhus in China-Vietnam border areas.
4.Zhenzhu Tiaozhi Capsules Reduce Renal Lipid Deposition and Inflammation in Mouse Model of Diabetic Kidney Disease via SCAP-SREBP-1c/NLRP3 Signaling Pathway

ObjectiveTo investigate the protective effects and mechanisms of Zhenzhu Tiaozhi capsules on the kidneys in the mouse model of diabetic kidney disease. MethodsThirty male C57BL/6J mice were selected as experimental objects. The model of diabetic kidney disease was induced by intraperitoneal injection of streptozotocin (STZ) at 40 mg·kg-1 for 5 days combined with a high-fat diet (HFD). Fasting blood glucose (FBG) ≥ 11.1 mmol·L-1, increased urine volume, and continuous appearance of proteinuria indicated successful modeling. Mice were grouped as follows: Blank, model, low- and high-dose (0.98 and 1.96 g·kg-1, respectively) Zhenzhu Tiaozhi capsules, and losartan potassium (30 mg·kg-1), with six mice in each group. After 12 weeks of continuous gavage, urine and kidney specimens were collected, and the 24-h urinary protein and the urinary albumin-to-creatinine ratio (UACR) in mice were measured. Hematoxylin-eosin (HE) staining, periodic acid-Schiff (PAS) staining, and Masson staining were performed for observation of histopathological changes in kidneys. Immunofluorescence assay was employed to detect the positive expression of the podocyte marker protein nephrin. Oil red O staining was used to detect renal lipid deposition. Enzyme linked immunosorbent assay was employed to measure the levels of interleukin-1β (IL-1β), interleukin-6 (IL-6), and tumor necrosis factor-α (TNF-α) in the renal tissue. Western blot was employed to determine the expression levels of sterol regulatory element-binding protein cleavage-activating protein (SCAP), sterol regulatory element-binding protein-1c (SREBP-1c), and NOD-like receptor protein 3 (NLRP3) in the renal tissue. ResultsCompared with the blank group, the model group showed increases in 24-h urinary protein and UACR (P<0.05), glomeruli exhibiting capsule adhesion, collagen fiber deposition, mesangial proliferation, and inflammatory cell infiltration, elevated levels of IL-1β, IL-6, and TNF-α (P<0.05), reduced positive expression of nephrin (P<0.05), increased lipid deposition (P<0.05), and up-regulated expression of SCAP, SREBP-1c, and NLRP3 (P<0.05) in the renal tissue. Compared with the model group, the treatment with losartan potassium or high-dose Zhenzhu Tiaozhi capsules for 12 weeks decreased 24-h urinary protein and UACR (P<0.05), and the treatment with low-dose Zhenzhu Tiaozhi capsules for 12 weeks reduced the 24-h urinary protein (P<0.05). Pathological staining results revealed that kidney damage in mice from all treatment groups was alleviated, with reduced inflammatory infiltration, collagen fiber deposition, and mesangial proliferation, and increased positive expression of nephrin in the renal tissue (P<0.05). In addition, all the treatment groups showed reduced lipid droplets (P<0.05), lowered levels of IL-1β, IL-6, and TNF-α (P<0.05), and down-regulated expression of SCAP, SREBP-1c, and NLRP3 (P<0.05) in the renal tissue. ConclusionZhenzhu Tiaozhi capsules can ameliorate kidney damage in the mouse model of diabetic kidney disease by inhibiting the activation of the SCAP-SREBP-1c/NLRP3 signaling pathway, which reduces renal lipid deposition and inflammation.

Forensic medicine has been designated as a first-level discipline,presenting new opportunities and challenges for the development of forensic medicine.Since the 1980s,the establishment of foren-sic medicine discipline and the cultivation of high-level forensic talents have become hot topics in the development of forensic medicine in China.Since the 13th Five-Year Plan,the forensic team of Shanxi Medical University has been aiming at the forefront,proposing the development goals of"Five First-class"and the discipline development path"Six Major Achievements".It has selected benchmark disci-plines,identified gaps in disciplinary development,unified thoughts,formulated completion timelines,concentrated superior resources,assigned tasks to individuals,and created an"Organized Fill-in-the-Blank Format"forensic medicine discipline construction model with the characteristics of the new era.The construction model of forensic medicine has achieved good results in the goals,discipline frame-work,scientific research,talent cultivation,discipline team and platform construction,forming a rela-tively complete discipline construction and management system,and accumulating valuable experience for the construction of first-level discipline and high-level talent cultivation of forensic medicine.

