ObjectiveTo clarify the anti-inflammatory and lung-protective effects of Yantiao prescription on lipopolysaccharide (LPS)-induced acute lung injury (ALI), and to explore the impact of Yantiao prescription on the metabolic pathways of arachidonic acid (AA) in vivo. MethodsThirty male C57BL/6J mice were randomly divided into the following groups based on body weight: normal group, model group, dexamethasone group (2 mg·kg-1), low-dose Yantiao prescription group (18 g·kg-1), and high-dose Yantiao prescription group (36 g·kg-1), with 6 mice in each group. The ALI mouse model was established by intraperitoneal injection of LPS. The treatment groups received oral gavage once a day for 7 consecutive days, and serum and lung tissue were collected at the end of the experiment. The content of pro-inflammatory cytokines tumor necrosis factor-α (TNF-α), interleukin-1β (IL-1β), and interleukin-6 (IL-6) in serum was detected by enzyme-linked immunosorbent assay (ELISA). Hematoxylin-eosin (HE) staining was used to assess lung tissue pathology. The wet/dry weight ratio (W/D) and myeloperoxidase (MPO) activity in lung tissue were measured. The content of AA metabolites in serum and lung tissue was measured by liquid chromatography triple quadrupole-mass spectrometry (LC-MS/MS). ResultsCompared with the conditions in the normal group, the content of serum pro-inflammatory cytokines TNF-α, IL-1β, and IL-6 in the model group was significantly increased (P<0.01). The alveolar structure in mice was severely damaged, with markedly thickened alveolar walls and extensive inflammatory cell infiltration. The W/D ratio and MPO activity in lung tissue were significantly increased (P<0.01). The content of AA metabolites, including prostaglandin D2 (PGD2), prostaglandin E2 (PGE2), 11(S)-hydroxy-eicosatetraenoic acid [11(S)-HETE], and 5-hydroxy-eicosatetraenoic acid (5-HETE) in serum and lung tissue was significantly increased (P<0.05), while the content of 11,12-epoxyeicosatrienoic acid (11,12-EET) and 14,15-epoxyeicosatrienoic acid (14,15-EET) in serum was significantly decreased (P<0.01). Compared with the results in the model group, the content of serum pro-inflammatory cytokines TNF-α, IL-1β, and IL-6 in the dexamethasone group, low-dose Yantiao prescription group, and high-dose Yantiao prescription group was significantly reduced (P<0.05). Mild thickening of alveolar walls, scattered inflammatory cell infiltration, and relatively intact tissue structure with improved alveolar architecture were observed. The W/D ratio and MPO activity in lung tissue were significantly reduced (P<0.01). The content of AA metabolites PGD2, PGE2, 11(S)-HETE, and 5-HETE in serum from the dexamethasone group was significantly decreased (P<0.05), while the content of 14,15-EET in serum significantly increased (P<0.01), and the content of 5-HETE in lung tissue significantly decreased (P<0.01). In the low-dose and high-dose Yantiao prescription groups, the content of AA metabolites PGD2, PGE2, 11(S)-HETE, and 5-HETE in serum and lung tissue was significantly decreased (P<0.05), while the content of 11,12-EET in both serum and lung tissue was significantly increased (P<0.05). ConclusionYantiao prescription has significant protective effects against LPS-induced ALI, which are related to its regulation of AA metabolic pathways in vivo.

Coronary artery fistula(CAF)is defined as the direct connection between coronary artery and cardiac cavity, great vessel or other vascular structures. Most CAF are congenital and often present as persistent murmur in asymptomatic children.Although abnormal shunt of most coronary fistulas has little effect on hemodynamics, some larger fistulas can also lead to a series of complications, such as congestive heart failure. Therefore, closed CAF has been recommended to treat or prevent myocardial ischemia, congestive heart failure, endarteritis, arrhythmia, thromboembolism, aneurysm dilatation and rupture and other complications.CAF is a rare congenital heart disease, and bilateral CAF originating from two coronary arteries is even rarer. At present, there are rare systematic reports on bilateral CAF.Therefore,a case of huge bilateral CAF at the same site was reported, and systematically, this kind of case was summarized,so as to improve the clinical attention of diagnosis and treatment of this kind of CAF.Nowadays, some new interventional devices and technologies are emerging with the continuous progress of cardiovascular interventional technology, making the treatment more accurate, safe and effective. At the same time, there is also a clearer understanding of the indications and operating standards for interventional treatment, which is more operable and predictable during the treatment process. Therefore, interventional treatment of CAF may partially replace surgical treatment as the preferred treatment method.

Objective:To investigate the risk factors of cardiovascular diseases(CVD)in patients with obstructive sleep apnea hypopnea syndrome(OSAHS).Methods:A total of 281 OSAHS patients admitted in Second People's Hospital of Huai'an City between May 2020 and March 2022 were selected and divided into CVD group(n=63)and no CVD group(n=218)according to presence of CVD.The OSAHS general data questionnaire designed by the re-search group was used to investigate the patients and univariate analysis was carried out,and the clinically significant single factors were included in the multivariate Logistic regression analysis.Influencing factors for CVD in OSAHS patients were analyzed. Results:Compared with no CVD group, patients in CVD group had significant higher age, body mass index(BMI),fasting plasma glucose(FPG)level, proportions of CVD family history, dyslipidemia and severe OSAHS(P<0.05 or<0.01).Multivariate Logistic regression analysis indicated that age,BMI,dyslipi-demia,FPG and severe OSAHS were independent risk factors for CVD in OSAHS patients(OR=2.213~2.482,P<0.05 or<0.01).Conclusion:Age,BMI,dyslipidemia,FPG and severe OSAHS are independent risk factors of CVD in OSAHS patients.Individualized prevention and control strategies can be formulated according to above fac-tors in clinic, which is of great significance for prevention of CVD occurrence.
9.Role of neuroplasticity in the recovery of cognitive function after stroke
Journal of Apoplexy and Nervous Diseases 2024;41(5):387-394
Post-stroke cognitive impairment(PSCI)has attracted substantial attention in the field of stroke research worldwide due to its high incidence rate and significant morbidity and mortality rates, but there are limited treatment meth-ods since the pathogenesis of PSCI remains unclear.Clinical observational studies have shown that patients achieve vary-ing degrees of cognitive recovery within the first 12 months after stroke, and such recovery is closely associated with the mechanisms of neuroplasticity.Gaining a deeper insight into the mechanisms of neuroplasticity is important for developing optimal strategies for nervous system repair after stroke and compensating for impaired brain functions. This article intro-duces the mechanisms of spontaneous neuroplasticity after stroke, including the plasticity of neuronal cells, synaptic neu-ral circuits, and brain networks, and their association with cognitive function and reveals the dynamic and intricate nature of neuroplasticity and brain function recovery.Subsequently, the article discusses the treatment methods for enhancing neuroplasticity, including pharmacotherapy, rehabilitation training, and noninvasive brain stimulation, and points out that early intervention may help to achieve a good outcome.Although there is a lack of clear clinical evidence for some new methods, inspiring results have been obtained in preliminary studies, which may help to initiate a new era in post-stroke therapy.