Insulin is an important protein hormone that participates in multiple metabolic pathways.Biosynthetic insulin has been widely used in the treatment of type 1 and type 2 diabetes.Currently,the number of reported cases of insulin overdose both at home and abroad is gradually increasing,and insulin homicide is no longer a means of"committing murder without leaving a trace".At present,there are no systematic protocols for the identification of insulin overdose in the field of forensic medi-cine in China.This article introduces the causes,toxicological characteristics,forensic examination,labo-ratory testing methods and indicator reference of insulin overdose.Based on the identification practice and research results and referring to relevant studies on insulin overdose at home and abroad,this pa-per aims to provide recommendations and references for the formulation of forensic identification guide-lines for insulin overdose cases.
7.Effects of perineal massage combined with Kegel exercises on preventing perineal injury: a Meta-analysis

Objective:To evaluate the effect of perineal massage during pregnancy, when combined with Kegel exercises, on preventing perineal injury, and to provide a basis for offering safer and more effective delivery intervention measures for expectant mothers.Methods:Randomized controlled trials on perineal massage combined with Kegel exercises for preventing perineal injury were retrieved from databases such as CNKI, Wanfang database, VIP database, CBM database, PubMed, Embase, Web of Science, and Cochrane Library, from their inception to December 28, 2024. Meta-analysis was conducted using RevMan 5.3 software.Results:A total of 15 articles were included, involving 2 393 research subjects. The results of the Meta-analysis showed that there were statistically significant differences in the first stage of labor ( MD = - 1.63, 95% CI - 1.93 - - 1.30, P<0.001), the second stage of labor ( MD = - 32.58, 95% CI - 42.26 - - 22.90, P<0.01), and the third stage of labor time ( MD = - 3.41, 95% CI - 9.91 - - 2.91, P<0.001); the degree of perineal laceration Ⅱ ( OR = 0.40, 95% CI 0.28 - 0.58, P<0.001), Ⅲ( OR = 0.38, 95% CI 0.24 - 0.60, P<0.001), and Ⅳ( OR = 0.13, 95% CI 0.03 - 0.51, P = 0.003); the rate of perineal incision ( OR = 0.15, 95% CI 0.07 - 0.34, P<0.001), the rate of perineal laceration ( OR = 0.34, 95% CI 0.25 - 0.46, P<0.001), the rate of intact perineum ( OR = 6.30, 95% CI 3.20 - 12.40, P<0.001), and the amount of perineal bleeding ( MD = - 29.72, 95% CI - 43.51 - - 15.93, P<0.001), the difference was statistically significant. Conclusions:Prenatal perineal massage combined with Kegel exercises is a safe, effective and easily implementable intervention method. It is highly effective in preventing perineal injuries and shortening the duration of labor. It not only reduces the risk of postpartum complications but also helps to optimize the delivery experience. Therefore, this combined intervention method can be regarded as one of the important measures for preventing perineal injuries and should be widely promoted in clinical practice.
8.Mechanism of Lizhong decoction in treating cold-damp diarrhea through network pharmacology,molecular docking and animal experiments

Aim To explore the key components and mechanisms of Lizhong decoction in treating rats with cold-damp diarrhea based on network pharmacology,molecular docking technology and animal experiments.Methods By literature review and database collec-tion,the components of Lizhong decoction,therapeutic targets,and the mapping with diarrhea disease targets were conducted to construct an intersection target pro-tein-protein interaction network for screening core tar-gets,and GO and KEGG pathway enrichment analysis was performed to build an"active component-target-pathway"network,followed by molecular docking vali-dation.Forty-eight rats were randomly divided into the normal control group(K),model group(DG),Lizhong decoction group(LZDG),and Pulsatilla decoction group(BTDG).Subsequently,a rat cold-damp diar-rhea model was established using Senna combined with low-temperature high-humidity environment,and the rats were intervened with Lizhong decoction and Pul-satilla decoction.HE staining was used to detect path-ological changes in intestinal tissue,ELISA was em-ployed to measure the levels of peripheral blood IL-6,IL-10,IL-1 β,and TNF-α,and western blot was used to determine the expression of colon tight junction pro-teins.Results Network pharmacology initially identi-fied 125 compounds in Lizhong decoction,5 186 drug target components,438 disease targets,and 60"drug-disease"shared targets.GO and KEGG enrichment a-nalysis showed that signaling pathways such as IL-17 and TNF were highly enriched.Molecular docking in-dicated that the core components of the drug had good binding activity with corresponding key targets.Liz-hong decoction could effectively improve the clinical symptoms of rats with cold-damp diarrhea,and com-pared with the DG group,the diarrhea rate,diarrhea in-dex,and other related indicators also gradually de-creased to normal levels.Compared with the DG group,the LZDG group showed reduced inflammation levels and a recovery in energy metabolism levels.Conclusion It can regulate targets such as MMP9 and IL-17 signaling pathways through multi-components like Calycosin and formononetin to exert its therapeutic effect on cold-damp diarrhea.
9.Mechanism of improving oxidative stress in diabetic kidney disease by regulating NOX family through ultrafiltration membrane extract of Angelica sinensis and Radix Hedysari

Aim To investigate the mechanisms of the ultrafiltration membrane extract of Angelica sinensis and Radix Hedysari extracts on oxidative stress in rats with diabetic kidney disease(DKD).Methods Forty-five SD rats were randomly divided into a control group(n=8)a model group(n=37).Rats in the model group were fed a high-sugar,high-fat diet for four weeks,followed by intraperitoneal injection of strepto-zotocin at a dose of 30 mg·kg-1 to induce diabetes in the rats.Three weeks later,rats with 24-hour urinary protein(24-hUP)levels more than or equal to 30 mg were injected via the tail vein with 0.05 mg·kg-1 of 10%high molecular weight dextran for three times to induce a model of blood stasis in diabetic kidney dis-ease(DKD).The rats were then evaluated for random blood glucose(GLU)levels,24-hUP,biochemical markers,histopathological staining,and the protein expression of nicotinamide adenine dinucleotide phos-phate(NADPH)oxidase(NOX)1,NOX2,NOX3,NOX4,and NOX5 in renal tissues using immunoblot a-nalysis.Results Compared to the control group,rats in the model group showed significantly increased GLU,24-hUP,SCr,BUN,TG,TC,bu markedly de-creased ALB2,HDL,LDL levels,and the relative ex-pression of NOX1,NOX2,NOX4,NOX5 proteins in-creased markedly(P<0.01);Comparison with the model group,rats in the treatment group exhibited sig-nificantly decreased GLU,24-hUP,SCr,BUN,TG,TC at 6 weeks and 8 weeks,but markedly increased ALB2,HDL,LDL levels,and the relative expression of NOX1,NOX2,NOX4,NOX5 proteins decreased significantly(P<0.05).Conclusions The ultrafil-tration membrane extract of Angelica sinensis and Radix Hedysari can effectively ameliorate oxidative stress and renal function in DKD rats,which may be associated with targets within the NOX family.
10.Curative Efficacy Analysis of Allogeneic Hematopoietic Stem Cell Transplantation for Acute Myeloid Leukemia with ASXL1 Mutation.

OBJECTIVE:
To explore the efficacy and apoptosis of allogeneic hematopoietic stem cell transplantation (allo-HSCT) in the treatment of acute myeloid leukemia (AML) with ASXL1 mutation.
METHODS:
The clinical data of 80 AML patients with ASXL1 mutation treated in our hospital from January 2019 to December 2021 were retrospectively analyzed. The clinical characteristics of the patients were summarized, and the therapeutic effect and prognostic factors of allo-HSCT for the patients were analyzed.
RESULTS:
Among the 80 patients, 38 were males and 42 were females, and the median age was 39(14-65) years. There were 17 patients in low-risk group, 25 patients in medium-risk group and 38 patients in high-risk group. ASXL1 mutation co-occurred with many other gene mutations, and the frequent mutated genes were TET2 (71.25%), NRAS (18.75%), DNMT3A (16.25%), NPM1 (15.00%), CEBPA (13.75%). Among medium and high-risk patients, 29 underwent allo-HSCT, while 34 received chemotherapy. The 2-year overall survival (OS) rate and disease-free survival (DFS) rate of the allo-HSCT group were 72.4% and 70.2%, while those of the chemotherapy group were 44.1% and 34.0%, respectively. The statistical analysis showed significant differences between the two groups (both P < 0.01). Multivariate analysis showed that age at transplantation >50- years and occurrence of acute graft-versus-host disease after transplantation were poor prognostic factors for OS and DFS in transplantation patients.
CONCLUSION
Allo-HSCT can improve the prognosis of AML patients with ASXL1 mutation.
